Abstract

This study aimed to investigate the Ameliorative effect of ethanolic extracted of silymarin from Silybum marianum seeds and to compare it’s with the commercial silymarin extract(legalon) against oral dosing of Nickel chloride effects on sperms concentration, motility ,viability, sperm abnormality and male fertility and gonadal hormones .Twenty adult male rabbits aged 5 to 6 month and weighted 1.250-1.500 kg divided into four equal groups,1stgroup served as control groupreceived 1 ml of normal saline (NaCl 0.9%),2ndgroup received (1mg/100g B.W )NiCl2 orally, 3rdgroup received same dose of NiCl2 plus (0.1mg/100g B.W)silymarin extract, the fourth group received same dose of NiCl2 plus (0.1mg/100g B.W) legalon for 35 days, Results showed negative effects of NiCl2 which causedsignificant(p≤0.05) decrease in sperm concentration ,viability, motility and fertility while sperm abnormality was significantly increase, also NiCl2 caused significant (p≤0.05) decrease in serum progesterone, estradiol and testosterone. while silymarin extract and legalon adverse the negative effects of NiCl2 and causing ameliorative effects on all the studied parameters .

Abstract

Defective Sperm parameters was suggested to play an important role in determining fertility and was proven to be closely related to fertilization and pregnancy rates in the natural fertilization process as well as in ART.To evaluate the influence of defective sperm parameters including concentration, morphology and motility on intracytoplasmic sperm injection outcome represented by fertilization rate, cleavage rate, embryo quality and pregnancy rate.This a cohort study included 60 infertile couple attended the clinics of infertility treatment center Al-Sadder teaching hospital and underwent intracytoplasmic sperm injection. They are classified into three groups according to their spermiogram and ICSI outcome evaluated for each group and compared with each other.The main results of this study showed no significant difference in fertilization rate,cleavage rate ,embryo quality and pregnancy rate among studied groups i.e., no significant difference in ICSI outcome between normozoospermic group and those with defective spermiogram. Intracytoplasmic sperm injection outcomes represented by fertilization rate, cleavage rate embryo quality and pregnancy rate were identical with no significant differences between couples with defective sperm parameters and those with normal spermiogram.

Abstract

This study was aimed to investigate the effect of sperm parameters on the response to in vitro sperm activation (ISA) and to compare sperm parameters according to the age and type of infertility.One hundred twenty-eight subjects employed in this study, with age range (18-49) years and duration of infertility (1-28) years. From each subjects semen sample was obtained and seminal fluid analysis (SFA) was done. Simple medium for assisted reproductive techniques; (SMART) was used for in vitro sperm activation by direct swim-up technique and centrifugation Swim-Up technique. Sperm concentration, sperm motility (%), sperm grade activity (%), normal sperm morphology (%) and sperm agglutination (%) were assessed. Crude data were statistically analyzed. From the results of present study, it was appeared that most sperm parameters were significantly (P<0.05) enhanced post-activation in vitro as compared to pre-activation using SMART medium by direct swim-up technique and centrifugation Swim-Up technique. Sperm concentration for post-activation was significantly reduced (P<0.05) compared to pre-activation groups and the percentages of progressive sperm activity with grade A, total progressive sperm motility (grades A and B) and normal sperm morphology were significantly increased (P<0.05). Based on the results of this study, it can be concluded that sperm motility and concentration were considered most important parameters that have the importance of activation and sperm morphology one of the sperm parameters that have role of the influence on the sperm qualities after activation.

