Assessment of anesthetic efficacy of 4%articaine and 2%Lidocaine during implant pilot hole preparation in the mandibular posterior region infiltration

Abstract

Aim.The aim of this study was to evaluate and compare the anesthetic efficacy of 4%articaine and 2% lidocaine (both with 1:100,000 epinepherine) for buccal andlingual infiltration in patients need implant placement.Materials and methods.Forty patients have edentulus regions posterier to mentalforamen were divided into 4 study groups and received buccal and lingualinfiltration of either 4% articaine or 2% lidocaine. Surgical procedure was begun5 minutes after solution deposition. Success was defined as no or milddiscomfort (VAS recordings) during during pilot hole drill.Results.The success rate for mandibular infiltration to produce anesthesia usingarticaine was 100% in premolar and molar area for the articaine solution andsuccess rate was 80% in preamolar and 30% in molar area. There was highsignificant difference between the articaine and lidocaine solutions (ANOVA P< 0.001).Conclusion. The efficacy of 4% articaine was superior to 2% lidocaine formandibular posterior regoin.

Abstract

Background: Excessive crestal bone loss after dental implant occurs due to many factors among which, is smoking.This study was conducted to evaluate the effect and role of smoking in breakdown of bone around dental implantand the possibility to be a factor of its failure.Method: Forty implants had been inserted in anterior and bicuspid region of upper arch of 40 systemically healthymale subjects, 20 were smokers and the other 20 subjects were non smokers. After loading the prosthesis, all subjectswere followed by clinical (checking for any mobility or gingival change) and radiographic examination for twosuccessive 6 months periods (the overall follow up period was 1 year). Mesial and distal bone levels were measuredby digitized x ray each six months in both smokers and non smokers. All measurements were recorded for eachpatient and a comparison of marginal bone level was done.Results: A marked excessive marginal bone loss was found at mesial side of smokers, mean (1.18± 0.033) mm after 6months of loading and mean of (1.52 ± 0.052) mm after 1 year, while in non smoker group, the mean of mesial boneloss after 6 months was (0.85 ± 0.026) mm and (1.12 ± 0.031) mm after 1 year. The mean distal bone loss in smokers atthe two periods was (1.23 ± 0.04) mm and (1.59 ± 0.046) mm, while in non smokers, the mean distal bone loss was(0.87 ± 0.023) mm and (1.1 ± 0.029) mm respectively. Differences in all measurements were significant P < 0.001.Conclusion: Smoking could be a factor results in future implant failure

Abstract

Background: Various anchorage techniques designed for canine retraction. Intraoral conventional techniques notalways enough, and now implants are widely used to increase the intra oral anchorage. The purposes of this studywere to measure and compare the rates of canine retraction and molar anchorage loss with titanium mini-implantanchorage and conventional molar anchorage.Materials and methods: The sample comprised 20 Iraqi patients (10 female, 10 male; range, 18-25 years; mean age,19.7 years) who were scheduled for extraction of upper first premolars, After leveling and aligning, titanium miniimplants1.6 mm in diameter and 8 mm in length inserted in the left side of maxillary arches between the roots of thesecond premolars and the first molars while the non-implant side include banding of maxillary right first and secondmolars with heavy ligation of maxillary right second premolar, first molar and second molar. X-ray guide and a bitewing radiograph used to determine the implant position. The implants indirectly and immediately loaded and themolars were also loaded immediately with closed coil springs with a force of 150 g for canine retraction. Using the preand post retraction study models, the amount of canine retraction was measured from canine cusp tip while molaranchorage loss measured from molar central fossa by using acrylic plug fabricated from acrylic with reference wires1.0 mm SS embedded in the acrylic.Results: mean canine retraction amounts and rates were larger on the implant-anchorage side than those on themolar-anchorage side in both genders. While mean molar anchorage loss amounts were smaller on the implantanchorageside than that on the molar-anchorage side in both genders and there was highly significant difference forcanine retraction rate and molar anchorage loss between both sides in both genders.Conclusion: the Canine retraction proceeds at a faster rate when titanium mini-implants are used for anchorage

