Abstract

background: Fluoresceins labelled monoclonal antibodies against cell surfaceantigens are able directly to examine the expression of multiple antigens on thesurface of lymphoid cells in Ulcerative colitis lesions.Materials and methods: Using dual-immunofluorescence for investigated thecoexpression of (CD3-CD19 and CD4-CD8) antigens on infiltrated lymphocytes of 47patients with ulcerative colitis and 10 subjects from control group. Total anddifferential T cells as well as plasma cell counts were recorded in all tissue samples.Non parametric Kruskal-Wallis test was used to compare the median cell countsamong different study groups.Results: T lymphocyte, T-cell subtype and plasma cell in UC were high with highlystatistical significant difference when compared with those in normal control subjectsT cell UC (301.39) Controls (35.48), T-helper 2 UC (228.41) Controls (24.75), Thelper1 UC (54.21) Controls (13.45) and plasma cell UC (45.46) Controls (12.94).Recorded values of were different and increased according to modified Geboeshistological grades of disease activity T lymphocytes (r=0.944) followed by T helper2 subtype (r=0.821), T helper 1 (r=0.653) and B lymphocyte (r=0.237).Conclusions: The increased number of lymphocyte subsets was found in the UClesions and correlated with different histological grades of disease activity.

Abstract

Background: Helicobacter pylori persistence may develop atrophic gastritis, gastric ulcer or cancer.Objectives:To determine the association between the presence of autophagy related gene 16 like 1 and thrionin 300 alanine mutation and H. pylori infection among atrophic gastritis patients.Methods:Gastric biopsy was taken from eighty patients and tested for urease, and blood samples were taken for serum separation for detection of Anti-H. pylori IgG by ELISA and DNA extraction from whole blood were used for sequence specific primer – polymerase chain reaction for autophagy related gene 16 like 1 and thrionin 300 alanine mutation allelic discrimination.Results:Among 40 H. pylori positive cases, the carriers of mutated allele were 62.5% compared with 36.25% in H. pylori negative cases (p = < 0.001, OR = 1.72, CI = 1.23-2.42).Conclusions:Among Iraqi atrophic gastritis, there is an association between H. pylori infection and mutated autophagy related gene 16 like 1 and thrionin 300 alanine mutation allele. Thrionin 300 alanine may confer higher risk for infection with H. pylori.Key words: Autophagy, H. pylori, atrophic gastritis.

Abstract

Currently, the evaluation and treatment of chronic inflammatory diseases relies on specific clinical signs accompanied with single or multiple marker(s) for that disease. Despite that, these diseases share a common innate immune mechanism involved upregulation of “Inflammasome” that have been proved in its implication in the pathologic mechanism (s) and direct irreversible conversion of pro-interleukin 1β and pro-interleukin 18 into active IL-1b and active IL-18 contributing in disease progression and tissue damage, suggesting a common theranostic target to reduce inflammatory mechanism.Keywords: NLRP3, inflammasome, theranostic markerCitation:Ghazi HF. Abstract

Background: there are different procedures for the diagnosis of females suspected with toxoplasmosis. However, time, cost, and accuracy of the test should meet patient’s needs.Material and methods: one hundred and eleven female with suspected toxoplasmosis were under go three different procedures for the diagnosis of Toxoplasma gondii infection. Latex agglutination test, rapid chromatographic (immunoblot) and ELISA test were done for all patients. The results were described as frequency and percentage of positivity; also, specificity and sensitivity of immunoblot were assessed according to the result of other tests.Results: The Latex test has shown 80% and 61.54% sensitivity and specificity respectively with IgG measured by ELISA while it has shown 100% sensitivity and specificity with IgM measured by ELISA. IgM immunoblot give a relatively higher sensitivity and specificity (95% and 98.89%) respectively than IgG immunoblot (88% and 89.29%) made with IgM and IgG ELISA respectively. Conclusion: Rapid chromatographic test considered as a good test for detection of IgG and IgM anti Toxoplasma gondii antibodies in both acute and chronic Toxoplasmosis.

