HLA prevalence in Iraqi patients with ischemic heart disease

Abstract

Background: The etiology of ischemic heart disease (IHD) is believed to have an immunological component. Association with human leukocyte antigens (HLAs) has been previously reported, particularly with DR6.Patients and methods: 75 cardiac patients were admitted to the coronary care unit, Baghdad Teaching Hospital over the period October 2008-May 2009 with the clinical diagnosis of acute coronary syndrome and STEMI myocardial infarction their ages range was (25-82) years the number of male was (55) (73.3%) and female was (20) (26.7%). All cases have routine ECG, cardiac marker’s measurements, routine haematological, Biochemical test and 2mls of blood reserved for HLA study.
Results: It was found that HLA-DR1 (8%) (P 0.001) has significant risk factor in the development of ischemic heart disease while HLA B (62%) (P 0.0009) has a protection factor in ischemic heart disease.
Conclusions: Our result suggests that strong relation between the incidence of acute ischemic episode (acute coronary syndrome and STEMS myocardial infarction and HLA-DR1) which mean that there may be predisposing genetic factor for the development ischemic heart disease.

Abstract

ABSTRACT:BACKGROUND:Immune system may interplay between Chlamydia pneumoniae infection and ischemic heart disease (IHD).Major histocompatibility genes regulate innate and adaptive immunity..OBJECTIVE:This study was established to shed light on the possible association between ischemic heart disease (IHD) with Chlamydia pneumoniae infection and HLA antigens.PATIENTS AND METHODS:Microlymphocytotoxicity assay has been applied for HLA-typing of 150 blood samples of 100 IHD patients and 50 healthy normal controls, In addition enzyme linked immunosorbant assay (ELISA) used to detedct C pnuminiae IgA and IgG.RESULTS:An increased frequency of HLA-A*2, B*35 alleland HLA-A*2-B*35 haplotype was observed for patients group versus control group with P-value (0.0001, 0.05, and 0.001) respectively.CONCLUSION:This finding demonstrated that the HLA-B*35 positive haplotypes confer the C.pneumoniae-related risk for IHD. HLA-DR3,DR4 and DR7 might play a role in AIH susceptibility

Abstract

Background: Ischemic heart diseases (I.H.D) become the most common cause of sudden death, and is also the most common reason for death of man and women over 20 years of age in the world. Many Factors Play a role in pathogenesis of I.H.D. among those could be Immune – inflammatory markers which may lead to development of this disease.The present study was conducted to obtain more clarification about the impact of some immuno – inflammatory markers (IL- 8 , IL- 4 and IFN- Ճ) on the Clinical expression of heart disease among Iraqi patients . Patients and methods: Hundred Iraqi patient with I.H.D. (80 male and 20 female) ages ranged between 20 to 80 year were involved in this study, include 4 subgroups: 17 with heart failure (H. F) ; 26 with myocardial infraction (M.I) ; 12 with stable angina (S.A) and 45 with unstable angina (U.A) who attending the Iraqi Center for Heart Disease and Baghdad Hospital from December 2006 to march 2007 . For quantitative estimation of serum levels of IL- 8, IL- 4 and IFN- Ճ by Enzyme Amplified Sensitivity Immuno Assay (EASIA) technique for both patients and healthy control groups. Results: Significant increased (P< 0.05) in the serum level of interleukin – 8 in all patient subgroups: H.F., M.I, S.A and U. A. respectively Compared with control group, also all patient subgroups: H. F., M. I, S.A and U. A. Consequently revealed Significant increased (P< 0.05) in the interleukin – 4 Concentration compared with control group. More over, highly Significant increased (P< 0.01) in the interferon – Ճ concentration in the patients Subgroups: H. F, M. I, S.A and U. A respectively compared with control group. Conclusion: Elevated level of inflammatory Markers (IL- 8, IL- 4 and IFN – Ճ) in all patients of I.H.D reflects the importance immuno – inflammatory elements as risk factor in the pathogenesis of Heart Disease. Abstract

Objectives :The aim of this study is to show the pattern of ischemic heart disease (IHD) cases admitted to CCU in Baquba teaching hospital and to study IHD cases in relation to certain variables (risk factors) which include age , sex , residency , occupation , physical activity , smoking , diabetes mellitus (DM) , hypertension and obesity.Methods :The study included 200 patients with IHD who were admitted to CCU in Baquba teaching hospital in a period of 3 months from 1st. February to 30th. April 2008 . We registered the risk factors of IHD as age , sex , DM , hypertension , smoking , obesity , physical inactivity and residency .Results :This study showed that most of IHD cases were diagnosed as unstable angina (52%) followed by myocardial infarction (MI) (37%) and the remaining cases were angina pectoris (11%) .IHD was more in males (56%) .Most cases were between (46-65) years old (57.5%) .IHD was more in urban (70.5%) . Smokers were (31.5%) , diabetics (25%) , hypertensive (54%) , Obesity (41%) , physical inactivity (59.5%) Conclusion :This study showed the pattern of IHD cases admitted to CCU in Baquba teaching hospital and the relation of risk factors and there percentage to IHD. So prevention, modification and improvement of risk factors can decrease numbers and severity of IHD in society .

Abstract

Background and Objectives: In the presence of ischemia, albumin, undergoes changes resulting in the formation of ischemia modified albumin .Increased serum concentration of ischemia modified albumin that was measured by albumin cobalt binding assay have been found in patients with myocardial ischemia. The purpose of this study was to investigate the relationship between serum albumin concentration and ischemia modified albumin concentration.Methods: Eighteen patients with stable angina and fifteen who didn’t have any evidence of coronary artery disease , were taken as control subjects, both groups underwent treadimall exercise stress test ,about 15 minuets after treadimall test blood samples were drawn from all the subjects, brought to laboratory to measure the serum ischemia modified albumin concentration and serum albumin concentration.Results: The study showed a significant difference in ischemia modified albumin between study groups, while no significant difference in serum albumin between both groups. There is a largest Receiver Operating Characteristics(ROC) area for ratio of serum albumin cobalt binding to serum albumin than serum albumin cobalt binding capacity alone.There is a weak positive linear correlation for both cupper and cobalt binding capacity among control subject ,where there is a weak negative linear correlation for both markers among cases with ischemic heart disease (IHD).Conclusion: The quantification of the role of serum albumin in combination with albumin cobalt binding capacity is expected to differentiate between ischemic heart disease patient and normal subject to larger extent.
