Abstract

Background: Asthma is an inflammatory airway disease; this inflammatory response can
be attributed to reduced lymphocyte apoptosis in peripheral blood and in airway tissues.
The mechanism behind this could be attributed to decreased Bcl2 protein and increase
Bax protein in peripheral blood lymphocytes of asthmatic patients.
Aim: to explore the mechanism behind decreased lymphocyte apoptosis in peripheral
blood of asthmatic patients at cellular level.
Method: Ninety four subjects; (44) control and (50) patients were included in this study
during the period from (2003) to (2004). The aspirated lymphocytes for each individual
were prepared and stained by immunocytochemistry to study the percentage of anti-
apoptotic Bcl-2 protein, pro-apoptotic Bax protein and their ratio.
Results: Our results showed that the percentage of Bcl2 protein in peripheral blood
lymphocyte of asthmatic patients were significantly higher than control (33.23±11.56,
21.47±4.16)(P=<0.00001).
The percentage of Bax protein in peripheral blood lymphocyte in asthmatic patients was
significantly lower than the control (21.43±6.88, 24.89±4.8) (P-O.0323). Bcl2/Bax ratio
was positive in asthmatic patients there was significant difference in comparison with the
control (1.68±0.57, 0.87 ± 0.17) (P=<0.00001).
Conclusion: This study clarifies the role of these 2 proteins in the process of reduction of
apoptosis in peripheral blood lymphocytes of asthmatic patients.

Abstract


Background: Many etiological factors have been implicated in the pathogenesis of Asthma like; viruses, bacteria and Chlamydia pneumoniae. This study focuses on the role of Chlamydia pneumoniae in asthma pathogenesis.Patients and Methods: The detection of Anti-Chlamydia pneumoniae antibodies IgA, IgM and IgG in patients by ELISA, for 35 patients, 18 asthmatic patients, 12 patients with chest infection with no history of asthma, and 5 apparently healthy as control subjects.
Results: The results showed that 80% and 70% of the patients were negative for anti- Chlamydia pneumonia antibodies IgA and IgM respectively, while significantly (p<0.05) high number (73%) of patients were positive for IgG, of which significant number (p<0.05) of them, 63% were asthmatic and 36% were non-asthmatic chest infection.
Conclusion: this study may support the association between Chlamydia pneumoniae and asthma.

Abstract

Background:Pseudomonas aeruginosa (P. aeruginosa) infections are clinical problem, it is a difficult to treat because of high resistant to many antibiotics (Multi-drug resistant) and a high risk of emergence of resistance during therapy. Carbapenems are therapeutic choice against infections caused by Gram-negative bacilli including strains of P. aeruginosa. New Delhi metallo-β-lactamase-1 (blaNDM-1) gene, an acquired class B carbapenemase. Dissemination predominantly involves transfer of the blaNDM-1 gene among promiscuous plasmids and clonal outbreaks. Bacteria with NDM-1 are typically resistant to nearly all antibiotics.Objective:To detect blaNDM-1 in the isolates of P. aeruginosa, which were recovered from various clinical samples from hospitalized patients in Wasit hospitals.Methods: This cross-sectional study involved 200 clinical samples were collected from three major hospitals in Wasit province. Samples were inoculated in Mackonkey and blood agar for primary isolation and then biochemical tests were used to confirm diagnosis of P. aeruginosa. The susceptibility test for 14 types of antibacterial drugs were tested by using disk diffusion method. Chromosomal and plasmid DNA were extracted by using special methods.Results: Out of 36 carbapenems resistant P. aeruginosa (CRPA) isolates, there were 18 isolates (50%) positive for blaNDM-1 gene.Conclusion: Rate of occurrence of blaNDM-1producers is highest among carbapenem-resistant P. aeruginosa isolated from clinical samples in Wasit hospitals. Abstract

Background: The sexually transmitted microorganism Chlamydia trachomatis causes a medical problem world wide, especially for pregnant women as this infection may lead to different complications in pregnancy.Patient and Methods: In this study, 57 pregnant women with suspected Chlamydial infection were included from consultant clinic Al-Kadhimiya teaching hospital in Baghdad, with 17 pregnant women as control group. ELISA tests were done to detect the level of anti-chlamydial antibodies, IgM, IgG and IgA.Results: In our study we found IgM seropositivity to C.trachomatis in 85.96% of patients and 14.03 % of controls and similar results were found regarding IgG seropositivity in patients and control groups. IgA seropositivity to C.trachomatis was positive in 59.64% of patients and 40.35 % of controls. In our study we also found higher rates for seropositivity regarding antichlamydial antibodies of IgG, IgM and IgA types in patients with a history of more than one abortion compared to those who have negative history of abortionConclusion: Antichlamydial antibodies of IgA, IgG and IgM types were found to be elevated in the study group and this may reflect the incidence of infection of Chlamydia trachomatis in our patients. Moreover it may reflect the relation of infection to repeated abortion as there was a high level of antichlamydial antibody in patients with history of more than one abortion.

