Abstract

Background: The immune response to Helicobacter pylori is a versatile group of mechanisms involving responses that are both protective and damaging to the host. H. pylori can directly bind to mast cells which produce pro-inflammatory factors which migrate and accumulate in the gastric mucosa It was found that infection with H. pylori is accompanied with increased total IgE and positive IgE specific for H. pylori in sera of some dyspeptic patients.Aims of the study: This study aimed to investigate the presence of IgE specific for H. pylori in gastric biopsies from dyspeptic patients regarding their gender. Patients and methods: Seventy six (76) adult patients from both genders were included in this study. Inclusion criteria included dyspeptic patients require upper gastrointestinal endoscopic examination. Patients were attending Gastrointestinal Endoscopy Unit at Baghdad Teaching Hospital during the period from December 2014 to May 2015. Gastric biopsies from patients with dyspepsia were investigated for IgE specific for Helicobacter pylori. using ELISA test. Sera from the patients were tested for IgM and IgG specific for H. pylori using ELISA test. Results: Study revealed that 75% of patients with serum positive IgM specific for H. pylori were showing positive IgE specific for H .pylori in their homogenates of their biopsies. While Sixteen (16) out of 60 (26.7%) positive H .pylori IgG patients were showing positive IgE specific for H .pylori in homogenates of their biopsies. Conclusion: The current study concluded that IgE specific for H. pylori undergo release locally in gastric mucosa of dyspeptic patients infected with H .pylori.

ديباجة: التفاعل المناعي لجرثومة الملويات البوابية متنوع فمنه الوقائي ومنه ما يسبب تلف الانسجة. جرثومة الملويات البوابية بإمكانها الالتصاق بالخلايا الصارية مباشرة منتجة عوامل التهابية التي تتجمع في الطبقة المخاطية للمعدة. لوحظ ان الخمج بالملويات البوابية يؤدي الى ارتفاع بمعيار الضد IgE الكلي والخاص بجرثومة الملويات البوابية في مصول بعض المرضى المصابين بهذه الجرثومة.اهداف الدراسة: هدفت هذه الدراسة للكشف ن الضد IgE الخاص بجرثومة الملويات البوابية في الخزع المعدية الذين يعانون من سوء الهضم وعلاقة ذلك بجنس المريض.المرضى وطرائق العمل: تضمنت هذه الدراسة ست وسبعين ( 76 ) مريضا بالغا من الجنسين الذين راجعوا شعبة منظار الجهاز الهضمي في مستشفى بغداد التعليمي خلال الفترة من الأول من كانون الأول / ديسمبر 2014 ولغاية نهاية شهر أيار/ مايو 2015 . كان من بين معايير تضمين المرضى في الدراسة مدى حاجة المريض لإجراء منظار على الجزء العلوي للمعدة والأمعاء. أجريت هذه الدراسة على الخزع النسيجية المأخوذة من المرضى الذين يعانون من عسر الهضم للتحري عن الضد (IgE)الخاص بجرثومة الملويات البوابية . وتم اختبار أمصال المرضى للكشف عن وجود الأضداد المناعية IgG ،IgM - الخاصة بجرثومة الملويات البوابية . النتائج : أوضحت النتائج ان(75%)من الاشخاص الموجبين للضد(IgM) كانوا موجبين للضد(IgE)الخاص بجرثومة الملويات البوابية في خليط عالق الخلايا بينما كان (16) من اصل ستين مريضا (60)(26.7%) موجبين للضد (IgG) كان خليط عالق الخلايا عندهم موجبا للضد(IgE)الخاص بجرثومة الملويات البوابية.الاستنتاجات : من هذه الدراسة نستنتج ان الضد(IgE)الخاص بجرثومة الملويات البوابية يمكن ان يفرز موضعيا في الغشاء المخاطي المعدي لمرضى عسر الهضم المصابين بجرثومة الملويات البوابية.

