Abstract

ABSTRACT
Background: Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a benign chronic ulcerative lesion
of the oral mucosa of an unclear pathogenesis characterized by a mixed infiltrate of inflammatory cells including
large mononuclear cells, lymphocytes and eosinophils. This study was designed to find any possible correlation
between the number of different cells infiltrates and the clinical features.
Method: A histopathological objective analysis was performed on ten highpower fields and the number of different
types of cells was correlated with the clinical data.
Results: No relationship was found between clinical and the quantifiable histopathological features studied.
Conclusion: No correlation is existed between the type of cell infiltrate and the size, site or the duration of the lesion
was found.

Abstract

Background: Fine needle aspiration cytology (FNAC) has been found to be very useful for the diagnosis of lesions as multiple myeloma (MM) and plasmacytomas in different parts of the body. The usefulness of such procedure has not yet been verified in the oral and maxillofacial region. This study was conducted to verify the value of FNAC in the diagnosis of oral and maxillofacial lesions in patients with plasma cell dyscrasias.Patients and methods: After clinical and radiological examination, FNAC was done by the use of 10 cc syringes with 22-23 gauge needles on 11 patients with lesions affecting the maxillofacial region. The smears stained with Leishman’s stain, to be examined microscopically.Results: Twenty eight patients were examined throughout 1.5 years period, 11 had soft tissue/or bone lesions affecting the maxillofacial region. 9 patients with MM and 2 had plasmacytoma. Of 9 patients with MM, the mandible was involved in 5 patients, 3 lesions affected the maxilla, and while in one patient the maxilla and mandible were both affected. Soft tissue lesions were seen in 6 patients. The results of FNAC showed that all lesions were due to involvement with myeloma cell infiltrates, with one exception of 2 osteolytic lesions which were due to odontogenic infection.Conclusion: FNAC is a very useful and safe procedure to diagnose different types of lesions affecting the oral cavity in patients with plasma cell dyscriasis

Abstract

Background:Verrucous carcinoma is a distinct variant of oral squamous cell carcinoma characterized by slow growth and rare metastases. It may present diagnostic difficulties as it may be inaccurately diagnosed as squamous cell papilloma.Objective:The study performed a comparative immunohistochemical staining for both entities to obtain a possible method of differentiation.Methods:The study involved 13 samples of oral verrucous carcinomas and 10 samples of oral squamous cell papillomas which were stained immunohistochemically with antibodies to the lymphatic endothelial marker D240.Results:In all samples, the entire epithelium of verrucous carcinomas was positively stained with D240 whereas only the basal cell layer of squamous cell papillomas was positive.Conclusion:D240 could be used as a differentiating marker between oral verrucous carcinomas and squamous cell papillomas.Keywords:Verrucous carcinoma, Squamous cell papillomas, D240.

Abstract

Background: techniques of image analysis have been used extensively to minimize interobserver variation of immunohistochemical scoring, yet; image acquisition procedures are often demanding, expensive and laborious. This study aims to assess the validity of image analysis to predict human observer’s score with a simplified image acquisition technique.Materials and methods: formalin fixed- paraffin embedded tissue sections for ameloblastomas and basal cell carcinomas were immunohistochemically stained with monoclonal antibodies to MMP-2 and MMP-9. The extent of antibody positivity was quantified using Imagej® based application on low power photomicrographs obtained with a conventional camera. Results of the software were employed to predict human visual scoring results with stepwise multiple regression analysis.Results: the overall prediction of epithelial score depicted as r square value was 0.26 (p<0.001) which was obviously higher than that of stromal score (0.10; p<0.01). Epithelial and stromal MMP-2 score prediction was generally higher than that of MMP-9. Collectively, ameloblastomas had a more efficient score prediction compared to basal cell carcinomas.Conclusion: there is a considerable variability in the prediction capacity of the technique with respect to different antibodies, different tumors and cellular versus stromal score

