Abstract

A serological study has been done in Al-Nassiriyha, southern Iraq, from December 2006 toAugust 2007 designed to determine the frequency of anti-cyclic citrullinated peptide antibody(anti-CCP) in a group of patients with rheumatoid arthritis for specific period of time comparedwith other rheumatic diseases and healthy controls. Also to show the diagnostic value ofanti-CCP antibody in relation to other markers [rheumatoid factor (RF), antinuclear factor (ANF),C-reactive protein (CRP) and cytokines such as interleukin–6 (IL-6), tumor necrosis factor–α(TNF-α)] to discriminate between those patients with and without RA.Blood samples were taken from 121 patients with rheumatoid arthritis, 88 patients of otherrheumatic diseases from AL-Hussein Teaching Hospital in AL-Nassiriyah and 120 healthycontrols from medical personnel and school students who were healthy. All the information wastaken from the patients and controls.An Enzyme-linked immunosorbent assay was used for estimation of anti-CCP, IL-6, TNF-α andslide agglutination test for the estimation of RF, CRP and ANF. The seropositivity to anti-CCP inRA patients group was 61.2% among them 23% with low levels of anti-CCP, 25% withmoderate levels and 51.3% with high levels (> 60 units /ml.).The high levels of anti-CCP persistfor long durations up to 10 years (54.7%). However, anti-CCP was rarely detected among non-RA rheumatic diseases and healthy controls (2.3% and 2.5% respectively). Anti-CCP showedthe greater specificity (97.6%) and sensitivity (61.2%) compared to the other marker used in thisstudy. The overall agreement between the tested parameters in their ability to detect RA patientwas higher for anti-CCP combined to IL-6 (91.7%) in comparison to the effect of other markerscombination. Although anti-CCP was more common in RA patient with 2-10 years duration ofillness, anti-CCP, IL-6 and TNF-α positivity as estimated by ELISA were persistently with hightiters regardless the duration of illness. The majority of RA patients with high anti-CCP levelswere among stage three of clinical RA. Smoking has a positive relation to the increased levelsof all tested parameters compared to the non-smokers.

Abstract

ABSTRACT: BACKGROUND :The incidence of rheumatoid arthritis[RA] varies from country to another which not depend on genetic bases alone but can explained by environmental exposure and genetic and environmental interaction, area of military conflict ,there was stress induced pathway that elevates the risk for rheumatoid arthritis, in Iraq rheumatoid was 1% of population in 1975.OBJECTIVE:To evaluate incidence of RA in Babylon –Iraq from 2001-2011PATIENTS AND METHODS:This study was done to examine the incidence of RA in Babylon between 2001-2011 by Retrospective cohort study in Merjan Teaching Hospital ,Rheumatology unit .Data included sex .birth year ,residency and latex test , from 2001 to 2011.All patients were diagnosed as Rheumatoid Arthritis according to American College of Rheumatology ACR 1987revised criteria of RA .the numbers of patients of RA who were received DMRADs included methotrxate therapy were recorded each year and incidence of each year was found then the cumulative incidence was calculated.RESULTS: There were1039 patients with rheumatoid arthritis from 53786 patients in rheumatology unit. 50.1% living in urban area while 49.9% in rural area. The incidence in 2001 was 1.60 while in 2011 was 3.02 for the same population and the cumulative incidence in 2011 was22.74. Latex test was 52% in2001 while in2011 was 58% in rheumatoid patients in the same year. The P value for age mean difference between female and male was [0.009].CONCLUSION:The incidence of RA was increasing from 1.60% in 2001 to 3.02% in 2011 and the cumulative risk wasb22.74% in Babylon –Iraq.KEY WARDS: rheumatoid arthritis.

Abstract

Rheumatoid factor (RF) produce in rheumatoid arthritis (RA)disease associated with effect of some environment factors and smoking . In this study, we investigated whether the association between RF production and smoking was influenced by duration of disease . the samples were taken from fifty patients with RA ,that achieved four or more criteria of the 2010 American college of Rheumatology , the sample of control 20 healthy person were used the level of RF –iso type concentration was detect in serum of patients of control group . the samples of the present study were taken from the rheumatology department of AL-Hussein General Teaching Hospital in Thi-Qar province from beginning of September 2012 into end of march 2013. The result shown high concentration of IgG -RF ,IgM-RF and IgA-RF (0.227,0.092and 0.245) respectively compare with control( 0.102,0.095 and 0.092). with high statistically significant (P<0.001).Between studied group and control with IgM-RF,IgG-RF, and slightly statistically significant between studied groups(P<0.05). ,in IgA.

