12.CO2 DIODE LASER HEMORRHOIDECTOMY: CLINICAL EXPERIENCE WITH 150 PATIENTS

Abstract

Background: Hemorrhoids are the most common benign anorectal conditions. About half of population are affected by hemorrhoids during their life time. Conventional surgical hemorrhoidectomy, which was the standard treatment for many decades are associated with remarkable and intolerable pain that may last up to 3 to 6 weeks, which makes more patients reluctant to surgery. Laser hemorrhoidectomy is relatively new promising modality with less postoperative pain and less complications with fast wound healing and early return to work.Objective: To assess the validity, feasibility and the outcomes of using the CO2 diode laser for treatment of symptomatic hemorrhoids.Methods: A prospective study conducted for the period from September 2013 to April 2015, included 150 patients (135 male and 15 female), with symptomatic first, second and third degree piles with age ranged from 24 to 83 years (mean 48.7 year) submitted to CO2 diode laser hemorrhoidectomy with either the coagulation mode or cutting mode of 30 Watt diode laser surgical machine. The procedures were done under local anesthesia as a day case surgery by single surgeon. Results: The results showed that the operative time ranges from 15 to 30 minutes (mean 23.5 minutes). Postoperative pain scores by visual analog scale (VAS) in the first, second and third postoperative days were 2.9, 2.1, 1.8 respectively. The pain decreased rapidly after the first week and reached to zero after 14 days postoperatively. The complications rate recorded in this work was 7.3%. All these complications were mild and can be dealt with conservatively; redo surgery was not required in any case. All patients were discharged 2 to 4 hours postoperatively and were followed for 3 to 6 months.Conclusion: Hemorrhoidectomy by CO2 diode laser is effective, and very quick outpatient procedure with very mild postoperative pain and low or negligible rate of complications. It associated with rapid healing and fast recovery that most patient can return to normal daily activity within 2 to 5 days. Abstract

The present study aimed to study different ways for treatment of Localized Juvenile periodontitis, cultural and microscopic results of specimens from base line, appeared the following bacterial strains: Bacteroides gracilis ,Fusobacterium nuclautum, Porphyromonas intermedia, Actinomyces, peptostreptococcus species, eikenella corrodens, streptococcus intermedius, Streptococcus salivaris. Which distribution in 50% gram positive and 50% gram negative. Cultural and microscopical result after ten days of the experimental design appeared significant difference P≥ 0.05, at the SRP group results of microscopic and culturing show Peptostreptoccal species, Prevotella intermedia, Fusobacterium nuclautum, Streptococcus Saliveris. While at the SRP and antibiotic treated group (Doxycyclin 100 mg) the pattern of microbial study appeared the following bacteria: Fusobacterium nucleotum, Peptostreptococcus, Streptococcus salivaris. At the other hand the results of third group SRP and diode Laser appeared no growth of microorganism at all site subjected to Laser 0.8 w, 940 nm for 20 sec after ten days of study.

Abstract

Background: lip lengthening procedure is one of the surgical options for the correction of gummy smile in patients with short upper lip.Methods: A comparative clinical study was conducted on 15 patients requiring lip lengthening procedure for the esthetic correction of excessive gingival exposure with gummy smile. Scalpel was used in seven patients and diode laser in the remaining eight patients. Under infiltration anesthesia, about one cm strip of mucosa was excised at the vestibular depth and the mucosa of the lip was sutured to the alveolar mucosa.Results: The diode laser group demonstrated less postoperative pain and swelling. Regarding postoperative ecchymosis, three patients in the scalpel group developed ecchymosis and no cases were noted in the laser group, the difference was statistically significant. No patient in the laser group developed wound dehiscence and only one patient in the scalpel group had wound dehiscence at third postoperative day. Observed at one month postoperatively, the laser treated groups had less obvious scar than the scalpel group.Conclusion: Diode laser is a valuable tool in performing lip lengthening for gummy smile