المقدمة: الكائن المجهري كلاميديا التراخومية والمنقول بالاتصال الجنسي يسبب مشاكل طبية عديدة في معظم انحاء العالم, لاسيما عند النساء الحوامل حيث ان هذه الاصابة قد تسبب مضاعفات عديدة ومختلفة للحوامل.المرضى وطريقة العمل: شملت هذه الدراسة 57 أمرأة حامل يشتبه بإصابتها بالكلاميدية مع 17 أمرأة حامل خالية من الاصابة. تم الفحص والتشخيص في العيادة الاستشارية للنساء الحوامل في المستشفى الكاظمية التعليمي في بغداد, حيث اجري فحص الاليزا (ELISA) لمعرفة مستوى مستضدات الكلاميديا من الانواع (A, G, M) في مصل الدم.النتائج: بينت النتائج بأن هناك 85.96% ارتفاع في مستوى مستضد (IgM) في المرضى مقارنة مع 14% لمجموعة السيطرة وكذلك نتائج مشابهة لمستضد (IgG). بينما نتائج مستضد (IgA) كانت 59,64% مرتفعة لدى المرضى و40.35% لمجموعة السيطرة. كما وأظهرت النتائج بأن هناك ارتفاع في نسب المستضدات الثلاثة للمرضى الذين لديهم تاريخ إجهاض متكرر بالمقارنة مع المرضى الذين ليس لديهم إجهاض سابق.الاستنتاجات: إن مستضدات الكلاميديا التراخومية مرتفعة في مجموعة البحث وهذا ممكن ان يعكس ارتفاع مستوى الإصابة بهذه الجرثومة. كما ويمكن أن نستنتج بأن قد يكون هناك ارتباط غير مباشرة بين نسبة الإصابة وعدد تكرار الإجهاض للمرضى المصابين.

Abstract

Background: Oral Lichen Planus (OLP) is a chronic inflammatory mucosal disease,presenting various clinical forms. Both antigen- specific and non- specific mechanismsinvolved in the pathogenesis of OLP.Matrix Metallo Proteinases (MMPs) and theirinhibitors are one of the mechanisms that would be activated in this disease.Objective: to explore the expression of Matrix MetalloProteinases (2&9) and theirinhibitors and correlation between them in oral lichen planus.Method: Twenty five paraffin embedded blocks of OLP and six of sequamous cellcarcinoma were collected from the records of Oral Diagnosis Department. Six negativecontrol cases were freshly obtained from Maxillofacial Center in the SpecializedSurgeries Hospital in Baghdad. All samples were investigated for the expression ofMMPs & Tissue Inhibitors of Matrix MetalloProteinases (TIMP) byImmunohistochemistry (IHC).Results: Expression of MMP-2,-9, TIMP-1 and TIMP-2 were detected at highersignificant level in keratinocytes and lymphocytes of OLP cases as compared to controlhealthy group (p<0.0001). There was no significant difference between the expression ofMMP-2, TIMP-1 and TIMP-2 in lymphocytes and keratinocytes. However MMP-9expression showed significant difference with MMP-2& TIMP-1&-2 in lymphocytes.There was non significant correlation between the expression of MMP-2, MMP-9, TIMP-1 and TIMP-2; and the morphological types of OLP.Conclusion: High expressions of MMPs and TIMPs in OLP may indicate their role inpathogenesis of the disease as there is no expression of these enzymes in the controlhealthy; so they could be a diagnostic of aid in OLP.