Abstract

Background: Vascular tumors and malformations, comprising a broad category of lesions often referred to asvascular anomalies. Hemangioma, represents a variety of vascular lesions (both malformations and tumor), whilelobular capillary hemangioma is a common vascular lesion of the skin and mucous membranes that occurs mainly inchildren and young adults. Lymphangiomas are malformations of the lymphatic system. At the level of lightmicroscopy the small lymphatics vessels may be similar to capillaries and sometimes are only tentatively identified bythe nature of their contents or by immunohistochemical staining procedure. This study aimed to assess the vascularand lymphatic vessels density in benign vascular lesions using CD34 and D2-40 immunohistochemical markers.Materials and Methods: Twenty two formalin-fixed paraffin-embedded tissue blocks of Hemangioma/vascularmalformation, thirty of lobular capillary hemangioma and another twenty of lymphangioma.Results: Lymphatic vessel density expressed by D2-40 immunomarker was found in all cases with mean (24.01±14.74)in lymphangioma ,for lobular capillary hemangioma it was (12.67±6.66) and for hemangioma was (9.77±6.82)where as the mean of microvessel density count measured by CD34 immunomarker was (49.87±31.97) for lobularcapillary hemangioma , in hemangioma it was (37.42±23.40) and (25.90±12.23 ) for lymphangioma.Conclusions: All vascular lesions are a mixture of blood and lymphatic vessels with different proportions,hemangioma shows high percentage of blood vessels and lymphangioma shows high percentage of lymphaticvessels.

Abstract

ABSTRACT:BACKGROUND:Central giant cell granuloma (CGCG) and peripheral giant cell granuloma (PGCG) are tumor like lesions that affect jaw bones, while giant cell tumor (osteoclastoma) is a tumor that affects the long bones (e.g. tubular bones). Its affection of the jaw bones is a matter of debate. Both are very similar in their histopathological features while they vary in their clinical behavior. GCT shows a more aggressive behavior than GCG.OBJECTIVE:To evaluate the expression of (Proliferating cell nuclear antigen) and (P53) in peripheral and central giant cell lesion of the jaw and giant cell tumor of long bones with correlation to histopathological parameters.METHODS:A total of 17 (GCT), 15 (CGCG) and 16 (PGCG) cases where enrolled in this study. Immunohistochemical staining with PCNA and P53 monoclonal antibody was performed.RESULTS:A non-significant difference in proliferative activities was recorded among different histological giant cell lesion subtypes. Giant cell granuloma expressed the same proliferative potential to that of giant cell tumor, moreover PCNA expression was not statistically correlated to different histopathological paramters of lesion subtype.On the other hand. The anti-apoptotic potential of giant cell granuloma which expressed by anti P53 monoclonal antibody was the same of that of giant cell tumor.CONCLUSION:Results of this study proved that the biological behavior namely P53 and PCNA activities was comparable between giant cell lesions and giant cell tumor. This suggest that these two conditions may act as one disease entity with a spectrum of clinical behavior, possibly due to certain differences in anatomical location which by itself affect its biological behavior. This hypothesis needs further verification concerning the clonality of the lesion to be accepted or refused

Abstract

Background: An important step of tumor progression in which Matrix metalloproteinase have been implicated is angiogenesis, because these enzymes degrade the extracellular matrix and provide a permissive microenvironment for the growth of new blood vessels.The present study conducted to evaluate the immunohistochemical expression ofMatrixmetalloproteinase -2(MMP-2) andangiogenic marker (CD34) in Oralsquamous cell carcinoma(SCC)versus verrucous carcinoma(VC) and to correlate their expressions with the clinicopathological parameters.Material & Methods: MMP-2 and CD34 expression was examined immunohistochemically in twenty four paraffin tissue blocks of squamous cell carcinoma and verrucous carcinoma (twelve cases of each). Results: All cases of Oral SCC exhibited positive immunostaining for MMP-2, while only one case of VC showed –ve expression. Interestingly all cases of VC showed –ve MMP-2 immunostaining of the basal cell layer. Generally lymphatic vessels were more than blood vessels in both VC&SCC cases. The mean MMP-2 immunoexpression was (59.00%) for both stage I &stage II, while the higher CD34 immuno expression was in stage I. The mean expression of MMP-2 was higher in well differentiated OSCCs, while for CD34 it was higher in poorly differentiated OSCCs followed by moderately differentiated, then well differentiated, however no statistically significant difference was found. Non- significant correlation was found concerning the expression of both markers for both lesions.Conclusion: No statistical correlation was found between MMP-2 expression and angiogenesis in OSCC and OVC.

