The Role of Metformin in patients with Primary Hypothyroidism

Abstract

Background: Insulin resistance is present in 50% or more of patients with primary hypothyroidism. Metformin can decrease TSH levels in these patients by a complex matter, this can be of great help in clinical practice.Objective: This study was designed to evaluate the effect of metformin in reducing TSH levels in patients with primary hypothyroidism.Methods: Hundred patients with primary hypothyroidism, 82 females, 18 males were included in this study, everyone was followed up for two months after adding metformin 850 mg twice daily in addition to thyroxin.Results: 36 patients (36%) have a normal baseline TSH and no change after 2 months, 64 patients (64%)have a change in TSH either a decrease(49 patients ,79.56%) or an increase (15 patients , 23.44 % ).Conclusion: Metformin therapy is highly effective in reducing TSH levels in patients with primary hypothyroidism

Abstract

The importance of this study appears in analyzing and diagnosing the primary education services in Diyala Provence and limiting how they are distributed and their sufficiency in producing the required services for the population at the level of Administerial Units for the Provence. It has been during the study the lack of school buildings were the original schools reached (80.9%) from the total of the Provence and (19.1%) hosting schools form the total of the Provence. The is an increase in the number of schools compared to the number of students were the ration of each student was (13 students/teachers) in all the Provence.

تبرز اهمية هذا البحث في تحليل وتشخيص خدمات التعليم الابتدائي في محافظة ديالى وتحديد كيفية توزيعها ومدى كفايتها في تقديم الخدمة اللازمة للسكان على مستوى الوحدات الادارية للمحافظة. وقد ظهر من خلال الدراسة قلة عدد المباني المدرسية اذ بلغ نسبة المدارس الاصلية (80.9%) من مجموع المحافظة والمدارس الضيف (19.1%) من مجموع المحافظة، وان هناك زيادة في عدد المدارس مقارنة بعدد التلاميذ اذ بلغ نصيب التلميذ من المدرسة نحو (257) تلميذ /مدرسة ، فضلا عن تحسن حصة المعلم الواحد من عدد التلاميذ اذا بلغ (13) تلميذ / معلم على مستوى المحافظة

Abstract

This study aims at identifying the Primary Thinking Skills Included in the Reading Books of the Primary Stage and the extent of their availability in the books of reading in the first three primary stages. To achieve this goal, the researcher used the analysis method, using other analysts, the coefficient of stability between him and the first analyst (0.80%), the researcher and the second analyst (0.85%), the first analyst, the analyst and the second (0.82%). To extract the results, the researcher used repetitions and percentages of primary thinking skills The results of the search for the highest skill achieved,(Application skill) by (59%)And then (skill of remembering) by (45%) Followed by (comparative skill) by (37%) Then (cognitive skill) by (22%) and (classification) by (13%) Then (coding skill) by (9%) And (induction skill) by (11%) And (ranking skill) by (8%) In addition (questioning) by (8%). In the light of the results of the research, the researcher recommended several recommendations, and suggested several proposals, a similar study of this study in the analysis of exercises other Arabic books for the primary stage, and in light of other thinking skills.

هدف البحث الحالي إلى تحديد مهارات التفكير الاولية المتضمنة في كتب المرحلة الإبتدائية , ومدى توافرها في كتب القراءة الإبتدائية للمراحل الثلاثة الاولى , و لتحقيق هذا الهدف قام الباحث بطريقة التحليل ,وبالاستعانة بمحللينِ آخرينِ ,وبلغ معامل الثبات بينه وبين المحلل الأول (0.80%) ,وبين الباحث والمحلل الثاني (0.85%) ,وبين المحلل الأول ,والمحلل والثاني(0.82%) ,ولاستخراج النتائج استعمل الباحث التكرارات ,والنسب المئوية لمهارات التفكير الاولية، واسفرت نتائج البحث عن أعلى مهارة محققة, وهي ( مهارة التطبيق) بنسبة (59) , ومن ثَمَّ(مهارة التذكر) بنسبة ( 45 ), ثم تليها ( مهارة المقارنة) بنسبة (37) ,ثم (مهارة الادراك) بنسبة (22), ومهارة (التصنيف) بنسبة(13) ,ثم (مهارة الترميز بنسبة (9), و(مهارة الاستقراء) بنسبة (11), و(مهارة الترتيب) بنسبة ( 8 ) , و(مهارة الأسئلة) متساوية بنسبة (8) ,وفي ضوء نتائج البحث أوصى الباحث بتوصياتٍ عدّة , واقترح مقترحاتٍ عِدّة منها ,إجراء دراسة مماثلة لهذه الدراسة في تحليل تمرينات كتب اللغة العربية الاخرى للمرحلة الابتدائية ,وفي ضوء مهارات التفكير الاخرى.