صممت هذه الدراسة لتقييم مدى تأثير متغيرات النطف على الاستجابة للتنشيط خارج الجسم ولمقارنة متغيرات النطف مع العمر ونوع العقم. شارك في هذه الدراسة مئة وثمانية وعشرون شخص، وكان متوسط أعمارهم (18-49) وفترة العقم (1-28) سنة. أخذت عينة السائل المنوي من كل مريض واجري لها فحص السائل المنوي. استعمل الوسط الزرعي البسيط للتقنيات المساعدة على الإنجاب لتنشيط النطف خارج الجسم باستخدام تقنية سباحة النطف للأعلى مباشرة وتقنية الطرد المركزي. تم تقيم تركيز النطف والنسبة المئوية للنطف المتحركة والنسبة المئوية لنشاط النطف والنسبة المئوية لشكل النطف الطبيعي والنسبة المئوية لتكتل النطف.كما تم تحليل النتائج الأولية إحصائيا. أظهرت نتائج الدراسة الحالية تحسن معنوي (P<0.05) في اغلب متغيرات النطف بعد التنشيط خارج الجسم عند المقارنة مع قبل التنشيط باستخدام وسط سمارت وتقنية سباحة النطف للأعلى مباشرة وتقنية الطرد المركزي. تركيز النطف بعد التنشيط انخفض بشكل معنوي( P<0.05) عند المقارنة مع قبل التنشيط والنسب المئوية لكل من الحركة التقدمية (A) للنطف والحركة التقدمية الكلية((A+B للنطف وشكل النطف الطبيعي أظهرت زيادة معنوية(P<0.0).لذلك نستنتج اعتمادا على معطيات هذه الدراسة بأن حركة النطف وتركيزها من أهم الصفات التي لها أهمية بالتنشيط خارج الجسم وشكل وشكل النطف الطبيعي واحد من الصفات التي لها دور بالتأثير على صفات النطف بعد التنشيط خارج الجسم.

Abstract

Immature chromatin condensation is a type of DNA damage in human spermatozoa may adversely affect reproductive outcomes.To assess sperm chromatin maturity in infertile men and to evaluate the relationship between sperm chromatin status and ICSI outcome.Fifty six infertile couple attending the clinics of fertility center in Al-sader teaching hospital were included in this study .Male partner were classified according to their spermiogram ,their semen sample tested by AB for chromatin maturity .The percentage of positive AB stain is significantly higher in patients with abnormal spermiogram than normozoospermic group.No significant difference in ICSI outcomes between patients with high proportion of abnormal chromatin and apposite group.Male with abnormal spermiogram show high percent of sperm chromatin immaturity which have no significant influence on the result of ICSI procedure.

Abstract

The aim of this study was to evaluate the association between the follicular fluid (FF) malondialdehyde (MDA) levels as a free radical marker and reduced glutathione (GSH) as antioxidant and pregnancy after intracytoplasmic sperm injection (ICSI).A total seventy-five infertile women aged between 22-45 years(31.43 ± 5.38 years), referred to the fertility clinic in Al-Sadder teaching hospital, , and undergone intracytoplasmic sperm injection throughout period from March 2013 to January 2014,were included in this study. follicular fluid (FF) malondialdehyde was measured by spectrophotometer based on the calorimetric reaction with thiobarbituric acid (TBA) and follicular fluid glutathione was measured by spectrophotometer depends on the action of sulfhydryl groups.Out of the 75 included patients, 15 (20%) achieved pregnancy after ICSI. Pregnant women were associated with significantly higher MDA level (P < 0.05) and lower GSH (P < 0.05) .There was a significant positive correlation between MDA and the number of pronucleus ( r= 0.326 ). The best cutoff value of FF.MDA and FF GSH associated with pregnancy were 3.49 µM and 26.90 µM respectively. Higher FF MDA and lower FF GSH levels are associated with pregnancy after ICSI. Oxidative stress parameters may be markers of metabolic activity within the follicle.

Abstract

Background: The role of TNF-α and TNF-α antagonists on seminal fluid parameters in men is controversial. TNF-α is a cytokine had a wide range of cellular effects. In testis, “TNF-α is produced by germ cells and it has a role in the regulation of spermatogenesis”. Objective: to study the effect of biological agent on sperm quality in a group of spondyloarthritis patients Patients and Methods: A prospective study was carried out in Baghdad teaching hospital during the period starting on 1st of January 2013 till the end of October 2013 .All semen samples of (60) spondyloarthritis patients and controls (60) was collected by masturbation after (72) hour of sexual abstinence. The semen samples was examined with regard to sperm concentration, motility, and morphology, according to WHO 1999 guidelines.. Results were compared with controlsResults: According semen parameters such as Volume, PH, Count, Motility Morphology in two groups of spondyloarthritis there were no significant differences noticed between TNF blocking agents and semen parameters when compared to controls group on( baseline ) but after (40 weeks), Regarding motility and morphology significant differences (P<0.001) were found between cases and healthy controls.Conclusion: Biological agents (infliximab&enbrel) had no affect on seminal fluid parameters such as volume, PH and sperm concentration, but it decreased motility of sperm and percent of normal morphology

