TY - JOUR ID - TI - Electrophysiological Study Of The Effect Of Posture On The Sympathetic Skin Response In Normal Subjects And In Diabetic Patients AU - Yesar M.H.Al-Shamma MBChB, PhD PY - 2008 VL - 11 IS - 1 SP - 284 EP - 300 JO - KUFA MEDICAL JOURNAL مجلة الكوفة الطبية SN - 1993517X 27094462 AB - Thirty nine diabetic patients, of both insulin dependent and noninsulin dependent types, and thirty four age and sex matched control subjects were included in this study in an attempt to clarify the effect of changing posture on the peripheral nerves in both groups and to clarify the effect of diabetes on the peripheral nerve by using electromyography. Peripheral nerve dysfunction was evaluated by clinical examination, and electrophysiological testing of the somatosensory and autonomic peripheral nerves in supine and in standing position.The sympathetic skin responses (SSR) evoked by electrical stimulation of right and left median nerves and right and left common peroneal nerves were also recorded and analyzed. The sympathetic skin response test of diabetic patients show a significant reduction in latency and amplitude of the SSR in both upper and lower limbs with more affection in the lower limbs, and it was absent in 7 patients in upper and lower limbs, and in 13 patients in lower limbs only. Also there is a significant reduction in the amplitude of SSR in both control and diabetic groups on standing from supine position. Lastly, we conclude that the electrophysiological assessment of the peripheral nerve is essential for the diagnosis of the peripheral neuropathy which often accompanied with diabetes mellitus. Also we found that the abnormal SSR indicates the early affection of small unmyelinated c fibers in case of diabetic neuropathy. Also, we found that the sympathetic nerve fibers are significantly affected by changing body posture from supine to standing position.

تضمنت هذه الدراسة تسعة وثلاثين من مرضى السكري بنوعيه معتمد الأنسولين وغير معتمد الأنسولين، وأربعاً وثلاثين شخصاً صحيحاً متوافقين عمراً وجنساً مع مرضى السكري في محاولة لتوضيح تأثير التغير في وضع الجسم على الإستجابة السمبثاوية للجلد باستخدام جهاز تخطيط الأعصاب والعضلات. يتم تقييم الخلل الوظيفي في العصب المحيطي بواسطة الفحص السريري والإختبار الكهروفسلجي للأعصاب المحيطية التلقائية والجسمية الحسية في وضع الإستلقاء والوقوف. الإستجابات السمبثاوية التي تستثار بواسطة التحفيز الكهربائي للعصب الأوسط الأيمن والأيسر والعصب الشضوي المشترك الأيمن والأيسر، تم تسجيلها وتحليلها في وضع الوقوف والإستلقاء. إختبار الإستجابات السمبثاوية للجلد لمرضى السكري أوضحت إنخفاضاً ذا مغزى في الكمون والقمة للإستجابات السمبثاوية للجلد في الأطراف العليا والسفلى، مع تأثير أكبر في الأطراف السفلى، وكان مفقوداً في سبعة مرضى في الأطراف العليا والسفلى، وفي ثلاثة عشر مريضاً في الأطراف السفلى فقط. كذلك كان هناك إنخفاض ذا مغزى في القمة في الإستجابات السمبثاوية للجلد في كلا المجموعتين من مرضى السكري والأصحاء في وضعي الأستلقاء والوقوف. أخيراً، نستنتج بأن التقييم الكهروفسلجي لوظيفة الأعصاب المحيطية ضروري جداً لتشخيص اعتلال الأعصاب المحيطية الذي غالباً ما يترافق مع داء السكري، كذلك وجدنا بأن استجابة الجلد السمبثاوية غير الطبيعية تشير إلى التأثر المبكر للألياف العصبية الصغيرة نوع ج غير المغطاة بالنخاعين في حال إعتلال الأعصاب المحيطية المرافق للسكري. كذلك وجدنا بأن الألياف العصبية السمبثاوية تتأثر تأثراً ذا مغزى بتغير وضع الجسم من الإستلقاء إلى الوقوف.
