Abstract

Background and objective: This study aimed to measure the prevalence of dyslipidemia in patients with rheumatoid arthritis compared with healthy control people and find out the correlation between dyslipidemia and disease activity.Methods: A prospective case-control was done. A total of 100 female patients with rheumatoid arthritis were included in the study. This study was conducted at the Rheumatology Out-patient Clinic in Rizgary Teaching Hospital, Erbil City over seven months from April 2013 to October 2013. Patients who fulfilling the 1987 revised American College of Rheumatology and 2010 American College of Rheumatology/European League against rheumatism classification criteria for rheumatoid arthritis and 100 healthy female age-matched controls with the mean age for both groups being 45. Fasting lipid profiles of cases and controls were estimated after an overnight fasting of 12 hours. Association between lipid profile and disease activity using disease activity score DAS 28, inflammatory markers (CRP and ESR) was also determined. Results: Rheumatoid arthritis patients showed a higher prevalence of associated dyslipidemia (47%) in comparison to control (5%), P = 0.004. There was a significant reduction in serum high density lipoproteins (P = 0.04), with significant elevation of serum total cholesterol, triglyceride, low density lipoprotein and very low density lipoprotein (P = 0.001, 0.004, 0.04 and 0.05, respectively) in comparison to controls. There was a significant relation between disease activity score 28 and serum high density lipoproteins (P <0.001) and there was a significant relation between ESR and triglyceride (P = 0.05).Conclusion: is frequent among the patients with rheumatoid arthritis and highly associated with active rheumatoid arthritis. Serum high density lipoproteins significantly reduced while other parameters of lipid profiles significantly increased in comparisons to control.

Abstract

This study was conducted to investigate the serum level of C-reactive protein, erythrocyte sedimentation rate, creatinine, glomular filtration rate, tumor necrosis factor-α (TNF-α) and interlukin-6 (IL6) in rheumatoid arthritis patients associated with renal impairment treated with antiTNF-α. This study included "two groups; 54 patients with rheumatoid arthritis treated with Etanercept (Enbrel) 50mg and 58 patients with rheumatoid arthritis on treatment other than Anti TNF-α as a comparative"group. The results showed that renal function tests of rheumatoid arthritis patients who received Anti TNF-α were different, before and"after treatment. A "significant decrease in the value of creatinine and in the level of GFR after 6 months of treatment was observed"(P≤ 0.0001). The erythrocyte sedimentation rate, C-reactive protein, interlukin-6 and TNF-α increased in rheumatoid arthritis patients with renal disorders before biologic therapy compared with the comparison group who were on treatment other than Anti TNF-α. Following the administration of Anti TNF-α"therapy. These inflammatory parameters were significantly diminished in the rheumatoid arthritis patients after the therapy. The treatment with Anti TNF-α seems to be a factor in restoring the levels of the inflammatory immune markers.

Abstract

Background and objective: Rheumatoid arthritis is a systemic inflammatory disease. Ocular manifestations involved with rheumatoid arthritis are keratoconjunctivitis sicca, episcleritis, scleritis, anterior and posterior uveitis, dry eyes and ulcerative keratitis. This study aimed to detect the prevalence of ocular manifestations in rheumatoid arthritis patients, find out the relation between the effects of disease modifying anti-rheumatic drugs, biological agents and ocular complications in rheumatoid arthritis patients and explore role of anti-cyclic citrullinated peptide antibodies.Methods: This is a cross-sectional clinical study of 60 rheumatoid arthritis patients who have attended Rizgary Teaching Hospital in Erbil. A data information about symptoms of ocular manifestations and history of ocular complications before starting disease modifying anti-rheumatic drugs and/or biological agents were taken. Musculoskeletal examination, serum Anti-CCP antibodies test were done. Ocular examination by ophthalmologist was done.Results: This study was conducted from April to November 2015 and involved 60 patients; 55 females and five males with a mean± SD age of 46 ± 11.46 years. Thirty nine (65%) patients had ocular manifestations of rheumatoid arthritis. The most frequent manifestation was dry eyes 30 (50.0%). There was no relation between severity of the rheumatoid arthritis disease and ocular manifestation (P = 0.529). There was a relationship between the Anti-CCP antibodies titer and presence of ocular manifestations in rheumatoid arthritis patients (P = 0.006). There were no curable effects of the disease modifying anti-rheumatic drugs, biological agents on ocular manifestations in rheumatoid arthritis patients (P = 0.787).Conclusion: Dry eye was the most common ocular manifestation, anti-CCP antibodies are a sensitive marker for ocular manifestations. Disease modifying anti-rheumatic drugs and biological therapies provide no significant efficacy in treatment or prevent the occurrence of ocular manifestations in rheumatoid arthritis patients.