Abstract

Background: Human Adenovirus (ADV) is one of highly prevalent viruses worldwide, after primary infection, it remains latent and then might reactivate in immunocompromised patients. High ADV viremia seen in renal transplant recipients (RTR) with clinical presentations range from asymptomatic viremia to respiratory and gastrointestinal disease hemorrhagic cystitis, graft dysfunction and severe disseminated disease.Objective: The objectives of this study are to determine the rate of occurrence of ADV viremia by quantitative Real time PCR (QRT-PCR) in RTR and correlate them with urine cytology results, renal function tests and patients' hematological parameters.Methods:Seventy-one renal transplant recipients (RTR) were enrolled in this study. Whole blood samples (3 ml) divided into two parts, one part for complete blood picture and differential count and other part from which plasma separated and subjected to viral DNA extraction and then ADV Taqman QRT-PCR analysis for viral load measurement. Five ml urine specimens were collected for Pap-stained urine cytology.Results: Out of 71 RTR, 15 (21.12%) had positive ADV viremia by QRT-PCR, with a mean viral load 4.0 x107± 1.9 x108 copies/ml, and 80% (12 out of the 15) of positive viremia patients aged more than 40 years (p=0.011). All of RTRs 15/15 (100%) had symptomatic urinary tract infection (UTI) (p=0.039), and 5 out of 9 patients who had lymphopenia had positive viremia (p=0.007). Pap-stained urine cytology smears showed that 39/71 (55.71%) of the RTRs had positive decoy cells (DC), but there was no significant correlation between ADV viremia and the presence of DC (p=0.107).Conclusion: The present study showed the prevalence of ADV viremia in RTRs, with very high viral load, which is associated with lymphopenia and overt clinical features, this suggests that ADV might be an important cause of morbidity in RTRs.Keywords: Adenovirus, renal transplantation, real-time PCR, urine cytologyCitation: Ahmed HM, Al-Obaidi AB, Hussein MR, Kadhim HS, Ghazi HF. Abstract

AbstractBackground:Although progress was made in an in vitro fertilization (IVF) techniques,the majority of transferred embryos fail to implant. Morphology embryoscoring is the standard procedure for most of IVF centers for choosing the bestembryo, but remains limited since even the embryos classified as (top quality)may not implant. Determining molecular mechanisms of human embryo implantationis an extremely challenging task due to the limitation of materials andsignificant differences underlying this process among mammalian species. L-Selectinand its ligand carbohydrate have been proposed as a system that mediatesinitial adhesion of human blastocysts to the uterine epithelia.Objectives:Quantitative evaluation of soluble L-Selectin molecule in an in-vitroculture media used for in vitro fertilization. Determine cut-off value of solubleL-Selectin quantity to be as qualitative predictor of successful implantationPatients and Methods: This prospective study was undertaken in the High Instituteof Infertility diagnosis and assisted Reproductive Technologies/ Al-NahrainUniversity/ Baghdad/ Iraq, during the period from September2017 to April2018.A total of 74 infertile women were underwent controlled ovarian hyperstimulation for intracytoplasmic sperm injection cycle. Flexible antagonist protocolwas used as ovulation induction protocol in all the cases .culture mediaL-Selectin level were measured on the day of embryo transfer by using Enzymelinked immune sorbent assay (ELISA) for all cases . Comparison in culture mediaL-Selectin was done to all cases.Results: There was no significant difference between pregnant and non-pregnantgroups in age , BMI, and level of basal hormone FSH, LH , AMH and E2 in theday of hCG injection, and significant difference in the level of L-Selectin inpregnant compared to non-pregnant groups.Conclusion: Level of L-Selectin concentration in embryo culture media can beused as predictor of ICSI success

Abstract

Background:inflammasome protein govern the inflammatory process. The elevationof NLRP3 inflammasome might be related to bad seminal fluid analysisresults.Objectives:This study aimed to evaluate seminal plasma NLRP3 protein and itsrelation with seminal fluid analysis parameters before and after in vitro spermactivation.Patients and Methods: a total of 50 infertile patients were collected form higherinstitute of infertility diagnosis and assisted reproductive technique, there semensamples were processed for seminal fluid analysis before and after in vitrosperm activation. NLRP3 inflammasome protein were measured in seminal plasma.Results:the mean level of NLRP3 protein was 5.12 ng/ml. the higher level ofNLRP3 protein (4.5ng/ml) associated with lower sperm concentration, grade A,B, C and higher level of grade D motility. However, 50% of those patients havean improvement in their seminal fluid analysis after in vitro sperm activation.Conclusion:Elevation of NLRP3 protein in seminal plasma may be associatedwith the increased leukocytospermia, bad sperm quality and unresponsiveness toin vitro sperm activation among infertile male.

Abstract

Background: Peripheral blood lymphocytes (PBL)of Rheumatoid arthritis (RA)patients have a property of phenotypic and functional activation. Glutathione S- transferase pi (GSTπ) has been implicated in playing an important role in the initiation and progression of cellular activation.Objectives: To determine the percentage of cellular expression of GSTπ in the lymphocytes of RA patients in comparison with controls and to explore the relation between its cellular expression and disease activity pattern.Patients and Methods: This prospective study included46 RA patients and 17 healthy controls. Blood samples were taken and from all subjects PBL were isolated and then smeared on slides. The cellular reactivity for GSTπ was determined by immunocytochemistry technique.Results: This study found lower expression of GSTπ in the RA patients with a statistical significant difference with control group, while no statistical difference was found in RA with high and minimum disease activity groups. No correlation was observed between GSTπ with Disease Activity Score (DAS). Conclusion: Although the decrease in the expression of GSTπ in PBLs was pronounced in RA patients, however it doesn’t correlate with disease activity state.Key word: Peripheral blood lymphocytes, Rheumatoid arthritis, Glutathione S-transferase, immunocytochemistry.

