Abstract

ABSTRACT
Background: Host responses to periodontal disease include the production of different enzymes that are released by
stromal, epithelial or inflammatory cells into saliva & gingival crevicular fluid (GCF).
Aim of the study: to assess the difference in the activity of CK &BUN enzymes in the saliva of male patients with
chronic periodontitis before & after non surgical periodontal treatment & to evaluate the correlation between these
enzymes& probing pocket depth &relative attachment level.
Materials & methods: Measurements of plaque index (PlI),gingival index(GI) , bleeding on probing (BOP) , probing
pocket depth (PPD) &relative attachment level (RAL) were taken from male patients suffering from chronic
periodontitis before perio dontal treatment & one month after treatment .
Saliva was collected for biochemical analysis of the enzymes creatine kinase & blood urea nitrogen.
Results: The results have shown that after conventional periodontal therapy, the activity of these enzymes was
significantly decreased .There was a correlation between the activities of these enzymes & probing depth .Also there
was a strong correlation between CK enzyme & relative attachment level.
Conclusion: Based on these results, it can be assumed that the activity of these enzymes in saliva, as biochemical
markers for periodontal tissue damage, may be useful in the diagnosis, prognosis & evaluation of periodontal therapy
effects in periodontal disease.

Abstract

Background: Periodontal pathogens can induce free radicals over-formation and thus may cause collagen andperiodontal destruction. Anti-oxidants are used as supplements to counteract the over production of free radicals inperiodontal disease, that can reduce of collagen destruction. Coenzyme Q10 serves as an endogenous antioxidant,regenerates other antioxidants, stimulates cell growth, and inhibits cell death. Because it is an antioxidant, coenzymeQ10 has received much research attention associated with periodontal diseases. Perio Q gel may possibly beeffective as a topical agent and as an adjunct to scaling& root planing in treatment of gingivitis and chronicperiodontitis.Aim of study:Determine the periodontal health status in a follow up study for 6 weeks of a group of patients withchronic periodontitis by measuring clinical periodontal parameters, which included (Plaque index, Gingival index,Bleeding on Probing, Probing Pocket Depth and Relative Attachment Level) and to evaluate the effect of intrapocket application of perio Q gel (coenzyme Q10) alone & as adjunct to scaling,& root planing on the periodontalclinical parameters in the management of patients with chronic periodontitis.Materials and methods: A total of 323 sites with pocket depth (5-8) mm in patients with chronic periodontitis wererandomly divided in three groups. The Gel group, 111 sites were treated with intra-pocket application of Perio Q gelalone. In the Combination group, 106 sites were treated with scaling and root planning (SRP) plus intra- pocketapplication of Perio Q gel, in Scaling and root planing group, 106 sites were treated with scaling and root planingalone. Clinical periodontal parameters such as Plaque index (PI), Gingival index (GI), Bleeding on probing (BOP),Probing pocket depth (PPD), Relative attachment level (RAL) were assessed at first visit, 3weeks and 6weeks.Results: on intra-group analysis, all groups showed highly significant reduction in PI, GI, BOP, PPD and RAL among thethree visits .On inter-group analysis, the results showed significant reduction in the clinical parameters PPD and RAL ofcombination group in comparison to SRP group.Conclusion: The results of the research were encouraging and suggested the possibility to use the gel as a sole agentto support standard treatment procedures in periodontitis. The clinical parameters significantly improved in the phaseof periodontal treatment, indicating that CoQ10 opens new treatment options by improving the host response todisease activity.