Abstract

The wide use of laser in medicine and surgery make it a well - known tool in all fields of medicine, which lead the physicians to put quires whether it may have side effects on the body as a whole and on the gestation and the fetus especially. The purpose of the being study is to explore the accessible confirmation to delimit the protection of laser therapy during the pregnancy and supporting of utilising the diode laser when selected in the therapy of cases followed the gestation.Twenty-eight local adult females’ rabbits employed in the present study, they were preserved naturally, furthermore formerly divided into four groups. For each group seven rabbits. The laser sours used was a diode laser with wavelength 904nm, Continuous operations, power density (2X10-3 watt/mm2). The period of radiation was 15 min/session by direct contact of the window of the laser sours to the area of the pelvis. The 1st.group radiated during the 1st.week of gestation after the copulation directly, 2nd.group radiated at the beginning of the 2nd.week of gestation, 3rd.group radiated at the beginning of the 3rd.week of gestation and 4th.group was radiated at the beginning of 4th.week of gestation. The average length of gestation for the females’ rabbits employed in this study was between 29–32 days and they gave birth to 2–6 litters in a sex average between males and females.The outcomes revealed that the laser irradiation didn`t create any side effect or clinical symptoms during the period of a whole gestation. It was found that the laser radiation didn`t elongate or shorten the course of gestation, on the reverse it remains at its normal limits. In addition, the numbers of litters remain at normal limits. There were no abortions or abnormalities in the litters. The percentage of males compared with females outlive at normal values.A conclusion can be progressed that the diode laser is innocuous enough to be applied if needed to treat pregnant female throughout the period of gestation externally any fear of abortion or deformities.

Stimulation of Hatching Efficiency in Capsulated Cysts of Artemia fransiscana Using 890 nm Diode Laser

Abstract

Artemia fransiscana is one of the most important live food for commercial larval aquaculture. The aim of this study is to investigate the effects of 890 nm diode laser irradiation on Artemia capsulated cysts using (1-10) minutes exposure time, and 2.26x10-3 J/cm2 Fluence. The Artemia samples were obtained from two locations: Dyalaa and Basraa. After irradiation, hatching percentage (H %) and hatching efficiency(HE) of Artemia were measured after 24 and 48 hours of incubation. The results of the effect of laser light on the capsulated cysts from Dyalaa showed that the optimum dose for enhancing (H %) after 24 hours of incubation is using 10 minutes exposure time, while after 48 hours of incubation the (H %) enhancement can be achieved using 6 minutes exposure time. The optimum exposure times for (HE) enhancement after 24 and 48 hours of incubation were 5 and 7 minutes. The results of the effect of laser light on the capsulated cysts from Basraa showed that after 24 hours of incubation, the optimum exposure times for enhancement (H%) was 9 minutes, while after 48 hours of irradiation the best exposure times was 5 minutes .Very effective enhancement of (HE) was noticed after 24 hours of irradiation at 3 minutes exposure time using 2.26x10-3 J/cm2 Fluence. No enhancement was observed after 48 hours of irradiationIn conclusion, 890 nm diode laser irradiation can be used successfully for increasing Hatching percentage (H %) and Hatching Efficiency (HE) of Artemia capsulated cysts using certain energy density and certain exposure times

