Abstract

Background: Malocclusion is one of the main problems concerning the oral cavity. This study examined the possible effect of mouth breathing habits in patients with respiratory tract allergies on the dental occlusion.Materials and methods: The study was conducted on 150 patients having allergic rhinitis, asthma or both ranging in age from 5-19 years. A group of 150 healthy individuals matched with gender and age to the study group was used as a control group. The total sample was questioned about their mode of breathing then their occlusion was examined clinically.Results: Significantly higher percentages of mouth breathers were found among the study group (P<0.05). Similarly CLII and CLIII dental occlusion, displaced teeth as well as crowding affected significantly greater percentages of allergic patients in comparison to the control group.Conclusion: Mouth breathing habits in patients with respiratory tract allergies may have an additive role in the development of some undesirable malocclusions

Abstract

background :Asthma is a chronic inflammatory disorder of the airways in which many cells and cellular elements play a role, leading to recurrent episodes of wheezing, breathlessness, chest tightness, and coughing. Although clinicians are satisfied with this definition, epidemiologists are still searching for their own. Studies on asthma used either questionnaire or histopathophysiologic way to define asthma each has its privileges and drawbacks. Asthma has become more common in both children and adults around the world in recent decades. Its prevalence has been dramatically increased during last few years, but still no definite asthma prevalence is specified in Iraq.Aim of the Study : To define the prevalence of some risk factors in Iraqi asthmatics, patterns of presentation, and to find the level of education about asthma among the patients, this study was conducted.Method :Two hundred twenty five asthmatic patients diagnosed by questionnaire, supported by clinical detection of wheeze, were included in a cross sectional study done in outpatients clinic, Baghdad Teaching hospital in Baghdad, collected from December 2007 to March 2008.Results : mean age for the study sample is 38 years, 50 are men and 175 are women. Normal body mass index reported in 86 patients, 78 patients (33.7 %) have positive family history of allergic diseases, 110 patients reported exposure to environment with house dust mite, and 49 patients (21.7%) were passive smoker. Persistent symptoms are commonest pattern of asthma reported in 157 patients. Eighty eight patients use their treatment regularly, and 31 patients used steroid inhalers. Proper usage of inhalers as assessed by demonstration found in only 39 patients. Spirometry or pulmonary function tests are performed only in 11 patients (4.9 %) for diagnosis or asthma follow-up.Conclusions : High body mass index, exposure to an environment in favored for house mite inhabitance, family history of atopy, history of allergic diseases, Gastro-intestinal symptoms suggestive of gastroesophageal reflux disease and Allergic rhinitis and sinusitis are the commonest risk factors reported by asthmatics.Irregular treatment with inhalers especially corticosteroid and their improper usage are still widely spread among our patients, also spirometry and other pulmonary function tests are still rarely used in diagnosis, and management of asthmatic patients.

SEROLOGICAL STUDY FOR SOME CHICKEN ALLERGENS IN ALLERGIC PATIENTS

Abstract

An allergic extracts from chicken feather and dropping were prepared with extraction, followed by purification using dialysis.Total and specific IgE ELISA was performed on a total of 190 serum samples collected from allergic and healthy individuales in center of asthma and allergic diseases in Basrha city during September 2011.ELISA test based on total IgE results revealed that the higher rate of allergic patients (47.37%) had allergy questionable while (35.26%) of them had allergy very probable and (17.37%) of them infected with allergy not probable. According to the relationship between the symptoms of allergy and total IgE based ELISA results, the higher rate of allergy very probable 95.5% was observed in symptomatic patients.According to the results of ELISA based on specific IgE the overall rate of chicken allergens in studied patients 94.7%. Chicken dropping allergens showed the higher overall 94.7% of distribution followed by the rate of feather allergens 73.68%. Depending on the sex, age (first age group range from>15-45 and second age group range from <45-75) and type of sensitivity to all tested allergens, males and the patients of second age group showed higher rate of sensitivity ( 80.35% and 80% respectively ), while in patients who were sensitive to single allergen (Dropping) the higher rate of sensitivity was observed in females and first age groups patients (23.88% and 15% respectively).The estimation of IgE seropositivity in symptomatic and asymptomatic individuals resulted the symptomatic patients showed the higher rate of seropositivity ( 95.8% )against chicken dropping allergens, while the higher rate of seropositivity (75%) was observed against chicken feather allergens in asymptomatic patients.According to the relationship between total and specific IgE based ELISA results, the patients with allergy very probable showed the higher rate of seropositivity for both dropping and feather allergens (98.5% and 76.1% respectively).The positive feather and dropping based ELISA results (mean± SD) of the allergic patients sera depending of sex and age revealed different mean ± SD value of optical density in concern to sex and age of allergic patients