Abstract

The objective of this study was to investigate the effect of Royal jelly RJ on morphology and motility of mice sperms. Sperms were collected from the cauda region of the epididymis of each 10 mice from the treatment and control groups. Direct activation techniques and evaluation of sperm morphology were carried out. Dhino microscope was used for sperm measurement. The inspection was carried out in Salamatic laboratory for pathological analysis in 2015.The result revealed that all of the sperm function parameters registered significant activation (P<0.05) in the treatment group. There was a significant increase in both the percentage of the sperm motility grade A and the progressive motility (A+B) of the treatment group. Spermatozoon with different lengths were noticed. The study explains that this difference may be due to the presence of different sperm developmental stages and not to the increase in the number and sizes of mitochondria in the mid-peace during activation or not to the sliding movement of microtubules in the axon of spermatozoon. These findings provide evidence that RJ can play an important role only in improving the male mice motility and which may open the way for further researches to demonstrate the possibility of using RJ in artificial insemination in field animals.

صممت هذه الدراسة للمقارنة بين نتائج متغيرات النطف قبل وبعد التنشيط بالغذاء الملكي للنحل. إستخدم في هذه التجربة عشرون من ذكور الفئران البالغة (عشرة لكل من مجموعتي التجربة والسيطرة) والتي جمعت من بيت الحيوان في المعهد العالي لتشخيص العقم والتقنيات المساعدة على الانجاب بجامعة النهرين. أجريت التجربة والتحليلات في مختبر سلامتك للتحليلات المرضية ببغداد - الرصافة 2015. أخذت النطف من ذيل البربخ. تم تقييم تركيز النطف, والنسبة المئوية للنطف المتحركة, والنسبة المئوية لنشاط النطف, والنسبة المئوية لشكلياء النطف الطبيعي. تم تسجيل الأبعاد الشكليائية للنطفة في الحالتين باستخدام الميكروسكوب نوع داينو. تم تحليل النتائج إحصائيا. أظهرت نتائج الدراسة الحالية تحسن معنوي (p<0.05) في متغيرات النطفة في مجموعة المعاملة بعد التنشيط (خارج الجسم) بمادة الغذاء الملكي مقارنة بمجموعة السيطرة حيث حصلت زيادة معنوية (P<0.05) بعد تنشيط النطف لكل من الحركة التقدمية A للنطف والحركة التقدمية الكلية (A+B) للنطف. أظهرت الدراسة وجود أطوال مختلفة للنطف في مجموعتي السيطرة و التجربة وزيادة غير معنوية في أطوال النطف لمجموعة التجربة, تعزي الدراسة هذه الزيادة إلى المراحل التطورية المختلفة لنشأة النطف وليس بسبب إزدياد أعداد وأحجام المتقدرات الموجودة في المنطقة الوسطية للنطفة بعد التنشيط أو بسبب حركة ألنبيبات الدقيقة ألإنزلاقية الموجودة في محور النطفة. ترى الدراسة أن إضافة الغذاء الملكي الى الوسط الزرعي للنطف يحفز حركتها وقد يؤدي إلى زيادة نشاطها, وإمكانية إستخدامه لزيادة نشاط النطف في التلقيح الاصطناعي في الحيوانات الحقلية.

