Tumor Suppressor Gene( E-Cadherin )and Its Role in Normal and Malignant Cells

Abstract

Tumor suppressor genes (E-cadherin) are particularly active area of research in development and tumorigenesis. The calcium-dependent interactions among E-cadherin molecules are critical for the formation and maintenance of adherent junctions in areas of epithelial cell-cell contact. Loss of E-cadherin-mediated-adhesion characterises the transition from benign lesions to invasive, metastatic cancer. Nevertheless, there is evidence that E-cadherins may also play a role in the wnt signal transduction pathway, together with other key molecules involved in it, such as beta-catenins and adenomatous poliposis coli gene products. The structure and function of E-cadherin, gene and protein, in normal as well as in tumor cells are reviewed in this paper.

Prevalence of Cardiac Dysfunction among Thalassaemic Patients in Babylon Governorate

Abstract

A prospective study was conducted on 216 patients with thalassaemia classified as 135 patients with -thalassaemia major, 57 patients with thalassaemia intermedia and 24 patients with sickle cell/ thalassaemia, who were attending the thalassaemia center at Babylon Maternity and Children Hospital, from period of August 2003 to April 2004. Their age ranged from (1- 30) years with a mean of 11.28 years. They were studied for cardiac dysfunction, 39 patients (18%) were found to have cardiac abnormality classified as (43.7%) as asymptomatic patients, (33.3%) as asymptomatic heart failure, and (23%) as asymptomatic heart failure. Risk factors for cardiac dysfunction were: 1. Age older than 10 years. 2. Frequent blood transfusion more than 120 times. 3. Serum ferritin more than 1800 - 2000 ng/L.

Once – Daily Therapy for Helicobactor Pylori Infection Using Tinidazol Containing Regimens

Abstract

In this study 80 patients were divided into four groups, (20 patients) for each, all of them were infected by H. pylori. The patients were treated by once daily regimens as follow: Group (I) (tinidazol+ omeprazol) Group (II) (I+amoxyllin) Group (III) (I +azethromycin) Group (IV) (I + clarethromycin) The cure rate was 21%,55%,83%,93% respectively ,the results were statistically significant when group II compared with group I ,III. & IV, PV <0.05. In spite of these results, this regimen needs more evaluation to be used routinely.

High Risk Factor Values of Cardiovascular Disease and Uric Acid Levels in Sera of Patients with Type 1 Diabetes Mellitus

Abstract

Diabetes mellitus is a chronic illness required ongoing medical care. As a result, laboratory testing has become an essential part of the evaluation and monitoring process in diabetes management. Further tests are required to establish the correlation between hyperglycemia and other disease parameter. Therefore, the correlation between cardiovascular disease risk factor values and uric acid levels in sera of adult patients with type 1 diabetes mellitus (without renal diseases) and healthy controls have been studied. Total cholesterol, HDL-cholesterol and uric acid in sera of 52 patients (36 males and 16 females) with poor hyperglycemic control type 1 diabetes mellitus and 30 healthy controls (19 males and 11 females) were determined. Results of present study show significantly higher risk factor values (P < 0.001), which means increasing in the probability of atherosclerosis incidence. Also hyperglycemia found to be associated with hyperuricemia in type 1 diabetic patients.

Isolation and Characterization of Morganella morgani from Alkaline Urine

Abstract

A total of 75 alkaline urine samples obtained from patients with urinary tract infections, only six isolates of Morganella morgani were isolated. All these isolates were urease producers, which was produced constitutively & was responsible for changing the pH of urine into alkaline. Also, some isolates were able to produce haemolysin (two isolates), uronic acid (one isolates) and morganocin (one isolates). Furthermore, Antimicrobiol sensitivity tests were performed on Morganella isolates & they showed high resistance rates against the commonly used antimicrobial agents: ampicilline, amoxicilline, trimethoprim, nalidixic acid & cephalexin, where as all isolates were sensitive at high rates to ciprofloxacin & cefotaxim which nowadays used successfully in treatment of UTI. This work was the first one performed one Morganella morgani in Hilla province Iraq.

