Diode laser microsurgery; a recent treatment for laryngeal cancer: Mosul experience

Abstract

Objective: To evaluate the results of treating laryngeal squamous cell carcinoma using diode laser.
Design: Prospective study.
Setting: Department of ENT-Al Jamhuri Teaching Hospital.
Patients and Methods: Thirty one cases of squamous cell carcinoma of the larynx with age range between 32-66 years. The stage of tumour was Tis: 3, T1: 12, T2:14, & T3:2 cases . Lesions were excised or evaporated using diode laser with safety margin and followed for at least 36 months to evaluate cure rate, recurrences and its relation to site and stage of tumour, need for tracheostomy, complications and quality of voice.
Results: Diode laser was highly effective in treating early laryngeal cancer.Cure rates after 36 month follow up were 100%, 92% & 78% for Tis, T1 & T2 respectively. For more advanced lesions (T3) , the role of laser was limited only for debulking to avoid tracheostomy or biopsy taking in deep seated tumours as alternative to open biopsy. No major intraoperative or postoperative complications happened and no tracheostomy needed. Patients were discharged after twenty four hours. Voice quality was good after surgery in most cases.
Conclusion: Diode laser is safe in treating squamous cell carcinoma of the larynx and highly effective in early stages (Tis, T1, T2) and can avoid tracheostomy or open biopsy in more advanced lesions.

Malignant tumours of nasal cavity and paranasal sinuses in north of lraq a fifteen years retrospective study

Abstract

Objectives: To identify the different pathological types of malignant tumors of nasal cavity' and paranasal sinuses, their different clinical aspects, treatment received and prognosis in north of Iraq.Design : Retrospective study.Settings: Files of patients treated in the hospital of oncology and nuclear medicine in Mosulover 15 years period (1980-1994).Main outcome measures: Types, incidence, age and sex distribution, anatomical sites, clinical presentation, staging, method of treatment and prognosis.Results: They constituted 0.51% of all malignancies and 2.13 % of head and neck cancers.Sinus tumours affected both sexes equally as well as both sides. Nasal cavity tumours weremore common in male (male/female=1.4/1) and in the right side (right/left=1.33/1).Thecommonest age at presentation was the fifth decade.The most common presenting symptom was facial and nasal pain with average delay indiagnosis of about 8.5 months. Tumours of epithelial origin formed the majority ofcases (85.68%).All maxillary and ethmoidal cases were advanced at time of diagnosis (T3 or T4), while 60%of nasal cavity tumours were T1 and T2. 21.87% of patients had palpable cervical lymph nodes at presentation.The majority of patients received radiotherapy alone, few cases had surgery alone or both, but prognosis was generally unsatisfactoryconclusion: Malignant tumours of nose and sinuses are rare in north of lraq, are usuallyadvanced at presention , and carry poor prognosis.keywords: Malignant Tumours , Nasal Cavity , Paranasal Sinuses

Total Laryngectomy outcome of 36 cases after 5 years at Mosul Centre

Abstract

Objective: To evaluate results of treating laryngeal cancer by total laryngectomy.
Methods: A prospective study of 36 patients with squamous cell carcinoma of the larynx treated with total laryngectomy over five years period.
Results: The age range of the patients was between 34-72 years with a male to female ratio of 11/1. After five years follow up of patients , 100%, 84% & 14% were free of disease for T2, T3 & T4 respectively. Results of treating cases at T3 stage seems to be better when surgery preceded radiotherapy than when radiation given before.
Conclusion: Combination of surgery and radiotherapy gives relatively high cure rates if stage of tumour was still within the larynx ( T3 or less). The results of this study shows that this was better when surgery preceded radiotherapy.

الهدف: تهدف هذه الدراسة لتقييم نتائج علاج ورم الحنجرة الخبيث بعملية استئصال الحنجرة.التصمبم: دراسة لسلسلة من الحالات.الطريقة: ستة وثلاثون مريض بسرطان الحنجرة عولجوا بعملية استئصال الحنجرة خلال فترة خمسة سنوات. النتائج : تراوحت اعمارالمرضى بين 34-72 سنة وكانت نسبة الذكور الى الاناث 11/1. بمتابعة المرضى لمدة خمسة سنوات على الاقل وجد ان 100% و 84% و14% كانوا خالين من المرض للمرحلة T2و T3 T4 بالتسلسل . ووجد ان النتائج كانت افضل نسبيا اذا سبقت العملية العلاج بالاشعاع.الاستنتاج: استخدام الجراحة و العلاج بالاشعاع معا يعطي نسبة شفاء عالية نسبيا اذا عولج المرضى في مراحل غير متقدمة للمرض. وكانت النتائج افضل اذا سبقت الجراحة العلاج بالاشعة.