Abstract

In this paper we introduced and study two concepts one of is a subclass of a class of weakly primary submodules called WE-primary submodules and the other of a subclass of weakly quasi-prime submodules called WE-quasi-prime submodules. Several properties of WE-primary and WE- quasi - prime submodules have been given, and some characterizations of each type of submodules are introduced moreover behavior of these submodules in some classes of modules are investigated. On the otherhand, the homeorphic and inverse image of these concept under certain conditions are proved

في هذا البحث قدمنا ودرسنا مفهومين الاول هو صف جزئي من صف المقاسات الجزئية الابتدائية الضعيفة.صف المقاييس الجزئية الظاهرية الضعيفة والذي يدعى المقياس الجزئي الابتدائي من النمط –WE والثاني هو صف جزئي من صف المقاسات الجزئية الأولية الظاهرية الضعيفة والذي يدعى المقياس الجزئي الاولي الظاهري من النمط – WE. اعطيت العديد من الصفات المقاسات الجزئية الابتدائية من النمطWE والمقاسات الجزئية الأولية الظاهرية من النمط – WE وبعض المكافئات لهذين النوعين من المقاسات الجزئية قدمت. بالإضافة الى ذلك سلوك هذه المقاسات الجزئية في بعض انواع المقاسات وجدت. من ناحيه اخرى الصورة والصورة العكسية لهذين المقياسين الجزئيين تحت تأثير التشاكل برهنت

Abstract

Background: The Service Provision Assessment (SPA) survey is a nationwide facility‑based survey that measures the ability of health facilitiesto provide quality preventive, diagnostic, and treatment services for key maternal and child health and HIV/AIDS programs. Objective: Thestudy was designed to assess some aspects of expressed maternal health services provided at ten primary health‑care (PHC) centers inBabylon province, Iraq, using interviews with health service providers and clients and observations of provider–client consultations to obtaininformation on the capacity of facilities to provide quality services, and the existence of functioning systems to support quality services.Materials and Methods: A cross‑sectional study was done in ten Babylon PHC centers from March to August 2018. The study included165 pregnant women. The questionnaire included sociodemographic factors (age, residence, employment, parity, and education) and datarelated to Service Provision Assessment questionnaire which include a facility inventory, client exit interviews, and antenatal care (ANC)observation and consultation. Results: The mean age of pregnant women attending ANC was 25.29 ± 5.48, 66.1% came from urban areas,the majority were not employed (83%), their educational level was secondary (38.8%), and 57% were multipara. The findings showed that themajority (89.1%) of patients think that health facility services were good. A sizeable majority (83.0%) of women were satisfied regarding healthservices including availability of medicines at the facility (91.5%), cleanliness of the facility (100%), privacy (93.9%), charging or paymentof fees for services received (0.6%), and discussion of problems related to the pregnancy (93.3%). Conclusion: This study showed a highlevel of satisfaction with the quality of maternal health services among antenatal attendees and highlighted the need to support interventionsthat increase the uptake of formal maternal health‑care services.