تعتبرArtemia fransiscana واحدة من اهم الاعلاف للحيوانات المائية ,السمك, الروبيان, السرطان و..الخ خلال ادوارها اليرقية. الهدف من الدراسة هو البحث في تأثير 890 نانوميتر دايود ليزر على الحويصلات المتكبسلة للـجنس Artemia بأستخدام (1-10) دقائق زمن تشعيع و 2.26 x 10-3 جول/ سم 2 كثافة طاقة. عينات الجنس Artemia مأخوذة من منطقتين هي ديالى والبصرة. بعد التشعيع, تم قياس كل من نسبة الفقس% وكفاءة الفقس بعد 24 و48 ساعة من الحضن.أظهرت نتائج تأثير ضوء الليزر على الحويصلات المتكبسلة لمنطقة ديالى بأن الجرع المثلى لزيادة نسبة الفقس % بعد 24 ساعة من الحضن كانت عند 10 دقائق زمن تعريض, بينما النسبة المئوية للفقس بعد 48 ساعة من الحضن ازدادت عند 6 دقائق زمن تعريض. اما وقت التعريض المثالي لكفاءة الفقس فقد شوهد عند 5 و 7 دقائق زمن تعريض بعد 24 و 48 ساعة حضن بالتعاقب.أما بالنسبة لحويصلات منطقة البصرة كانت اعلى نسبة فقس لوحظت عند 9 دقائق زمن تعريض بعد 24 ساعة حضن بينما سجلت بعد 48 ساعة حضن افضل نسبة فقس % عند 5 دقائق زمن تعريض. لوحظ زيادة فعالة لكفاءة الفقس بعد 24 ساعة حضن عند 3 دقائق زمن تعريض, لكن لم يلاحظ أي زيادة لكفاءة الفقس لجميع اوقات التعريض للتشعيع بالليزر بعد 48 ساعة اذا ما قورنت مع مجموعة السيطرة.نستنتج من ذلك يمكن استخدام ليزر الدايود ذات الطول الموجي 890 نانوميتر لزيادة النسبة المئوية للفقس وكفاءة الفقس للحويصلات المتكبسلة ذات الكفاءة الواطئة للـ Artemia بأستخدام جرع ليزر بخصائص مثلى.

Abstract

Objective: To evaluate the results of treating laryngeal squamous cell carcinoma using diode laser.
Design: Prospective study.
Setting: Department of ENT-Al Jamhuri Teaching Hospital.
Patients and Methods: Thirty one cases of squamous cell carcinoma of the larynx with age range between 32-66 years. The stage of tumour was Tis: 3, T1: 12, T2:14, & T3:2 cases . Lesions were excised or evaporated using diode laser with safety margin and followed for at least 36 months to evaluate cure rate, recurrences and its relation to site and stage of tumour, need for tracheostomy, complications and quality of voice.
Results: Diode laser was highly effective in treating early laryngeal cancer.Cure rates after 36 month follow up were 100%, 92% & 78% for Tis, T1 & T2 respectively. For more advanced lesions (T3) , the role of laser was limited only for debulking to avoid tracheostomy or biopsy taking in deep seated tumours as alternative to open biopsy. No major intraoperative or postoperative complications happened and no tracheostomy needed. Patients were discharged after twenty four hours. Voice quality was good after surgery in most cases.
Conclusion: Diode laser is safe in treating squamous cell carcinoma of the larynx and highly effective in early stages (Tis, T1, T2) and can avoid tracheostomy or open biopsy in more advanced lesions.

Abstract

Aims of the study: To compare the new diode laser (1064nm) incision with conventional blade inci-sion wounds after oral soft tissue biopsy in terms of haemostasis, local anesthetic required, duration of incision and post operative pain. Materials and Methods: This study was conducted in the Oral and Maxillofacial Surgery Department at College of Dentistry/University of Mosul and Oral and Maxil-lofacial Surgery Department in Al-Salam Teaching Hospital between February 2010 and May 2010. Thirty patients were enrolled and divided into two groups, each consisted of fifteen patients. The first group included fifteen patients requiring biopsy (based on their indications for biopsy) in the oral cavi-ty, where the incision was done using surgical blade no.15. The second one included fifteen patients who needed biopsy in the oral cavity (based on their indications for biopsy), here the incision was made by using diode laser (FOXTM, A.R.C. laser, Germany) wavelength 1064nm with output power (3.5Watt) CW in contact with the tip of bare fiber (200micron). All biopsies in the two groups were excisional and done under local anesthesia using infiltration technique. Results: In comparison be-tween the two groups: the amount of local anesthetic required in the second group (diode laser wound group 0.73 ml) was less than that required by the first group (blade incision wound group 1.6 ml) and showed statistically high significance at p value < 0.05. The mean of the difference in the weight of the gauze before and after the operation in the second group(0.16 gm) was less than the difference in the weight of the gauze in the first group(0.96 gm) which showed statistically high significance at p value <0.05. The mean of the duration for incision was statistically not significant between the two groups and the mean of pain score at the first post-operative day was statistically significant in the second group in comparison to the first group but there was no statistical difference in the pain score level at the third postoperative day between the two groups. Conclusion: The use of Diode (1064nm) laser to perform oral biopsy has several advantages over the blade incisions wound including less amount of local anesthesia required, enhanced haemostasis (suturing after surgery was not necessary) , minimal postoperative pain.

