Abstract

Background:Progesterone hormone is important in the preparation for and in maintaining of pregnancythrough different mechanisms but mainly by shifting the balance from Th1 to Th2 mediatedimmunity to avoid rejection of the fetus; in this study we tried to find out whether there is arelation between the level of the expression of progesterone and its stimulant HCG hormone andrecurrent pregnancy loss (RPL).Patients and Methods:Immunohistochemistry technique was performed to detect and determine the expression ofprogesterone and HCG hormones using paraffin embedded sections of curate samples obtainedfrom 40 women, who where divided into three groups: 24 women with RPL, 10 women withabortion for the first time, and 6 women with induced abortion.Fac Med Baghdad200Results:The levels of the expression of both progesterone and HCG hormones were found to besignificantly down-regulated in the first group as compared with the second group (p=0.00), andthe third group (p=0.00), with a highly significant positive correlation between these twoparameters (r=0.866, p<0.01).Conclusion:Low expression of progesterone in women with RPL could raise the possibility ofunderlying immuno-endocrine pathology responsible for this recurrent loss due to the importantrole of progesterone as an immunosuppressive hormone help in maintaining pregnancy.Key wards: Progesterone, HCG, RPL

Abstract

Objective: The study was designed to correlate the residual beta cell secretary capacity with the ovarian function in Iraqi diabetic women.Setting: Department of physiology, Medical College of Al-Mustansiriyah University in cooperation with the Iraqi National Diabetes Center (NDC) of Al- Mustansiriyah University in Baghdad from November 2004 till November 2005.Outcomes measures: Residual Beta cell function was estimated by measuring C-peptide level by radioimmunoassay technique while ovarian function was estimated by measuring serum levels of FSH, LH, Progesterone and Estrogen using enzymatic Link Fluorescent assay technique.Results: Data showed that the C-peptide level in type 1 diabetic subjects was significantly lower than the control subjects (p <0.0001).C-peptide was significantly correlated to glycemic control expressed by HbA1c while it was not significantly correlated with fasting plasma glucose.Also C-peptide showed no significant correlation with the, FSH, LH, progesterone, and estradiol levels in diabetic subjects and the healthy control group.Conclusions: The present data reflects the deleterious effect of beta cell secretary capacity dysfunction on hyperglycemia and emphasizes that this decrease in beta cell capacity had no effect on ovarian function except for Progesterone.Keywords: C-peptide, FSH, LH, progesterone, estradiol.

الهــدف :- صممت هذه الدراسة لتقيم العلاقة بين الوظيفة الافرازية الثمالية لخلايا بيتــا في البنكرياس ووظيفة المبيض عند بعض النساء العراقيات المصابات بالنوع الاول من داء السكري .مكان البحث :- أجريت هذه الدراسة في قسم الفسلجة لكلية طب المستنصرية بالتعاون مــــــع المركزالوطني للسكري التابع للجامعة المستنصرية في بغداد للفترة من تشرين الثاني للعام الدراسي 2004 لغاية تشرين الثاني 2005 .القياسـات :- تم الاستدلال على الوظيفة الثمالية لخلايا بيتا في البنكـــــرياس بقياس البيتيد الرابط بطريقة المقايسة المناعية الشعاعية بينما تم الاستدلال على وظيفـــة المبيض بقياس الهرمونات الانثوية المفرزة من المبايض ( الاستروجيـــــن والبروجستيرون ) والهرمونات الانثوية المفرزة من الغدة النخامية هرمـون الاباضة والهرمون المحفز لنمو البيوض بطريقة المقايسة المناعية الانزيمية .النتـائـج :- أظهرت النتائج بأن مستوى الببتيد الرابط عند المصابات بالنوع الاول مـــــــن السكري اقل وبدلاله معنوية واضحة من مستواه عند مجموعة الاصحاء .وأن الببتيد الرابط مرتبط وبدلاله معنوية مع السيطرة الايضية P<0.0001بدلالة خضاب الدم المسكر بينما كان الترابط بدون دلاله معنوية اذا كانت السيــطرة الايضية بدلاله مستوى الكلوكوز في بلازما الدم في حالة الصيام كذلك فأن الببتيــــد الرابط لم يظهر أي ارتباط ذو دلاله معنوية مع الوظائف المقاسة للمبيض عند النساء المصابات بالنوع الاول من داء السكري او النساء في المجموعة الضابطة من الاصحاءألاستنتاج :- وثقت هذه الدراسة التأثير التراجعي لارتفاع نسبة السكر في الدم على القابلـــــــية الافرازية لخلايا بيتا وتؤكد كذلك بأن النقص في القابلية الافرازية لهذه الخلايا ليس له تأثير على وظائف المبيض .

Abstract

ABSTRACT:BACKGROUND:Rheumatoid arthritis and systemic lupus erythematosus are multifactorial autoimmune diseases.Some recent reports indicated a hormonal disturbances affected the balance between Th1 and Th2lymphocyte response.OBJECTIVES:To investigate the immunological and hormonal disturbance in patient with rheumatoid arthritis(RA) and systemic lupus erythematosus (SLE).METHODS:Serum samples, were collected from patients with RA, SLE and control, then the tests forantinuclear antibodies, anti double strand DNA, anticardiolipin antibodies are done by usingEnzyme linked immuno assay (ELISA) method. Also, hormonal studies including estrogen,progesterone and prolactin level are done by using the Radio immunoassay technique (RIA).RESULTS:The results indicated the increasing of ANA, anti ds-DNA, and anticardiolipin antibodies. Also,elevation in the levels of estrogen, progesterone and prolactin in patients with RA and SLEcomparing with control.CONCLUSION:Immunological and hormonal disturbances in patients with RA and SLE were documented throughthrough the increasing of ANA, anti dsDNA anticardiolipin antibodies and elevation of the level ofestrogen, progesterone, and prolactin.