Abstract

Background: Chronic periodontitis (CP) is greatly prevalent condition of inflammatory behavior. Salivary biomarker total antioxidants capacity (T-AOC) status, may be related to both periodontal condition and oral hygiene.Aims of the study: To assess the level of salivary T-AOC of patients with chronic periodontitis in comparison to healthy control and to correlate between the level of this marker with the clinical periodontal parameters (plaque index (PLI), gingival index (GI), bleeding on probing (BOP), probing pocket depth (PPD), and clinical attachment level (CAL)).Materials and Methods: Ninety subjects of males and females with an age ranged between (35-55) years were participated in this study. Participants were divided into two groups: the first group was CP group that consisted of fifty-five subjects and the second group consisted of thirty-five subjects as control group with healthy periodontium and both groups systemically healthy.The whole unstimulated salivary samples were collected, and then periodontal evaluation that including the assessment of clinical periodontal parameter (PLI, GI, BOP, PPD, and CAL) were done for all participants. Enzyme-linked immune– sorbent assay (ELISA) used to determine the level of T-AOC in saliva.Results: The two studied groups showed a highly significant difference regarding the salivary level of T-AOC, and it revealed that the mean value of salivary level of T-AOC was statistically lower in CP group than the control group. Regarding Pearson Correlation Coefficient, this study revealed that there is strong negative correlations between clinical periodontal parameters (GI, BOP, PPD, and CAL) with salivary level of T-AOC.Conclusion: Salivary T-AOC could be used as a reliable marker of chronic periodontitis activity

Abstract

Background: Dental calculus is mineralized dental plaque formed on teeth and dental prosthesis surfaces in the oral cavity. Urinary stone is a crystal aggregation formed in urinary system due to minerals saturation present in urine. The structure of dental calculus is similar to that of urinary stone.Objective: To assess oral hygiene and gingival status in patients with urinary stone. And compared with healthy subjects.Patients and Methods: Sixty participants, 25-40 years, were involved in this study who were divided into study and control group. The study group involved patients with urinary stone while the control group involved healthy subjects. Clinical parameters including plaque, calculus and gingival indices were recorded for all participants. The correlation between the recorded clinical parameters was estimated.Results: The study group mean (±SD) plaque (1.435±0.499), gingival (0.995±0.288) and calculus (1.28 ±0.66) indices were found to be significantly high compared with control group(0.868±0.265), (0.602±0.265) (0.501±0.457 ) respectively. The formed calculus was positively associated with accumulated plaque (r: 0.608, p < 0.05) and gingival inflammation(r: 0.612, p < 0.05).Conclusion: Urinary stone patients were associated with more plaque and calculus accumulation, and had worse gingival inflammation compared to healthy participants. Thus, they need to be more aware of their gingival health status and improved preventive care of oral health.

Abstract

Background: Pregnancy is a stressful state of increased inflammatory activity, and pregnancy – associated hormonechanges can influence periodontal tissues, these inflammatory activity lead to production of inflammatorymediators. Interleukin 1 beta (IL-1β) is a potent pro-inflammatory cytokines that is consistently associated withperiodontal diseases. This study was designed to determine the periodontal health status and detect the serum levelof IL-1β in the healthy pregnant women at first, second and third trimester and compare it with healthy non pregnantwomen, and determine its correlation with different clinical periodontal parameters.Materials and Methods: Subjects included in the study were sixty six (66) healthy pregnant women with an age rangeof 20-35 years old. They were divided into three subgroups according to gestational age, as twenty two (22) womenin each trimester. Also the sample included fifteen (15) married, non pregnant women and didn’t takecontraceptive pills, as control group with same age rang of 20- 35 years. Clinical periodontal parameters weremeasured in this study (plaque index, gingival index, bleeding on probing, probing pocket depth and clinicalattachment level). Blood samples were collected from all women under study (pregnant & non pregnant women) toasses concentrations of IL-1β by mean of enzyme – linked immune sorbent assay (ELISA).Results: Highly significant statistical differences were observed among the study groups regarding the gingival index(GI)with p-value( 0.007) and the percentages of bleeding on probing (BOP) p- value(0.00), a significant differenceregarding the probing pocket depth ( PPD) with p-value (0.046) ,and non significant statistical differences regardingthe plaque index (PLI), p-value(0.6) and clinical attachment level (CAL) with p-value (0.371). Interleukin 1-beta ( IL-1β) serum level showed a highly statistical significant difference among the study groups with p-value (0.00).Apregnant women showed higher level than non pregnant with a higher value in the second trimester.Conclusions: The present result revealed that the serum level of IL-1β was higher in pregnant women than nonpregnant women with a highly significant difference. The IL-1β serum concentration reaches the maximum value inthe second trimester of pregnancy. Nevertheless there were weak correlations between clinical periodontalparameters and serum level of IL-1β