Abstract

The present work covers the analytical design process of three dimensional (3-D) hip joint prosthesis with numerical fatigue stress analysis. The analytical generation equations describing the different stem constructive parts (ball, neck, tour, cone, lower ball) have been presented to reform the stem model in a mathematical feature. The generated surface has been introduced to FE solver (Ansys version 11) in order to simulate the induced dynamic stresses and investigate the effect of every design parameter (ball radius, angle of neck, radius of neck, neck ratio, main tour radius, and outer tour radius) on the max. equivalent stresses for hip prosthesis made from titanium alloy. The dynamic loading case has been studied to a stumbling case. The load has been applied on the cap tip as a concentrated load distributed on the interface of ball and socket. The results show that the decreasing of max. Fatigue stress by (175) MPa could be obtained by increasing the outer tour radius from (10)mm to (15) mm and that will change the max. Fatigue zone location from the tour section to the neck. The ball radius and neck angle must be as lower as possible to decrease the fatigue stresses. The most dominate parameter to increase the safety factor is the radius of neck.

هذا العمل يغطي تصميم بديل مفصل الورك ثلاثي الابعاد مع التحليل العددي لاجهادات الكلال. معادلات التوليد النظرية التي تصف الاجزاء البنائية للبديل (الكرة, العنق, الوصلة, المخروط, الكرة السفلى) تم تقديمها لتكوين نموذج البديل بصورة رياضية . اما ادخال السطح المولد الى محلل العناصر المحددة (Ansys version 11 ) لمحاكاة الاجهادات الديناميكية المتولدة ودراسة تأثير كل متغير (نصف قطر الكرة العليا, زاوية ميل العنق, نصف قطر العنق, نسبة العنق, نصف قطر الوصلة الرئيسي, نصف قطر الوصلة الخارجي) على الاجهادات العظمى المكافئة لبديل ورك مصنوع من سبيكة التيتانيوم حالة التحميل الديناميكية درست لحالة التعثر . تم تسليط حمل مركز على قمة غطاء المفصل وموزعاً على سطح الكرة والتجويف. اظهرت النتائج بأنه يمكن تقليل اجهاد الكلال الاعظم بمقدار 175 ميكا باسكال بزيادة نصف قطر الوصلة الخارجي من (10) الى (15) ملم و ذلك سوف يغير موقع منطقة اعظم اجهاد كلال من مقطع الوصلة الى منطقة العنق. نصف قطر الكرة و زاوية العنق يجب ان يكونا باقل مايمكن لتقليل اجهادات الكلال. المتغير الاكثر تاثيرا في زبادة معامل الامان هو نصف قطر العنق.

Abstract

To evaluate the direction dependence of osstell's transducer during the assessment of dental implant stability by wireless resonance frequency analyzer. Materials and Methods: Thirty tapered-SLA dental implants (Super line®) were installed in ten ribs of freshly slaughtered oxen of 2 - 2.5 years old. Three implants were installed in each rib using the fit-sized drilling protocol without countersinking. The stability was assessed by wireless resonance frequency analyzer and five records were taken for each implant by holding the Osstell's tip in different directions in respect to the long axis of the "smart peg". These directions were: (S=superior) parallel to long axis of the " smart peg" and superior to it. (A=anterior) perpendicular to long axis of the " smart peg" and anterior to it. (P=posterior) perpendicular to long axis of the " smart peg" and posterior to it. (M=mesial) perpendicular to long axis of the " smart peg" and mesial to it. (D=distal) perpendicular to long axis of the " smart peg" and distal to it. Results: The implant stability quotient values (ISQ) were almost identical in anterior and posterior directions of Osstell's tip and also closely comparable in mesial and distal directions. Although the ISQ values that recorded in superior directions were slightly higher than in other directions, no significant difference was found among the different directions of probe orientation (ANOVA P> 0.05). Conclusion: Any direction of osstell's transducer will be suitable to assess implant primary stability.

Abstract

Potentiodynamic polarization measurements were used to study the effect ofEllagic acid on biocorrosion of ( 316 L SS ) in Simulated body fluid (Ringer Solution)was used as the electrolyte .The temperature of the polarization test cell wasmaintained at 37±1OC by thermo stated water bath to simulate the human body .Theresult from corrosion behavior investigated by electrochemical measurements appearthe corrosion current density of ( 316 L SS ) specimens in Ringer solution containing Ellagic acid was much lower than the values obtained in Ringer solution withoutEllagic acid .