Abstract

The research aims at setting up measure for the tendencies of primary stage students towards environment. It also aims at knowing pupil's tendencies according to variable of sex (males and females). The principal sample of the research consisted of (400) pupils: 200 male, (200) female. The sample had been (20040) fifth-class pupils which was chosen randomly: (8020) male and (12020) female from (324) schools.The researches followed main steps to set up a measure for the tendencies towards environment. The validity of its items has been conducted through apparent validity, the validity of its setting up and through deducting the distinguishing power of its items and calculating its the constancy by the method of half division. The coefficient of its constancy reached to (0.70) by the use of person correlation coefficient.The constancy coefficient became (0.88) after its correction by the use of serman correlation coefficient. When measures were applied in its final form on the research sample, the results were as follows:

يهدف البحث الى بناء مقياس لاتجاهات تلاميذ المرحلة الابتدائية نحو البيئة ومعرفة اتجاهات التلاميذ نحو البيئة تبعاً لمتغير جنس التلاميذ (ذكور وإناث) وتكونت عينة البحث الأساسية من (400) تلميذ وتلميذه بواقع (200) تلميذ و(200) تلميذه، اختيرت بشكل عشوائي من مجتمع تلاميذ الصف الخامس البالغ (20040) تلميذ وتلميذه منهم (8020) تلميذ و(12020) تلميذه موزعين على (324) مدرسة ابتدائية.واتبع الباحثان خطوات اساسية لبناء مقياس الاتجاهات نحو البيئة حيث تم اجراء الصدق لفقراته من خلال الصدق الظاهري وصدق البناء، واستخراج القوة التمييزية لفقراته وحساب الثبات له بطريقة التجزئة النصفية وقد بلغ معامل ثباته باستخدام معامل ارتباط بيرسون (0,70) وبعد تصحيحه باستخدام معامل سيرمان بروان اصبح معامل الثبات (0,88) وبعد تطبيق المقياس بصيغته النهائية على عينه البحث كانت النتائج كما يأتي:

The Efficacy of Direct Trocar versus Veress Needle Method as a Primary Access Technique in Laparoscopic Cholecystectomy

Abstract

Objective: The objective of this study was to compare the safety and efficacy of direct trocar method and veress needle as the primary entry technique to establish pneumoperitoneum. Study Design: This was a randomized controlled study. Patients and Methods: A randomized controlled study was done on 150 patients who complained from gallstones and attended to our surgical team, from the period of May 1,2013 to May 31, 2017, the patient age range was 19–65 years with a mean age of 41 ± 0.9 years, after confirming the diagnosis of gallstones,informed consent was obtained from the patients. Those with previous abdominal surgery or serious comorbidities were excluded from the study.A total of 150 patients were divided randomly with 75 patients had the access into their abdomen by veress needle and the other 75 patients by direct trocar method. Variables comparing the safety and efficacy of the two methods were studied. Results: Direct trocar method was faster (2.3 ± 1.1 standard deviation [SD] min) than veress technique (5 ± 0.9 SD min), CO2 leak occurs just in one case of trocar method while it does not occur in veress technique, extraperitoneal insufflations occur in four cases (5.3%) operated by veress method, and only in one case of direct trocar method, port site infection develops in two cases (2.6%) of direct trocar method and in single case of veress cannula. Only one case of port site hematoma occurs in direct trocar method. Conclusion: Direct trocar method is a safe, fast, and easily done technique for the establishment of primary entrance into the peritoneal cavity and hence pneumoperitoneum, there were no significant variations in minor complications and without major ones.

Abstract

The study deals with the notion of an approximaitly primary submodules of unitary left R-module M over a commutative ring R with identity as a generalization of a primary submodules and approximaitly prime submodules, where a proper submodule N of an R-module M is called an approximaitly primary submodule of M, if whenever ay∈N, for a∈R, y∈M, implies that either y∈N+soc(M) or a^k M⊆N+soc(M) for some positive integer k of Z. Several characterizations, basic properties of this concept are given. On the other hand the relationships of this concept with some classes of modules are studied. Furthermore, the behavior of approximaitly primary submodule under R-homomorphism are discussed.

Abstract

Let Ḿ be a unitary R-module and R is a commutative ring with identity. Our aim in thispaper to study the concepts T-ABSO fuzzy ideals, T-ABSO fuzzy submodules and T-ABSOquasi primary fuzzy submodules, also we discuss these concepts in the class of multiplicationfuzzy modules and relationships between these concepts. Many new basic properties andcharacterizations on these concepts are given.