Abstract

Background:Surgical diode lasers have been used in oral surgical procedures with beneficial effects as compared to the conventional techniques.Objective:To evaluate the efficacy and safety of 810 nm surgical diode laser in the field of oral and maxillofacial surgery.Methods:Forty patients who had different oral lesions were attending consultation clinic of the Maxillofacial Surgery Department in the Al-Kadhimyia Teaching Hospital. The patients have been treated by 810 nm diode laser. The power of the diode laser was 2-5 Watts in continuous mode. Excisional biopsies were sent for histopathological examination. Intraoperative and postoperative clinical examinations were done.Results:The clinical observations revealed no bleeding intraoperatively and postoperatively, no infection and minimal swelling was seen postoperatively.Conclusion:The benefits of 810 nm diode laser application in oral and maxillofacial surgery have been justified based on its efficacy and safety. There is a good acceptance for this new modality of treatment by the patients.Keywords:Surgical diode laser, soft tissue lesion, oral surgery

Abstract

The triggering effect for the face pumping of Nd:YVO4 disc medium of 4×5×0.5 mm was investigated using bulk diode laser at different resonator cavity length in pulse mode and at repetition rate of 1.3kHz. The maximum emitted peak power was found to be 100, 82, and 66 mW for resonator lengths of 10, 13.5, and 17.5 cm respectively, while the threshold pumping power was found to be 41mW. The maximum emitted peak power obtained was 300 mW when using external triggering and 10cm length, with repetition of 3Hz.

في هذا العمل تمت دراسة تأثير القدح في عملية الضخ المباشر الامامي لعينه القرص (Nd:YVO4 ) بابعاد )4x5x 0.5 mm( باستخدام ليزرالصمام الثنائي لابعاد مختلفة لمرنان الضخ النبضي بتكرارية kHz 1.3. ووجد ان اعلى قيمة للقدرة المنبعثة هي بحدودmW 100,82,66 لابعاد المرنان cm 10,13.5,17.5 على التوالي بينما وجد ان قدرة العتبه كانت بحدود mW 41 وان اعلى ذروة قدرة انبعاث كانت mW 300 عند استخدام اشارة قدح خارجية ولطول المرنان cm10 وتكرارية Hz3 للضخ النبضي.

Abstract

The bentonite is an adsorbent material that is used frequently in industrial fields. bentonite was used with laser to treat wastewater, when laser is a new technique that has ability to remove the dye. Using of laser as a source to refresh with high efficiency has the important in industrial field.In this study laser diode with power 50 mw, He-Ne laser with power 10 mw and DPSSL with power 500 mw were used with the help of ultrasonic bath and H2O2 to promote the work of the laser. AFM and FTIR testing were made to bentonite to study the characteristics of it. Minitab program was used to facilitate calculations and choose the optimum parameters of experiments. The optimum input conditions of experiments to remove the dye from wastewater were chosen. The best concentration of bentonite is 4 g/l, the best temperature of wastewater is 42◦C, the best time of work is 27 min and the best concentration of H2O2 is 10%. It found from study that the increase of each parameters cause decrease in output concentration of wastewater. The accuracy of work was determined from Minitab program, it was 94.54%.