Abstract

Background and objective: Rheumatoid arthritis (RA) is an autoimmune disease of unknown etiology. Vitamin D3 plays an important role in bone metabolism and may also have immunomodulatory effects. This study was designed to estimate the prevalence of vitamin D3 deficiency in patients with rheumatoid arthritis as compared to healthy controls and to analyze the association between vitamin D3 with disease activity.Methods: This case-control study included 50 consecutive rheumatoid arthritis patients, who fulfilled EULAR-ACR-2010 and ACR 1987 criteria for Rheumatoid Arthritis, and 50 age matched controls. Their mean age and standard deviation (SD) was 46.42±10.86 years. They were not on vitamin D3 supplements. They were selected from the Rheumatology and Medical Rehabilitation Department of Rizgary Teaching Hospital in Erbil City. The activity of disease was assessed by disease activity score and clinical disease activity index parameters. The vitamin D3 was measured for all patients and controls.Results: A total of 100 participants were enrolled in the study. The vast majority of them were female, and 70% of cases had deficient vitamin D3 levels, while 46% of control group suffered from vitamin D3 deficiency (P <0.05). There was a non-significant statistical relationship between vitamin D3 levels and duration of disease, disease activity score and clinical disease activity index.Conclusion: Level of vitamin D3 was low among cases compared to controls and it was statistically significant. Vitamin D3 was not correlated with the disease activity.

Abstract

Background and objective: Bone loss regarded as a major health concern which is thought to be associated with a number of chronic illnesses including rheumatoid arthritis. This study was designed to determine the frequency of low bone mass as well as the influence of disease duration on bone mass in the patients with rheumatoid arthritis.Methods: Sixty patients with rheumatoid arthritis who were visiting Rizgary Teaching Hospital in Erbil from April 2015 to March 2016were included in a cross-sectional study. Bone mineral density was measured using dual energy X-ray absorptiometry.Results: There was increased frequency of bone loss in patients with rheumatoid arthritis. There was a positive association between bone mineral density of the spine and femoral neck with disease activity measured by disease activity score and disease activity index (P <0.05). There was a significant association between bone mineral density and disease duration. Conclusion: Significant proportions of patients with rheumatoid arthritis have osteoporosis at the femoral neck and lumber spine, and disease duration longer than five years is associated with a significant increase in bone loss.

Abstract

Background: chronic periodontitis and rheumatoid arthritis are widely prevalent diseases and are characterized by tissue destruction due to chronic inflammation. Recently, there is growing evidence that the two diseases share many pathological features the aims of the study To determine the periodontal health status in patient have chronic periodontitis with rheumatoid arthritis and compare it with those having chronic periodontitis without Rheumatoid arthritis and determine the serum levels of interleukin -17(IL-17), tumor necrosis factor- alpha (TNF-α ) in both groups and compare with the control group (subject samples neither have periodontitis nor arthritis ) and correlate these immunological markers with the periodontal parameters Plaque index , gingival index , bleeding on probing, probing depth, clinical attachment level and number of missing teeth.Materials and methods: Eighty (80) males and females subjects with age range (30-45) years were recruited in this study they were divided into three main groups The chronic periodontitis with rheumatoid arthritis group consist of thirty (30) subjects and second group consist of thirty (30) subjects have chronic periodontitis and third group consist of twenty (20) subject case control group. All subjects had normal weight and height range according to BMI (body mass index) that it value is (18.5-25), Clinical periodontal parameters used in this study were Plaque index, gingival index, bleeding on probing, clinical attachment level index, probing pocket depth and number of missing teeth was measured in all groups at four surfaces of all presented teeth Blood samples were collected from all individuals and examined to determined serum level of interleukin -17 and tumor necrosis factor-a by mean of enzyme-linked immune–sorbent assay.Results: The present study showed patients with chronic periodontitis and rheumatoid arthritis had higher prevalence of sites presenting dental plaque, a higher rate of gingival inflammation and bleeding on probing greater probing depth, greater attachment loss and high number of missing teeth compared to those had chronic periodontitis only and control subjects . Also highly significant differences between studied group regarding serum level of IL-17 and TNF-α atp < 0.001, as well as, it revealed that mean serum levels of IL-17 were statistically higher in chronic periodontitis with rheumatoid arthritis group (607.9 ± 79.9) than Chronic Periodontitis group (421.4 ± 5.9) and Control groups (15.9 ± 2.7) similarly serum level of TNF-α (402.2 ± 41.2 319.4 ± 526 85.3 ± 4.9) respectively at p < 0.001. Regarding correlation, the current study observed strong positive correlation between serum levels of IL-17 andTNF-α with PL.I, GI, BOP, PPD CAL and the number of missing teeth in the PRA at p<0.001. Also this study reveal significant correlation between the two immunological markers (TNF-α and IL-17) in chronic periodontitis with rheumatoid arthritis group and in Chronic Periodontitis group.Conclusion: It was concluded that there was higher potentiality to chronic periodontitis involvement among rheumatoid arthritis patients, that correlated positively with increase the level of serum levels of IL-17 and TNF-α accordingly with high score of clinical parameters that had recorded. That mean TNF –a and IL-17 may play an important role in increase the severity of periodontitis as well as rheumatoid arthritis

