Treatment of Freckles Among Patients with Dark skin Complexions. A Comparison of Intense Pulsed Light, Q-Switched 532 nm Laser and Phenol 80% Spot Peel

Abstract

ABSTRACT:BACKGROUND: Intense pulsed light (IPL), Q-switched 532 nm laser and phenol peels are useful for pigmentary lesions but with limited comparative studies in dark skin peoples and a presumed pigmentary changes. OBJECTIVE:To compare the efficacy and safety of IPL, Q-switched 532 nm laser and phenol 80% peel in the treatment of frecklesMETHODS:Twenty six patients with freckles were enrolled in the study. They had Fitzpatrick skin type III and IV. From each patient, the freckles on the right and left cheeks were treated with a single session of IPL or phenol 80% spot peel respectively. Freckles on the midface were treated with Q-switched 532 nm laser. Patients were seen after 1, 2, 6 and 16 weeks RESULTS:Nineteen (73.07%) patients were females and 7 (26.92%) were males, their ages ranged from 19-36 (27.53±5.88) years. Immediate darkening was seen at the IPL treated lesions. Frosting was the result at the laser and phenol ones. Dry scab developed on all lesions and falls after 5-7 days. At 16 weeks of follow up, score 1 was maintained in 18 (69.23%) versus 6 (23.07%) patients at the IPL and phenol sides respectively, while at the laser treated lesions 19 (73.07%) patients were having score 2. No one had score 1. After 16, good satisfaction was obtained in 61.53%, 38.46% and 26.92% on the IPL, phenol and laser sites respectively.CONCLUSION:ll 3 modalities are beneficial, but intense pulse light was more effective and safe..

Abstract

ABSTRACT:BACKGROUND:The meaningful association of androgenetic alopecia and coronary heart disease had been well documented, but few studies had been focused on the importance of lipid parameters in patients with androgenetic alopecia.OBJECTIVE:To investigate the lipid profile and its relation to female pattern alopecia.PATIENTS & METHODS:This is a case controlled study conducted at the Department of Dermatology & Venereology- Baghdad Teaching Hospital, between January 2001 and April 2002. Sixty female patients with androgenetic alopecia were enrolled in this work. From each patient a detailed history and full clinical examination were performed regarding all demographic points relative to the disease, grading of alopecia and measurement of serum lipoproteins was done. Female pattern alopecia was classified according to Sharquei’s classificationSixty age and weight matched females with normal hair status were considered as a control group. Measurement of serum lipoproteins also performed for them.RESULT:Sixty patients, their ages ranged between 20-60 years with mean + SD of 30.3 + 9.4 years. Twenty (33.3%) patients were having grade I, 20 (33.3%) patients grade II and another 20 (33.3%) patients grade III.. The mean levels of total serum cholesterol, triglycerides, low density lipoprotein and very low density lipoprotein in all patients and those with grade II and III separately were significantly higher when compared to the control group. On the other hand, the mean levels of high density lipoprotein in all patients and those with grade I and III separately were lower in comparison to the control.CONCLUSION:The atherogenic index or risk ratio was found to be significantly high in patients with female patteren alopecia and this goes parallel with the severity of baldness.

Abstract

Background: Fibromyalgia syndrome and psoriasis share various soft problems. In addition, patients with psoriasis can experience subjective joint swelling and pain (Fibromyalgia syndrome) and objective swelling (Psoriatic arthritis) with the latter requiring systemic disease modifying antirheumatic drugs while the former requires anti-fibromyalgia measures. Objective: To assess the rate of fibromyalgia syndrome in a sample of Iraqi patients with psoriasis.Patients and Method: A sample of one hundred and twenty Iraqi patients with psoriasis and another one hundred and twenty healthy individuals matched for age and sex, serving as control group were studied. Full history was taken and complete clinical examination was done and various laboratory investigations were carried out for all individuals in both groups. The American College of Rheumatology 1990 criteria for Fibromyalgia syndrome were applied for individuals of both groups. Results: Fibromyalgia was present in 32 (26.7%) psoriatic patients compared to 13 (10.8%) individuals of the control group (P=0.0016). We found that psoriatic arthritis as well as treatment with systemic therapy (chemotherapy) are significant predictors for development of Fibromyalgia in a rate of 17 (14.2%) (p. value =0.001), 20 (16.7%) (p. value =0.0001) respectively. Conclusion: There is a significant association between Fibromyalgia Syndrome and psoriasis.Keywords: Fibromyalgia, Psoriasis, Psoriatic arthritis.

الخلفية: تشترك متلازمة الألم العضلي الليفي مع داء الصدفية بمختلف مشاكل الأنسجة إلرخوة (العضلات، الاوتار واللفافة) كما ان مرضى الصدفية قد يعانون من الم و تورم المفاصل غير الموضوعي (متلازمة الألم العضلي) او الموضوعي (التهاب المفصل في الصدفية)، حيث يتطلب الأخير العقاقير المضادة للروماتيزم بينما يتم علاج الأول ببرامج علاجية مختصة بمتلازمة الألم العضلي، وهذا ما جلب انتباه الباحثين والأطباء السريريين لبحث العلاقة بين داء الصدفية و متلازمة الألم العضلي.الهدف:هو تقييم معدل انتشار متلازمة الألم العضلي الليفي لدى المرض العراقيين المصابين بداء الصدفيةطريقة البحث: تمت دراسة مئة وعشرين مريضاً يعانون من داء الصدفية ومئة وعشرين شخصاً سليماً تم دراستهم كمجموعة ضبط. وتمت مطابقة المجموعتين من حيث العمر والجنس. تم استخدام طريقة التصنيف حسب توصيات لجنة المعايير المتعددة المراكز بالكلية الأمريكية لمبحث الرثائيات لعام 1990 لمتلازمة الألم العضلي الليفي على كافة الأشخاص المفحوصين. تم أجراء الفحوصات اللازمة للتشخيص. تم اخذ التاريخ المرضي لكافة المرضى المفحوصين وتم فحص الجميع لثماني عشرة نقطة أيلام (120 مريضاً مصاباً بالصدفية و120 شخصا سليماً اتخذوا كمجموعة ضبط. النتائج: وجد إن هنالك 32 مريضاً (26.7%) مصاباً بداء الصدفية يشكون من الألم العضلي الليفي مقارنة مع 13 شخصاً (10.8%) من مجموعة الضبط (0.0016P=). وجدنا إن الصدفية الرثائية (التهاب المفصل في الصدفية) مع استخدام العلاج الكيميائي تعتبران من العوامل المهمة في تطور متلازمة الألم العضلي الليفي.الاستنتاجات: هنالك ترابط معتد ذا مغزى بين متلازمة الألم العضلي الليفي والصدفية. مفتاح الكلمات: وهن العضلات الليفي، داء الصدفية، التهاب المفاصل الصدفي