Postmenopausal Bleeding: Clinical Significance and Histopathological Evaluation

Abstract

Normal menstruation is defined as the bleeding from secretory endometrium associated with an ovulatory cycle not exceeding a length of 5 days. Any bleeding not fulfilling these criteria is referred to as an abnormal uterine bleeding. Some of these are the result of an identifiable pathological lesion, such as endometriosis, submucous myoma, endometrial polyp, or cancer, particularly in the postmenopausal patient. The aim of the present study was to investigate the clinical significance and endometrial pathology in patients with postmenopausal bleeding (PMB) in terms of etiology, risks factors, incidence of malignancy, and histopathological evaluation. 202 cases of PMB admitted to Basrah Maternity Teaching Hospital from 1990-1999 underwent a detailed history, clinical examination and full investigation, including full laboratory investigation, pelvic ultrasound, and examination under anesthesia (EUA) with dilatation and curettage and tissue sampling. The age range of the patient was from (35 to 72 years) with a mean of (49 years). The Results showed that Benign pathology was found in (184 / 209) cases. These included senile atrophic endometrium, normal functioning endometrium, endometrial hyperplasia, endometritis, polyps, cervicitis, and cervical polyps. Malignant pathology was found in (18) cases including (8) cases of cancer of the cervix and (10) cases of adenocarcinoma of the endometrium. It is concluded that postmenopausal bleeding is an important symptom and requires careful and prompt evaluation to eliminate the possibility of malignancy as quickly as possible.

Maternal and Cord Blood Fructosamine in Normally Pregnant Women During Delivery

Abstract

To compare between serum fructosamine values in maternal and cord blood and the relationship with birth weight, serum albumin or total protein concentration as well as the influence of albumin or total protein on fructosamine values and the need for correcting their values accordingly using different equations. Plasma glucose (PG) and serum fructosamine (FAm), albumin and total protein were measured in maternal and cord blood from 20 full-term pregnant women delivered by normal vaginal delivery women. Calculation of corrected fructosamine (FAc) was made according to serum albumin or total protein values using different equations. The pregnant women were attending Al-Batool Maternity Hospital in Mosul during April 2001. When comparing the maternal and cord blood parameters, there was no significant difference in PG and albumin concentrations, while there was a slight significant difference between them in FAm (p<0.01), and a highly significant difference in total protein (p<0.001). There was no significant correlation between the weight of the baby and FAm of the maternal blood (r=0.08) or cord blood (r=0.18). There was also no significant correlation between FAm and albumin in the cord blood (r=0.31) or maternal blood (r=0.07). After correction, comparison between FAm and FAc showed a slight difference between FAc and FAm in the maternal blood (P< 0.05 based on albumin and p<0.001 based on total protein). In the cord blood, FAm showed also a highly significant difference from FA (P< 0.001). There was no significant difference between FAc in the cord blood and in the maternal blood using all equations. Cord blood fructosamine was lower than maternal blood fructosamine with no significant correlation was found between weight of baby and maternal fructosamine or cord blood fructosamine. Correction of measured fructosamine accordingly improves its usefulness as an index of glycated protein for assessing glycaemic state.

Diabetes Mellius is The Predominant Risk Factor of Dupuytren , s Disease in Babylon Aprospective Matched Case Control Study

