TY - JOUR ID - TI - Occurrence of Amoebic Versus Bacillary Dysentery among Children Under Five Years of Age in Baghdad City AU - Ali Awad Zaidan (FICM/FM) Riyadh KL Al Jiboory (FICMS) PY - 2013 VL - 9 IS - 2 SP - 35 EP - 39 JO - Al-Kindy College Medical Journal مجلة كلية الطب الكندي SN - 18109543 25214365 AB - Background: During Annual statistical report published by the Iraqi ministry of health the year 2004 showed that mortality rate was 0.15 per 1000 of diarrheal episodes among children under five years.Objectives: To study the occurrence of Shigellosis and Entamoeba histolytica in a sample of children from certain hospitals in Baghdad and determine its relation to some demographic factors.Methods: This cross sectional study was carried out in Baghdad city involving 400 children with bloody diarrhea under five years of age attending four hospitals, 130 cases from Central Pediatric Hospital, 110 cases from Al Mansoor Pediatric Hospital, 90 cases from Al Kadhimya Teaching Hospital and 70 cases from Mohammed Baaqir Al Hakeem Hospital for the period from the first of January through October 2005. Stool specimens were examined microscopically for detection the amoebic dysentery and negative cases were sent for stool culture for detection of Shigellosis. Results: The study revealed that Entamoeba histolytica was the most frequently identified cause of bloody diarrhea 362 (90.5%), while shigella was identified in 38 cases (9.5%). young children below two years of age were more liable to have diarrhea than other age group. Both shigella and amoebic cases shared the following risk factors, lack of breast feeding, unboiled water, low level of education of mother and overcrowding.Conclusions: Child health services should concentrate on health education of mother to encourage breast feeding, boiling of water, general personal and environmental hygiene to prevent spread of these infection.

من خلال الاحصائيةالسنوية التي تصدر من قبل وزارة الصحة العراقية لعام 2004 وجد ان نسبة الوفيات هي 0.15 لكل 1000 حالة اسهال بين الاطفال دون سن الخامسة من العمر.الهدف: لتعين نسبة حدوث الزحار الأميبي والزحار البكتيري وتحليل بعض عوامل الخطورة وكذالك الأعراض السريرية المختلفة لللأسهال الدموي عند الأطفال دون سن الخامسة من العمر.نوع الدراسة:دراسة مقطعيةمكان الدراسة اجريت الدراسة في مدينة بغداد حيث شملت 400 طفل دون سن الخامسة والذين يعانون من الأسهال الدموي من الراقدين والمراجعين للعيادة الخارجية لاربعة من المستشفيات حيث تم اخذ 130 حالة من مستشفى الطفل المركزي و110 حالة من مستشفى المنصور للاطفال و90حالة من مستشفى الكاظمية التعليمي و70 حالة من مستشفى محمد باقر الحكيم للفترة من الاول من كانون الثاني لغاية الثلاثون من تشرين الاول.الطريقة: تم فحص البرازالعام للمشمولين في هذه الدراسة للكشف عن حالات الزحار الاميبي, ثم زراعة العينات السالبة مختبريا للكشف عن حلات الزحار البكتيري.النتائج: اظهرت الدراسة بان نسبة( 5. 90 %) من حلات الاسهال الدموي هي زحار اميبي وان نسبة الزحار البكتيري هي( 9.5% )0وكذلك اظهرت هذه الدراسة بان الاطفال دون السنتين من العمر هم اكثر من غيرهم عرضة للاصابة بالاسهال الدموي. ومن نتائج الدراسة ايضا وجود تشابه في علاقة عوامل الخطورة مع حالات الاصا بة لكل من الزحارالاميبي والزحار البكتيري وخاصة الرضاعة الصناعية, وتدني مستوى ثقافة الام, وشرب الماء الغير مغلي وكذلك كثرة عدد افراد الاسرة.الاستنتاج: لذلك يجب التركيز على التثقيف الصحي للامهات وتشجيعهم على الرضاعة الطبيعية, وغلي الماء قبل الاستعمال, بالاضافة الى النظافة الشخصية والبيئية لمنع انتشار المرض.
