Oral hygiene and salivary immunoglobulin among acute lymphocytic leukemic patients undergoing chemotherapy courses

Abstract

ABSTRACT
Background: Chemotherapy impaired the normal function of the human immune system. The patients' ability to
accomplish adequate oral hygiene may be limited. When chemotherapy is indicated, it is imperative that health of
the oral cavity be assessed initially as well as throughout therapy. This study was conducted to evaluate the oral
hygiene and salivary immunoglobulin in patients undergoing chemotherapy courses.
Subjects and methods: The study groups included 30 acute lymphatic patients, they were under chemotherapy. The
control group includes 30 subjects matching with study group in age and gender. Plaque status was evaluated
according to the Silness &Loe Index, dental calculus according to Ramfjord index, while gingival condition was
assessed according to Loe & Silness. After oral examination, stimulated saliva samples were collected from the subjects
and performed under standard condition following instruction cited by Tenovuo & Lagerlof, and chemically analyzed
for the detection of salivary immunoglobulin (IgA & IgG).
Results: Higher plaque, calculus and gingival index were recorded among acute lymphatic patients compare to the
control group, statistically significant difference with calculus index between groups. A low level of IgA, and IgG were
seen among the study group compared to the control groups in both genders. A non-significant correlation was
found between salivary immunoglobulin and oral cleanliness among acute lymphatic patients.
Conclusions: Salivary immunoglobulin levels affect severally by chemotherapy. Salivary IgA, and IgG defect seem to
play a role in the development of poor oral hygiene among acute lymphatic patients.

Abstract

Background: Nutritional Rickets is a condition produced by an absence of Vitamin D, calcium or phosphate. It clues to relaxing and fading of the bones. Dental expression of children with rickets contains enamel hypoplasia and delayed tooth eruption. This study was conducted in order to assess caries experience (dmfs) and enamel defects among study and control groups, and to evaluate and compare the levels of selected salivary biomarkers between children with nutritional rickets and apparently healthy children.Material and methods: Assessment of caries according to WHO in 1987, and assessment of enamel defects according to enamel defect index EDI of WHO in 1997. In addition a stimulated saliva samples were collected according to Palone et al from 30 children diagnosed with nutritional rickets and 30 control children as control group. Salivary vitamin D, calcium, phosphate and alkaline phosphtase were analyzed.Results: Caries experience represented by dmfs was significantly higher among control group compared to study group, while enamel hypoplasia was higher in study group than control group. Salivary inorganic component (Ca, PO4 ALP) revealed obvious variations between study and control group. Salivary vitamin D concentration was lower in study group compared with control group.Conclusion: Based on the results, it can be concluded that nutritional rickets impact on certain salivary biomarkers which can be considered for evaluating the diagnosis and prognosis of caries experience and enamel defects in nutritional rickets children.

Abstract

Background: Autism is developmental disorders characterized by severe defects in social interaction and communication. A wide spectrum of medical and behavioral symptoms is exhibited by children with autism, which make routine dental care very difficult in them.Aims of this study: This study conducted to investigate the caries-experience, tooth wearing, in addition to developmental deformity of enamel among autistic children, in comparison to normal non autistic children.Subjects, materials & methods: The study group included 43 autistic children, with an age range of 4-16 years. They were with confirmed diagnosis of autism. Children were divided into three groups according to the severity of the disease. The control group included 43 children matching with age and gender. Assessment and recording of caries — experience were through the application of decayed. Missing and filled teeth index (DMTT&dmft) WHO (1987), tooth wearing was recording according to Smith index (1984), and developmental deformity of enamel was recorded according to Modified Developmental Defect of Enamel index (DDE) (1989).Results: A low level of dental caries was seen among autistic groups compared to the control, with differences were statistically highly significant in primary and permanent dentition. High mean values were recorded concerning teeth wearing (both dentition) among the severe autistic group compared with non autistic children with statistically highly significant difference. No developmental deformity of enamel was recorded among autistic and normal children.Conclusions: Increase values of teeth wearing with low level of caries - experience was recorded among autistic children compared to healthy children.Key words: Autism, dental caries, teeth wearing, enamel developmental defect.