Abstract

Eighty eczematous patients were included in this study from both sexes andtwo age groups, adults were (52) patients aged (18-70) years and children were(28) included aged (3-17) years. They were attending Department of Dermatologyin Ramadi General Hospital Ramadi, Al-Anbar Governorate West of Iraq,during the period extended from April2005-March 2006. Patients were submittedto a clinical examination and questionnaire test. Data were analyzed using Chi -square and T test.Results showed that females within age group (3-17) years showed higherrate of eczema than males. Adult males showed higher of eczema , (18: 80 )patients were showing familial link, adult females were more allergic to variety offood. Out of the total studied patients (61.25%) showed seasonal eczema while38.75% of them showed eczema at any season

الخلاصة17 ) سنة - تضمنت الدراسة مرضى من الجنسين( 28 ) من الأطفال الذين تراوحت أعمارهم( 370 ) عاما جميعهم راجعوا قسم الأمراض الجلدية في - و( 52 ) من البالغين الذين تراوحت أعمارهم( 18مستشفى صدام العام في الرمادي / محافظة الانبار غرب العراق خلال الفترة من نيسان 2005 إلى آذار2006 ، علما أن جميع المرضى خضعوا لاختبار استمارة الاستبيان والفحص السر يرى .استخدم مربع كاىلمقارنة النتائج. بينت النتائج أن الإناث من الأطفال كانت لديهم أعلى نسبة إصابة من الذكور أما الذكورالبالغين فكانت لديهم الإصابة أعلى من الإناث البالغات كما وبينت الدراسة أن( 18:80 )مريضا كانت لديهمعلاقة بين عامل العائلة والإصابة . تبين أن الإناث البالغات كان لديهن تحسس لمختلف الأغذية . كما تبينأن( 61.25 )% من المرضى كانت لديهم علاقة بين الاكزما وفصول السنة في حين أن( 38.75 )% منالمرضى كانت لديهم اكزما على مدى فصول السنة

Abstract

Objective: The aim of the study is to determine the most common allergens triggering allergic disease in Diyala Governorate.Materials and Methods: This study including 391 patients (148 male and 243 female ) with bronchial asthma attending asthma and allergy unit of Baquba teaching hospital consultation clinic from 4/1/2009 to 31/12/2009. skin prick test were done for them ; the results were fixed according to the reactivity of the allergens used in the study.Results: Most common offending allergens were house dust mite( 25.3%), Bermuda 23% , grasses 19.6% , mugwart 16.6% , fagace 15.8% , chenopoda 15.6% , date palm and batulacea 12.7% , olacea 12% , house dust 11.5% , feather 9.9% , alternaria 9.2% , plantine 8.6% , aspirgillus 6.9% , clodosporium 5.3% , cat hair 4.6% , horse hair 4% , penicillinum 3.3% and dog hair 3%In the study it was also observed that patients of bronchial asthma had associated allergic rhinitis in 49.1% of cases .Conclusion: This study and other studies will definitely be helpful in identifying the most common triggering allergens causing various allergic disorders including bronchial asthma in Diyala Governorate ; and this will be helpful in the prevention of asthma and other allergic disorders.

Abstract

Background: The prevalence of asthma is increasing world wide which impose an increasingly large burden on the health services, and the mortality rate from asthma has risen sharply which may reflect the disease severity. The aim of this study is to identify the risk factors of asthma, assess the quality of life (QoL) of asthmatic patients. Patients & Method: The data were collected from Asthma and Allergy Center in Suliamania from February 2007 to June 2007 in which 173 cases were included all aged 18 years old and above. Results: After analyzing the data we found that (61.3%) were females and (38.7%) were males and only (29.5%) of the participants had a positive family history of asthma. QoL of asthmatic patients was significantly affected (P-value<0.05) by socio-demographic characteristics such as (gender, level of education, and occupation). About (27%) of the cases used combined medication for treatment. Conclusion: Asthma significantly affects QoL and general health status. Gender, education, occupation and severity were confirmed to have major impact on QoL. Female, low socio-economic status and being a housewife are among the most important risk factor of asthma while dust exposure, activities like exercise were among the commonest triggers of asthma .Key words:- Asthma, Quality of life(QoL), Allergic rhinitis

