Evaluation of treatment of intra-bony defects with a mixture of β-tricalcium phosphate - hydroxyapatite granules and oily calcium hydroxide suspension

Abstract

Background: An oily calcium hydroxide formulation proved over the last years to be highly efficient in promotingbone regeneration in closed defects as periapical lesions, cysts, or post-extraction defects. The aim of the presentstudy is the assessment of the outcome of treatment of deep intrabony periodontal defects with an Open FlapDebridement) (OFD) + combination of {(30% Hydroxyapatite HAp + 70% β-Tricalcium Phosphate granules mixed withan Oily Calcium Hydroxide Suspension (OCHS )} and compare the results with {(OFD) alone)}. The combination ofOCHS& TCP was used in humans with a sort of positive results, and more conduction of studies was recommended.Material and method: The sample of this study composed of sixteen patients; each had at least two intrabony defectteeth. For each patient, one tooth was treated by OFD only (control group 16 teeth) and the other teeth (studygroup 16 teeth) were treated with OFD + a mixture of (OSTEON II- Korea) granules comprise of (30% HydroxyapatiteHAp + 70% β-Tricalcium Phosphate) and OCHS (Osteoinductal®, Osteoinductal GmbH., Muenchen, Germany). Theclinical parameters that were measured and recorded included (Plaque index PLI, Gingival index (GI), Gingivalrecession(GR) , Bleeding on probing (BOP), probing pocket depth (PPD), clinical attachment level (CAL) andradiographical width & depth of the bony defect. These parameters were recorded before treatment (base line) &six months after the treatment.Results: The results after treatment revealed a highly significant reduction in all of the parameters except the PLI inboth study & control groups compared to baseline (p < 0.001). The clinical results were indicated that the studygroup showed significant reductions of PPD and CAL mean values more than improvements obtained by the controlgroup with significant difference (p < 0.05).Conclusions: An excellent postoperative improvement in clinical parameters was noticed in the study group morethan control group and the differences were significant. Thus the present study has revealed that the treatmentmodality of OFD + a combination of granules comprise of (30% Hydroxyapatite HAp + 70% β-Tricalcium Phosphate)mixed with oily Calcium Hydroxide suspension is successful, predictable and more beneficial than (open flapdebridement) (OFD) alone in the treatment of deep intrabony periodontal defects

Abstract

Alveolar atrophy following tooth extraction remains a challenge for future dental implant placement. Immediate implant placement and post extraction alveolar preservation are 2 methods that are used to prevent significant post extraction bone loss. Immediate extraction site grafting is a simple and straightforward procedure. Purpose: To determine the efficiency of Resorb β Tricalcium phosphate (βTCP)bone graft material in preservation alveolar bone after tooth extraction.Materials and Method: Thirty Patients requiring tooth extraction and alveolar bone regeneration for the resulting bone defects by Resorb β TCP(Bone Medik –DM Bone, Biphasic Calcium Phosphate ,Synthetic Resorbable material Si-HA (60%):β –TCP(40%)bone graft material were included in this study. Results: Histopathological examination of the biopsy specimen showed new bone deposition. Radio graphical radio densities of the regenerated bone were evident in treated sockets during the follow up period. Statistical analysis indicated that the healing percentage was 70%-80% in 13 patients and the main among 25-35 age group. Conclusions: Resorb β Tricalcium bone graft material was successful material in reconstruction of bone defects following tooth extraction when compared with other graft materials.

الملخصان انكماش العظم المحيط بالسن بقي مشكلة لوضع غرسات الاسنان في المستقبل. ان الوضع المباشر لغرسة الاسنان وتعويض انكماش العظم المحيط بالسن بعد القلع هي طريقتان تستخدم لمنع فقدان الواضح للعظم المحيط بالسن بعد القلع. ان استخدام المادة المكونة العظام في مكان القلع مباشرة هي طريقة سهلة و مباشرة. الغرض: لتحديد فعالية المادة المكونة العظام التراي كالسيوم فوسفيت القابلة للامتصاص في الحفاظ على العظم المحيط بالسن بعد قلع الاسنان. المواد و طريقة العمل:30 مريض بحاجة الى قلع اسنان و تعويض العظم المحيط بالأسنان و تعويض عيوب العظام باستخدام مادة التراي كالسيوم فوسفيت المكونة للعظام القابلة للامتصاص.النتائج: ان الفحص النسيجي للعينات اظهرت تكون عظم جديد و العتمات التي ظهرت في الاشعة للعظم المتكون في تجويف السن المعالج في فترات المتابعة.الاستنتاج: المادة المكونة للعظام البيتا تراي كالسيوم فوسفيت القابلة للامتصاص هي مادة ناجحة في اعادة بناء عيوب العظام بعد القلع عند مقارنتها مع مواد اخرى مكونة للعظام
