The role of sex hormones, waist circumference, BMI andLeptinemia in patients with acute renal failure.

Abstract


Background: Acute renal failure is characterized by impaired excretory endocrine
homeostatic and metabolic functions of the kidneys.
Objective: The aim of this study is to look for the role of sex hormones, waist
circumference and leptinemia in patients with acute renal failure.
Subjects and Methods: 55 subjects were involved in this study, 30 had acute renal
failure and the remaining 25 subjects were normal healthy individuals who served as
a control. The age range of the patients was between 25 and 45 years with a mean of
37±8.2 years.
Five mls of blood were withdrawn from each subject by vein puncture. Enzyme
linked immuno assay (ELISA) was used for the measurement of serum leptin level.
Radio immuno assay (RIA) technique was used for the measurements of testosterone
and estradiol levels.
Data were expressed as mean±SD. Results were evaluated using the student t-test for
paired data. Conventional methods were used for the correlation and regression
analyses.
Results: Patients with acute renal failure show a significant increase in leptin level,
BMI and waist circumference than healthy group. On the other hand, testosterone
level shows significant decrease while estradiol serum level shows a significant
increase when compared with the healthy group in both genders although more
significant in females.
Females with acute renal failure have significantly higher level of leptin than in males
with acute renal failure as well as the anthropometrical measurements (BMI & waist
circumference).
Conclusion: Increased level of serum leptin and estradiol in acute renal failure is a
reflection of increased waist circumference of the patients and the effected renal
parenchyma which make the ability of the kidney for biodegradation very poor. On
the other hand testesteron has a significant effect on serum leptin levels as can be
seen through its positive effect in males rather in females.

Abstract

Background: Chronic renal failure is a predominant reduction in glumerular filtration rate (GFR) sufficient to produce alterations in well-being and organ function. Endocrine abnormalities common in chronic renal insufficiency is the association of higher serum leptin levels in their patients. Leptin is a protein hormone produced mainly by adyposites. Serum leptin concentrations positively correlate with body mass index (BMI) and body fat content. Changes of androgens synthesis and metabolism develop early after the onset of renal insufficiency and are likely to be caused by primary hypogonadism and /or disturbances of the hypothalamic pituitary –axis.Aim: The aim of the present work is to analyze the contribution of serum testosterone and astradiol concentrations with leptinemia in hemodialyzed patents together with waist circumference and BMI. In order to shed some light on the mechanism of biodegradation of leptin by the kidneys in both genders and how it is affected by body composition and weight.Subjects and Methods: A total of 45 subjects were involved in this study, 30 of them had chronic renal failure and the remaining 15 subjects were normal healthy individuals served as a control. Five mls of blood were withdrawn from each subject by vein puncture, patients and control. ELISA technique was used for the measurement of serum leptin level while RIA technique was used for the measurements of testosterone and estradiol levels. Data were expressed as mean±SD. results were evaluated using the student t-test for paired data. Conventional methods were used for the correlation and regression analyses.Results:Results obtained showed that the level of serum leptin and serum estradiol in patients with chronic renal failure were significantly higher than that in healthy subjects, whereas testosterone level was significantly lower in patients than in healthy subjects.The anthropometrical measurement in patients with chronic renal failure had significantly higher values of waist but significant lower values of BMI. Male with chronic renal failure showed a significant lower level of serum leptin concentration than in females. However the anthropometrical measurements (BMI & waist) of males were significantly higher than those in the females. Serum leptin level was positively correlated with serum estradiol whereas it had an inverse correlation with testosterone. Leptin had positive correlation with each of the studied anthropometrical measurements (BMI & waist). All results are thoroughly discussed in the text. Conclusion :Increased levels of serum leptin and estradiol in CRF patients is a reflection of the increased waist circumference and the poor ability of the kidney for biodegradation of both these hormones. On the other hand testosterone had a significant effect on serum leptin levels as can be shown through its positive effect in males rather than in females.

Abstract

ABSTRACT:BACKGROUND:Dyslipidemia is a common health problem.OBJECTIVE:The present study was to estimate the prevalence of dyslipidemia as defined by the NCEP ATP3 criteria among people living in Karbala city.SUBJECTS AND METHODS:The investigations were performed on a sample of 130 subjects selected arbitrarily from those attending the public clinic in AL- Hussainy teaching hospital in Karbala for consultation and from some subjects accompanying the patients. Fasting blood samples for routine lipid analyses from each subject were obtained after informing them about the project and having their approval.RESULTS:In this sample of subjects it was found that 72.3% of them had a plasma TC level lower than 200mg/dl. This study also determined a prevalence of hypertriglyceridemias in 23.3% of study subjects who have a borderline TG level. The prevalence of high and very high TG among them were 6.9% and 4.4% respectively. The frequency of LDL-C levels above 160 mg/dl was found to be about 1% of the sample. Borderline levels of LDL-C was found in 7.3%of the sample. In the meantime the prevalence of low HDL-C levels in this study was found to be 39.2% of the subjects who had a serum level of HDL-C less than 40mg/dl.CONCLUSION:In conclusion this study demonstrated a very high prevalence of dyslipidemia among Iraqi adult subjects living in Karbala city. Abstract

ABSTRACT:BACKGROUND: Breast cancer is the most common malignancy in women, and a major cause of mortality and morbidity despite the advances in diagnosis and treatment. There is evidence that changes in HER2 protein expressions are associated with breast cancer progression.OBJECTIVE: To investigate whether measuring this tumour marker in serum of breast cancer patients before and after treatment might also be useful markers in the diagnosis, screening and monitoring the malignant tumour progression and response to therapy. METHODS: Serum samples were obtained from (28) apparently healthy women (Control Group) with a mean age of 40.9 ± 7.6 years and (60) female patients complaining from primary breast cancer (Patients Group) with a mean age of 48.3 ± 8.9 years. They were divided according to their clinical end point into: Pre-Surgical Group, Post-Surgical Group and post- chemotherapy Group. Serum Her-2/nue level was measured using ELISA kits.RESULTS: Level of Her2/neu (3130.4 pg/ml) was significantly higher in after 6 cycles of chemotherapy group than each of control (1400.8 pg/ml), before surgery (1597 pg/ml) and after surgery (1487.4 pg/ml) (P < 0.05). Her2/neu is effective test only after 6 cycle chemotherapy with an accuracy of 95.2%. The best performance for Her2neu was observed at values ≥ 1464 pg/ml (sensitivity = 95% and specificity = 61%). There were significant influences of the studied personal and the pathological characteristics of the tumour upon the biomarker levels where the levels were significantly higher with the increase of tumour pathological stage and in the presence of positive status for Her2neu receptors (P < 0.05).CONCLUSION:In this study there was a statistically significant association between tissue HER-2/neu and serum HER-2 /neu levels in the extracellular domain. It could be concluded that using serum Her-2/neu in patients after six cycles chemotherapy could predict response to therapy..