التمهيد :- ان ازدياد انتشار مرض ( الربو القصبي ) عالمياَ يؤدي الى عبء ثقيل على الخدمات الصحية و الزيادة المفاجئة لنسبة وفيات المرض تعكس شدة المرض . إن نوعية الحياة المتعلقة بالمرض شهدت إهتماماَ في السنوات الاخيرة , إذ أصبحت قياساَ لتاثيرات الامراض المزمنة .الهدف :- هو استكشاف العوامل المؤثرة للربو القصبي و بيان نوعية الحياة في مدينة السليمانية .الطرق :-تم جمع معلومات هذا البحث من مركز الحساسية و الربو في مدينة السليمانية خلال الفترة مابين الاول شباط الىالثلاثون من حزيران لعام 2007 .تم ملء استمارة الاستبيان خلال مقابلة مع 173 مريضاً في الدراسة الوصفية بعمر 18 سنة فما فوق و كانت الاستمارة مكونة من جزئين . الجزء الأول استفسار حول المعلومات الشخصية و البيئية للمريض , والجزء الثاني استفسار حول نوعية حياة المريض . النتائج :- بعد تحليل النتائج تبين أن (61.3%) من المرضى كانوا نساء و كان لدى (29.5%) من المرضى تأريخ عائلي للمرض .أثرت الصفات الشخصية و الاجتماعية للمرضى ( الجنس ,مستوى التعليم والمهنة) على نوعية الحياة وبمستوى معنوية عالية(P-value < 0.05) . (27%) من المرضى استعمل أكثر من دواء لعلاج حالة الربو.الاستنتاجات :- ان لمرض الربو تأثيرَ كبير على نوعية الحياة و الصحة العامة للمريض . الجنس, التعليم, المهنة وشدة المرض لهما تأثير كبير على نوعية الحياة بينما المرأة , المستوى الاجتماعي-المعاشي المنخفض وربة البيت هي من اهم العوامل المسببة للربو . كان التعرض للغبار والتمارين من اكثر المحفزات لحدوث الربو .

Abstract

ABSTRACT:BACGROUND:Although tonsillectomy is one of the most frequent surgical procedure practiced its indication and operative procedure remains controversial. There is an increased economic pressure to be done as an outpatient procedure . Hemorrhage is main complication other rare complication is asthmatic attacks precipitation in allergic patients.OBJECTIVE:To determine the indication. Operative procedure.Complication namely hemorrhage and allergic aggravationMETHODS:Aretrospective review of T’S under L.A done for 962 patients from june1993-august2008 in Baquba and Sulaiymani teaching hospital partially because of G.Adrug shortage during sanction or because of G.A contra-indication absolutely or relatively .precise block anesthesia of lesser palatine and glossopharyngeal sensory nerve supply to tonsil is practiced with removal of tonsil by dissectionRESULTS:Most patients operated on because of recurrent acute tonsillitis (926) of these 20 patients with nasal allergy 4patients of them with bronchial asthma, (12)patients with associated cardiac or renal problem, (16)patients with UAO or sleep apnea,(4)patients with enlarged suspected malignancy, (4)patients with peritonsillar abscess, (50)patients developed exaggerated gage reflex but not hindering the operation , (12)patients developed primary bleeding, (2)patients secondary bleeding. None of allergic patients developed asthma during the practice period.CONCLUSION:T’S by L.A have a place in surgical practice especially in allergic patients.Allergic aggravation and asthmatic precipitation may be due to G.A and not T’S per se.The operation is of shorter duration and hospitalization than under G.A. Peroperative and secondary bleeding are less than G.A and of less pain severity post-operatively than under G.A.Exaggerated gage reflex not hindering the operation

Abstract

Background: Allergy is a hypersensitivity reaction mediated by immunological mechanisms which can be antibody or cell-mediated.Objective: This study aims at evaluating serum immunoglobulins A and E levels in patients suffering from Allergic in comparison with healthy individuals. Methods: This work was applied on 87 Allergiec patients admitted to the Babylon Center of Allergic and 40 apparently health controls with age range (10-69 years). . Results:The Immunological parameters showing that there is a significantly increased (p< 0.05) in IgE(448.75) IU/ml compared to control group (52.11) in age group10-19 and a significantly increased (p< 0.05) in IgA(380.5) mg/ml compared to control group (110.54 ) in age group 50-59 and no significantly increased (p> 0.05) of E-rosette test level in the allergic patients (32.601%) compared to control group (17.224%) in age group 50-59 years .Conclusion: We conclude that there is an association of all Allergic IgE level increase it in differant age also IgA level in serum of Allergic

