Abstract

Background: Autoimmune hepatitis (AIH) is a rare chronic liver disease of known etiology, characterized by hypergammaglobulinemia, characteristic auto antibodies, and a favorable response to immunosuppressive treatment.Strong circumstantial evidences denoted that there is quite long list of environmental factors such as (food additives and drugs), viruses and toxins which play an important role in precipitating this disease.
Patients and Methods: the study was performed on 13 Iraqi patients with acute viral hepatitis, attending the Teaching Hospital for Gastroenterology and Liver Disease and Al-Khadymia Teaching Hospital in the period between November 2003 and July 2005. ANA, SMA and anti-LKM 1 were detected by immunoflurescent technique whereas SLA/LP Ab was detected by Euroline method.
Results: the thirteen patients with acute viral hepatitis were developing AIH after 3 months.Conclusion: acute viral hepatitis is a trigger of AIH.

A comparison of the prevalence of anti-Liver/Kidney Microsome antibody type-1(LKM-1) in individuals with chronic hepatitis C and those with autoimmune hepatitis

Abstract

Summary:Background: Autoantibodies are frequently found in patients with chronic hepatitis C, which suggests that HCV elicit an immune response in the host. ). The relationship of type-2 AIH to chronic hepatitis C (HCV) is an interesting and as yet unresolved problem. Importantly, antibodies to liver/kidney microsome type-1 (LKM1), the serologic marker of type 2-AIH, have been recognized in serum of some patients with chronic hepatitis C . Methods: Anti-liver kidney microsome type 1 (LKM-1) autoantibodies were studied by indirect immuno florescence assay (IIF) and confirmed by immunoblot in the serum of 73 Iraqi patients with autoimmune hepatitis (AIH) in comparison with 50 patients control (HCV infection) and 50 healthy individuals.Results: anti-LKM 1 with high titers (1/160) present in serum of all patients with AIH-2, whereas they present in titer (≤1/80) in serum of 19 patients with chronic HCV, while all healthy control group are negative.Conclusion: AIH is easily distinguished from chronic viral hepatitis since patients with AIH are more commonly women than those with chronic viral hepatitis, and they have higher serum levels of aspartate aminotransferase, bilirubin, gamma-globulin, IgG, alkaline phosphates, and higher frequency of multilobular necrosis on histologic examination than counters with chronic viral hepatitis. Abstract

The diagnosis of Autoimmune hepatitis type-1 was studied in 167 blood samples which collected from Aleppo university and Zahi Azrak Hospitals in Aleppo, patients were complain in symptoms of hepatitis during the period from 4 – 2008 to 4 - 2009 . By determining special auto antibodies existence and in the comparison technique will be used the (ELISA), and the (IFA) technique. The results did not showed any infection with autoimmune hepatitis type-2, while the disease percentage in the type-1(AIH) 27.0%, the ANA was 13.33%, ASMA percentage 33.33%, and the AMA , 13.33%, The results did not show LKM-1 & SLA. (LKM-1 special in the Autoimmune hepatitis type-2) of the studied patients.

تم فحص 167 عينة دم لتشخيص التهاب الكبد المناعي الذاتي من النمط الأولType-1 ، والتي جمعت من المرضى الذين يشتكون من أعراض التهاب كبد والمراجعين لعيادات مستشفى حلب الجامعي ومستشفى زاهي ازرق (مستشفى الحميات) في حلب، وذلك خلال الفترة بين 4-2008 و 4-2009 بغية التحري عن الاضداد الذاتية الخاصة بالتهاب الكبد وذلك باستخدام تقنية ELISA وتقنية الفلورة المناعية غير المباشرة.IFA لم يتم إثبات وجود النمط الثاني Type-2 من التهاب الكبد المناعي الذاتي بالتقنيات المستخدمة. وقد كانت نسبة الإصابة بالنمط الأول 27.0%، كما تبين أن نسبة الاضداد الذاتية للنواةANA 13.33%، أما نسبة الاضداد الذاتية للعضلات الملساءASMA فكانت 33.33%، ولأضداد الميتوكوندريا AMA 13.33%، ولم تظهر الاضداد الذاتية من نوع SLA للنمط الأول، وكذلك أضداد LKM-1 والخاصة بالنمط الثاني من التهاب الكبد المناعي الذاتي أي ايجابية لدى العينات المفحوصة .
