Abstract

ABSTRACTThe data of this study of cleft lip & Palate are related to a sample of 638 patients who were born with this deformity andwere referred to the plastic unit between1994-2003. These patients were evaluated in respect to age at presentation,family history, and type of deformity, sex ratio, laterality and clinical severity. All patients were operated on then followedup for a period of 5 months to 2.5 years. The results of the analysis of the data show that there is steady increase in thenumber of referred cases each year. The bulk of the orofacial defect is the isolated cleft lip, which account for 44.4% ofthe cases with male predominance except in cleft palate. Left sided cleft with or with out palate was found to be as twicecommon as right side cleft lip. The unilateral cleft is three times as bilateral; while in cleft lip and palate it's twice ascommon. The increase in the number of referred cases may reflect the increase in the birth rate and also increase in thenumber of cases due to increase environmental pollution.

المعلومات الواردة في هذا البحث حول الشق الولادي للشفة العليا و سقف الفم مأخوذ من مجموعة 638 مريض ولدوا بهذا التشوه وتمت إحالتهم لشعبة الجراحة التقويمية من مختلف مناطق جنوب العراق خلال الفترة مابين 1994- 2003. تمت دراسة هؤلاء المرضى بالنسبة للعمر في المراجعة الأولى، وجود تشوه مماثل في العائلة، نوع التشوه، نسبة نوع التشوه بين الذكور و الإناث، النسبة لجهة اليمين أو اليسار وأخيرا شدة التشوه. كل المرضى المشمولين بالدراسة تم فحصهم وإجراء العملية اللازمة لهم ومتابعتهم للفترة ما بين خمسة شهور و سنتان ونصف من قبل نفس الجراح. وقد تبين من نتائج هذه الدراسة إن هناك زيادة سنوية في عدد المرضى المراجعين وان غالبية التشوهات هي شق الشفة العليا وحيدة الجانب (44.4%) وان الذكور يكونون الغالبية بين الجنسين إلا في شق سقف الفم حيث الغالبية من الإناث. وتبين أيضا أن شق الشفة العليا للجهة اليسرى مع أو بدون شق سقف الفم هو ضعف مثيلتها في الجهة اليمنى. و شق الشفة العليا للجهة الواحدة يعادل ثلاث مرات شق الشفة للجهتين ولكنها تعادل مرتين في حالة وجود فتحة في سقف الفم. إن الزيادة السنوية في عدد المرضى قد تعكس زيادة في عدد الولادات وقد تعكس أيضا زيادة في التلوث البيئي؟.

Abstract

The Van der Woude syndrome is a rare autosomal-dominant craniofacial disorder. In its classical form consisting of a cleft lip or cleft palate, distinctive pits of the lower lips and hypodontia. This is a review of a family with VWS. The family pedigree, past history and clinical features were reviewed. The related literatures were studied, which show the variable presentations including lip pits alone, absent teeth, or isolated cleft lip and palate of varying degrees of severity, and the points for early diagnosis.

Abstract

This is a retrospective analytic study of the data collected from inpatients admitted to Basrah Teaching hospital during the period 1992 �1998. They were 1726 patients (not include those treated as outpatients) classified as males & females, adults and children (<12 years of age) with an average annual admission of 246.5 patients. The bulk of the cases are adult females and children males. Sex ratio is 1.1: 1 but with variation according to age. Adult to children ratio is 1.4: 1. Seasonality is not that clear but showed some increase in winter months. The flame burn cases formed the bulk of the burn being 52.9% followed by scalds and electric burn. The most common place of burn is home and especially the kitchen and bathroom. Overall mortality is 23.1% with age and sex variation.

Abstract

Gigantomastia or “Gravidic macromastia” during pregnancy is a massive diffuse enlargement of the breast during gestational period. It is a rare condition of undetermined aetiology, which may be due to hormonal excess, or hypersensitivity of the target organ to normal hormonal level. Histologically it is due to glandular hyperplasia with an increase in connective tissue. This paper reports an unusual case of gigantomastia in 22 years old lady, who is gravida 2, para nil, with total breast weight of 15,850 kg. Ulceration and haemorrhage of the breasts complicated the picture and end in abortion followed by subtotal mastectomy and free implantation of nipple areola complex.