الخلفیة. الحمل ھو حالة مرھقة من زیادة النشاط الالتھابي , كما ان التغیرات الھرمونیة المصاحبة للحمل تؤثر في انسجة ماحول الاسنان . یؤدي ھذا النشاط الالتھابي الى انتاج وسطاء التھابات مثلالسایتوكینات .المدور الخلوي 1 بیتا ھو من السایتوكینات الموالیة للالتھابات القویة الذي یرتبط باستمرار مع امراضانسجة ماحول الاسنان .تم تصمیم ھذه الدراسة لتحدید الحالة الصحیة لانسجة ماحول الاسنانوكشف المستوى المصلي للسایتوكین 1 بیتا في النساء الحوامل في فترات مختلفة من الحمل ومقارنتھا مع النساء غیر الحوامل , وتحدید علاقتھ مع معلمات ماحول الاسنان السریریة المختلفة .35 ) سنة تم تقسیمھن الى ثلاث مجموعات فرعیة وفقا لعمر او مرحلة الحمل الى 22 امراءة حامل في كل - المواد وطرائق العمل. شمل البحث ستة وستین( 66 ) من النساء الحوامل تراوحت اعمارھن بین ( 2035 ) سنة . كانت معلمات - فصل من فصول الحمل الثلاثة . وشملت العینة خمسة عشر( 15 ) من النساء المتزوجات غیر الحوامل ولم یاخذن حبوب منع الحمل , كمجموعة مقارنة بنفس متوسط الاعمار من ( 20ماحول الاسنان المستخدمة في ھذه الدراسة ھي مؤشر الصفیحة الجرثومیة و مؤشر التھابات اللثة والنزف عند التسمیر وعمق جیوب اللثة وفقدان الانسجة الرابطة. تم جمع عینات الدم من جمیع النساء قیدالدراسة ( الحوامل وغیر الحوامل) لتقییم التركیزات المصلیة للسایتوكین 1بیتا باستخدام تقنیة مقایسة الانزیم المرتبط الممتز المناعیةالنتائج .لوحظ وجود فروق ذات دلالة احصائیة عالیة بین مجموعات الدراسة یشأن مؤشر التھاب اللثة ومؤشر النزف عند التسمیر ,ھناك فرق كبیر فیما یتعلق بعمق جیوب اللثة . لاتوجد فروق فیما یتعلق بمؤشرالصفیحة الجرثومیة وفقدان الانسجة الرابطة. اظھر المستوى المصلي للمدور الخلوي 1بیتا فروقات ذات دلالة احصائیة عالیة بین مجموعات الدراسة مع . واظھرت.اعلى قیمة للمدور الخلوي 1بیتا في الفصل.p= الثاني من الحمل . 0.00الاستنتاج كشفت النتیجة الحالیة أن المستوى المصلي من المدور الخلوي 1بیتا كان أعلى في النساء الحوامل من النساء غیر الحوامل مع فروق ذات دلالة إحصائیة عالیة.التركیز المصلي للمدور الخلوي 1بیتایصل القیمة القصوى في الثلث الثاني من الحمل . ومع ذلك كان ھناك وجود علاقة ضعیفة بین المعلمات السریریة ومستوى المصل من المدور الخلوي 1بیتا.