Abstract

In Merjan teaching hospital we recorded (3200) cases of hand complaints in period from Nov. 2002 – June 2004. Of them, Dupuytren’s hand diseases were represented in (84) cases 2.6% the (study group). which were investigated for the association and severity, with diabetes mellitus ,smoking and other risk factors , this study group also were matched for age and gender with other three groups : Group I: (230) Patients who had diabetes mellitus for more than (10) years duration, who were neither smokers nor alcoholic or epileptics. Group II: (300 ) community controls , who were smokers “ more than 20 cigarette per day for more than 10 years “,who were neither diabetics nor alcoholic or epileptics . Group III: ( 420 ) community controls , who were randomly selected . The results showed that the Dupuytren’s disease is strongly associated with diabetes mellitus, where in study group: 25% of Dupuytren’s were diabetics, while 11.9% of Dupuytren’s were smokers 2.3% were epileptics on phenytoin. Group I: 9.5% of diabetics patients had got Dupuytren’s Group II :2.6% of smokers had got Dupuytren’s disease . Group III: The total number of Dupuytren's disease were ( 7 ) cases 1.6 %. Grouped as follow ( 4 ) cases were diabetics , ( 1 ) case was smokers. And (2) cases had not apparent risk factors . On basis of the results, the Dupuytren’s disease is strongly associated with diabetes mellitus, although milder , appeared earlier in age than other risk factors and mostly bilateral. There was no significant difference regarding incidence between different types of jobs , or between dominant hands and non dominant ones . The purpose of the present prospective case matched study , is to elucidate the role of diabetes mellitus as main predisposing risk factor of Dupuytren’s disease In Babylon.

Morphometric and pathological Study of Urinary Bladder Carcinomas: Significance of Schistosomal Fibrosis

Abstract

Objective measurement of microscopic features has been advocated for decades as a method to make more reproducible and "scientific" the practice of histopathology, but it is only recently that technical advances in computing have rendered this procedure suitable for diagnostic and prognostic determinations in surgical pathology. Urinary bladder cancer is the third most common malignancy in Iraq. Heavy Schistosoma haematobium infection predisposes to bladder cancer which is usually of squamous cell type and accounts for about one quarter of deaths from this infection.We investigated the extent of fibrosis in forty urinary bladder carcinomas (15 Schistosomal associated SASCC and 25 Schistosomal non-associated transitional cell carcinoma SNATCC). The cases were subjected to quantitative assessment of their fibrosis by (1) colorimetric micromethod for collagen measurement (2) morphometric assessment of collagen by use of image analyzer. The results obtained by both methods showed that SASCC were more fibrogenic than SNATCC and displaying more desmoplasia. It is concluded that the total amount of collagen in addition to the distribution pattern of the fibrotic process in schistosomal cases around the newly formed blood vessel and lymphatic both play a role in determination of the unique behavior of such neoplasm.

Abstract

Ninety six patients with large bile duct obstruction were admitted to the surgical ward in the specialized teaching hospital for gastro-enterology and hepatology between January 2002 and march 2004 - Each of them was offered a therapeutic procedure (surgical or otherwise), that successfully drained his/her obstructed biliary system. - Each of them was assessed clinically and biochemically before and after that therapeutic procedure and the follow-up continued for 10 days to assess the immediate clinical and biochemical responses. - Most of the clinical and biochemical parameters chosen for the assessment improved towards normality during the observation period. - Aberrant results were noticed, specified, and analyzed, in order to understand the underlying pathological factors leading to them. - The results were discussed. Recommendations were suggested to improve postoperative assessment of such patients.

Immunology of E. coli Persistant pyuria

Abstract

Fourty Persistent Pyuria Patients were diagnosed as E. coli persistant pyuria . Of which 33 were Immune (In a sense of mounting normal adapted immune response) and seven were immunocompromised (In a sense of mounting subnormal adapted immune response .The female / Male ratio was 13/27 . The patients age range was 30-70 years .The median total serum protein was 7.274 g/dl . Likewise the median serum total globulin was 4.302 g/dl . while the median of systemic to the median of mucosal globulin ratio was 0.69 . These scores were generally higher than those of normal control subjects . The median immunoglobulin class concentrations were 2.339, 0.195 and 0.309 g/dl to classes IgG , IgM and IgA respectively . E. coli immunodominant epitopes were intiating specific circulating and mucosal agglutinin as well as significant leucocytes inhibition factors in immune patients and nonsignificant in immunocompromized . Thus, E. coli epitopes could be of T dependent type and activate either or both Th1 and Th2 lymphocyte, or it might be of T independent epitope type

