Validity of Hounsfield Units from computed tomographic images of mandibular bone in detection of osteoporosis

Abstract

Background: The figure for the clinical application of computed tomography have been increased significantly inoral and maxillofacial field that supply the dentists with sufficient data enables them to play a main role in screeningosteoporosis, therefore Hounsfield units of mandibular computed tomography view used as a main indicator topredict general skeleton osteoporosis and fracture risk factor.Material and Methods: Thirty subjects (7 males &23 females) with a mean age of (60.1) years underwent computedtomographic scanning for different diagnostic assessment in head and neck region. The mandibular bone quality ofthem were determined through Hounsfield units of CT scan images and were correlated with the bone mineraldensity values obtained from t-scores of lumbar spine using dual x-ray absorptiometry scans (DEXA).Results: There was a highly significant positive correlation [p-value 0.000 (HS)] of bone mineral density that measuredby t-score of dual x-ray absorptiometrical scan and Hounsfield units with very strong relation in measuring the bonedensity (r test) = 0.969, this close relation lead to predict osteoporosity and the chance of fracture occurrence usinga statistical equation that classified the patients as osteoporotic.Conclusion: Hounsfield units obtained from computed tomography scans that are made for any purposes canprovide an alternative clinical parameter to predict osteoporosis at no additional cost to the patient and noadditional radiation.

الخلفیة : لقد تزاید التطبیق السریري للتصویر المقطعي بشكل كبیر في مجال الفم و الوجھ والفكین مما قدم لأطباء الأسنان بیانات كافیة تمكنھم من لعب الدور الرئیسي في فحصھشاشة العظام ، وبالتالي اصبح استخدام وحدة ھاونسفیلد في التصویر المقطعي للفك السفلي یوصف بكونھ مؤشرا رئیسیا للتنبؤ بھشاشة العظام الھیكلي العام و معامل خطر لحدوثالكسور.المواد و الطریقة : ثلاثون شخصا ( 7 ذكور و 23 إناث) بمتوسط عمر ( 60.1 ) سنة تم استخدام المسح الشعاعي الطبقي لھم لغرضالتشخیصالتقییمي في منطقة الرأس و الرقبة وتحدید نوعیة العظم للفك السفلي من خلال وحدات ھاونسفیلد لصور الاشعة المقطعیة وربطھا مع قیم كثافة العظام التي تم الحصول علیھا من فحص العمود الفقري القطني باستخدامالمسح الامتصاصي المزدوج للأشعة السینیة .مع وجود علاقة احصائیة قویة جدا في قیاس p value) ( النتائج : كان ھناك ارتباط ذات دلالة عالیة من الإیجابیة بین و حدات ھاونسفیلد وقیمة كثافة المعادن في العظام 0.0000.969 ، وھذا یؤدي إلى علاقة وثیقة التنبؤ وفرصة حدوث الكسر باستخدام المعادلة الإحصائیة التي تصنف المرضى على اساس ھشاشة العظام . = (r-test) كثافة العظامالاستنتاج : ان استخدام وحدات ھاونسفیلد في المسح التصویري المقطعي التي یتم إجراؤه لأي غرض توفر البدیل السریري للتنبؤ بمرض ھشاشة العظام دون أي تكلفة إضافیة اواي اشعاعات إضافیة.للمریض

Radiographic findings of oral and dental aspects of chronic renal failure of Iraqi patients under hemodialysis therapy

Abstract

Background: Chronic renal failure can give rise to a wide spectrum of oral manifestations, affecting the hard or softtissues of the mouth. The majority of affected individuals have disease that does not complicate oral health care. Themain aim of this study is to assess the response to therapy and to determine the radiographic manifestations in teethand jaws of individuals with chronic renal failure undergoing hemodialysis and to find possible preventive andtherapeutic strategies.Materials and methods: Thirty male patients with chronic renal failure who were continuously receiving therapy withongoing hemodialysis were included in this study and were divided into two subgroups according to the period oftherapy (those who had renal dialysis for less than a year, and 1 to 2 years). All patients were requested forpanoramic (OPG) radiographs with standardized parameters according to user manual of the machine thenquantify the frequency of abnormal dental and periodontal radiographic findings in both arches i.e. the alveolarbone resorption, presence of hypercementosis of the roots, lamina dura reduction or loss and dental condition, thenthe data were analyzed statistically by comparison with control normal subjects.Results: It has been established that the duration with the increase the abnormality in dental and periodontalradiographic findings as alveolar bone resorption, presence of hypercementosis of the roots, lamina dura reductionor loss, as well as other dental diseases.Conclusion: Panoramic radiographic view used in this study considers as a good monitor for jaw and dental changesassociated with patients affected with chronic renal failure treated with hemodialysis, and the results showed strongcorrelation between duration of hemodialysis and all studied variables. Clinicians should use oral health assessmenttools to determine individual treatment and approaches to promote the oral health of patients undergoinghemodialysis and improve their quality of life

Orthopantomogrphic pre-surgical assessment of mandibular third molar teeth form and structures using surgical findings as a gold standard

Abstract

Background: Mandibular third molars are the most frequently impacted teeth in human and their surgical extractionhas become one of the commonest dentoalveolar surgeries. Accurate assessment of the position and morphologyof the mandibular third molars is necessary to make a sound decision about the proposed surgical procedure.Today, orthopantomography is the imaging method of choice to provide information for adequate assessment ofthe impacted lower third molars, the related teeth, anatomical features, and the surrounding bone. The aim of thisstudy is to determine the diagnostic accuracy of orthopantomographic view compared with postsurgical removalclinical finding in assessing the crown position, number and morphology of roots of the impacted lower third molar.Materials and methods: Total sample of 50 patients (25 males and 25 females), age range from 19 to 35 years oldwith impacted lower third molars assessed radiographically by using Standardized orthopantomography forevaluation of crown position and roots number and morphology in comparison with surgical findings.Results: According to the data obtained in this study, the comparison between the radiographic interpretation andthe clinical findings revealed a complete agreement for crown position (100% K- value) while based on roots numberthe (K –value was 0.7, 10.66, 0.80) for teeth with one, two and complex roots respectively and according to rootsmorphology (K –value was 0.64, 0.67, 0.81) for normal, fused and dilacerated roots respectively with more frequentfalse negative findingsConclusion: Although Orthopantomograph have a reasonable diagnostic value in the preoperative evaluation ofthe impacted lower third molars, but for more precise information modern radiographic modalities is advised to beused.
