The Role of Resistin in Patients with Primary Fibromyalgia

Abstract

Background: Fibromyalgia syndrome (FMS) is a common rheumatologic syndrome with multiple manifestations and associated with many diseases, it characterized by chronic wide spread muscular pain and tenderness. Resistin is an adipokine discovered in 2001 and considered as a link between obesity and type 2 diabetes by antagonizing and resisting the action of insulin as it name encompass ;resistin (resist insulin) . It is secreted mainly from adipose tissue in mice and from white blood cells besides the adipose tissue in humans. Research reviled that resistin has a role in some inflammatory diseases like Rheumatoid Arthritis (RA).
Objective: The objective of this study is to measure Resistin concentration and to evaluate its role in the pathogenesis of primary FMS.
Patients and methods: Fifty patients with primary FMS were included in this study (37 females and 13 males) the age range of (17-65) years (Mean ± SD) (40.13 ± 12.0) years, and thirty healthy individuals volunteers (21 females and 9 males), whose age and sex matching with FMS patients, age ranging (18 - 63) years, (Mean ± SD) (36.1 ±10.0) years. Resistin concentration was measured in sera of patients and controls by ELISA kit. Anthropometric measurements like body mass index (BMI) and waist circumference (WCr) were taken, besides other features like sleep disturbance, emotional distress, and fatigue were reported.
Results: Resistin concentration was higher in FMS patients than controls (2.15±0.9) ng /ml vs. (1.57±0.6) ng /ml. This elevation was highly significant statistically (p=0.003).Other measurements in patients group like BMI, WCr were (28.39±5.0) Kg/m2 and (100.34±13.21) cm respectively .These values were highly significant when compared to their control group (p=0.002) and (p=0.008) respectively . Serum resistin level was positively correlated with both waist circumference (r=0.411,p=0.003) and age (r=0.405 ,p=0.004) . Waist circumference was positively correlated with age (r=0.604, p=0.000).Clinical features like sleep disturbance, emotional distress ,and fatigue showed highly significant difference between the two groups . No significant differences were reported with respect to age and sex.
Conclusion: The result of the current study suggest that Resistin might have a role in the pathogenesis of FMS.

Abstract

Background: Osteoarthritis (OA) is disorder of diarthrodial joints characterized clinically by pain and functional limitation. Rheumatoid factor (RF) represents one of routine laboratory tests that done for all patients have joint complaints. Chloroquine phosphate (CQP) is a disease modifying antirheumatic drug (DMARD) used for patients suffer from knee osteoarthritis (KOA) in order to reduce their RF value and improves the disease status. Objective: To evaluate the effect of chloroquine phosphate on rheumatoid factor (RF) level in serum of patients with knee osteoarthritis KOA) Design: case report.Subjects and methods: RF value were assessed quantitatively by ELISA technique before and after treatmtnt for a total of fifty five patients with KOA (30 femal and 25 male) their age ranged from (50-66 years) selected randomly from out patient clinic in Baghdad Teaching Hospital , Medical City Baghdad; suffering from KOA. All patients were treated with oral dosage form of CQP for one month twice daily.Results: Mean serum RF level was significantly reduced (p<0.05) in serum of patients after treatment with CQP for one month.Conclusion:CQP is a disease modifying antirheumatic drug (DMARD) used for patients suffering from KOA in order to reduce their RF value and improves the disease status.

Abstract

Background: New data suggests that joint damage in Knee Osteoarthritis (KOA) may be
caused by systemic factors like adipose tissue products; Adipokines, which may provide a
metabolic link between obesity & KOA. Recently, one of the known adipokines named
LEPTIN has been linked to KOA because it can be detected in serum & synovial fluid of
patients with KOA.
Objective: To evaluate the contribution of Leptin & serum lipids to the pathophysiology of
Osteoarthritis in Iraqi patients with Knee OA.
Subjects& Methods: The study was carried on 90 subjects divided into four groups:
Knee Osteoarthritis cases group (n=60).
Control group (n=30).
Obese subjects group (n=60).
Non-obese subjects group (n=30).
KOA cases were diagnosed clinically whereas obesity was specified by BMI ≥25kg/m2. For
all subjects studied measurements of fasting serum leptin and lipid profile have been done.
Results: Mean serum leptin level was significantly higher in KOA cases compared to control
group (P<0.001), and higher in obese than non-obese subjects (P<0.001). Serum leptin level
also showed a strong positive correlation with BMI (r=0.501, P<0.01).
Conclusions: Leptin may play an important role in the pathogenesis of KOA. In addition
abnormal lipid profile and obesity are important risk factors for KOA.