In vitro evaluation of some antibacterial drugs against Staphylococcus aureus isolated from recurrent boil

Abstract

A comparison of the antibacterial activity for each of amikacin, amoxicillin, ampicillin, cephalexin, chloramphenicol, clindamycin, neomycin and rifampin was evaluated against Staphylococcus aureus isolated recently from recurrent boil. The antibacterial sensitivity was determined by agar diffusion assay using antibiotic disc on Muller-Hinton agar. Rifampin and clindamycin were the most effective antibacterial drugs against this microorganism, Cephalexin and chloramphenicol approximately showed the same activity and they were more effective than amikacin. Neomycin, on the other hand had the lowest activity than the previous drugs. Staphylococcus aureus that isolated from recurrent boil shown to be resistant for both amoxicillin and ampicillin.

Serum Iron as an independent predictor of Survival in Childhood Acute Leukemia

Abstract

The present investigation was motivated from the consideration that a host environment rich in iron, might offer favorable growth conditions for leukemic cells in addition to infection, thus affecting survival. Hypothesizing that any effect of an increased serum iron on the risk of bacterial infection would be particularly important in immunosuppressed patients during chemotherapy. Serum iron measurments were obtained on fifty – eight (58) pediatric patients with acute Lymphocytic Leukemia (ALL) before and after chemotherapy and after achieving clinical remission. The relationship between serum iron and survival was observed when patients were stratified according to length of time from diagnosis, risk group and French – American – British (FAB) classification. A significant difference (P< 0.001) was found between the survival of patients according to whether their first measured serum iron was greater than 165 µg/dL or less than 165 µg/dL, with no deaths in the group with serum iron less than 65µg/dL. It is suggested that infectious complications (pneumonia , sepsis, etc..), FAB classification should be taken into consideration in evaluating serum iron levels as prognostic variable in survival of acute lymphocytic leukemia; however, these findings suggest the need for a prospective study.

Hematological changes in children suffering from visceral leishmaniasis (Kala azar)

Abstract

This study was designated to investigate the changes that occur in certain clinical and hematological features in visceral leishmaniasis (VL) among Al-Mahawyl children, during the period 20/11/2000 - 20/4/2001. The total numbers used was 53 of both healthy and infected children from VL . Their ages were 1 month – 3 years. They were divided into two age groups. First group 1 month – < 1 year and second group 1 – 3 years old. The commonest presenting clinical features were fever, pale and hepatosplenomegally. The hematological changes studied RBCs count, Hb and PCV in both age groups of visceral leishmaniasis were significant (P < 0.01) decrease than control. The values of total WBCs, the percentage of neutrophil in both age groups of VL showed significant (P < 0.01) decrease. While the percentage of lymphocyte and ESR of this study indicated a significant (P < 0.01) increase in both age groups of VL. Whereas the values of platelets count showed significant (P < 0.01) decrease. The main changes may be attributed to hemolysis, hepatosplenomegally and ineffective erythropoisis as well as iron and folate deficiency.

The Role of Renal Ultrasound in Urinary Tract Infection in Children

Abstract

To study the role of renal ultrasound in urinary tract infection in children, any child with proved urinary tract infection is examined clinically and sent for renal ultrasound and results were analysed. A total of 132 patients with urinary tract infection age 1 month — 14 years, 40(30%) patients were found to have urinary tract abnormalities on ultrasound and the commonest abnormality was renal stones and the commonest organism linked with renal abnormality was E Coli. Renal ultrasound is a useful tool to diagnose urinary tract abnormalities, however, it is not useful to detect renal scars and vesicoureteric reflux.

