Non-opioid analgesia with Regional block: The effect of ketamine as adjuvant with 0.5% bupivacaine in infraclavicular brachial plexus block

Abstract

Aims: To determine the effect of the addition of 25 µg/kg body weight ketamine to 0.5% bupivacaine in infra-clavicular brachial plexus block. Patients and Methods: In a randomized double blinded, placebo controlled clinical trial, Forty patients, from the age of 20-60 years, with ASA I, II and III, undergoing upper limb (below elbow) surgery, had been chosen to be enrolled in the study. They had been divided randomly, into 2 groups. Group 1 will receive 30 ml of solution 1 (25 ml of 0.5% bupivacaine with 5 ml of normal saline) as placebo or control group. Group 2 will receive 30 ml of solution 2 (25 ml of 0.5% bupivacaine with 25 µg/kg body weight ketamine) as ketamine group. A nerve stimulator (B/Braun Stimuplex® HNS 11) had been connected to 22G-10 cm length insulated needle. Time of motor impairment was assessed by lack of muscular coordination and heaviness feeling of the limb. Time of parasthesia was assessed by loss of pin brick sensation.Results: There was a quite noticeable prolonged time of analgesia in the ketamine group. The mean ± SD was 698 ± 139 minutes in ketamine group while the mean ± SD in placebo group was 140 ± 40 minutes (p<0.05). Twelve patients out of 20 had recorded 800 minutes of analgesia (60%) in ketamine group. There were no other clinical differences in side effects or motoric latency in between both groups. Conclusions: Low dose of ketamine added as an adjuvant to 0.5% bupivacaine in infra-clavicular brachial plexus block, will prolong the time of post-operative analgesia without any side effects.

High- temperature oxide superconductivity of Bi-Sr-Ca-Cu-O system and Tl-Ba-Ca-Cu-O system without any rare- earth elements were discovered by Maeda et al (1) and Hermann (2),respectively. Maeda and co-workers observed a superconducting onset temperature of about 120K,which was higher than that of Ba2YCu3O7 by more than 10K.Takano et al.

Abstract

The present work aims to study the performance of a trickle bed reactor for phenol degradation in refinery wastewater by investigating the applicability of a commercial 0.5% platinum/alumina catalyst, which is used currently for desulfurization process in the North Refinery Company-Iraq. Synthetic wastewater experimentally was prepared to simulate the specification of samples taken from North Company Refinary. Results showed that initial phenol concentration had adverse effect on phenol removal. The results exhibited that the highest phenol conversion of (98.47%) was obtained over 0.5% Pt/γ-Al2O3 at the studied conditions (i.e., operating pressur 0.8 Mpa, operating temperature 1200C, LHSV 2.5 h-1, and air superficial velocity 0.25 m/s), with initial phenol concentration of 200 mg/L. The present method characterized by low residence time, the end-products are environmantally harmless no sludge production which requires further treatment

تضمن البحث دراسة كفاءة اداء عمود الطبقة الوشلة لازالة الفينول من مياه الصرف للمصافي النفطية بأستخدام %Pt/Al2O30.5 كعامل مساعد والمستخدم حاليا في وحدة ازالة الكبريت " desulfurization process " في مصافي الشمال. النتائج أظهرت ان اعلى نسبه تحول للفينول ((98.47 تم الحصول عليه تحت الظروف التاليه (LHSV 2.5 سا-1, درجة حرارة=120 م°, الضغط التشغيلي =0.8 ميكاباسكال والتركيز الأبتدائي للفينول=200 مغم/لتر وسرعة الغاز 0.25 m/s). وان النواتج النهائية في هذه الطريقة المستخدمة في البحث لا تحتاج الى معالجات اضافية اخرى.

Abstract

In this research study Hardness (shore D), Water absorption, Flexural, Impact Test, and Fracture Toughness of polymer nano composites.The polymer nano composites based on unsaturated polyester resin reinforced with Kevlar fibers (K.F). The samples are attended by hand lay – up method according to (Rule mixture) for various volume fractions of unsaturated polyester resin, fiber and carbon nanotube. The polyester resin was matrix strengthened with 3% volume fraction from Kevlar fiber and (0.5%, 1%, 1.5%, 2%( volume fractions of carbon nanotube. The water absorption, hardness (shore D), flexural test, impact testand toughness fracture properties were studied. Results showed that the water absorption increase with volume fraction increase of fiber with Carbon Nanotube, the sample (polyester+3%K.F+0.5% CNTs) has lower water absorption than other samples. The hardness (shore D), flexural test, impact testand toughness fracture for the sample (polyester+3%C.F+0.5% CNTs) has higher value for Nano- composites. Nano composites, Carbon nanotube, Kevlar fiber, Hardness (shore D), Water absorption, Flexural Strength, Impact Test, Toughness Fracture.

تم في هذا البحث دراسة الصلادة (شورD), امتصاصية الماء, مقاومة الكسر, اختبار الصدمة و متانة الكسر لمتراكبات نانوية ذات اساس راتنج البولي استر غير المشبع مقواة بألياف الكفلر. العينات جهزت بطريقة الصب اليدوي وفقالقاعدة الخلط لكسور حجمية مختلفة من راتنج البولي استر غير المشبع والالياف وانابيب الكاربون النانوية. راتنج البولي استر كان ارضية مقواة مع 3%كسر حجمي من الياف الكفلر و (0.5 ,1 ,1.5 , 2 ) % كسر حجمي لانابيب الكاربون النانوية. تم دراسة خواص امتصاصية الماء, صلادة (شور(D, اختبار الصدمة ومقاومة الكسر. اظهرت النتائج بان امتصاصيه الماء تزداد مع زيادة الكسر الحجمي للالياف مع انابيب الكاربون النانوية, العينه (بولي استر + 3% الياف كفلر + 0.5 % انابيب اكاربون النانوية) تمتلك اقل امتصاصية ماء من العينات الاخرى. الصلادة (شورD), فحص الكسر, فحص الصدمة ومتانة الكسر للعينه (بولي استر +3% الياف كفلر + 0.5 %انابيب الكاربون انانوية) تمتلك اعلى قيمه للمتراكبات النانوية.