Abstract

Earlobe keloids are commonresponse to ear piercing, especially infemales and darker skin individuals9.Keloids on the ears present severaltherapeutic challenges. They arecommon after small skin excisions andother procedures, including drainage ofauricular hematomas, repair of otherauricular traumas or as secondarykeloid formation after prior keloidexcision. Several treatment modalitiesare used for keloid such as surgeryalone or surgery combined with othermeasures with varying success rates.Today there is no agreement aboutwhich treatment modality willsignificantly solve the problem10.This paper report a case of a largebilateral post burn ear keloid on thehelical rim which was successfullytreated with surgery and intra-lesionalinjection of 5FU and Triamcinolon.

INJECTION OF BOILING HYPERTONIC SALINE IN THE TREATMENT OF CUTANEOUS HAEMANGIOMA

Abstract

Abstract This study deals with a new way for the treatment of cutaneous haemangiomas in children and adults by using a boiling hypertonic saline, the material used is safe and nontoxic even if absorbed. A total of 309 patients with 354 haemangiomas were studied. The sample consisted of 233 females and 76 males ranging in age from 2 months to 50 years. Patients were divided into three groups; Group I: observation, Group II: excision and reconstruction, Group III: injection & excision. Each patient in group III received 2-4 shots of boiling saline intra-lesional at an interval of 3-4 weeks, to allow time for the post injection swelling to subside and fibrous tissue to mature. The female to male ratio was 3.1:1. The lesion appears within the first weeks after birth in 189 children (61.3%). Among the 309 patients, 288 (81.4%) haemangiomas were located on the head and neck, 39 (11%) on the trunk, 25 (7%) on the extremities, and 2 (0.6) in the perineal area. The three groups are as follows: group I: observation; 161 patients (52%). group II: excision and reconstruction. 42 patients (13.7); group III: injection & excision, 106 patients (34.3%). The presenting symptoms at consultation were parents worry 200 patients (64%), obstruction of orifices 38 (12.3%), ulceration 32 (10.6 percent), bleeding 26 (8.2%), infection 10(3.2%), and pain 3 (0.8percent). The complications after treatment of the 106 patients were; bleeding 3 patients (2.3%), infection 7 (6.8%), skin necrosis 20 (19.3%); the remaining patients 76 (71.6%) tolerate the injection easily and show no effect. The evaluation of patients treated with boiling saline sclerotherapy is based on improvement of volume, color, and texture as follow, poor 10 (9.1%); good 54 (51.1%); and excellent 42 (39.8%). In conclusion, treatment of cutaneous Haemangioma is indicated when there is no sign of involution. Injection sclerotherapy with boiling saline proved to be safe, nontoxic, cheap and effective.

Abstract

The right wrist of twenty cadavers were examined to measure the three dimensions of the carpal tunnel by using a Vernia (INOX), the cross sectional area and volume were then calculated for males and females separately. All were above 40 years. Males were found to have larger carpal tunnel than females of the same age group, both in cross sectional area and volume. It was found also that the volume of the tunnel change according to the age. The pattern by which the volume changes is different in males and females. In active age group i.e. fourth to sixth decade, the tunnel volume in females’ collapse but it expand in males, this may explain the high incidence of carpal tunnel syndrome in females in this age group and the low number of affected males. The age / volume relation may shed some light on the etiology of the “Idiopathic” carpal tunnel syndrome

Abstract

Eighteen patients who sustained severe open leg fractures (Gustilo�s III) were treated in ALBasrahGeneral Hospital and Al-Sadder Teaching hospital between 2007 and 2008. Theywere12 males and 6 females, average age were 25 years. Middle third fractures occurred in 8cases (44.4%) and the comminuted geometry of fracture was in 14 patients (77.7%). All patientswere treated by meticulous wound excision with stabilization of fracture and then wound cover,which is done early in 6 patients (before 7 days) and late in 12 patients (after 7 days). Localmuscle flap was done in 9 patients (50%), local fasciocutanous flap in 6 patients (33.3%) andfree flap in 3(16.7%). The complications occurs more in late local reconstruction as flap infectionin (55.5%), nonunion in (33.3%), flap necrosis in (22.2%) and osteomylitis in (33.3%) while inearly reconstruction it is found only (16.6%) as a complication of each of the above. In muscleflap, flap infection found in (22.2%), flap necrosis in (11.1%) and nonunion in (22.2%) which waslower than other type of reconstruction, the muscle flap was more reliable flap in early period ofreconstructions. In conclusion, primary reconstruction of Gustilo's type III open tibial fractureshad advantages compared with secondary reconstruction.
