Manhal فلاح سالم منهل PY - 2012 VL - 54 IS - 1 SP - 38 EP - 41 JO - Journal of the Faculty of Medicine Baghdad مجلة كلية الطب SN - 00419419 24108057 AB - Background: Urinary tract infections (UTIs) are common in dialysis patients, are associated with increased rate of complications, and may be difficult to diagnose due to often subclinical presentation.Objectives: To examine theprevalence of urinary tract infections in hemodialysis patients with renal failure, and to evaluate the diagnostic accuracy of pyuria as a screening test in hemodialysis patients. Patients and Methods: A total of forty hemodialysis patients (27 males and 13 females) with renal failure were selected according to the study criteria. Clean-catch mid-stream urine specimens were obtained from study patients, their urinalysis and bacterial culture were performed according to standard techniques. Bacteriuria is defined as detection of  105 cfu/ml of a single microorganismin culture of urine specimens.Pyuria is defined as the presence of 10 neutrophils per high power field of voided mid-stream urine.Results: Urine from 25(62%) patients had no significant organism growth, whereasurine from 15(37%) patients grew  105 cfu/ml; 6 patients had been infected with E. coli, 5 patients with Klebsiella spp., one patient with Acinetobacter, one patient with α-hemolytic Streptococci, one patient with coagulase negative Staphylococci, and one patient with Proteus spp.. Fourteenout of 40 (35%) patients had pyuria and only 9 (64%) out of 14 patients had a positive urine culture for infection. The sensitivity and specificity of pyuria screening for UTI was 60% and 80%, respectively. The positive and negative predictive values are 64% and 77%, respectively. Conclusions: The result of this study demonstrates that pyuria was a good marker for significant bacteriuria in these patients. Cultural techniques are needed for susceptibility testing of bacterial isolates to guide antimicrobial therapy. The antimicrobial susceptibility tests in this study revealed that Amikacin, Gentamicin, Ceftazidime, Cefoxitin, and Imipenem act well on isolated bacteria.

خلفية الدراسة : تعتبرأخماج الجهاز البولي من الحالات الشائعة لدى المرضى الخاضعين لغسل الكلية، فضلا عن ارتباطها بمضاعفات متزايدة ، وربما يكون من الصعب تشخيصها بسبب عدم ظهورها سريريا على المريض أهداف الدراسة : لمعرفة نسبة شيوع أخماج الجهاز البولي لدى المرضى المصابين بالعجز الكلوي والخاضعين لغسل الكلية ولغرض تقييم الدقة التشخيصية للإدرار القيحي كإختبار أولي للمرضى نفسهم . المرضى وطرائق العمل : تم اختيار 40 مريضا (27 ذكرا و13 انثى) مصابين بعجز كلوي وحسب معايير الدراسة . تم جمع عينات الإدرار الوسطي وتم إجراء التحليل الكامل مع الزرع البكتيري طبقا للمعايير البكتريولوجية . تم تعريف الإدرار الجرثومي بأنه تحديد أكثر من 105 cfu /ml لجرثومة واحدة في زرع عينات الأدرار . تم تعريف الإدرار القيحي بأنه وجود أكثر من 10 خلايا بيضاء متعادلة في قوة التكبير العالية للمجهر لعينة الإدرار . النتائج : نتائج فحص الإدرار من 25 (62%) مريض لم تظهر نموا جرثوميا مهما ، في حين أظهرت نتائج الزرع عند 15 (37%) وجود أكثر من 105 cfu/ml ، ستة من هؤلاء كانوا مصابين بجرثومة الأشرشيا القولونية ، وخمسة منهم كانوا مصابين بجرثومة الكلبسيلا ومريض واحد فقط بجرثومة أسينيتوباكتر والمكورات المسبحية نوع ألفا ، والكورات العنقودية السالبة لفحص الكوأكيوليز والبروتيوس . أظهرت النتائج كذلك بأن هناك 14 (35%) مريضا كان لديهم أدرارا قيحيا و9 (64%) من هؤلاء كان لديهم زرعا موجبا للإدرار . كانت القدرة التحسسية والتخصصية لفحص الإدرار القيحي في التحري عن أخماج الجهاز البولي 60% و80% على التوالي . كانت القيمة التنبؤية الموجبة والسالبة للفحص 64% و77% على التوالي .الإستنتاجات : يستنتج من هذه الدراسة أن فحص الإدرار القيحي يشكل علامة جيدة للإدرار الجرثومي لدى هؤلاء المرضى . أن طرق الزرع مطلوبة لمعرفة أختبار فحص حساسية المضاد الحيوي للعزلات الجرثومية . لقد أظهرت هذه الدراسة أن الميكاسين والجنتامايسين والسفتازيديم والسيفوكسيتين والإمبينيم تعمل بشكل جيد ضد هذه العزلات .
