A Comparative Study between Healthy and Reversible Obstructive Uropathy Patients under Anesthetic Muscle Relaxation by the use of Atracurium Besylate

Abstract

This study was designed to identify the correlation among the duration of action and the degree ofmuscle relaxation of atracurium by the use of a peripheral nerve stimulator in healthy surgicalpatients and in patients need surgery due to reversible obstructive uropathy . The total number ofpatients was 33.They were divided into two groups, group I (22 patients) and group II (11 patients)All the surgical operations included in the research, last for not less than 75 minutes . In bothgroups I and II Atracurium was given as a bolus intravenous injection in a dose of 0.5 mg/kg .Thebolus dose of the muscle relaxant was followed by 5 i.v injections (0.1 mg/kg)for each top up doseaccording to the monitored neuro-muscular transmission by the peripheral nerve stimulator. In thefirst group the duration of action of the bolus dose was 21.87±3.03 minutes as determine bypositive TWR and positive T3 by TOF. The duration of action of 1st ,2nd ,3rd and 4th intravenousdoses administered after the bolus dose were 13.5±2.08,12.37±2.40,13.19±2.84 and 13.28±1.40minutes respectively. In the second group ( patients with reversible obstructive uropathy) whoreceived the bolus dose of atracurium followed by 5 intravenous doses ,the duration of action of thebolus dose was 19.79±2.33 minutes ,while the duration of action of 1st ,2nd ,3rd and 4thintravenous doses administered after bolus dose were 12.89±0.76,13.28±1.45,12.28±2.87 and13.81±1.88 minutes respectively.