Abstract

Further, circulatory levels of pro-inflammatory cytokines like IL-8 and others may be altered in FMS patients, possibly associated with their symptoms.The objective of the study is to measure IL-8 concentration and to evaluate its role in the pathogenesis of primary FMS.OBJECTIVE:Fifty patients with primary FMS were included in the study (37 females and 13 males) the age range of (17-65) years (Mean ± SD) (40.13 ± 12.0) years, and thirty healthy individuals volunteers (21 females and 9 males), whose age and sex matching with FMS patients, age ranging (18 - 63) years, (Mean ± SD) (36.1 ±10.0) years.METHODS:IL-8 concentration was measured in sera of patients and controls by ELISA kit. No significant differences were reported with respect to age and sex.CONCLUSION:The result of the current study suggest that interleukin-8, (IL-8) might have a role in the pathogenesis of FMS.

Abstract

ABSTRACT:BACKGROUND:Nodal Osteoarthritis is a subset of OA characterized by polyarticularinterphalangeal and thumb base OA ,Heberden’sand Bouchard’s nodes formation.C-reactive protein (CRP) is one of the first acute phase response proteins to be elevated in an inflammatory processes.OBJECTIVE:The aim of this study is to investigate whether the inflammatory processes have roles in the development of nodal osteoarthritis (NOA) in Iraqi femalepatients using serum highly sensitive C-reactive protein as aninflammatory marker.SUBJECTS AND METHODS:The study included eighty four females, divided into two main groups,sixty were patients with (NOA), and twenty-four represented the control group. Other subdivisions were applied according to body mass index and menopause status. Measurement of serum (hsCRP), wasapplied to all subjects included in this study, by ELISA sandwich method.RESULTS:Serum highly sensitive C-reactive protein (hsCRP) level was significantly higher (18.08±8.15) (P value < 0.05) in patients with NOA in comparison to control group (2.23±1.73) .CONCLUSION:Highly significant elevated level of serum (hsCRP) observed in the NOA patients compared to control suggest that the inflammatory processes have roles in the development of NOA.

Abstract

This study was conducted to evaluate the prevalence rate of toxoplasmosis among 294 rheumatoid arthritis (RA) patients treated with methotrexate (MTX), 50 RA patients without treatment and 50 samples as healthy control. Blood samples were collected and the presence of T.gondii IgG and IgM antibodies was determined by using Enzyme linked immunosorbent assay (ELISA). Tumor necrosis factor alpha (TNF-α) was also estimated in serum of all subjects by using ELISA method too. The seroprevalence of toxoplasmosis IgM and IgG in RA+MTX was 60(20.408%), and 98(33.33%), in RA patients 4(8%), and 18(36%) while, it was 2(24%), 6(12%) in healthy group. Tumor necrosis factor alpha (TNF-α) was also estimated in serum of all subjects by using ELISA method too. The mean levels of TNF-α in seropositive anti-Toxoplasma IgM and IgG of RA+MTX patients were 3.781 pg/ml ± 0.571) and (36.98 pg/ml ± 0.58), in RA patients (25.404 pg/ml ± 1.748) and (40.12 pg/ml ± 1.7) while, they were (5.04 pg/ml ± 0.643) and (10.7 pg/ml ± 1.7) in healthy group. The results showed significant difference (P<0.05) was found between treated and untreated patients.

