Abstract

ABSTRACTBackground: Bladder cancer is third most common malignant tumors in both men and women in Iraq. Transitional cell carcinoma ( TCC) account for > 90% of bladder cancer. Survivin, family member of inhibitor of apoptosis (IAP) that play important role in both cell division and inhibit of apoptosis, Survivin express in vast majority of human cancer but not in normal adult tissues. Objective: to evaluate the possible role of Survivin in patient of transitional cell carcinoma of bladder. Materials and methods: formalin fixed, paraffin embedded tissue from (45) patients with transitional cell carcinoma of bladder from Al-Kadhimiya Teaching Hospital in Baghdad were included in this study. In addition, (16) apparently normal bladder biopsies were as a control groups. Tissue blocks were sectioned for detection of Survivin by Immunohistochemistry(IHC).Results: Survivin was not expressed in normal bladder urothelium, and it was over expressed in 71.1% (32/45) of TCC patients (p=0.001) . Overexpression of survivin was associated with tumor grade (p=0.029), although there was no significant association (p=731), but there was different in reading between Survivin expression and muscle invasion.Conclusion: Survivin is highly expression in TCC in compared with normal urothelium, and play key roles in survival of neoplastic cells by negative regulating apoptosis, and its correlated with poor prognosis in cancer.

الدكتور ظافر عبدالله فرحان ,الدكتور حيدر صباح كاظم  , الدكتور اسامة سعدي عبدالمحسن الخلاصة : يعد سرطان المثانة ثالث اكثر الاورام الخبيثة شيوعا في العراق لكلا الجنسين ، يشكل سرطان الخلايا الانتقالية اكثر من) ٪ 90) من سرطان المثانه . ان معلم السيرفايفين هو احد بروتينات مانعه الاستماته ) الموت المبرمج للخلايا), ويلعب دور مهم في انقسام الخلايا ومنع عملية الموت المبرمج للخلايا. حيث له القابليه على عبور غالبية السرطانات ولكنه يفقد هذا العبور في الخلايا النسيجية الطبيعية.الهدف من الدراسة : تحديد الدور المحتمل لمعلم السيرفايفين Survivin في تشخيص سرطان الخلايا الانتقالية (TCC) في المثانة البولية.المواد وطرق العمل : جمع 45 عينة نسيجية شمعية مثبتة بالفورمالين تم استردادها من المواد الأرشيفية لمستشفى الكاظمية التعليمي في مدينة بغداد ، خلال الفترة من كانون الثاني 2010 إلى آذار من العام 2011. اضافة الى 16 عينة من نسيح المثانة الطبيعي التي جمعت كمجموعة سيطرة. تم اختيار هذه الكتل النسيجية لتحديد السيرفايفين بطريقة التصبيغ المناعي النسيجي الكيميائي.النتائج :وجدت الدراسة ان معلم السيرفايفين ليس له القابلية لعبور الانسجة الانتقالية الطبييعة في المثانة البولية , بينما كانت له القابيلة على العبورفي 32 عينة من اصل 45 (71.1%) وكانت القيمة الاحصائية (P:0.001) وان هذا التعبير له علاثة وثسقة برتب السرطان (P:0.029), بالرغم من عدم وجود علاقة احصائية معنوية (P:0.731) , لكنه هناك اختلاف في القراءة بين تعبير السيرفايفين و غزو العضلات لسرطان الخلايا الانتقالية للمثانة البولية .الاستنتاجات:ان معلم السيرفايفين كان عالي التعبير في سرطان الخلايا الانتقالية مقارنه مع الخلايا الطبيعية للنسيح الانتقالي بالمثانة البولية , وهذا يلعب دور مهم في بقاء الخلايا السرطانية بوساطة التنظيم السلبي لعملية الاستماته. وكذلك يلعب دور مهم في المآل الردي للسرطان

Abstract

Transitional cell carcinoma (TCC) of the bladder in patients has historically a favorable prognosis. Bcl-2 and p53genes are implicated in cell cycle regulation with roles on programmed cell death. Presence of nuclear accumulationof p53 and cytoplasmic accumulation of bcl-2 were proposed to confer a growth advantage to tumor cells. In this study,we investigated the roles of p53 and bcl-2 as prognostic factors in TCC of bladder.Method: Investigated 25 patients with transitional cell carcinoma (TCC) and 27 patients with cystitis (control group) forthe expression of Bcl-2 and P53 by immunohistochemical staining with specific monoclonal antibodies.Results: Bcl-2 was significantly expressed in TCC compared to those with cystitis (control). Moreover, P53 was significantlyexpressed in TCC compared to those with cystitis (control). A significant positive correlation between these twoparameters in Tcc.Conclusions: These observations detect that increased the expression of Bcl-2 and P53 in TCC play a potential role inthe development of bladder cancer.