العامل الروماتيزمي (RF) الذي ينتج في مرض التهاب المفاصل الرثوي بالارتباط مع بعض العوامل البيئية والتدخين , نحن نتحرى فيما اذا كان هنالك ارتباط بين إنتاج العامل الروماتيزمي والتدخين متأثرا بفترة المرض .تم تخذ عينات من خمسون مريضا بالتهاب المفاصل الرثوي والذين أنجزوا أربعة أو أكثر من معايير الكلية الأمريكية لإمراض الروماتيزم للعام 2010 وكما شملت العينة 20 شخص من الأصحاء ظاهريا كعينات سيطرة استخدمت للكشف عن مستوى تركيز الأنواع المتماثلة للعامل (RF )عينات هذه الدراسة جمعت من قسم إمراض المفاصل في مستشفى الحسين التعليمي في محافظة ذي قار للفترة من بداية أيلول 2012 إلى نهاية آذار 2013.وجد ان تركيز مرتفع من ( IgM-RF ,IgG -RF و IgA-RF) بمعدلات هي ( 0.227, 0.092 , 0.245) على التوالي مقارنة بمجموعة السيطرة (0.102, 0.095, 0.092 ) على التوالي و هذه الدراسة أظهرت فرقا معنويا عاليا مقداره (P<0.001) بين مجموعة السيطرة ومجموعة المرضى .لكل من (IgM-RF ,IgG –RF)و فرقا معنويا طفيفا (P<0.05) بين مجموعة الدراسة بالنسبة للعامل (IgA-RF).

Abstract

Rheumatoid arthritis (RA) is a common autoimmune disease characterized by chronic inflammation and joint destruction. The aim of this study was to investigate levels of Rheumatoid factor (RF) and C- Reactive protein in Iraqi RA patients. Blood samples were obtained from 50 RA patients (males and females) referred to Rheumatology consultation clinic in Baghdad teaching hospital, Medical city, with a mean age of 40.88±18.75 years. On the other hand, blood samples were obtained from 50 healthy controls with a mean age of 38.90±14.73 years. Results showed that Rheumatoid factor (RF) was detected in all serum samples taken from RA patients (100% positive results) with 100% negative results in healthy controls while C- Reactive protein (CRP) was detected in 70% of serum samples taken from the same patients.

)هو احد امراض المناعة الذاتية الشائعة والذي يسبب التهاب مزمن يصيب RA( التهاب المفاصل الروماتويدي )في CRP( )والبروتين سي الفعال RF( المفاصل .كان الهدف من هذه الدراسة هو التحقيق من مستويات العامل الروماتويدي مريض (ذكور و إناث )من 50 المرضى العراقيين المصابين بألتهاب المفاصل الروماتويدي .تم الحصول على عينات دم من سنة .من 18.75 ± 40.88 عيادات استشارية المفاصل في مستشفى بغداد التعليمي، مدينة الطب، بمتوسط عمر تتراوح مابين سنة .تم الكشف عن 14.73 ± 38.90 عينة ضبط صحيحة بمتوسط عمر 50 ناحية أخرى، تم الحصول على عينات الدم من )٪ نتائج إيجابية100( )في جميع عينات المصل المأخوذة من مرضى التهاب المفاصل الروماتويدي RF(العامل الروماتويدي ٪ من عينات 70 )في CRP( ٪ نتائج سلبية في عينات الضبط الصحيحة بينما تم الكشف عن البروتين سي الفعال 100مع المصل المأخوذة من نفس المرضى.

Abstract

This study was conducted to determine the relationship between two most common diseases in Iraqis patients , which are Diabetic mellitus (DM) and Rheumatoid Arthritis (RA); seeking rheumatoid factor in hyperglycemic sera. The results revealed that ; 62.5% of hyperglycemic (HG) patients had positive rheumatoid factor (RF) . No difference in number between both gender of HG patients (20 males and 20 females ) , RF reaction was nearly similar in males and females of HG patients ( 12 &13 respectively ) . Only 40% out of patient controls had positive RF . None of the apparently healthy subjects had positive RF .

