Abstract

ABSTRACT:BACKGROUND:Cancer of breast is by far one of the most important clinical problems that concerns the surgeon. It isthe commonest form of cancer in females, when detected early and given the proper treatment can becured. Color Doppler imaging is modern technique that can be used in preoperative assessment ofbreast lumps.OBJECTIVE :To asses the role of color doppler imaging in differentiating malignant from benign breast lumps incomparison with the histopathology in AL- Najaf teaching hospital.METHODS:Total number of patients was 80 female of different age groups were assessed during the period fromfirst of February 2004 to the first of February 2006 , presented with variable palpable breast masses.After clinical assessment, all our patients underwent , color doppler imaging and then subjected to theexcisional biopsy and histopathology for confirmation of the diagnosis.RESULTS:The results of color doppler imaging for all our patients were found as follow, benign lumps in 54cases (67.5%),malignant lumps in 26 cases (32.5%),two cases diagnosed as benign by color dopplerimaging proved to be malignant by histopathologygiving ( 2.5%),false negative rate. All obtained results of sensitivity, specificity and over allaccuracy of color doppler imaging respectively as follow (92.85%),(100%),and(97.5%).CONCLUSION:Color doppler imaging is highly sensitive and specific method in evaluating malignant breast massesand is painless, non-invasive and time saving procedure, so we can reduce the rate of unnecessarysurgery for histopathology.

Abstract

Abstract
Back ground:
Earlier, the studies had been shown an increased prevalence of previously diagnosed hypothyroidism in gallstone patient and a delayed emptying of the biliary tract in hypothyroidism, explained partly by the missing prorelaxing effect of thyroxine on the sphincter of oddi contractility . Other explanations include the known link between thyroid failure and disturbances of lipid metabolism that may consecutively lead to change of the composition of the bile and motility of biliary tract.
Aim:
To show the prevalence of previously undiagnosed hypothyroidism in patients with gallstones .
Patient and method:
A cross sectional study was done in Al-Sader Teaching Hospital in Al-Najaf city between 15th of February 2008 and 1st of November 2009 of 225 cases were taken to show relation between gallstone and hypothyroidism. For every patient with diagnosed gallstone, full history and clinical examination was taken and sent for ultrasound of neck for goitre detection and laboratory blood test for T3, T4 and TSH.
Results:
Out of 225 patients with gallstone 198 (88%) were females and 27 (12%) males. Thyroid disorder in form of hypothyroidism was found in 24 (10.6%), (from this percentage 22 (9.7%) were females and from this 18 (8.0%) were subclinical and 4 (1.7%) were clinical hypothyroidism and males were 2 (0.9%) with subclinical cases. From 225 cases with gallstones, 22(9.7%) cases complaining from goitre. Peak age between 51-60 years.
Conclusion and recommendation:
There is no significant association between hypothyroidism and gallstones in both genders. Gallstone patients between 51-60 years should be checked for serum TSH, T3 and T4 because of high incidence of hypothyroidism among this age group.

مقدمة------ أثبتت بعض الدراسات بوجود علاقة تصاعدية بخمول الغدة الدرقية لدى المرضى المصابين بحصاة المرارة وبعلاقة بين خمول الغدة الدرقية وبطء تفريغ المادة الصفراوية لدى هولاء المرضى والتي يمكن تفسيرها بغياب تاثير هرمون الثايروكسين (هرمون الغدة الدرقية) على ارخاء معصرة قناة الصفراء (معصرة اودي).التفسير الاخر لهذه العلاقة هي اضطراب ايض الدهون مما يؤدي الى تغيير بتركيبة المادة الصفراوية وترسب حصاة المرارة.الهدف------لقياس درجة شيوع وتشخيص خمول الغدة الدرقية لدى المرضى المصابين بحصى المرارةالطريقة والمرضى-----------------تم أخذ ٢٢٥ عينة عشوائية من مرضى مستشفى الصدر التعليمي في النجف من الخامس عشر من شباط ٢٠٠٨ الى الاول من تشرين الثاني ٢٠٠٨ لغرض اجراء هذه الدراسة. لكل مريض بحصى المرارة تم اجراء الفحص السريري وواخذ تاريخ المرض بدقة ثم اجري لهم سونار الرقبة وعينة دم لفحص وظائف الغدة الدرقية.النتائج-------من اصل ٢٢٥مريض ١٩٨كانو نساء و٢٧كانو رجال.وجد عدد المصابين بخمول الغدة الدرقية ٢٤ مريض (٦,١٠٪) , من هذه النسبة ٢٢(٩٫٧٪) كانو نساء منهن ١٨(٨٪) كن لايبدين اعراض المرض و٤(١٫٧٪)باعراض كاملة للمرض ومريضان من الرجال وكانو لايبدون اعراض المرض٢(٠٫٩٪). من اصل٢٢٥مريض ٢٢(٩٫٧٪) كانو يعانون من تضخم الغدة الدرقية. وكان اعلى مستوى اصابة بين عمر ٥١-٦٠ سنة.الاستنتاجات-----------لاتوجد هناك علاقة هامة بين مرضى حصاة المرارة وخمول الغدة الدرقية في كلا الجنسين.يجب قياس وظيفة الغدة الدرقية للمرضى المصابين بحصى المرارة بين الفئة العمرية ٥١-٦٠ سنة بسبب ارتفاع الاصابة بخمول الغدة الدرقية في هذه الفئة.

Abstract

ABSTRACT: BACKGROUND: The incidence of clinically apparent thyroid nodules in the general population is 4.5%.The overall incidence of malignancy in solitary thyroid nodule ranges between 10% to 15%. U/S and FNAC are used to differentiate benign and malignant nodules with high percentage of sensitivity and specifity. OBJECTIVES: 1. To evaluate the utility of FNAC in the preoperative diagnosis of solitary thyroid nodule. 2. To evaluate efficacy of FNAC and U/S in differentiation between benign and malignant nodule. METHODS: 130 euthyroid patients with solitary thyroid nodule attending the department of general surgery in the teaching hospital in Najaf province during the period from November 2001 to November 2003. After clinical assessment all our patient underwent FNAC for their thyroid nodule, and then subjected to the U/S examination. RESULTS: Out of these130 patients, the female to male ratio was 7:1, correlation of FNAC with HPE showed a sensitivity of 83.3% and specificity of 100%. CONCLUSION : Our study as compared to other studies else where showed that U/S and FNAC are very important diagnostic tools in investigation of clinically euthyroid solitary thyroid nodules. They will give optimal results and avoid unnecessary surgery in certain patients without missing any malignancy in others