Abstract

Rheumatoid arthritis (RA) is a common inflammatory disease associated with many extraarticularfeatures. The aim: of the current study is to assess the influence of omega-3 fatty acids(EPA, DHA) on serum alkaline phosphatase in patients with active RA. A single blinded placebocontrolled clinical trial with 12 weeks follow up period at which 50 patients with active RAusing methotrexate were randomized into 2 groups to receive either omega-3 (1000 mg) capsulethree times daily or capsules prefilled with glucose as placebo and were evaluated at zero time(baseline) and after 12 weeks for alkaline phosphatase level in serum. In addition to twenty fivehealthy subjects as control group. The age of RA patients in omega-3 group ranged from 25-80years (50.36±2.46), whereas the age of RA patients in placebo group ranged from 35-68 years(50.08±1.84 ), while the age of apparently healthy subjects in control group where ranged from40-60 years (94.20±1.12). The RA disease activity was measured using disease activity score of28 joints(DAS28-ESR) and clinical disease activity index (CDAI). After 12 weeks of startingadjuvant treatment with either omega-3 or placebo, the results showed that there were nosignificant difference between the effect of omega- 3 and placebo on alkaline phosphatase level.In Conclusion, omega-3 significantly decreased alkaline phosphatase level in patient with activerheumatoid arthritis.

Abstract

Background and objective: The prevalence of obesity is rising across the world and it is regarded as a major health concern which is thought to be associated with a number of chronic illnesses including rheumatoid arthritis. This study aimed to find out the prevalence of obesity in patients with rheumatoid arthritis and to apprehend the association of obesity with disease activity and latex positivity.Methods: One hundred twenty patients with rheumatoid arthritis who were regularly visiting Rizgary Teaching Hospital in Erbil were included in a cross-sectional study using specially designed mixed questionnaire. Body mass index and waist circumferences were measured, and their association with disease activity score 28 (DAS28) and clinical disease activity index (CDAI) were estimated. Furthermore, we evaluated their association with positive latex test.Results: There was increased frequency of obesity in rheumatoid arthritis patients, and there was a positive association between body mass index and waist circumference with disease activity measured by DAS28 and CDAI (P <0.001). There was no significant association between obesity and latex positivity which has been assessed separately for males and females (P = 0.898 and 0.086, respectively).Conclusion: Obesity is frequently found in rheumatoid arthritis patients. It is associated with higher disease activity, but not with the latex positivity in these patients.