يعد مرض السكري من أمراض العصر المزمنة والمقيتة , ومن مسببات الالتهابات. لذا تمت دراسة علاقة هذا المرض بداء التهاب المفاصل الريثاني (Rheumatoid Arthritis RA) من خلال دراسة عامل الالتهاب (rheumatoid factor RF) في مصول مرتفعي السكر( hyperglycemia). تم انتقاء (40) مريضا"يعاني ارتفاع السكر على أساس الفحص السريري والمختبري وعشرة أشخاص يعانون أمراضا" مختلفة غير السكري كمجموعة سيطرة وعشرة متبرعين من السويين ظاهريا" ولأعمار مضاهية للمرضى . تم الفحص السريري في العيادة الخارجية لمدينة الطب / بغداد . اجري العمل في المختبرات التعليمية لمدينة الطب . ومن هذا العمل المحدود لوحظ ما يلي : اظهر 62.5% من المرضى مرتفعي السكر(hyperglycemic HG) قيمة موجبة للعامل (RF) بينما اظهرفقط 40% من مجموعة السيطرة القيمة الموجبة للعامل ولم تظهر في نفس الوقت أي قيمة موجبة للعامل (RF) لمجموعة المتبرعين من السويين ظاهريا". كان عدد الرجال والنساء من مرضى مرتفعي السكر متساويا" ( 20 : 20 ). وقد اظهر الجنسين تفاعلا" ايجابيا" +ve للعامل الرثوي (RF) متقاربا" ( 12 رجلا" : 13 أمرأة ) .

Abstract

Osteoarthritis (OA) is a series of aggressive destructive inflammatory processes. Synovitis is common both at an early and a late phase. This disease may be uniquely singular in some site but phylogenetically related at some point in time to produce a common outcome of dysfunction, disability, socioeconomic destruction and sometimes socioeconomic failure. Articular cartilage, subchondral bone and synovial membrane are the site of major abnormalities in this disease process. Rheumatoid factor (RF) represents one of the routine laboratory tests that made for all patients have joint complaints.Chloroquine phosphate (CQP) is agent belong to disease modifying osteoathritic drugs (DMOADs). Chloroquine and their derivatives have been used for their anti-inflammatory effect in juvenile chronic arthritis, Sjogren,s syndrome, discoid and systemic lupus erythematosus.The aim of this trial depend on using this drug in osteoarthritic patients for two months then estimate the level of RF check and RF(A,G,M).The result showed a significant correlation between CQP and RF check as well as RF type M in patients suffered from OA, so the level of these two parameters are decreased significantly in period of treatment thus leads to ameliorate the patients status and the joints pain will decreased.

ألفصال العظمي عباره عن سلسلھ من العملیات الھادمھ العدائیھ الالتھابیھ للمفاصل . التھاب الغشاء المصلي ممكن حصولھ فيالمراحل المتطوره او المتأخره من ھذا المرض . ھناك نوعین من اسباب حصول الفصال العظمي قد بكون متمیزا ووحیدا في بعضالاماكن اومعتمدا على التطور النوعي في مواضع اخرى مما یسبب العجز وعدم الكفاءه في اداء الوظائف والعوز الاجتماعيالاقتصادي مؤدیا لفشل حركة المفاصل والعظام المحیطھ بھا كذلك الغشاء المصلي لحصول تغییرات لاطبیعیھ فیھ. تعیین عاملالروماتیزم من التحلیلات المختبریھ المعمول بھا للمرضى اللذین یعانون من مشاكل في المفاصل.عقار فوسفات الكلوركوین اثبتاستعمالھ كعامل مغیر ومضاد للالتھاب في الفصال العظمي.اما عقار الكلوراكوین النقي ومشتقاتھ استعمل سابقا في امراض عدیده منھاالالتھابات الرثویھ المزمنھ الولادیھ, متلازمة جوجرین, داء الذئبھ الاحمراریھ الجھازي و القرصي. ھدف ھذه التجربھ ھو استخدامفوسفات الكلوراكوین لفترة شھرین لغرض تعیین عامل الروماتیزم المرتبط الكلي والخاص بكل بروتین مناعي نوع( أي, جي, أم).اظھرت النتائج بان ھناك علاقھ وثیقھ ومعنویھ بینھما.أجریت التجربة على 25 متطوعاً من الأصحاء و 65 مریضاً في مستشفى مدینةالطب/قسم الروماتیزم والتأھیل الطبي تم تحلیل دم المرضى والأصحاء لتعیین نسبة العامل الرثوي الكلي ثم اجراء تحلیل آخر علىالمرضى اللذین یكون لدیھم تركیز ھذا العامل أعلى من 20 مایكرو/مل من الدم للكشف على المكونات المفصولة من البروتیناتالمناعیة المحتویة على الغلوبین وتعیین مستویاتھا و أنواعھا.صرف للمرضى عقار الكلوراكوین لمدة شھرین بجرعة 2 حبة یومیاً منقبل الطبیب الاختصاصي بعدھا عینت تراكیز ھذه البروتینات. لوحظ أن ھناك انخفاض ملحوظ و معنوي في نسبتھا مما یقلل من تھدمالأنسجة العظمیة والغضروفیة وتحسن حالة المرضى اضافة الى تقلیل الالم .