Abstract

Background: physicochemical and biochemical coating techniques that are investigated now a day to enhancebone regeneration at the interface of titanium implant materials. The combination, however, of both organic andinorganic constituents is expected to result into truly bone-resembling coatings and as such to a new generation ofsurface-modified titanium implants with improved functionality and biological efficacy. This research was conductedto study the expression of osteocalcin and growth hormone receptor as bone formation markers in coated anduncoated implant in interval periods (3days,1,2and 6 weeks).,Materials and methods: Commercially pure titanium (CpTi) implants coated with hydroxyapatite by EPD method andwith fibronectin protein, were placed in the tibia of (16) New Zeland white rabbits , immunohistochemical tests fordetection of expression of osteocalcin and growth hormone receptor were performed on all the implants of bothcontrol and experimental groups (3days,1,2 and 6 weeks) healing intervals. Mechanical test (torque removal test)was performed as an indicator for the presence of osseointegration and as a test for the mechanical property ofbone-implant interface to be primarily propping the interface machanics.Results: The removal torque mean values in all studied groups uncoated and coated were increasing withadvancing time (higher at 6 than 2 weeks periods) and coated implant showed high value in comparsion to control.Result shows that Immunohistochemical findings revealed high positive expression range from strong to moderate forosteocalcin and growth hormone receptor in coated implant in comparison to uncoated. These results indicatingthat a mixing of bioactive HA ceramic and FN increased the activity of coated layer which improved the boneformation and maturation in bone-implant interface and enhance mechanical interlocking with bone.Conclusions: The present study concludes that organic and inorganic surface modification for titanium implantsurface by HA and FN enhances bone formation and increase osseointegration

The second topic dealt with metaphor and its divisions. الحمد لله رب العالمين ، والصلاة والسلام على النبي خير البشر ما هطلت الغمائم بتهتان المطر، وهدلت الحمائم بأفنان الشجر، وعلى آله وأصحابه الميامين الغرر ، وبعد:فلم ينل الحديث النبوي الشريف قسطه الكافي من الدرس البلاغي ، إذ انصبت جل الدراسات البلاغية على القرآن الكريم ، فقد كانت تراودنا فكرة تناول موضوع في الحديث النبوي الشريف منذ زمن ، فوقع الاختيار على كتاب (طرح التثريب في شرح التقريب) ، وذلك لما لهذا الشرح من قيمة علمية أصيلة ، فهو موسوعة لغوية ونحوية وفقهية فضلاً عما يحتويه من الموضوعات البلاغية القيمة المبثوثة في ثناياه ، فقد أحاط مؤلفه بعلوم عصره وآراء شيوخه وسالفيه ، ولما كان هذا الكتاب لم ينهض لدراسته أحد من الباحثين فيما يخص الجانب البلاغي توكلنا على الله تعالى لنبدأ مشوارا مع هذا الكتاب وشارحيه بعلم من أهم علوم البلاغة ألا وهو علم البيان .فكان عنوان البحث : (علم البيان في كتاب طرح التثريب في شرح التقريب)0وقد قسمنا البحث إلى ثلاثة مباحث يسبقها تمهيد وتختمها خاتمة ، أمَّا التمهيد فقد جاء على قسمين : الأول : عن الشارحّين وحياتهما (الحافظ العراقي و ابنه أبي زرعة) ، فالشرح مشترك لهما ، وجاء القسم الثاني في تعريف علم البيان من حيث اللغة والاصطلاح .أما المبحث الأول فاختص بالتشبيه وأقسامه . وتناول المبحث الثاني المجاز وأقسامه . أما المبحث الثالث فكان على قسمين : الأول : في الكناية وأقسامها ، والثاني : في التعريض .وختاما نقول : هذا ما وفقنا إليه الله سبحانه وتعالى أن نقدمه للقارئ ، فإن أصبنا فمن الله وحده ، وإن كانت الأخرى فحسبنا أننا اجتهدنا ما استطعنا ولله الحمد أولا وآخرا ، وصلى الله تعالى على سيدنا محمد وعلى آله وصحبه أجمعين.