The Distribution Of Malaria In Rabiea Region In Northern Iraq

Abstract

The region of Rabiea in northern Iraq, was having increasing number of malaria cases in January 1995 concerning farmers and their families. A screening was done among them using parahsitological methods of laboratory diagnosed malaria .P. vivax was the only species of anopheles were recoverd namly , A. superpictus A. sacharovi and A. pulcherimus. The strains are the first time recoverd in this region in northern Iraq. The overall rate positivity (7.44%) was highest among persons aged (21-30) years (43.85%) and lowest among (51-60) years (2.47%) and above 61 years old was (0.48). This study demonstrated statically insignificant difference in infection between males and females (Table 2). Seasonal distribution of infection with vivax washighest during Autumn months (52.88%) followed by summer (30.27%), winter (8.68%) and spring (6.94%). Statically study revealed just significant in summer and autumn (table 3). The gradual control of this outbreak of malaria infection in 1995, through the years 1996 till the end of december 2000 might be due to the efforts of ministry of health by using insecticide and other rmethods including treatment directed against the infected people with malaria.

Distribution of Pathogens and Their Antibiotic Sensitivities in Urinary Tract Infection in Children Under Five Year in Hilla, Iraq

Abstract

A cross sectional study was done on out patient children, where urine culture and sensitivity test was done on samples taken by suprapubic aspiration .The positive cultures were studied in relation to age and gender. The study shows a difference in the distribution of uro- pathogens and it’s microbial sensitivities. Females predominate upon males in all age groups. E.coli, klebsiella and proteus are the most common pathogens 44%, 18%, 17%, respectively. The most active antibiotics are nalidixic acid, Gentamicin and nitrofurantion 82%, 81%, 80% respectively.

Abstract

In this study we reviewed the management of 164 patients with heamorrhoids underwent rubber band ligation on out patient basis and review of related literatures. These patients with haemorrhoids underwent rubber banding as out patient. The male patients put in a knee-chest position while female patients were in left lateral position. Thay were given diclofenac sodium 75 mg . IM. We Applied baron band ligation device. We treated all hemorrhoids in a single session,(including multiple hemorrhoids). All cases passed uneventfull postoperative period. It is a simple procedure; it is done as out patient procedure with minimal complications.

Abstract

To assess the ability of colour/pulsed Doppler Ultrasound to assess the impedance of blood flow through spiral arteries in the central and peripheral area of the placental bed. This study has shown that there is statistically significant lower impedance values of blood flow in spiral arteries were obtained in central compared with peripheral area of placental bed, so we can compare the spiral artery flow velocity waveform in normal pregnancy and complicated pregnancies.

The Sunflower Oil (SFO) as mucosal Immuno adjuvant In Rabbit O. caniculus

Abstract

The wheat gluten was separated by alkalinzation methods method, wheat gluten peptide antigen (WGPA) was found as a proleine rich gluten. WGPA solutions were used as a test imunogen for immunopriming of rabbits (O. caniculus). Six groups of rabbits, each of two were assigned as; Intramuscular Subcutaneous IM-SC saline control group (I), Oral saline control group (II), IM-SC/ WGPA group (III), Oral/ WGPA group (IV), IM-SC-/ WGPA-SFO group (V) and Oral/ WGPA-SFO group (VI). WGPA specific immunesera were separated decomplementized, saved in aliquots at -18 C o till use. The immunoadjuvant effect was scored by comparing titers in the group V and III as well as VI and IV. The WGPA specific polyclonal antibody titers in the sera of the groups V were as 40960 and III 5120. Likewise, in the groups VI and IV were 40960 and 10240 respectively. The mucosal specific anti WPA antibody titer in group III. Was similarly, it was 3776 in the group VI as it was reaching 1920 in the group IV. Thus, SFO could be used as a candidate enhancer of mucosal immune responses to the mucosally applied vaccines, for animal walfare at first, then after sophistication for human walfare.

Abstract

A 47-year- old man presented in August,2004 with ,a three years duration , of insidious Low back pain and stiffness. His back stiffness was more severe and pronounced at waking-up in the morning , the pain and stiffness eased with activity as the day is going on. The pain was dull in nature , radiated to his buttocks , and poorly localized. He had no recent illnesses . His past medical history included a lithotomy from Urinary bladder , 5 years ago. Also he noticed progressive darkening of the skin over his face for the last several years.