Abstract

The study aimed to estimate the role of glucagon-like peptide-1 (GLP-1) and visfatin as a novel pro inflammatory marker in Rheumatoid Arthritis (RA ) according to the activity scores of disease to assess the possibility of introducing glucagon-like peptide-1 and visfatin in the diagnosis and monitoring of RA patients and to found the correlation of visfatin level with GLP-1 and AIP in patients prone to atherosclerosis ,fifty healthy individuals as control group (G1) and fifty rheumatoid arthritis (RA) patients (G2) were enrolled in this study with middle age ranged (30 – 40) years and BMI ≥24 kg/m2. ESR ,RF, lipid profile, CRP,insulin, visfatin and GLP-1were determined. Results in table (1) revealed highly significant increase in ESR,RF,TC,TG,LDL,VLDL and AIP level in rheumatoid arthritis (G2) comparing to (G1) while significant decrease noticed in (G2) group comparing to (G1). Results in table(2) showed highly significant increase in insulin ,CRP, visfatin and GLP-1 level Results in table(3) illustrated correlation relation data and results revealed highly significant ,were r=0.035 for GLP-1 with visfatin for (G1), and highly significant , r= -0.089 for (G2) as shown in figure(1-A) and(1-B), the result also highly significant were r= -0.183 for GLP-1 with CRP for control group and significant ,r= 0.043 for GLP-1 with CRP for RA group as shown in figure(2-A) and(2-B), and significant, r= -0.056 for GLP-1with AIP for G1 and no significant, were r= 0.042 for GLP-1 with AIP for G2 as shown in figure (3-A)and (3-B). conclusion that summarized from this study is glucagon-like peptide-1 and visfatin plays a role in RA pathogenesis that can be considered marker in RA . and may be used as a drug in future for RA patients.

تهدف هذه الدراسه الى تحدبد دور الفزفاتين كداله اساسيه للالتهابات المتواليه في مرضى التهاب المفاصل الرثوي وفقا لدرجات نشاط المرض لتقييم امكانيه استخدام الفزفاتين في تشخيص ومراقبه مرضى التهاب المفاصل الرثوي وايجاد معاملات الارتباط لمستوى الفزفاتين مع GLP-1 ,AIP في المرضى العرضين للاصابه بتصلب الشرايين,تم جمع نماذج الدم من خمسين شخصا من الاصحاء ظاهريا G1 وخمسين شخصا من مرضى التهاب المفاصل الرثوي G2 بلغت متوسط اعمارهم(30-40) سنه بكتله جسميه 24≤ كغم/م2.تم قياس مستويات ESR,RF,CRP,GLP-1 ومستويات الدهون,الانسولين,الفزفاتين.النتائج في الجدول (1) تظهر زياده معنويه عاليه في مستويات ESR,RF,TC,TG,LDL,VLDL,AIP قي المجموعه G2 مفارنه بالمجموعه الظابطه G1اما نتائج الجدول الثاني(2) تظهر زياده معنويه عاليه في مستويات الانسولين ,الفزفاتين CRP,GLP-1 ,ويوضح الجدول الثالث بيانات العلاقات الارتباطيه واظهرت النتائج معامل ارتباط معنوي 0.035 لل GLP-1مع الفزفاتين للمجموعه G1 ومعامل ارتباط معنوي-0.089 للمجموعه G2 كما موضح بالشكل (1-A) ,(1-B) ,كما اظهرت النتائج ومعامل ارتباط معنوي-0.183 لل GLP-1مع CRPللمجموعه G1 ومعامل ارتباط معنوي 0.043 لمجموعه المرضى كما موضح بالشكل (2-A) ,(2 -B) اما معامل ارتباط GLP-1 مع AIP بالنسبه للمجموعه الظابطه كانت معنويه -0.056وغير معنويه -0.042 لمجموعه المرضى كما موضح بالشكل (3-A) ,(3 -B) , ونستنتج من هذه الدراسه ان الفزفاتين يلعب دورا فعالا في الحالات المرضيه من التهاب المفاصل الرثوي وعلاقته ببقيه المعاملات التي تعتبر مؤشرات للاصابه بالجلطه القلبيه ويعتبر مؤشر مرضي قوي للRA ويمكن ان يستخدم مستقبلا كعلاج لمرضRA.