Abstract

Objective To estimate the types of hand deformities in patients with rheumatoid arthritis (RA). Methods: A group of 100 patients who had been admitted to the department of rheumatology and rehabilitation center in Sulaimany general hospital and those who had been attended to the rheumatology and rehabilitation center between June 2006 and January 2007 were included in this study.86% were females and 14% were males . Patients who had undergone articular surgery and patients below 16 years had been excluded. Results Hand deformities recorded in 47% of cases. The frequency of deformities was as follow; swan neck deformity (48.90%), boutonniere deformity (43.04%), Z deformity(59.5%), ulnar deviation of the metacarpophalangeal joints (MCPJ) (78.7%), radial deviation of the wrist (91.48%), subluxation of the wrist (27.6%), and subluxation of the MCPJ(36.17%).There was a significant correlation between disease duration and development of deformities,other factors that had been studied but had no effect on the development of deformities were rheumatoid factor positivity ;occupation of the patients ;effect of weather;and geographical distribution of the patients. Conclusion Rheumatoid arthritis is an important disease that affects the hands and cause deformities. Understanding these deformities is necessary for the proper management of these patients. This study is representative for RA related hand deformities and factors that contribute to their development.

Abstract

BACKGROUND: Pathogenetic therapy of RA is a complex task,according to modern views on the development of* Dept. of Microbiology, Medical College, Al-Nahrain Univ. ** Dept. of Clinical Biochemistry, Medical College, Al-Nahrain Univ. *** Dept of Basic Science, College of Dentistry, Univ. of Baghdad. (1). autoimmune inflammation in RA, cytokineimbalance plays a great part in RA pathogenesis Rheumatoid arthritis is a chronic systemic inflammatory disease affects many tissues and organs, butprincipally attacks flexible (synovial) joints. Methotrexate is the most commonly used disease-modifyingantirheumatic drug for the treatment of rheumatoid arthritis. OBJECTIVE:The aim of this study was to evaluate the effect of methotrexate on serum levels of IL-1α and IL-8 inrheumatoid arthritis. SUBJECTS AND METHODS: Blood samples were collected from 50 patients with rheumatoid arthritis (25 patients without treatment and 25 patients are received methotraxate) and from 30 healthy age and sex matched individuals served ascontrols. Serum IL-1α and IL-8 were measured by means of enzyme-linked immune-sorbent assay.RESULTS: The present results showed that serum levels of IL-1α and IL-8 were significantly higher in RA patientsthan in healthy controls (P<0.01), furthermore, level of IL-1α was significantly decrease in patientstreated with methotraxate as compared to those patients who have received no treatment (P<0. 01). On theother hand serum level of IL-8 did not showed any significant differences between patients treated withmethotraxate and those patients without treatment (P>0. 05).CONCLUSION: These finding demonstrate that methotrexate turns out to be a good inhibitor for IL-α production. Abstract

The study included 30 cases, 15 patients suspected with rheumatoid arthritis (RA) from margan hospital and Hilla general hospital in Babylon governorate, and 15 apparently healthy subject with no history of RA and were selected as control group. The immunoglobulins concentration of sera were estimated by using single radial immunodiffusion assay . The study showed significant elevation in concentrations of immunoglobulins IgG and IgA in patients in comparison with control group , while there was non- significant elevation in concentration of immunoglobulin IgM in comparison with control group.

شملت الدراسة 30 حالة ، 15 مريضاً يشكو أعراض التهاب المفاصل الرثوي في مستشفى مرجان التخصصي ومستشفى الحلة التعليمي في محافظة بابل و 15 من الاشخاص الاصحاء بدون علامات سريرية سابقة لوجود التهاب المفاصل الرثوي كمجموعة سيطرة . تم تقدير تركيز الكلوبيولينات المناعية المصلية للمرضى والاصحاء باستخدام طريقة الانتشار المناعي الشعاعي المنفرد، و أظهرت الدراسة وجود ارتفاعاً معنوياً في تراكيز الكلوبيولينات المناعية IgG و IgA عند المرضى مقارنة مع مجموعة السيطرة ، في حين لوحظ وجود ارتفاع غير معنوي في تركيز الكلوبيولين المناعي IgM مقارنة مع مجموعة السيطرة .