Abstract

Background and objectives: Lymphoproliferative neoplasms are among the commonest human neoplasia. Immune dysregulation is frequently associated with many of these diseases, particularly those related to B-cell origin. This study aimed to find out the immunological status of newly diagnosed patients with lymphoproliferative disorders.Methods: This study was carried out in Erbil province during the period from November 2010 to April 2011. Fifty two newly diagnosed patients (34 males and 18 females) with lymphoproliferative disorders referred to Nnakaly Hospital for blood disease and 20 healthy control subjects had been included in this study. Determination of immunoglobulin subclasses by radial immunodiffusion in addition serum protein electrophoresis has been performed for each sample for the detection of monoclonal bands.Results: Serum IgM and IgA level were decreased in all groups of patients with different lymphoproliferative disorders compared to control group. Serum IgG level showed insignificant reduction in acute lymphocytic leukemia and chronic lymphocytic leukemia patients, but it was increased in non Hodgkin lymphoma, Hodgkin lymphoma and multiple myeloma patients. Paraproteinemia had been seen in all patients with multiple myeloma while only 10% of chronic lymphocytic leukemia and non Hodgkin lymphoma showed paraproteinemia. No M-band had been noticed in acute lymphocytic leukemia and Hodgkin lymphoma.Conclusion: There were some conflicting data regarding serum immunoglobulin levels in newly diagnosed patients with lymphoproliferative disorders in Erbil, Iraq.

Abstract

BACKGROUND: B‑chronic lymphocytic leukemia (CLL) is a monoclonal malignancy characterizedby an accumulation of small mature‑looking B lymphocytes in the blood, bone marrow, and othertissues. It is typically characterized by CD5+, CD23+, CD22−, and CD79b−, with weak expressionof surface immunoglobulin (Ig).OBJECTIVES: The aim of the study was to assess the expression of IgM and IgD heavy chain byflow cytometry in untreated newly diagnosed B‑cell CLL and correlate the heavy chain isotopesexpression with CD38 expression.MATERIALS AND METHODS: A prospective cross‑sectional study was conducted on thirty patientswith new diagnosis of B‑CLL. The study was conducted at nursing home teaching laboratory inmedical city hospital in Baghdad for the period from November 2016 to March 2017. Five milliliter ofvenous blood in ethylenediaminetetraacetic acid was collected from thirty patients for detection ofexpression of IgM, IgD, and CD38 by flow cytometry. The patients were randomly selected regardingthe age, sex, and stage of the disease.RESULTS: The mean age of all included patients was (61.73 ± 6.92) with more disease predominantin male than female. The most common presenting feature of the patients was lymphadenopathyfound in 16 patients (43%). Regarding staging system, six (20%) patients were in with Binet Stage Cand five patients (17%) were in modified Rai Stage 4. CD38 and IgM expression showed a significantrelation to hemoglobin and platelets (P < 0.05). There was a significant positive correlation betweenIgM and CD38 while it was negative between IgD and CD38.CONCLUSIONS: The current study showed that the expression of CD38 had a significant correlationwith IgM expression as well as there was positive correlation with the stage of the disease whichmay point out to inferior prognosis for those patients with CLL.

A Compare Study Effects of Ginkgo biloba Extract and Corticosteroid in Induce Allergic Asthma in Rabbits in Regards to Physiological and Immunological Parameters

Abstract

Thirty adult male of New Zealand Albino white rabbits were used in this article and these rabbits are haphazardly divided into five different experimental groups. Ovalbumin was used in a dose (0.1 mg /kg) to induce allergy and sensitization challenge was confirmed by the presence of the clinical symptoms of wheezing, sneezing, chest tightness, shortness of breath, changes in the level of total white blood cell, neutrophils, eosinophils, Immunoglobulin E and Interleukin-4. Eight days after allergy induction, Ginkgo biloba giving in the dose (25 mg/kg) and in a combination with prednisolone in the half of recommended dose (12.5 + 0.5) mg/kg (Ginkgo biloba + prednisolone) was giving orally for 8 days. The results showed that ovalbumin could induce sensitivity and significantly increased the level of total white blood cell, neutrophils, eosinophils, Immunoglobulin E and Interleukin-4. Oral treatment with Ginkgo biloba in the dose (25 mg/kg) and in a combination with prednisolone as the half of recommended dose (12.5 + 0.5) mg /kg of Ginkgo biloba + prednisolone showed high significant decrease (p≤ 0.01) in the number of total white blood cell, neutrophils and eosinophils and serum levels of Immunoglobulin E and Interleukin-4levels. The effects of this plant extract were comparable with the observed effect of prednisolone-treated group to be present with high efficacy of herbal medical therapy than that of prednisolone treatment a combined with adverse effects when use alone.
