Rare morphologic variations of mandibular canal course: radiographic study

Abstract

Background: mandibular canal course variations are found very rarely, nevertheless, their recognition may play avery important role prior to any surgical procedure involving the lower jaw. This study aimed to identify the variationsin the different parts of the radiographic course of mandibular canal using digital panoramic imaging.Materials and Methods: The courses of the mandibular canal in 319 panoramic images of Iraqi subjects wereevaluated carefully and any rare radiographic finding was recorded when identified.Results: A total of (5.95 % of the radiographic images examined in the present study demonstrated mandibular canalvariations. The most common variation observed in the present study was bifid mandibular canal (1.88 %), followedby indistinct mental foramen (1.57%), then indistinct mandibular canal walls (1.25%) and indistinct mandibularforamen (1.25%).Conclusion: The course and configuration of the mandibular canal should be carefully observed in different locationsbecause it posses many variations. Recognition of these variations is important in preventing the intra and postoperative complications in certain dental procedures

على الرغم من أن التباین في مسار قناة الفك الاسفل ھو من الحالات النادرة الا ان اكتشاف ذلك التباین لھ دور بالغ الاھمیة قبل اجراء اي عملیة جراحیة في الفك الاسفل. الغرضمنھذه الدراسة ھو تحدید التباین الموجود شعاعیا في مناطق مختلفة على طول مسار قناة الفك الاسفل باستخدام التصویر بالاشعة البانورامیة الرقمیة.تم تقییم مسار قناة الفك الاسفل بدقة في 319 صورة اشعة بانورامیة لمواطنین عراقیین وقد تم تسجیل جمیع الحالات النادرة في مسار قناة الفك الاسفل حیثما وجدت.اثبتت النتائج ان % 5.95 من العینة الكلیةالمفحوصة لدیھم تباین في مسار قناة الفك الاسفل. كانت قناة الفك الاسفل المنشطرة ھي النوع الاكثر انتشارا بین تلك التباینات اذ شكلت (%.( 1.88 ) یتبعھا عدم وضوح الثقب الذقني (% 1.57 ) ثم عدم تمییز جدار قناة الفك الاسفل (% 1.25 ) واخیرا عدم وضوح ثقب الفك الاسفل (% 1.25ان المسار والشكل العام لقناة الفك الاسفل یجب ان یلاحظا بدقة في مواقع مختلفة بسبب التباینات المتعددة لھذه القناة. ان لتمییز ھذه التباینات اھمیة في تلافي المضاعفات التي قد تحدثاثناء وبعد بعضعملیات طب الاسنان

Abstract

The mandibular canal appears below or superimposed over the apices of themandibular molar teeth but it has some variations regarding its distance to both lowerfirst molar and the base border of the mandible that may make a problem specially forthe oral surgeon in his work during operations like implantation, so, this study wasdone to estimate the position of the mandibular canal and its relation to the lower firstmolar and the base border of the mandible by the aid of digital panoramicradiographs.The sample of this study was collected from patients who attended Al-Karamaspecialized center for dentistry. Forty patients were selected in this study with the agerange between 20-60 years (males and females) that divided in to four groupsaccording to a special criteria. Forty digital views (OPG) were taken for Iraqi patients,using computerized digital panoramic x-ray machine. All radiographs were examinedand then the position of the mandibular canal for each patient was estimated.The results revealed that the mandibular canal is most commonly located awayfrom the root apices of lower first molar, and the distance between the mandibularcanal and the base border of the mandible is indirectly proportional with age.
