Pre-Emptive Analgesic Effect of Tramadol and Ibuprofen After Impacted Mandibular Third Molar Extraction

Abstract

Aims: to evaluate the efficacy of analgesia produced by preoperative administration of tramadol or ibuprofen on surgically extracted mandibular 3rd molars .Materials and Methods :surgical removal of lower 3rd molar was performed in 30 patients , they were divided into three groups of ten patient for each. Group I: preoperative 100mg tramadol IM injection; Group II: preoperative 800 mg Ibuprofen orally; Group III: control group with no preoperative treatment .Analgesia was assessed by visual analogue scale (VAS), Verbal pain Scale(VPS), patient satisfaction(PS), amount of analgesic consumption and time elapsed before first intake were recorded after 24 hours. Results: There was significant differences between tramadol and control group in all types of pain scales in favor of tramadol group . There was significant difference in the visual analog scale (VAS) ,number and time of tablets needed during 24 hours between Ibuprofen and control group . There was significant difference between Tramadol and Ibuprofen groups in both of (VAS) and (VPS) in favor of Tramadol group. Conclusions: The use of tramadol or ibuprofen preemptively is effective method of postoperative pain control in impacted lower 3rd molar surgery .

Abstract

Aims of the study: The study aims to evaluate the effect of naproxen and ibuprofen on post maxillo-mandibular fixation (MMF) limited mouth opening. Materials and Methods: The study was per-formed at AL-Jumhoori Teaching Hospital in Mosul City on 30 patients with mandibular fracture and or maxilla who were treated with MMF. The medicaments were allocated to 30 patients using a double blind randomization study who were divided into three groups .Group 1(10 patients) received Ibu-profen. Group 2(10 patients) received Naproxen and Group 3 (10 patients) received placebo . Immedi-ately after MMF release, the maximum mouth opening was measured as the inter-incisal distance be-tween the right upper central incisor and the right lower central incisor. After one hour ,the inter incisal distance was measured again and recorded .The data were analyzed using paired T-test and Duncan multiple analysis. Results: Both Naproxen and Ibuprofen have a statistically significant effect on the improvement of post-MMF limited mouth opening compared to placebo . At the same time there was no significant difference between the effect of Naproxen and that of Ibuprofen on the improvement of post MMF-limited mouth opening .The study also showed that there is inverse correlation between the MMF period and the maximum mouth opening immediately after MMF release. Conclusions: There is no significant difference between the effect of naproxen and that of ibuprofen on the improvement of limited mouth opening after MMF release. The available and cheaper drugs can be prescribed.

Relation of the Mandibular Canal to the Root Apices of Different Types of Impacted Lower 3rd Molar

Abstract

Aims: The study aims to determine the radiographic relationship of the inferior dental canal to the roots of impacted lower third molars. Materials and Methods: Sixty one patients were examined radio graphically by using conventional orthopantomograph (OPG) for detecting the relationship of the root apices of different types of impacted lower third molars to the inferior dental canal. Results: The sam-ple enrolled included 61 patients (30 males and 31 females) with a total of 100 impacted lower third molars. Age of patients included in the study ranged from 17 to 48 years within an average age of (32.5 %). Bilateral impaction was noticed in 39 patients (63.9 %), whereas unilateral impaction was observed in 22 patients (36.1 %). The greater number of impacted lower 3rd molar were of mesioangular angula-tions and were accounted in 40 impacted teeth (40 %) of the sample ,followed by vertical impaction 29 %, and horizontal impaction 23 %, while the inverted and distoangular angulations represented 4 % each. Seventy three roots (38 mesial and 35 distal roots) were categorized as "adjacent" relationship and 63 roots (32 mesial and 31 distal roots) categorized as "notching" and 53 roots (26 mesial and 27 distal roots) were categorized as "super imposition" with only one root ,mesial root categorized as "grooving". The remaining 10 roots (3 mesial and 7 distal roots) could not be placed under any of these categories. Therefore, they were placed in the category X or none. Conclusion: Within the limitations of this study, the mesioangular impaction is the most problematic type of impaction and "adjacent" relationship of mandibular third molar to inferior dental canal is most common type.

Abstract

Aim of the study: To compare the success rate of root end filling material using MTA( mineral trioxideaggregate) and zinc free amalgam in re-apicectomized teeth. Materials and methods: The studysample consisted of 30 patients having previously failed surgical endodontic treatment. The samplewas divided into 2 groups, the 1st group treated with MTA root end filling, while the 2nd group treatedwith amalgam root end filling. The patients were followed up after 6 months to assess the periapicalarea both clinically and radiographically. Results: There was no statistically significant difference betweenthe success rate of MTA treated group and that of amalgam treated group, although MTA has ahigher success rate than amalgam treated group .the success rate of MTA treated group was (86.6 %)while the failure rate of MTA treated group was (13.3%), the success rate of zinc free amalgam treatedgroup was ( 66.6%) and the rate of uncertain healing was (13.3%) while the rate of failure was (22%).Conclusions: MTA can be a good alternative to amalgam as a retro grade filling material, but amalgamis inexpensive, available ,and most of the dentist are familiar with the use of it.

Abstract

Aims: The present study designed to assess the effects of a bone substitute consisting of β-Tricalcium Phosphate (β-TCP) on acceleration of bone healing of surgically created defects. Materials and Methods: Ten rabbits were included in the study. Before the operation, each rabbit was weighed and given anesthesia accordingly. Surgery was performed under aseptic conditions. The left and right mandibles were exposed through a sub-mandibular incision. Two rectangular full thickness defects (10×5 mm) were created in each side. The anterior defect was left untreated to serve as a control while the posterior one was filled with β-TCP in each side. Five rabbits were sacrificed one week after surgery and the other five rabbits were sacrificed one month after surgery. The bony mandibles were dissected from the heads and immediately immersed in 10% formalin solution and sent for histological analyses. Results: Microscopic examination of the experimental section of one week group showed more organization, lesser degree of inflammatory cell infiltrate, and more granulation tissue formation than the control group. Similarly, microscopic sections from the experimental osseous defects taken at 1 month interval showed more pronounced callus formation than the control group. Conclusions: βtricalcium phosphate accelerate bony healing at both one week and one month interval which is especially important in dental implants and in oral and maxillofacial reconstruction, for both the functional and psychological aspect.

Abstract

The current study was performed to investigate the possible association between nasopharyngeal airway inadequacy due to enlarged adenoid and some dentoalveoular characteristics in subjects 3–17 years old in Mosul City. Twenty four patients (12 males and12 females) were randomly selected from patients who admitted to Ear Nose and Throat Department at Al–Zahrawi Teaching Hospital in Mosul City and who indicated for adenoidectomy to relieve severe nasal obstruction. Sixteen variables were recorded and analyzed using Statistical Package for Social Sciences system loaded on Pentium IV computer. The results proved that a significant correlation existed between nasopharyngeal airway inadequacy due to enlarged adenoid and some dentoalveoular characteristics and females were having high incidence of tonsillar enlargement than males.
