Shall we Keep Non-Steroidal Anti-Inflammatory Drugs behind the Counter?

Abstract

A five hundred and thirty one (531) on the counter (OTC) non-steroidal anti-inflammatory drugs (NSAIDs) users, were directly interviewed by pre-structured questionares. The study was conducted at Rheumatology and rehabilitation unit of Merjan teaching hospital in period from March 2008 to August 2008. The items of pre-structured questionare, measures the knowledge and the attitude of patients toward the use of these medications. The results showed that most of studied users were not using these agents correctly and frequently ignore the potential adverse effects. Lack of knowledge of most of users, and lack of direct or labeled instructions were considered the corner stone for abusing these hazardous agents.

Diabetes Mellius is The Predominant Risk Factor of Dupuytren , s Disease in Babylon Aprospective Matched Case Control Study

Abstract

In Merjan teaching hospital we recorded (3200) cases of hand complaints in period from Nov. 2002 – June 2004. Of them, Dupuytren’s hand diseases were represented in (84) cases 2.6% the (study group). which were investigated for the association and severity, with diabetes mellitus ,smoking and other risk factors , this study group also were matched for age and gender with other three groups : Group I: (230) Patients who had diabetes mellitus for more than (10) years duration, who were neither smokers nor alcoholic or epileptics. Group II: (300 ) community controls , who were smokers “ more than 20 cigarette per day for more than 10 years “,who were neither diabetics nor alcoholic or epileptics . Group III: ( 420 ) community controls , who were randomly selected . The results showed that the Dupuytren’s disease is strongly associated with diabetes mellitus, where in study group: 25% of Dupuytren’s were diabetics, while 11.9% of Dupuytren’s were smokers 2.3% were epileptics on phenytoin. Group I: 9.5% of diabetics patients had got Dupuytren’s Group II :2.6% of smokers had got Dupuytren’s disease . Group III: The total number of Dupuytren's disease were ( 7 ) cases 1.6 %. Grouped as follow ( 4 ) cases were diabetics , ( 1 ) case was smokers. And (2) cases had not apparent risk factors . On basis of the results, the Dupuytren’s disease is strongly associated with diabetes mellitus, although milder , appeared earlier in age than other risk factors and mostly bilateral. There was no significant difference regarding incidence between different types of jobs , or between dominant hands and non dominant ones . The purpose of the present prospective case matched study , is to elucidate the role of diabetes mellitus as main predisposing risk factor of Dupuytren’s disease In Babylon.

Abstract

A 47-year- old man presented in August,2004 with ,a three years duration , of insidious Low back pain and stiffness. His back stiffness was more severe and pronounced at waking-up in the morning , the pain and stiffness eased with activity as the day is going on. The pain was dull in nature , radiated to his buttocks , and poorly localized. He had no recent illnesses . His past medical history included a lithotomy from Urinary bladder , 5 years ago. Also he noticed progressive darkening of the skin over his face for the last several years.

Abstract

Back ground: In patients with a history of systemic cancer(out side the spinal column) , presented with persistent back pain, the early detection and management of spinal metastases is of utmost important to help with pain control, prevent further spread and development of neurological complications.Objective: To determine and evaluate the importance of spinal percussion, as a physical sign , and as evidence of spinal metastases.Methods: A prospective study of 61 patients with a history of systemic cancer , presented with back pain and suspicion of spinal metastases, was carried out.Full medical history and physical examination, including entire spinal percussion with a reflex hammer, were done for all patients.All patients had ESR , serum alkaline phosphatase estimation and plain radiographic study of entire spine.All patients had MRI scanning.Results: The spinal percussion sign profiling as a red flag, carried a high sensitivity of 90% and specificity of 89.4% aiding in identifying the spinal metastases.Conclusions: This test will help in prediction which patient with a history of primary cancer presented with back pain , have a high possibility of spinal metastases that need the priority , or at least to put them on the first line of waiting list of MRI for final diagnosis

ان اهمية الاكتشاف المبكر لانتشار المرص السرطاني الى العمود الفقري كمسبب لالام الظهر واعطاء العلاج اللازم له اهمية في تقليل الالم ومنع المضاعفات العصبية تمت دراسة و متابعة61. مريضا مصابا بمرض السرطان لاحد اعضاء الجسم( خارج العمود الفقري) يعانون من الام الظهر. تم اخذ التاريخ المرضي الكامل واجراء الفحص السريري, متضمنا فحص العمود الفقري بواسطة المطرقة الطبية. كذلك اجري فحص نسبة ترسيب الخلايا الحمراء ومستوى alkaline phosphatase serum.كذلك تم اخذ الصور الشعاعية للعمود الفقري بمنظرين امامي-خلفي وجانبي لجميع المرضى في الدراسة.تم عمل فحص الرنين المغناطيسي للعمود الفقري لجميع مرضى الدراسة.ان نتائج الدراسة بينت بان حساسية الفحص الطرقي للعمود الفقري هى 90% ونوعيته4..89 % لذلك اعتبر هذا الفحص السريري من العوامل التنبؤية المهمة لاحتمالية عالية بوجود انتشار الورم السرطاني الى العمود الفقري مسببا لالام الظهر., ممكنا الطبيب المعالج باستبيان اكثر الحالات احتمالية يكون فيها الام الظهر فيها بسبب انتشار الورم للعمود الفقري واعطاء هؤلاء المرضى ,على الاقل, الاولوية في الدور لفحص الرنين المغناطيسي لكون هذا الفحص ,أي الرنين المغناطيسي ,محدد من حيث العدد وياخذ وقتا غير قصير لاجرائه في الوقت الحاضر.

Abstract

At hemophilic centre in Merjan Teaching Hospital a ( 56) patients with severe hemophilia were investigated using a pre-structured questionnaire,were measuring mainly (4) parameters: Factor availability , academic achievement and work attendance , immediate and long term rehabilitation and participation in sport and physical activities. Due to perennial and then subsequent shortages of replacement therapy, and lacking of immediate and long term rehabilitation measures , most of patients developed various form of chronic arthropathies and disabilities with poor school and work attendance and most of them confined to their home with no employment , and were not participated in sport or physical activities.

Abstract

Aim: To determine if there is any association between lymphopenia and clinical features, disease activity and damage in systemic lupus Erythematosus (SLE). Methods: The study group consists of 50 Iraqi patients with SLE mostly from Babylon Governorate who were diagnosed depending on the American College of Rheumatology revised criteria (ACR) for SLE; they were attending Marjan Teaching Hospital during the period from 1st Jan. 2007 to 1st Jul. 2008. The clinical manifestations were obtained by detailed medical history and physical examination. The disease activity was assessed with the systemic lupus erythematosus disease activity index (SLEDAI) and the Physician's Global Assessment (PGA). The disease damage was determined with the Systemic Lupus International Collaborating Clinics Damage Index (SLICC-DI). Lymphocytes count was expressed in two categories: normal (more than 1500/cmm) and lymphopenia (less than 1500/cmm); the count of less than 500/cmm was considered as marked lymphopenia. The relationship of lymphopenia with clinical, pharmacologic treatment variables, SLEDAI, PGA and SLICC-DI scores was examined. Results: Lymphopenia was found to be positively associated with skin manifestations, renal involvement, serositis and the use of steroids. Marked lymphopenia was associated with even higher SLEDAI, PGA and SLICC-DI scores. Conclusion: Lymphopenia was associated with some clinical manifestations of SLE and with more disease activity and damage. Marked lymphopenia was associated with even higher disease activity and damage .