Pattern of pulmonary function test in rheumatoid arthritis patients

Abstract

Background: Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by a painful symmetrical peripheral polyarthritis. Extra-articular involvement in RA is a common condition. Pulmonary involvement occurs in 20 to 67 % of RA patients and it is responsible for about 10-20 % of mortality in RA patients.Objectives: To evaluate the extent and pattern of pulmonary involvement in RA patients by using spirometry. Methods: A case control study conducted at the rheumatology unit of Al Sader teaching hospital, forty RA patients [33(82.5%) female and 7(17.5%) male] and sixty apparently healthy subjects {50(83.3%) female and 10(16.7 %) male} were included in the study. All subjects had subjected to a pulmonary function test (PFT) for pulmonary function evaluation using MIR spirolab III. Measured and predicted forced vital capacities (FVC), forced expiratory volume in the first second (FEV1) and FEV1 / FVC ratio were considered as the parameters in this study for both groups.Results: Nine patients were found to have a restrictive pattern of pulmonary function test, while obstructive pattern was found in only one patient. On the other hand, the pulmonary function tests were normal in all of sixty healthy control. The mean of FEV1 in patient group (2.07 ± 0.56L) and of FEV1% (95.8%  25.2) are significantly lower than those in the control group (2.46 ± 0.65 L; 105.3%  13.09). In addition, mean FVC in the patient group (2.35 ± 0.62L) and its mean percentage (92%  22.68) showed a clear significant reduction compared to those in control group (2.71 ± 0.67 L; 100.53  12.12). No significant difference (P = 0.173) has been observed in FEV1 / FVC ratio between patient (88.17  7.47) and control group (89.87  4.99).Conclusions: Different pattern of pulmonary function abnormalities could be manifested in RA patients and the restrictive pattern represents the most common feature. Spirometry is indicated as a baseline assessment and for follow up of RA patient to enhance early detection and management of the pulmonary involvement.

التهاب المفاصل الروماتزمي هو مرض التهابي مزمن يسبب التهاب المفاصل الطرفية المتناظرة المؤلم. اصابة اعضاء اخرى في الجسم غير المفاصل يعتبر حالة شائعة لدى المصابين بهذا المرض. حيث ان الإصابة الرئوية تحدث في 20 إلى 67٪ من مرضى التهاب المفاصل الروماتزمي، وهي مسؤولة عن حوالي 10-20٪ من الوفيات لمرضى التهاب المفاصل الروماتزمي .الأهداف: لتقييم مدى ونمط الاصابات الرئوية في مرضى التهاب المفاصل الروماتزمي في مدينة بصرة عن طريق فحص وظائف الرئة .الطريقة: الدراسة هي دراسة مقارنة بين الحالات المرضية والاصحاء واجريت في وحدة الروماتزم في مستشفى الصدر التعليمي في مدينة البصرة وشملت اربعون مريضا بالتهاب المفاصل الروماتزمي }33 (82,5%) أنثى و 7(17،5%) ذكور{ و ستون شخصا اصحاء }50(83،3%) أنثى و 10(16،7%) ذكور{. وقد خضع جميع الاشخاص المشمولين بالدراسة لاختبار فحص وظائف الرئة لتقييم وظيفة الرئة باستخدام جهاز سبايرولاب حيث تم قياس حجم هواء الزفير الكلي القسري ((FVC، وحجم الزفير القسري في الثانية الأولى (FEV1)، ونسبة القيمتين FEV1/FVC)) لكلا المجموعتين.النتائج: وجدان تسعة مرضى لديهم نمط تحددي في اختبار وظيفة الرئة، في حين تم العثور على نمط انسدادي في اختبار وظيفة الرئة في مريضة واحدة. من ناحية أخرى، كان اختبار وظيفة الرئة طبيعيا في كل الاشخاص الاصحاء. وكانت قيمة متوسط FEV1 في مجموعة المرضى (2,07 ± 0.55) لتر ومتوسط نسبةFEV1% (95،8±25،2%) وهو اقل من المجموعة المقارنة للأشخاص الاصحاء (2.46 ± 0.65 لتر) بمتوسط نسبة FEV1٪ (105,3± 13,9٪). بالإضافة إلى ذلك، أظهر متوسطFVC في مجموعة المرضى (2,35± 0,62) لتر بمتوسط نسبة 92٪ بانخفاض واضح مقارنة مع مجموعة المقارنة للأشخاص الاصحاء الذين كان متوسط قيمة FVC (2.7 ± 0.67 لتر) ومتوسط نسبة FVC٪ عند (100،53٪). هناك اختلافات طفيفة (غير معنوية) في نسبة FEV1 / FVC بمتوسط قيمة 88.17٪ في مرضى التهاب المفاصل الروماتزمي و 89.87٪ في المجموعة المقارنة للأشخاص الاصحاء.الاستنتاجات: انماط مختلفة من اصابات الرئة يمكن أن تظهر لدى مرضى التهاب المفاصل الروماتزمي ويمثل النمط المحدد الاصابة الأكثر شيوعا. ونتيجة لذلك يوصى بأجراء فحص وضائف الرئة باعتباره فحص اساسي لمرضى التهاب المفاصل الراتزمي لتعزيز الكشف المبكر للإصابات الرئوية.الكلمات المفتاحية: التهاب المفاصل الروماتزمي، فحص وظائف الرئة
