Immunohsitochemical Study of CA19-9 Overexpression In Colorectal Adenocarcinoma and Its Correlation With Some Pathological Parameters

Abstract

Background: Colorectal carcinoma is third most frequent cancer in the world in both sexes and the third most frequent cause of cancer related deaths .Its incidence and mortality rates vary markedly around the world. The risk of developing colorectal carcinoma increasing with age. Most cases occur in the sixty and seventy years old , while cases before age fifty years old are uncommon unless there is a family history of early colon cancer. The pathogensis of colorectal carcinoma is complex and developed in multisteps process in which several gene mutations will occur an coordinate with each other in genotyping and phenotyping outcome.Aim of the study:- To assess CA19-9 over expression in colorectal adenocarcinoma and its correlation to some of pathological parameters.Materials and methods: this study was included fourty cases of colorectal carcinoma, collected randomly from the period of February 2010- May 2012 from Al Hilla Teaching Hospital.40 patients (30male and 10 females) of colorectal adenocarcinoma ,their median age 50 years range from (30 – 70 )years . A group of 10 patients with nonspecific colitis were included as a control group. A manual avidin biotin peroxidase complex procedure(ABC) system was used in the imunohistochemical analysis (Dako Cytomation Copenhagen ,Denmark).Results: CA19-9 over expression was positive in (70%) of colorectal adenocarcinoma, while there was no expression in benign colonic lesions. CA19-9 immunohistochemical staining was positively correlated with grade and stage of colorectal adenocarcinoma.Conclusion: These finding support the role of CA19-9 in carcinogenesis of colorectal adenocarcinoma regarding behavior and aggressiveness, and thus CA19-9 could be considered as a poor prognostic parameter in colorectal adenocarcinoma .Keywords: colorectal carcinoma, A manual avidin biotin peroxidase complex procedure(ABC)

يشكل سرطان القولون والمستقيم الخبيث ثالث اكثر الاورام السرطانية شيوعا في كلا الجنسين وثالث اكثر الاسباب السرطانية المؤدية الى الوفاة في العالم . يعتبر نسبة حدوث سرطان القولون و نسبة الموت المتسبب منه في زيادة مستمرة في العالم كذلك هو ثالث الانواع المشخصة في الرجال و الثاني بالنسبة للنساء . ان نسبة تطور والاصابة بهذا المرض تزداد بازدياد التقدم بالعمر و ليزداد ظهوره في سن الستين و السبعين .و امكانية الاصابة به قبل سن الخمسين غير شائعة الا اذا كان هنالك تاريخ مرضي لاصابة مبكر ة بهذا المرض و كذلك وجود اسباب اخرى . يعتبر سرطان القولون معقد من خلال حدوثه و نشوئه و يتضمن عدة خطوات متسلسلة لطفرات جينية تعمل معا لتتسبب بظهور خلل جيني و مظهري .

Abstract

Background Carcinoma of the breast is one of the most important diseases for women, in the world .The disease is the commonest cause of death among women aged 40-50. Breast carcinoma is uncommon in women younger than age 30, thereafter, the risk steadily increases throughout life, but after menopause the upward slop of the curve almost plateaus.The incidence of breast carcinoma is increasing in all industrialized countries, changes in diet, reproductive patterns and altered exposure to endogenous and exogenous substances with hormonal activity have been suggested as contributing to this increase.Aim of the study: To assess estrogen and progesterone receptor over expression in breast carcinoma and its correlation with ages of patients, histopathological types & grades of tumors. Materials and methods: This study was included 58 paraffin embedded samples from female patients with breast carcinoma were collected randomly from period of June2010- November 2012 in AL-Hilla Teaching hospital. The clinical information were collected including ages of the patients, histopathological types, & tumor grades from clinical reports of the hospital . A manual avidin biotin peroxidase complex procedure(ABC) system was used in the imunohistochemical analysis (Dako Cytomation Copenhagen, Denmark).Results: The estrogen, progesterone receptors determined by immunohistochemical method revealed ER+/PR+ in 15.5% of cases, ER+/PR- in 3.4%, ER-/PR+ in 29.3 and ER-/PR- in 51% of cases , all ER+/PR+ are of invasive ductal carcinoma,most cases with ER+/PR+ are of premenapuasal and perimenapuasal age group 36.2% & 32.7% respectively and most cases with grade III are ER-/PR-Conclusion: Present study reported high percentage of ER-/PR- cases of breast ca ,all ER+/PR+ cases are of invasive ductal ca, the population of high grade IDC were predominantly ER-/PR-

سرطان الثدي يعتبر من أهم الامراض واشهرها التي تصيب النساء في العالم. ويعتبر من أهم الأمراض المؤديه الى الموت في النساء في عمر 40-50 سنة . سرطان الثدي غير شائع بين النساء بعمر مبكر أقل من 35سنة لذلك فأن نسبة زيادة خطورة الأصابة بهذا المرض تزداد بزيادة التقدم في السن وتقل بعد سن اليأس. أن نسبة الأصابة بسرطان الثدي تزداد في الدول المتقدمة وذلك يرجع الى عدة أسباب منها الأختلاف الغذائي ,التغييرات الجنسية والهرمونية. أن الهدف من أجراء هذه الدراسة هو تقييم نسبة ظهور عوامل مثل الأستروجين والبروجسترون في سرطان الثدي وعلاقته بعمر المريضة , نوعية النسيج المرضي ودرجة التمايز بالنسبة لسرطان الثدي.

Abstract

Lung cancer is the most common cancer in men while is third highest incidence among women and is second after breast cancer in mortality. Most cases occur in older people in age 65 or older, while less than 2% are younger than 45. The pathogenesis of NSCLC is complex and developed in multistep process in which several gene mutations will occur and coordinate with each other in genotyping and phenotyping outcome. Non-small cell lung carcinomas (NSCLC) which represent 85% of all lung cancers, it represents the most common type, usually grows and spreads more slowly than other types, histologically is divided into Adenocarcinoma (ADC), Squamous cell carcinoma (SCC) and large cell carcinoma (LCC). Its incidence and mortality rates vary markedly around the world. The aim of this study was to assess Anaplastic Lymphoma Kinase (ALK) over expression in Non-small cell lung carcinomas (NSCLC) and its correlation with type, grade and stage of the tumor.Forty cases of NSCLC was included in this study, collected randomly from the period of February 2016- April 2017 from Al-Harirey Teaching Hospital in Baghdad and Al-Sadder Teaching Hospital in Al-Najaf governate. The (40) patients (22 male and 18 females) of NSCLC, their median age 58.3 years range from (34-80) years. A group of 12 cases with benign brain tissue were included as a control group. A manual Envision procedure was used in the imunohistochemical analysis (Dako Cytomation Copenhagen, Denmark). ALK over expression was positive in (12.5%) of NSCLC, ALK immunohistochemical staining was more in ADC than SCC and its not correlated with grade and stage of NSCLC. These finding support the role of ALK in carcinogenesis of ADC and less commonly SCC of the lung.