Embryo Grade I is The Most Important ICSI Parameters That Predict Intracytoplasmic Sperm Injection(ICSI) Outcome

Abstract

The aim of this study was to compare different intracytoplasmic sperm injection (ICSI) parameters with its outcome and predict the most important factor that relate with successful rate.A total seventy-five infertile women aged between 22-45 years(31.43 ± 5.38 years), referred to the fertility clinic in Al-Sadder teaching hospital, , and undergone intracytoplasmic sperm injection throughout period from March 2013 to January 2014,were included in this study. Fertilization rate (FR) and cleavage rate was calculated. The embryos grading was evaluated according to their morphology and percentage of fragmentation. The biochemical pregnancies was confirm on the fourteenth day of embryo transfer by measurement of serum human chorionic gonadotrophine ( B- HCG ). Out of the 75 included patients, 15 (20%) achieved pregnancy after ICSI. Pregnant women were associated (P < 0.05) . Total oocyte retrieved , stage metaphase II oocyte , pronucleus , cleavage rate , grade I ,total number of embryos ,and number of transferred embryo significantly higher in pregnant women when compared with non-pregnant women(P<0.05) Analysis of ROC curve revealed that area under the curve for number of grade I embryos was 0.868 for predicting pregnancy followed by followed by MII (0.815), number of total oocyte .retrieved and PN (0.773), ET( 0.767) and total number of embryos(0.758) respectively.Data show that the number of grade I embryos is the better at predicting for successful pregnancy outcome.(higher test result give more positive test).

Abstract

background: infertile couples due to male factor reasons present with azoospermia asa cause of their infertility can positively benefit from intracytoplasmic sperminjection (ICSI) for fathering genetics offspring's.Aim of study: to compare the intracytoplasmic sperm injection outcomes in obstructive and non-obstructive azoospermic men.Materials and methods: A retrospective study compromised 42 couples seeking for treatment due to azoospermia as a cause of infertility. 19 men out of 42 had obstructive azoospermia (OA) and the remainder 23 man had non-obstructive azoospermia (NOA). All those males underwent surgical sperm retrieval techniques either testicular sperm extraction (TESE) in all NOA cases and some OA cases or percutaneous epididymal sperm aspiration (PESA) in the rest of OA cases. Sperms retrieved surgically were used either fresh or after cryopreservation for ICSI and the main outcomes; fertilization, cleavage and pregnancy rates and embryo quality were compared statistically. Data were expressed as mean ±SD and number and percent, and groups were compared by compares of two independent means. Dif-ferences were considered significant at p <0.05.Results: the results revealed significant reduction (p <0.05) in the fertilization rate in couples with NOA (38.2%) compare to (59.5%) for OA group, as well as a significant drop in cleavage (80.6% vs 68.9%) and pregnancy (31.57% vs 13.04%) rates was also shown in NOA group. The mean number of embryos (5.88±3.46 vs 3.33±2.54, p <0.017) obtained by ICSI and percent of good quality embryos (55.2% vs 31.37%, p <0.014) were significantly higher in OA group.Conclusion: type of azoospermia has greater effect on the success of ICSI cycles. The immaturity of testicular sperm in NOA can affect fertilization, embryo development and pregnancy rate.

Abstract

C-reactive protein is a protein that act as a sensitive marker in inflammatory processes, rises following hormonal stimulation it may affect fertility and intracytoplasmic sperm injection (ICSI) outcome. this study aimed to measure follicular fluid proinflammatory mediator as C-reactive protein and their relation controlled ovarian hyperstimulation protocols and to interpret these findings according to the cause of subfertility and subfertility treatment outcome. This study was carried out on 110 subfertile women aged between 18-45year (30.01±6.02), referred to fertility clinic in Al-Sadder teaching hospital at AL-Najaf city who classified into four group according to the cause of subfertility, estradiol(E2) also measured on day of HCG and CRP were measured in follicular fluid collected on day of pickup using special ELISA kit then correlate the result to ICSI outcome. The Result of this study reported that the pregnancy rate was 34.55% for embryo transferred after 48-72 hours, there was non-significant increment in pregnant than non-pregnant women regarding FF-CRP (P>0.05), also non-significant difference in FF- CRP in subfertile women according to cause of subfertility(p=0.303), higher FF-CRP value were in ovulatory factor female. Regarding ICSI parameters, there was positive non-significant correlation between FF- CRP and embryo number, Grade I, Grade II, embryo transferred, fertilization rate. so conclusion of this study minimal increment in these inflammatory markers would aid and share in success ICSI outcome.
