Abstract

Factor XIII deficiency is a rare disorder of coagulation characteri-zed by moderate to severe bleeding tendency with increased susceptibility to intracranial hemorrhages, almost normal coagulation screening tests, clot lysis in 5 M urea solution & a tendency to abnormal wound healing. It is inherited as an autosomal recessive trait. However, acquired causes for its deficiency are rarely encountered. We describe here an inherited form of F XIII deficiency in two members (a boy & a girl) in a one family in Basrah.

Abstract

Introduction Giovanni Di Guglielmo first described erythroleukemia, a leukemia composed of purely erythrocytic precursors in 1926, and the disorder is often still referred to as acute Di Guglielmo syndrome. It is classifiedas an M6 subtype of acute myelogenous leukemia (AML) in the French-American-British (FAB) classification system based on morphologic andcytochemical criteria1a. Acute erythroleukemia accounts for 3-5% ofall de novo AMLs and 20-30% of secondary leukemias. It is very rare inchildren. The incidence of erythroleukemia increases in peopleolder than 50 years2 Mazzella et al (2000) described 2 peaks, one in theseventh decade of life and a second, smaller peak in the fourth decade of life.

6- A STUDY OF THE INCIDENCE OF BONE MARROW NECROSIS IN PRIMARY & SECONDARY MALIGNANT BONE TUMORS 32 ,

Abstract

During one year period, 21 newly diagnosed patients with different malignant bone tumors weresubjected to this study, 52.4% (11) of them were males & 47.6% (10) were females with a meanage of 50.2 years. Detailed history & physical examination were taken including the age, sex,the pathological type of the tumor, whether it is a primary or secondary & the site of the tumor,the existence of bone pain, fever, weight loss, pallor, pathological fractures, neurological deficit,any adjuvant treatment the patient had been put on, the existence of sickle cell disease, diabetesmellitus and any history of a previous surgery. All cases were subjected to the followingtests: complete blood count (CBC) including the estimation of erythrocyte sedimentation rate(ESR) & reticulocyte count, estimation of lactate dehydrogenase(LDH) & alkaline phosphatase(ALP) enzymes & serum calcium. All cases were subjected to bone marrow examination includingboth aspirate & trephine biopsy (with its touch imprint). Smears & sections were examinedthoroughly to assess marrow status with emphasis on the existence of bone marrow necrosis(BMN).Results showed that 19.1 % of cases had bone marrow necrosis. Their mean age was 50.25years, with equal sex distribution, half of them had primary tumors, and 75 % of them had theirtumors situated in the spine. Clinically: all of them had weight loss, fever & bone pain while 75% of them had pathological fractures. The mean values of the investigations done for themwere: Hb 94 g/L, PCV 0.29, reticulocyte count 2.43 %, WBCs 14.75 x 109/L. Peripheral bloodfilm for all showed leuco-erythroblastic picture. They showed an elevated mean serum LDH(307.50 IU/L) & ALP (116.06 IU/L) levels.Cases that showed no evidence of BMN constituted 80.9 % of total. They had a mean age of5.15 years. 52.9 % were males while 47.1 % were females. Just more than half of cases (52.9%) had primary while 47.1 % had secondary type of tumors & 47.05 % of them were located inthe spine. Clinically all of them had bone pain, 94.1 % had weight loss, 52.9 % pathological fractures,29.4 % pallor & 23.5 % had fever. Their mean laboratory values were: Hb 108.1 g/L, PCV0.33 %, reticulocyte count 1.36 %, WBCs 7.11 X 109/L, LDH 214.88 IU/L, ALP 83.48 IU/L.Comparative study between cases with BMN & those without showed a significant relationshipbetween the following parameters & the presence of BMN at a level (p<0.05): fever, leukocytosis,reticulocytosis, and high LDH levels. These results were comparable with some & contradictingwith other studies.The paucity of studies in this field created difficulties to relate the results of this study to others,especially in our Country. Further studies in the same field will probably clarify.In conclusion, bone marrow necrosis, is not uncommon among primary & secondary malignantbone tumors. It is associated with severe clinical-pathological features that may reflect a pooroutcome. Prolonged, follow up studies are in need to clarify this point.

Abstract

Lymphomas of the bone and musculoskeletal tissue are uncommon. Yet, they constitute a certain frequency that they merit certain consideration. A study was conducted to clarify the behavior of such type of lymphomas in the South of Iraq, epidemiologically, clinically, pathologically, biochemically and immunologically. Results show that they form minority among total lymphoma cases, but they were the third common type among extranodal lymphomas with male gender, adult age and urban residency predominance. There was a predominance of low performance status, absence of B symptoms, neurological and musculo-skeletal manifestations, secondary bone, especially the spine, involvement. Non- Hodgkin lymphoma (NHL) was the most predominant, mainly the intermediate grade, with diffuse large cell lymphoma (DLCL) and Burkitt lymphoma (BL) as the main histological types encountered. Hematologically, there was mild anemia, rapid ESR (> 50 mm), normal platelets count in the majority, leucocytosis in < half and monocytopenia predominance. Biochemically they were characterized by normal liver function tests with the exception of high serum alkaline phosphatase, normal total serum proteins, hypoalbuminemia, normal serum urea, creatinine & uric acid, hypercalcemia, with serum LDH and B2MG elevation in the majority. Serum anti EB virus antibodies were seen in 1/4th of cases and the majority were of the higher IPI scores (both high intermediate & high). Those results were comparable to some and contradicting to other studies elsewhere.

Abstract

Beta 2 microglobulin is a known marker used in the follow up and monitoring therapy of patients with hematological malignancies. However, its assistant role in the diagnosis of some of them, like lymphomas is less highlighted. Thus, this study was designed as a part of a larger, wide scale study, to clarify the help of B2MG in the support of the diagnosis of different types of lymphoma. A total of 669 newly diagnosed, pre-treated lymphoma cases were investigated for B2MG using the ELFA/mini VIDAS system and the results showed an elevated level of B2MG among both Hodgkin & non-Hodgkin, adults and childhood and nodal and extranodal lymphomas, with a significant increase among non-Hodgkin and extranodal type of lymphomas, a finding that may make its use as a diagnostic marker is helpful. Those results were comparable to some and contradicting to other studies. Further future studies are in need to consolidate this.

Abstract

Abstract The role of platelet-rich plasma in the pain relief and treatment of many orthopedic problems had gained lot of studies & practice. Yet, it hadn’t been practiced in our locality. Thus, the study of its role in the treatment of certain enthsiopathies (plantar faciitis, achillis tendinitis and lateral epicondylitis) had been planned for. A total of 63 cases of the three diseases were chosen for a case control study. They were divided into two groups: the case group who had been treated with local injection of autologous platelets rich plasma (prepared by the Trima accel cell separating machine) and the control group who were treated by local steroid injections. Pre and three post-treatment follow up of cases were done to assess the pain perception level using the simple visual analog scale (VAS). Results had shown a statistically significant reduction in pain among cases compared to control. These results were comparable to many studies elsewhere in the world. This had led us to conclude the advice to encourage this type of therapy on a large scale of patients in the future with more detailed further studies about.