Abstract

Background: Visfatin is a novel adipokine that mainly secreted by visceral adipose tissue, had an important role ininflammation and immune system. Creatine Kinase (CK) which is an enzyme that is involved in energy metabolism,found in large amounts in myocardium, brain and skeletal tissues.This study is carried out To evaluate the periodontal health status of the study groups (chronic periodontitis andchronic periodontitis with coronary atherosclerosis) and control groups, to measure the salivary levels of visfatin andCreatine Kinase in these groups and compare between them, and to determine the correlations between salivaryvisfatin and Creatine Kinase levels with the periodontal parameters in the three groups.Materials and Methods: eighty participants, males and females were recruited in this study with age ranged from (30-60) years, they were divided into three groups: the first study group was the Chronic periodontitis group (n=30), thesecond study group was chronic periodontitis and coronary atherosclerosis (n=30) and the control group(n=20)which was healthy systemically with healthy periodontium. Periodontal health status was determined by measuringplaque index(PLI),gingival index (GI), probing pocket depth(PPD), bleeding on probing (BOP) and clinicalattachment level (CAL),salivary samples were taken from each participants, salivary visfatin levels were determinedby enzyme-linked immune-sorbent assay(ELISA) while the activity of salivary Creatine Kinase was determinedspectrometrically by using the International Federation of the Clinical Chemistry (IFCC) method on Hitachi 911Automatic analyzer.Results: The results of the study showed that the mean values of PLI, GI, visfatin, Creatine Kinase and the percentagesof sites according to PPD scores, CAL scores, BOP were higher in the second study group with chronic periodontitisand coronary atherosclerosis than in the other groups with highly significant differences between the groups at(P≤0.01). Also by using Pearson Correlation Coefficient, salivary visfatin levels were correlated positively with allclinical periodontal parameters with a strong and positive correlation between salivary visfatin levels and CAL scoresand PPD scores. Salivary Creatine Kinase levels were correlated positively with all clinical periodontal parameters witha strong and positive correlation between its levels and mean values of GI and percentages of BOP.Conclusion: The present study showed that salivary visfatin can be used as a marker for the development ofcoronary atherosclerosis and its levels are associated with the degree of periodontal destruction and showed thatCreatine Kinase may be used as a marker for coronary atherosclerosis and chronic periodontitis.

Abstract

Background: Anti-oxidants are used as supplements to counteract the over production of free radicals in periodontaldisease.Co-Q10 functions as an intercellular antioxidant by acting as a primary scavenger of free radicals (FRs) andreactive oxygen species (ROS), this study aimed to evaluate the effect of intra pocket application of perio Q gel(coenzyme Q10) alone and as adjunct to scaling and root planing on the periodontal clinical parameters in thetreatment of patients with chronic periodontitis and compare the better improvement on the clinical periodontalparameters among different treatment modalities at 3 and 6 weeks.Materials and methods: A total of 323 sites with pocket depth (5-8) mm in patients with chronic periodontitis wererandomly divided into three groups. Abstract

Periodontal diseases are inflammatory diseases that affect theperiodontium which lead to alveolar bone and attachment loss.Breast cancer is the most common cancer in women worldwide.Expression of (VEGF) is elevated in both diseases and it is themarker of angiogenesis related to cancer development. 1. Todetermine the periodontal health status and the serum levels of(VEGF) in women with breast cancer in comparison to womenwithout breast cancer. 2. To correlate between serum levels ofVEGF with clinical periodontal parameters (PLI, GI, BOP, PPD,and CAL) in women with breast cancer in comparison to womenwithout breast cancer. Eighty women with age ranged from (30-60) years old, 40 women with breast cancer and another 40 womenwithout. Periodontal health status were taken for all participantsand then they were subdivided into four groups: the (BC +CP)group which included participants with breast cancer and chronicperiodontitis (n=20), the (BC+ G) group included participants withbreast cancer and gingivitis (n=20), the (CP) group which includedparticipants with chronic periodontitis only (n=20) and the (G)group which included participants with gingivitis only (n=20).Other than breast cancer, all subjects were systemically healthyfemales. Periodontal health status was determined by clinicalperiodontal examination of (PLI, GI, BOP, PPD and CAL). Bloodsamples were collected from each participant, serum levels ofVEGF were determined by enzyme -linked immune-sorbent assay(ELISA). The results of this study showed that the median valuesof PLI and GI were slightly increased in BC+G group incomparison to other groups with non-significant differences, thepercentages of BOP showed highly significant differences amongthe groups (P <0.01). The median values of PPD showed slightincrease in BC+CP group than CP group with non-significantdifferences. Median values of CAL were increased in CP groupthan BC+CP group with non-significant differences. VEGF serumlevels were elevated in BC+CP group than other groups with nonsignificantdifferences. Also by using Spearman’s rank CorrelationCoefficient, serum levels of VEGF were correlated positively withall clinical periodontal parameters. VEGF serum level could beassociated with progression of both periodontal diseases and breastcancer.