الهدف : قياس مستوى تعبير بروتين Bcl-2 وp53 لمرضى سرطان المثانة الانتقالي .
الطريقة : استخدمت طريقة التحليل الكيميائي المناعي لكشف وتحديد مستوى بروتين Bcl-2 ,p53 في المقاطع النسيجية المطمورة في البرافين والتي تم الحصول عليها من 25 مريضا مصابا بسرطان المثانة الانتقالي Tcc )) والمجموعة الثانية تشمل 27 مريضا مصابا بالتهاب المثانة حيث اعتبروا محموعة سيطرة. النتائج : التحليل الكيميائي المناعي لمستوى بروتين Bcl-2 يزداد زيادة معنوية في حالة سرطان المثانة الانتقالي عن مجموعة السيطرة . ومستوى تعبير بروتين الp53 يزداد ايضا معنويا مقارنة مجموعة السيطرة .وهناك ارتباط موجب بين BCL-2 و P53 .
الاستنتاج : بروتينات لBcl-2 ,P53 تلعب دورا مهما في مرضى سرطان المثانة الانتقالي وان تعبير Bcl-2 يرتبط ارتباطا موجبا مع تعبير 53P .

Abstract

ABSTRACT:BACKGROUND:Carcinoma of the bladder is the second most common cancer in Iraq. The status of the bladderurothelium other than the principal lesions can provide distinct diagnostic information with regard totreatment response and outcome. Positive mucosal biopsy results are a significant indicator ofintravesical recurrence. Currently, in our urological practice, the importance of performing this step incystoscopy is relatively underestimated.OBJECTIVE:The objective of this study is to carry out a histological assessment of 'extra-tumor' bladder mucosa fordysplasia, and correlate the resulting findings of dysplasia with the grade, the type, and the macroscopicappearance of the tumor.MATERIALS AND METHODS:A total of 350 biopsies were taken by cystoscopy from 68 patients admitted with bladder tumors, eitherby cold-cupped biopsy forceps or resectoscope as part of TUR of the tumor. Normal-looking mucosa ofthe bladder was included in each patient's biopsy in a four quadrant way. All biopsies were examinedby the same histopathologist. All patients were managed according to their presentation, cystoscopicand histopathological findings.RESULTS:Statistical analysis showed that the highest significant difference was obtained in patients with tumorgrade, low against high grade (p<0.005). However, a noticeable difference was found in the other twocomparison groups, namely, the type, transitional against squamous and the macroscopic appearance,papillary against solid (p<0.05).CONCLUSION:The high grade, solid tumors and squamous cell carcinoma are associated with general bladder mucosaldysplastic changes which may exclude the bladder preserving procedures as an option of surgicaltreatment. In Iraq, more extensive studies are necessary to come out with appropriate guidelines for abetter treatment and outcome of bladder carcinoma which is a major health problem

Abstract

this study was conducted to estimate the over expression of CK 20 protein in human bladder carcinoma (transitional cell carcinoma in comparison to squamous cell carcinoma) and to show its possible correlation to the pathological parameters (gradeand stage) of TCC cases . We evaluated the available tissue blocks of 50 patients with bladder carcinoma lesions (40 with TCC & 10 cases with SCC) who had referred to Al-diwaniya Hospital between January 2007 and December 2010.The mean age of the patients was 55 years (range, 33 to 77 years). The TCC were classified according to grades into grade I, II and III in 20 (50%), 10 (25%), and 10 (25%) cases, respectively and according to stages into Ta in 10 (25%), T1 in 10(25%), T2 in 10(25%) and T3 in 10(25%). From 40 cases of TCC, A total of 28 (70%) patients were positive for over expression of CK20 and no expression were found in cases SCC. High histological grades of the TCC were associated with decrease expression of CK 2 .There were 20 (100%) in GI , 5 (50%) in GII , 3 (30%) in GII (P = .0000) while CK20 expression decreased as the tumor stages increased , it was 10 (100%) in Ta , 8 (80%) in T1 , 6 (60%) in T2 , 4 (40%) in T3 , and there is a statistically significant correlation with the stages of TCC of bladder (p=0.0229 ) .Aim of the Study: This study was conducted to estimate the over expression of CK-20 protein in human bladder TCC in comparison to bladder SCC lesions and to show its possible correlation to the pathological parameters of TCC like (grade and stage) .Key ward: - Bladder TCC, Bladder SCC, Cytokeratin 20, IHC