الهدف: التوحد يتصف باضطرابات النمو مع صعوبة التفاعت الاجتماعي. ايضا يتصف بمدى واسع باعراض طبية وسلوكية خاصة , مما يجعت العناية والفحص للفم والاسنان شديدة الصعوبة.ان اهداف هذه الدراسة تحديد شدة التسوس مع شدة احتكاك سطح السن , بالاضافة الى تحديد خلل نمو طبقة المينا عند اطفال التوحد مع مقارنتهم باطفال اصحاء .تم فحص 43 طفت يعانون من اضطرابات التوحد للاعمار من 4 – 16 سنة , قسموا الى مجموعتين استنادا شدة المرض , مع مقارنتهم باطفال اصحاء مطابقين لهم بالعمر والجنس .تم تحديد شدة التسوس من خلال المقياس (dmfs, DMFS) , مع قياس هحتكاك اسطح الاسنان , بالاضافة الى قياس خلل نمو طبقة المينا.النتائج: اظهرت النتائج ان التسوس كان قليلا عند اطفال مرضى التوحد مقارنة مع الاطفال الاصحاء المفحوصين, مع وجود علاقة معنوية عالية .سجلت النتائج قيمة معنوية عالية عند اطغال التوحد (شديدي التوحد) بما يتعلق بشدة احتكاك اسطح الاسنان مقارنة مع الاصحاء , مع وجود علاقة معنوية عالية . لم تظهر النتائج اي خلل في نمو طبقة المينا عند جميع الاطفال المفحوصين .الاستنتاج: للحصول على نتائج اوسع بما يتعلق بصحة الفم والاسنان وما يتعلق بشدة النمو والتسوس , نحتاج الى عينات اكبر من المصابين , مع الحصول على نتائج تحليلية للعاب.

Gingival health condition and salivary physical properties among a group of patients with intermaxillary fixation in tow times intervals (Follow up study)

Abstract

Background: One of the methods used in the treatment of maxillofacial fracture is intermaxillary fixation(IMF), the most common type is the Erich arch bar with interdental wiring. This study was conducted to investigate the impact of intermaxillary fixation on gingival health condition among a group of patients with facial fracture in relation to salivary physical properties.Materials and methods: Thirty patients with an age range of (17-37) years old with facial fractures and indicated for IMF. Plaque index and gingival index (Loe, 1967) were used to assess both of them before application and after removal of IMF. Unstimulated saliva sample collection was carried out under standardized conditions according to Navazesh and Kumer (2008) before IMF application and after removal to measure salivary flow rate and viscosity. Analysis of data was carried out using SPSS (version 18).Results: A higher mean values of plaque and gingival index were recorded after IMF removal than that before IMF application, with highly significant changes (p<0.01). In regarding to salivary physical properties, salivary flow rate mean values was lower after IMF removal than before IMF application with statistically highly significant changes ( p<0.01). Concerning salivary viscosity, it has been found that a higher mean values after IMF removal than before IMF application with highly significant changes (p<0.01).Conclusion: The results of current investigation revealed that intermaxillary fixation (Arch bar) have a significant effects on gingival health in addition to disturb normal level of certain salivary physical properties

Abstract

Background: Nutrition can affect the development and integrity of the oral cavity as well as the progression of oral diseases such as dental caries which was the most predominant and wide spread not life threatening human diseases especially in developing countries as in Iraq. This study was conducted to assess the occurrence, prevalence and severity of dental caries condition and their relations to nutritional status among intermediate schools females in Al-Najaf city in Iraq.Materials and methods: This study was conducted among intermediate schools females aged 13, 14 and15 years old and the total sample consisted of 754 students. The assessment of nutritional status was performed using body mass index (BMI) following Centers for Disease Control and Prevention growth chart (2000). Diagnosis and recording of dental caries was according to the criteria of WHO 1987.Results: The percentage of well-nutrition was (96.2%) while the prevalence of malnutrition was (3.8%). Result showed that only 5.17% of the total sample was caries-free. The mean dmft for deciduous teeth was equal to (1.77±0.15) and dmfs (3.92±0.39), while concerning permanent teeth the mean DMFT was equal to (4.68±0.10) and DMFS (6.22±0.16), significant differences were seen between DMFT, DMFS and age while opposite was found with nutritional status, and no significant differences were seen between caries experience of primary teeth and age and this similar was found with nutritional statusConclusion: This study revealed that a higher prevalence of dental caries among well-nourished females students aged 13-15 years. Therefore, there is need for an improving public and school preventive programs, and encouraged to orient health knowledge in a positive direction

