Serum levels of Interleukin 6 and Homocysteine in Type 2 Diabetic Patients with renal failure complication

Abstract

ackground: Cardiovascular diseases are more common among type 2 diabeticpatients than healthy subjects without a family history of diabetes. Seruminterleukin-6 (IL-6) and homocysteine (Hcy) levels are markers of endothelialdysfunction and cardiovascular disease.Aim: this study demonstrated to evaluate levels of IL-6, Hcy and their associationwith cardiovascular risk in type 2 diabetic patients with renal failure complication.Material & Methods: The circulating IL-6 and Hcy levels were measured in 45 type2 diabetic patients and 25 control subjects without a known family history of diabetes.The BIOSOURCE IL-6 ELISA kit used to measure IL- 6, high performance liquidchromatography HPLC technique used to measure serum Homocystein, andenzymatic methods are used for blood suger, urea, creatinine, and lipid profilemeasurements.Results: A significant differences were found in serum levels of IL6 and Hcybetween the two groups diabetic and healthy control (p>0.05). IL6 levels correlatedsignificantly with Hcy (p=0.02), hemoglobin A1C (p=0.03), and serum Hcy correlatedsignificantly with HbA1C (p=0.03) levels in patients with type 2diabetes mellitus.Conclusion: These results suggest that serum IL-6 and Hcy levels do not directlycontribute to the development of endothelial dysfunction and cardiovascular riskfactors in type 2 diabetic patients with renal failure complication.

Abstract

background: Diabetic retinopathy is the most important microvascular complication of diabetes mellitus and the inflammation have potent role in the pathogenesis and development of the disease .Tumor necrosis factor alpha is a prominent proinflammatory cytokine that mediated in all inflammatory process so it has unwanted role in diabetic complication, retina inflammation and endothelia disturbance. So the prediction of the TNFα role in the retinopathy will help to find the major treatment approach.Objective: this study conducted to evaluate the effect of TNFα in diabetic retinopathy and to find the association between TNFα and insulin sensitivity.Material & method: this study involved 120 persons were 40 healthy control subjects and 80 diabetic patients which divided into 40 type2 diabetic patients with retinopathy and 40 type2 diabetic patients without retinopathy .The serum level of TNFα ,insulin and lipid profile was measured for all patients and controls and used the SPSS program for the statistic calculation.Results: The serum level of TNFα was significantly higher in diabetic retinopathy patients in comparison to diabetic only patients and control subjects. Also there was negative correlation between TNFα and insulin sensitivity, the lipid profile was significantly higher in diabetic patient compared to control subject and the LDL cholesterol was significantly high in diabetic retinopathy patients in comparison to diabetic only and control subjects.Discussion: The inflammation play prominent role in the development of diabetic complication. TNFα is the most important inflammatory marker that present in the serum of diabetic patients. The TNFα affect mainly on the glucose and lipid metabolism and it worsen the insulin resistance and it correlate inversely with insulin sensitivity and due to this effect it will lead to develop of diabetic complication.Conclusion: The serum TNFα level was elevated in type2 diabetic patients and it may have role in developed the complication of diabetes and it was inversely correlated with insulin sensitivity.

Abstract

AbstractBackground: Type 2 diabetes mellitus (DM2) is suddenly increasing in incidence, and is associated with increase risk for cardiovascular disease, because of the macrovascular and microvascular injury. Although the main physiological abnormalities are insulin resistance and impaired insulin secretion, the specific determinants of these metabolic defects remain uncertain. The experimental evidences suggest that inflammation may play an intermediary role in pathogenesis, there by linking diabetes with a number of commonly coexisting conditions thought to originate through inflammatory mechanisms. Serum asymmetric dimethylarginine (ADMA) level is biomarker of endothelial dysfunction and cardiovascular disease. Aim: The aim of this study was to evaluate levels of ADMA, and its association with the complications of type 2 diabetic patients.Materials and Methods: The ADMA serum levels were measured in 75 of type2 diabetic patients and 75 healthy control subjects by using high performance liquid chromatography (HPLC).Results: A statistically significant differences was found in serum levels of ADMA between diabetic and healthy control groups (p>0.001). Serum levels of ADMA correlated significantly with the levels of (HbA1c(% in type 2 diabetic patients.Conclusion: These results suggest that serum ADMA levels directly contribute to the complications of cardiovascular disease in type 2 diabetic patients.Key Words: type 2 diabetes mellitus, a symmetric dimethylarginine, (HbA1c (%, lipid profile.

الملخص:الخلفية: ان حدوث الزيادة المفاجئة في داء السكري من النمط الثاني يزيد من خطورة الامراض القلبية الوعائية, وبالرغم من ان اهم الشواذ الفيزيولوجية لداء السكري من النمط الثاني هي مقاومة الانسولين وضعف في افراز الانسولين, فأن التحديدات الخاصة بقيت غير مؤكدة. والدلائل تشير ان الالتهاب يلعب دور الوسيط للإصابة بداء السكري ويربطهٌ بعدد من الحالات الشائعة يٌعتقد انها تٌنشىء ميكانيكة الالتهاب. ان تقييم مستوى غير المتناظرة ثنائي مثيل ارجنين ADMA)), الواسم الحيوي الذي ينبئ بزيادة خطر الامراض القلبية لدى مرضى السكري النمط الثاتي.الهدف: ان الهدف من هذه الدراسة تقييم مستوى غير المتناظرة ثنائي مثيل ارجنين ADMA)), وخضاب الدم المعسل (% (HbA1c وهما واسمان حيويان ينبئان بزيادة خطر الامراض القلبية لدى مرضى السكري النمط الثاتي ومقارنتها مع مجموعة السيطرة من الاصحاء.النماذج وطرق القياس:تتضمن هذه الدراسة قياس مستويات غير المتناظرة ثنائي مثيل ارجنين ADMA)), وخضاب الدم المعسل (% (HbA1c في امصال (75) من مرضى السكري النمط الثاني و(75) من الاصحاء بأستخدام تقنية كروماتوغرافيا السائل عالي الاداء. النتائج:ان الدراسات الاحصائية التي اجريت على هذا البحث اثبتت وجود فرقا معنويا بمستوى غير المتناظرة ثنائي مثيل ارجنين ADMA)), وخضاب الدم المعسل (% (HbA1c بين المجموعتين ( المرضى و الاصحاء ) p>0.001. كما اوجدت علاقة معنوية ايجابية بين هذين الواسمين الحيويين في المرضى والاصحاء.الاستنتاج:ان نتائج هذه الدراسة تقترح بأن مستوى غير المتناظرة ثنائي مثيل ارجنين ADMA)) هو واسم حيوي ينبأ بحدوث مضاعفات قلبية خطيرة لمرضى السكري النمط الثاني.