أجريت هذه الدراسة لتقييم معدل انتشار داء المقوسات الكونيدية في294 مريض بمرض التهاب المفاصل الرثياني المعالجين بالميثوتركسيت, و50 مريض بمرض المقاصل التهاب الرثياني غير معالجين و50 عينة كعينات سيطرة. تم جمع العينات وتحديد وجود الأجسام المضادة لطفيلي المقوسات الكونيدية من النوع (ج) و (م) باستخدام تقنية الاليزا. كذلك تم قياس عامل التنخر الورمي بتقنية الاليزا أيضا. وكانت الأجسام المضادة لطفيلي المقوسات الكونيدية من النوع (ج) و (م) في مجموعة التهاب المفاصل الرثياني المعالجين بالميثوتركسيت 60(20.408%)و 98(33.33%), ولمرضى التهاب المفاصل الرثياني غير المعالجين 4(8%)و 18(63%) أما في مجموعة السيطرة 2(24%)و6(12%). وكان معدل مستوى عامل التنخر الورمي في الأشخاص المصابين بطفيلي المقوسات الكونيدية من النوع (ج) و (م) في مجموعة التهاب المفاصل الرثياني المعالجين بالميثوتركسيت ( (3.871pg/ml±0.571و (36.98pg/ml±0.58), ولمرضى التهاب المفاصل الرثياني غير المعالجين (25.404 pg/ml ±1.748), (40.12 pg/ml ± 1.7) وفي مجموعة السيطرة 0.643) (5.04 pg/ml ±, و (10.7 pg/ml ± 1.7). أوضحت النتائج وجود فروق معنوية (P<0.05 ) بين المرضى المعالجين وغير المعالجين .

ASSOCIATION OF PORPHYROMONUS GINGIVALIS WITH RHEUMATOID ARTHRITIS

Abstract

Background:Rheumatoid arthritis (RA) is chronic systemic inflammatory disease. Porphyromonus gingivalis (P. gingivalis) produce peptidyle arginine enzymes, which lead to citrullination of human protein then lead to formation anticitrullinated peptide antibody (ACPA).Objective:To investigate the role of P. gingivalis as environmental factor for RA and association of P. gingivalis with development of ACPA.Methods:This study included 31 newly diagnosed RA patients with periodontitis, which included 22 females and 9 men in addition to 30 individual as healthy controls, which included 20 females and 10 men. The exclusion criteria included autoimmune disease (systemic lupus erythematosus, Bachet disease, ankylosing spondylitis, multiple sclerosis), attending Department of Rheumatology in Baghdad Teaching Hospital during period from May 2014 to January 2015. The age range was 20 to 68 years. Disease activity score 28 (DAS28) was calculated for each patient. Five ml of blood sample was taken for detection of ACPA antibody while gingival cervicular fluid was taken by paper point for detection of P. gingivalis by polymerase chain reaction (PCR) with specific primer for fimbrial antigen (fimA).Results:The frequency of positive cases with P. gingivalis were 13/31(41.90%) while in healthy controls was zero with significant P value (<0.001). The association between anti cyclic citrullinated peptide (ACCP) antibody and frequency of positive cases for P. gingivalis was significant (P<0.041). The association of P. gingivalis positivity and DAS28 was non-significant (P= 0.003).Conclusions:P. gingivalis showed positive association with RA in newly diagnosed patients. The frequency of positive cases for P. gingivalis revealed association with positivity of anti-CCP.Key words:RA, ACCP, P. gingivalis

Abstract

Background: The tumor necrotic factor alpha (TNF-α) was associated with rheumatoid arthritis (RA) pathogenesis and inflammation of joint, for this consider important target for drugs (TNF inhibitors) as for example infliximab.Objective: To investigate the role of TNF-α-308 polymorphism G>A in unresponsiveness to biological treatment (infliximab) in RA patients.Methods: This study included 29 RA patients treated with infliximab attending Department of Rheumatology in Baghdad Teaching Hospital and diagnosed according to American College of Rheumatology (ACR) with 30 persons as healthy controls. The age range was 20 to 68 years. The DAS28 was calculated for each RA patients. Blood samples were taken from them during the period from May 2014 to January 2015. The blood samples were tested by polymerase chain reaction (PCR) for TNF-α-308 G>A then by restriction fragment length polymorphism (RFLP) for A allele and G allele. HLA-DR genotyping by PCR-SSO.Results: The frequency of A allele was 23 (39.7%) in RA patients while G allele was 35 (60.3%) in RA patients, whereas in in controls A and G alleles frequencies were 5 (8.3%) and 55 (91.7%) respectively. The A allele was associated with high DAS28 19 (82.6% while for G allele was 1 (2.9%) and DAS28 showing significant association with TNF-α-308 polymorphism G>A. The distribution of A allele in female RA patients was 16 (36%) whereas G allele was 28(64%), while in male RA patients, A and G allele were 7 (50%) for each of them. The HLA-DRB1 not showing significant association with TNF-α-308 polymorphism G>A.Conclusion: The TNF-α-308 polymorphism G>A has effect on response to biological treatment (infliximab). The A allele of TNF-α was associated with unresponsiveness to infliximab and RA patients who carry this allele have high DAS28 score.