الخلفية: لخلايا الدم اللمفية المحيطية لمرضى التهاب المفاصل الرثوي لها ميزة النشاط الوضيفي والشكلي. للانزيم المحول للكلوتاثيون دور مهم في حث واستمرار النشاط الخلوي.أهداف الدراسة:معرفة نسبة اظهارللانزيم المحول للكلوتاثيون في خلايا الدم اللمفية المحيطية لمرضى التهاب المفاصل الرثوي وعناصر السيطرة والعلاقة مع نشاط المرض السريري.المرضى والطرائق: ستة وأربعون مريض مصابونبإلتهاب مفاصل الروماتويدي قَورنوا ب17 شخصا سويا كمجموعة ضبط من الأعمار المماثلة. الخلايا اللمفية فصلت من عينات الدم المحيطي ثم تم تقييم نسبة الاظهار للانزيم المحول للكلوتاثيون بأستعمال طريقة التصبيغ المناعي الخلوي الكيميائي.النتائج: كان لمرضى إلتهاب المفاصل الرثوي نسبة اظهاراوطأ للانزيم المحول للكلوتاثيون بفرق احصاء عالي مقارنة بمجموعة الضبط. بينما لايوجد فرق معنوي بين المجموعة عالية النشاط والمجموعة الواطئة النشاط، ولم يلاحظ وجود إرتباطَ نسبة اظهار أعلى للانزيم المحول للكلوتاثيون بدالة نشاط المرض.الاستنتاجات: لوحظ انخفاض في نسبة اظهارللانزيم المحول للكلوتاثيون في خلايا الدم اللمفية المحيطيةلمرضى إلتهابالمفاصل الرثوي وكذلك ليس لها علاقة بنشاطالمرض السريري.مفاتيح الكلمات: التهاب المفاصل الرثوي، الأنزيم المحول للكلوتاثيون، التصبيغ المناعي الخلوي الكيميائي، خلايا الدم اللمفية المحيطية.

IMPACT OF RAPID ANTIGEN DETECTION TESTS IN THE DIAGNOSIS OF STREPTOCOCCAL TONSILLOPHARYNGITIS IN CHILDREN

Abstract

Background:Tonsillopharyngitis among children considered as a major public health problem in the community, because of post-streptococcal complications, cost, time consuming diagnostic tests and un-necessary antibiotics. Objective:To determine the validity of Rapid Antigen Detection Test (RADT) in the diagnosis of streptococcal tonsillopharyngitis in comparison with clinical and bacterial culture method.Methods:Throat swabs were taken from 214 children with tonsillopharyngitis in Baghdad from 20th November 2015 to 30th May 2016. Tonsillopharyngitis diagnosed by clinical, culture on blood agar and RADT.Results:S. pyogenes were recovered from 116 (54.2%) cases. S. pyogenes infection alone or with overgrowth of other bacteria is associated with recommendation for tonsillectomy, and indices for MCIsaac clinical prediction for Streptococcal pharyngitis. RADT detection of S. pyogenes revealed 83.62% sensitivity and 95.92% specificity. MCIsaac clinical prediction 89.66% sensitivity and 44.9% specificity. The combined MCIsaac-RADT algorithm had increased sensitivity with an increased point of score and specificity found to be inversely associated with points of score.Conclusion:For routine work, this study supports clinical screening with MCIsaac score for all children with tonsillopharyngitis and subsequent testing by Strep A RADT to facilitate the decision for antibiotic use.Keywords:Rapid antigen detection, streptococcus pyogenes, tonsillopharyngitisCitation:Hadi ZH, Ghazi HF, Abdulla MM. 5.SERO-PREVALENCE AND PLASMA VIRAL LOAD OF EPSTEIN BARR VIRUS AMONG IRAQI BLOOD DONORS

Abstract

Background:Epstein-Barr virus (EBV) is one of the most common latent viruses inside the humans' B-lymphocytes and it has been documented as a causative agent of many cancers. The virus may be transmitted when infected blood transfused to immunocompromised as well as immunocompetent individuals.Objective:To estimate the prevalence of EBV among apparently healthy blood donors by enzyme-linked immunosorbent assay (ELISA) and by quantitative real time polymerase chain reaction (RT-PCR).Methods:Four hundred fifty (450) blood donors were enrolled in this study. Plasma samples were screened by ELISA technique for detection of EBV viral capsid antigen (VCA-IgG). DNA extracted from 50 representative samples of these 450, and plasma EBV viral load was investigated by RT-PCR.Results:The overall sero-prevalence of EBV IgG was 79.8%, with a significantly higher prevalence among females than males. RT-PCR results were negative for all of the 50 representative samples.Conclusion:The high EBV sero-prevalence rates among Iraqi subjects raise the possibility of increasing the risk of EBV-associated malignant diseases.Keywords:Epstein-Barr virus, seroprevalence, VCA-IgG, real-time PCR, blood donorsCitation:Amjad Q.