Abstract

Background: Human Adenovirus (ADV) is one of highly prevalent viruses worldwide, after primary infection, it remains latent and then might reactivate in immunocompromised patients. High ADV viremia seen in renal transplant recipients (RTR) with clinical presentations range from asymptomatic viremia to respiratory and gastrointestinal disease hemorrhagic cystitis, graft dysfunction and severe disseminated disease.Objective: The objectives of this study are to determine the rate of occurrence of ADV viremia by quantitative Real time PCR (QRT-PCR) in RTR and correlate them with urine cytology results, renal function tests and patients' hematological parameters.Methods:Seventy-one renal transplant recipients (RTR) were enrolled in this study. Whole blood samples (3 ml) divided into two parts, one part for complete blood picture and differential count and other part from which plasma separated and subjected to viral DNA extraction and then ADV Taqman QRT-PCR analysis for viral load measurement. Five ml urine specimens were collected for Pap-stained urine cytology.Results: Out of 71 RTR, 15 (21.12%) had positive ADV viremia by QRT-PCR, with a mean viral load 4.0 x107± 1.9 x108 copies/ml, and 80% (12 out of the 15) of positive viremia patients aged more than 40 years (p=0.011). All of RTRs 15/15 (100%) had symptomatic urinary tract infection (UTI) (p=0.039), and 5 out of 9 patients who had lymphopenia had positive viremia (p=0.007). Pap-stained urine cytology smears showed that 39/71 (55.71%) of the RTRs had positive decoy cells (DC), but there was no significant correlation between ADV viremia and the presence of DC (p=0.107).Conclusion: The present study showed the prevalence of ADV viremia in RTRs, with very high viral load, which is associated with lymphopenia and overt clinical features, this suggests that ADV might be an important cause of morbidity in RTRs.Keywords: Adenovirus, renal transplantation, real-time PCR, urine cytologyCitation: Ahmed HM, Al-Obaidi AB, Hussein MR, Kadhim HS, Ghazi HF. Abstract

Background:Ovarian cancer is one of the most common causes of gynecologic neoplasm all over the world.Objective:The objective is to shed light on the role of p53 protein and patient`s age in the pathogenesis of ovarian lesions.Methods:Paraffin embedded blocks of 62 patients with ovarian lesions were studied. Thirty-five cases of surface epithelial ovarian tumors, (31 cases of invasive surface epithelial ovarian tumors, and 4 cases of borderline intermediate malignancy cases of neoplastic ovarian cystic lesions). In addition, eighteen cases of benign neoplastic ovarian cystic lesions and 9 cases of non- neoplastic functional one were enrolled in this study. All of cases included, were stained with p53 by immunohistochemistry.Results:Immunohistochemistry for p53 showed that malignant cases were positive for p53 while all benign cases were negative for p53 and the borderline cases were also negative for p53. The non-neoplastic cases were negative for p53. There is a significant statistical difference in P53 expression in malignant cases compared to other groups (P < 0.001). A significant difference in mean age of malignant and border line cases in comparison with benign and non-neoplastic cases; (P < 0.001).Conclusion:Protein p53 may play a role in the pathogenesis of malignant ovarian cancer but not in benign lesions. The age of the patient has a role as a risk factor in ovarian lesions.Keywords:Ovarian lesion, ovarian cancer, p53, immunohistochemistry.

5.SERO-PREVALENCE AND PLASMA VIRAL LOAD OF EPSTEIN BARR VIRUS AMONG IRAQI BLOOD DONORS

Abstract

Background:Epstein-Barr virus (EBV) is one of the most common latent viruses inside the humans' B-lymphocytes and it has been documented as a causative agent of many cancers. The virus may be transmitted when infected blood transfused to immunocompromised as well as immunocompetent individuals.Objective:To estimate the prevalence of EBV among apparently healthy blood donors by enzyme-linked immunosorbent assay (ELISA) and by quantitative real time polymerase chain reaction (RT-PCR).Methods:Four hundred fifty (450) blood donors were enrolled in this study. Plasma samples were screened by ELISA technique for detection of EBV viral capsid antigen (VCA-IgG). DNA extracted from 50 representative samples of these 450, and plasma EBV viral load was investigated by RT-PCR.Results:The overall sero-prevalence of EBV IgG was 79.8%, with a significantly higher prevalence among females than males. RT-PCR results were negative for all of the 50 representative samples.Conclusion:The high EBV sero-prevalence rates among Iraqi subjects raise the possibility of increasing the risk of EBV-associated malignant diseases.Keywords:Epstein-Barr virus, seroprevalence, VCA-IgG, real-time PCR, blood donorsCitation:Amjad Q.
