Pain intensity and control with fixed orthodontic appliance therapy (A clinical comparative study on Iraqi sample)

Abstract

Background: The purpose of this prospective, randomized clinical trial was to investigate the level and intensity ofpatients` pain and discomfort, and to compare between the use of Bite Wafer and Paracetamol in reducing painand discomfort associated with initial orthodontic tooth movement in both adolescents and adults.Sample: 110 subjects with two age groups, 52 adolescents with age range from 12 to less than 18 years and 58 adultswith age range 18-24 years, successfully completing the study. For each subject fixed orthodontic appliance (Roth0.022) was bonded and round 0.014 NiTi arch wire was ligated with elastic ligature. The subjects in the Bite Wafergroup were instructed to chew on it whenever they feel pain for the next 7 days, and document the time andeffectiveness of it in the questionnaire. The Paracetamol group subjects instructed to use Paracetamol 500mg torelive pain and record times and effectiveness of its use in questionnaire.Results: The peak of pain was occurred in the first day and declined gradually till totally disappeared at the sixth dayafter initial arch wire placement. A marked reduction of pain intensity was noticed in both adolescents and adultsgroups, using Bite Wafer, from the first to the sixth day which is much higher than Paracetamol group especially inadolescents. No gender differences (P>0.05) was noticed in this study.Conclusion: Although both Bite Wafer and Paracetamol reduced pain gradually, Bite Wafer reduced pain moreobviously and safely in comparison to Paracetamol especially in adolescents

Abstract

ABSTRACT: Orthodontic tooth movement is a physiologic response to externally applied forces; the motive forces are primarily mechanical. The optimal application of orthodontic force enables maximum movement of teeth with minimal irreversible damage of the periodontal ligament (PDL), alveolar bone, and teeth. Since arch wires are the main forcesystem in orthodontics, it is important in clinical practice that they deliver appropriate, predictable and repeatable forces during treatment. These specialized wires even promise shape memory properties and the possibility of super elastic behavior, which significantlyimpacts clinical practices. Since, standard stainless steel and titanium arch wires are still the materials of choice in many stages of treatment. They provide an attractive combination of stiffness, resilience and formability. However, clinical practitioners have commented on the variability of arch wire behavior for years. Inconsistent arch wire properties can contribute to unpredictable treatment duration and results.This paper examines the mechanical and physical characteristics of stainless steel and titanium wires to quantify their variability in engineering terms. From the results for both types of wires, the testing method provides the information required by designers wishing toimprove the arch wire properties and provide valuable information to clinicians for their practice.Keywords: orthodontics, mechanical properties, tensile properties, orthodontic wires,titanium, stainless steel.

الخلاصة:حركة الأسنان أثناء التقویم تمثل عملیة استجابة فسلجیة لقوى خارجیة، والتي غالبا ما تكون هذه القوى میكانیكیة. التطبیق الأمثل لقوى التقویم تسمح بحركة قصوى للأسنان مصاحب بضرر بسیط للأسنان،الأربطة المحیطة بالأسنان، وعظم السنخى.تعتبر الأسلاك المتقوسة المصدر الأساسي لتسلیط القوة في جهاز التقویم. فمن خلاله یمكن أن نسلط قوة مناسبة خلال جلسات العلاج حیث ان لشكل السلك وسلوكه المرن تأثیر واضح على فترة العلاج. إلى حد هذه الفترة تعتبر أسلاكالتیتانیوم والستینلس ستیل الخیار الأمثل في المراحل العلاجیة المختلفة لتقویم الأسنان وذلك لأنها تجمع بین خاصیة الصلابة، المرونة، وقابلیة التشكیل في جهاز التقویم. من ناحیة أخرى، یعاني أطباء الأسنان وخلال سنوات من عدم القدرة على تحدید فترة التقویم حیث یعزى ذلك لتضارب الخواص المیكانیكیة للأسلاك المستخدمة في التقویم من سلك لآخر.انطلاقا من الدور الذي یؤدیه السلك في أداء جهاز التقویم انشأ هذا البحث والذي اهتم بدراسة الخواص المیكانیكیة لأسلاك التیتانیوم والستینلس ستیل لتحدید المتغیرات التي تحدث ضمن الحدود الهندسیة. كما حددت النتائج الخواص المیكانیكیة الواجب توفرها في ألأسلاك المستخدمة في التقویم.

A lateral cephalometric and arch width Study between class II division 1 malocclusion and Class II division 2 malocclusion (A comparative study)

Abstract

Background: the purpose of this retrospective study was to compare somecraniofacral cephalometric measurements and arch width dimensions betweensubjects with class II division 1 and class II division 2 malocclusion in Iraqipopulation.Materials and methods: 60 digital true lateral cephalometric radiographs 30 for classII division 1 and 30 for class II division 2 subjects (15 males, 15 females for eachgroup ) were selected from the files of the patients attending the orthodontic clinicin the college of dentistry, university of Baghdad with an over jet more than 6 mmand class II molar and canine relation. Also for each group a study model weretake to measure the arch width for both upper and lower arches. The inter molar,inter premolar and inter canine measurements were carried out on study models.The radio graphs were digitized by using the Auto CAD program in the personalcomputer all the data compared by independent t- test.Results and conclusions: For the dental arch width measurements the statisticallysignificant difference was found between the mandibular inter canine width andmaxillary inter molar width. The cephalometric results that class II division 1showed a retrognathic mandible, and this due to the difference in SNB anglebetween the two groups, the class II division 2 subjects had anterior position of themandible relative to the cranial base, more concave profile, great posterior faceheight, horizontal growth, the mandibular inclination is interiorly, more deep bite,and reduction in the mandibular inter canine width and maxillary inter molarwidth .