Abstract

BACKGROUND: Vascular tumors are a heterogeneous group of diseases with biological behavior ranging from a hamartomatous growth to frank malignant. The pathophysiology of lymphangioma, vascular malformation and hemangioma is interconnected, blood vessels known to be the site of origin of hamartomas, venous malformations and some neoplasms as benign, tumor-like growth of vessels (hemangiomas). Angiogenesis is the process of formation of new blood vessels from an existing structure.Aims of study Assessment of angiogenic potential in benign vascular lesions (hemangioma, lymphangioma and lobular capillary hemangioma) of head and neck region.Materials and Methods: Twenty-two formalin-fixed paraffin-embedded tissue blocks of Hemangioma/vascular malformation, thirty of lobular capillary hemangioma and another twenty of lymphangioma to be stained with Endothelial cell-Specific Molecule-1 (ESM-1) monoclonal antibody.Results: Microvessel density expressed by Endothelial cell-Specific Molecule-1 (ESM-1) immunomarker was found in all cases with mean density of (37.44±23.16) for lobular capillary hemangioma and (25.02±13.89) for hemangioma and (6.34±3.52) for lymphangioma. According to post hoc test ESM-1 marker expression showed a high significant difference between (hemangioma and lymphangioma=0.001), (lymphangioma, pyogenic granuloma=0.000), and it was significantly different between (hemangioma, pyogenic granuloma=0.011)Conclusions: The obvious capillary growth in lobular capillary hemangioma revealed that lobular capillary hemangioma showed the highest activity of angiogenic potential in comparison to hemangioma and lymphangioma

Abstract

A total of 49 patients were included in this study, 29 patients with oral benign and malignant lesions, and 20 healthy individuals were included as control.All these cases were examined by using cytobrush technique.The study aimed to evaluate the sensitivity and specificity of cytobrush method in the detection of precancerous and cancerous lesions of oral mucosa.The results of cytobrush method were compared with those of scalpel biopsy of suspicious oral lesions.The brush method has high sensitivity and specificity rate (88% sensitivity, 100% specificity). The cytobrush method was a highly accurate method of detecting oral precancerous andcancerous lesions (93% accuracy rate).Although the histological pattern was the definitive diagnosis in those patients, but when the lesion was doubtful, the cytological examination (cytobrush technique) was not only supportive but also diagnostic. It can aid in confirming the nature of apparently benign oral lesionsand, more significantly, revealing those that are precancerous and cancerous when they are not clinically suspected of being so.All cytobrush positive results should be referred for scalpel biopsy and histology to completely characterize the lesion.

شملت هذه الدراسة 49 شخصا، تسعة وعشرون منهم يعانون من تقرحات فموية حميدة أو خبيثة، أما العشرون الآخرون فهم أصحاء وتم اعتبارهم كمجموعة سيطرة. كل هذه الحالات تم فحصها باستعمال طريقة الفرشاة الخلوية. الهدف من الدراسة هو تقييم حساسية وخصوصية طريقة الفرشاة الخلوية في تحديد التقرحات السرطانية وما قبل السرطانية التي تصيب بطانة الفم، ونتائج هذه الطريقة قورنت مع الطريقة المشرطية لأخذ العينة من التقرحات الفموية المتوقعة.طريقة البابانيكولا استعملت لغرض صبغ المسحات التي أخذت بطريقة الفرشاة الخلوية، ويتم معاينتها تحت المجهر الضوئي.أظهرت نتائج البحث أن هناك معدل عالي لحساسية وخصوصية هذه الطريقة ( 88% للحساسية و 100% للخصوصية) ، لذلك فان طريقة الفرشاة الخلوية ذات دقة عالية في تعيين التقرحات الفموية السرطانية وما قبل السرطانية ( 93% معدل الدقة).نستنتج من ذلك انه بالرغم من ان الحالة النسجية تعطي التشخيص النهائي للحالة، إلا أن طريقة الفرشاة الخلوية ليست فقط داعمة للتشخيص بل هي طريقة تشخيصية أيضا.

Abstract

A case of a 41 years old male with a nodular lesion at the tip of the tongue is presented. Incisional biopsy showedmalignant growth with nonspecific features. Although the lesion showed no pigmentation; positivity for Melan-Awas detected leading to the diagnosis of Amelanotic melanoma. The patient was treated later with excision andneck dissection.
