Abstract

Nonsteroidal anti-inflammatory drugs (NSAIDs) represent the most frequently prescribed medicine in the world. Their main disadvantages were related to their effect on the gut and kidney. Since the early 80s of the last century, drugs manufacturers try to discover a NSAIDs which does not affect the gut and kidney .The gut toxicity may be decrease by concomitant use of antisecretery agents, but this increase their cost and decrease the compliance. The fully selective cyclooxygenase inhibitors has been introduced recently as an alternative to old generation NSAIDs.Unfortunaltly subsequent long-term evaluation produced disappointing results, and opened the door for a lot of discussion and controversies.

Abstract

Iron deficiency anemia (IDA) is one of the most common presenting features of coeliac disease (CD). The aim of this study is to find the prevalence of CD among patients with otherwise unexplained IDA and reviewing the literatures. This was a cross sectional study of patients with IDA. Gastroduodenoscopy was performed to determine the cause of anemia.Four biopsies were taken from the second part of duodenum. Small intestinal histologic features were interpreted according to the revised Marsh criteria. Total number of patients was 47, 28 males and 19 females. Mean age of 26.9±13.3 years. Twenty of 47 patients (42%) had biopsy result consistent with CD .All CD patients were at age of 28 year and below. Only one patient was at age of 45 year. Out of 20 CD patients, 16 were having Marsh grade IIIB and 4 with Marsh grade IIIC.Fourteen were males and 6 were females All patients with other wise unexplained IDA should have upper gastrointestinal endoscopy with at least four biopsies from the duodenum to exclude CD. DIABETIC FOOT: CORRELATION BETWEEN CLINICAL ABNORMALITIES AND ELECTROPHYSIOLOGICAL STUDIES

Abstract

Diabetic foot ulceration is a serious and expensive complication with considerable morbidity that affects up to 15% of diabetic patients during their lifetime and 80-85% of amputations are preceded by foot ulcers. The aim of this work is to study the correlation between severity of clinical abnormalities and electrophysiological studies in diabetic foot ulcers. This study was a cross sectional evaluation of 44 patients with diabetic foot ulcers seen in 2 hospitals in Basrah (Al-Faiha General and Basrah Teaching) from October 2003 to July 2004. All patients were type 2 diabetes mellitus. The sensitivity of numbness, burning feeling, pricking feeling and worse symptom at night was 84.6%, 69.2%, 61.5% and 51.5% respectively. While sensitivity of decreased pin prick sensation, absent vibration sense, absent ankle jerk, decreased temperature sensations and absent position sense was 100%, 87.2%, 71.8%, 56.5% and 12.8% respectively. Sensitivity of combined clinical symptoms was 66.6%, with specificity of 40%, and predictive value of 89.6% while that of clinical signs 48.7% and 60% respectively and predictive value of 90.4 %. There was no significant difference in severity of electrophysiological abnormalities in the affected and non-affected feet. Clinical findings was correlated well with the severity of electrophysiological changes in patients with diabetic foot ulcers.

Abstract

ABSTRACT Metabolic syndrome increases the risk for coronary heart disease and stroke by three folds with marked increase in cardiovascular mortality. The aim of this work is to study metabolic syndrome in adult’s persons in Basrah in single center. A cross sectional hospital based study of adults persons including persons seen in the in-patient and out-patient clinic of the Al-Faiha General hospital over a period from January to August 2004, who agree to participate. The presence of 3 metabolic abnormalities is enough to establish the diagnosis of metabolic syndrome. The metabolic abnormalities were abdominal obesity, high serum triglycerides, low high-density lipoprotein cholesterol, hypertension, and high fasting plasma glucose. Total number of persons was 500, of them 152 women and 348 men. Age range 20-88 year, with mean age of 49.9± 11.3 year. Metabolic syndrome was seen in 332 persons (66.4%), 214 of males (61.4%), and 118 females (77.6%). There was a clear increase in the prevalence of the metabolic syndrome with increasing age up to the age of 79 year. In conclusion: This study reported high figure of metabolic syndrome. Adoption of Westeraern life in our society with overweight, physical inactivity, sedentary behavior, and unhealthy dietary habits may be the cause.

فرع الطب كلية الطب جامعة البصرهالمتلازمه الايضيه تزيد من الاصابه بامراض القلب التاجيه والسكته الدماغيه بثلاثة اضعاف مع زيادة واضحه بالوفيات الناتجه عن امراض القلب والاوعيه الدمويه.كان الهدف من هذا العمل هو لدراسة المتلازمه الايضيه في البالغين في البصره في مركز واحد.كانت هذه دراسه مقطعيه عرضيه جرت في المستشفى على مرضى بالغين فحصوا في العياده الخارجيه والردهات في مستشفى الفيحاء العام في البصره للفتره من كانون الثاني الى اب 2004 للمرضى الذين قبلو المشاركه. وجود 3 اضطرابات ايضيه كانت كافيه لتشخيص المتلازمه الايضيه و كانت الاضطرابات الايضيه هي السمنه الحشويه، ارتفاع الدهون الثلاثيه، وقلة البروتينات الشحميه عالية الكثافه، فرط ضغط الدم، وارتفاع السكر بعد الصيام. كان مجموع المرضى 500 منهم 152 امراه و348 رجل، العمر كان 20-88 سنه مع معدل عمر 49.9±11.3 سنه. وجدت المتلازمه الايضيه في 332 شخص (66.4%) منهم 214 رجل (61.4%) و118 امراه (77.6%). كان هنالك زياده واضحه في انتشار المتلازمه الايضيه مع ازدياد العمرالى حد عمر 79 سنه.الاستنتاج :هذه الدراسه سجلت رقم عالي للمتلازمه الايضيه.اعتماد نمط الحياة الغربيه في مجتمعنا مع زيادة الوزن وقلة الحركه وسلوك الجلوس مع اطباع الاكل غير الصحيه ربما تكون السبب.