Abstract

Background: Leukemia is a broad term given to a group of malignant diseases characterized by diffusereplacement of bone marrow with proliferating leukocyte precursors. Chemotherapy has been increasingly used totreat malignant conditions. The systemic sequelae as a result of these immunosuppressive techniques induce manyoral and dental complications. This study was conducted to evaluate the effect of chemotherapy on oral healthstatus and activity of salivary alkaline phosphates enzyme in patients with acute lymphocytic leukemia.Materials and methods: The study groups included 28 patients with acute lymphocytic leukemia; they were underchemotherapy, aged 20-25 year old. The control group includes healthy subjects matching with study group. Plaque,gingival, calculus and loss of attachment status were recorded. After oral examination, stimulated saliva sampleswere collected from the subjects (study & control groups) and performed under standard condition followinginstruction and chemically analyzed for the detection of salivary alkaline phosphates enzyme.Results The study showed higher mean values of plaque index, gingival index, attachment loss and concentration ofsalivary alkaline phosphates enzyme among leukemic group than control group with statistically high significantdifferences.Conclusions: It was concluded that patients with acute lymphocytic leukemia have poor oral hygiene and needintense oral hygiene program before, during and after chemotherapy

Effect of Zamzam Water on the Microhardness of Initial Carious Lesion of Permanent Teeth Ename(An in vitro study)l

Abstract

Chemical tests concluded that Zamzam water is absolutely good and suitable for drinking as it has very high percentage of sodium, calcium, magnesium, and many other minerals. The purpose of this study was to test the effect of Zamzam water on the microhardness of permanent teeth enamel.Teeth samples (n= 30) with artificially initiated carious lesions were divided randomly into three groups, study group treated with Zamzam water (n= 10), control positive treated with sodium fluoride (n=10), and control negative treated with deionized water (n=10). Teeth were subjected for microhardness assessment before and after pH cycling and treatment with the three agents.The microhardness of enamel surfaces treated with Zamzam water is shown statistically significant increase in the values following demineralization which was not different from that of sodium fluoride.Zamzam water cause an increase in the microhardness of the enamel surface after pH-cycling.

Salivary Alkaline Phosphatase and Periodontal Disease

Abstract

Saliva is an easily collected fluid that contains locally and systemically derivedmarkers of periodontal disease. The purpose of this study was to compare the level ofsalivary alkaline phosphatase activity in healthy subjects and patients with periodontaldisease (gingivitis and periodontitis), and to evaluate the relationship of this enzymewith different clinical parameters in all patients included in the study.The study included 65 patients referred to Department of Oral Diagnosis, Collegeof Dentistry, University of Baghdad, they were divided into three groups, 20 healthysubjects, 20 subjects with gingivitis and 25 one with periodontitis. The clinicalparameters including plaque index (PI), gingival index (GI), probing pocket depth(PPD) and clinical attachment loss (CAL) were recorded and salivary samplescollected for analysis of alkaline phosphatase (ALP) levels.A highly significant increase in mean values of clinical parameters was found inpatients with periodontal disease (gingivitis and periodontitis) compared to healthysubjects, also a highly significant increase in the mean values of alkaline phosphataseactivity was illustrated as the severity of periodontal disease increased. ALP level wasclosely related to the gingival index, probing depth and attachment loss in patientswith periodontal disease, while no relation was shown to plaque index.Salivary alkaline phosphatase could be used as a useful marker for monitoringperiodontal disease..