أعدت دراسة مناعية لمعرفة مدى شيوع الأجسام المضادة IgE,IgA كذلك اجري اختبار E-rosette لمصول مرضى الحساسية في محافظة بابل اللذين كان عددهم87 مريض و للفئات العمرية بين ( 10-69)سنة مقارنة بالاصحاء اللذين بلغ عددهم 40 شخص لكلا الجنسين .تم تقدير نسبة IgEو IgA للمرضى والأسوياء باستعمال تقنية الامتزاز المناعي المرتبط بالأنزيم ELISA) ), أظهرت الدراسة أن هنالك فارق معنوي ( P > 0.05 ) بين مصول مرضى الحساسية مقارنة بالسيطرة وجد اعلى تركيز IgE بلغ 448,75وحدة دولية /مل مقارنة بالاصحاء اللذين بلغ تركيز مصولهم 52.11 عند الفئة العمرية 50-59وفي حين بلغ تركيز IgA بلغ 380.5 ملغم /مل مقارنة بالاصحاء الذين بلغ تركيزهم 110.54 ملغم/مل عند الفئة العمرية 50-59 سنه IgA في حين بلغت نسبة E-rosette للمرضى 32.601% مقارنة بالاصحاء التي بلغت نسبتهم 17.224%عند الفئة العمرية(50-59) سنة . كما بينت الدراسة الحالية وجود ارتفاع في معدل تركيز الأضداد IgEو IgA في مصول المرضى الحساسية بالمقارنة مع مجموعة السيطرة .

Abstract

Allergy is a hypersensitivity reaction mediated by immunological mechanisms which can be antibody or cell-mediated. This study aims at evaluating serum immunoglobulins E levels in patients suffering from allergic in comparison with healthy individuals. This work was applied on 97 allergiec patients(28 male,16 femal) admitted to the Babylon Center of Allergic and 20 apparently health controls with age range (10-69 years). . The Immunological parameters showing that there is a significantly increased (p< 0.05) in IgE(448.75) IU/ml compared to control group (50.11) in age group1-10 .The results was revealed that rural-urban ratio was higher –urban 66, 68% than in rural 31,32%for allergiec patients. We conclude that there is an association of all Allergic IgE level increase it in differant age in serum of Allergic Pationts

مرض الحساسية يعد من المشاكل الكبيرة التي يعاني منها الشعوب لاسيما في العراق حيث جمعت 97 عينه دم ( 28 ذكور و16 اناث) من المرضى ممن احيلو لمركز الربو والحساسيه في بابل تراوحت اعمار المرضى الذين شملت الدراسة من1 -60 سنة. وبينت تقنية الاليزا ارتفاعاً واضحاً في تراكيز الكلوبيولين المناعي لمرضى الحساسية، فوجد تاثيرا معنويا في زيادة تركيز للفئات العمرية كافة.أظهرت الفئة 1-10سنة أعلى تركيز IgE، اذ بلغ معدل التركيز فيها 448,75 وحدة عالمية/مل مقارنة بمجموعة السيطرة. 50,11 وحدة عالمية /مل . أظهرت النتائج أنَ الفئة العمرية1-10 اكثر عرضة للحساسية اذ بلغت نسبة المصابين 29.8% ثم الفئة . %بنسبة 9.27 واقلها في الفئة العمرية 51-60 41-50 بنسبة 18.5% بينت الدراسة الزيادة في الاصابات لسكان المدينة اذ بلغت 66 اصابة أي بنسبة 68 % بينما اصابات سكنة الريف بلغت31 اصابة بنسبة 32 %من بين 98 مريض بالحساسية.و بينت الدراسة الحالية وجود ارتفاع في معدل تركيز الأضداد IgE في مصول المرضى الحساسيه بالمقارنة مع مجموعة السيطرة.

Leptin, Obesity and IgE in patients with Asthma and Allergic Rhinitis

Abstract

Background: Asthma is a chronic inflammatory disease of the respiratory airways with a worldwide prevalence. This study was performed to evaluate the role of leptin on asthma and /or allergic rhinitis, and to study asthma-obesity-leptin relationship. Methods: A cross sectional study was performed on asthmatic patients (190), allergic rhinitis patients (110) and healthy control subjects (48). Results: Serum leptin was significantly higher at asthma with overweight, asthma with metabolic syndrome, allergic rhinitis with overweight patients than of control subjects, while it was of no significant statistical difference at asthma with normal weight, allergic rhinitis with normal weight patients than of healthy control individuals. Body mass index was significantly higher in asthma with overweight patients; asthma with metabolic syndrome and allergic rhinitis with overweight. It was of no significant difference in asthmatic with normal weight patients, allergic rhinitis with normal weight than of control subjects. FEV1 was significantly lower in asthma with normal weight patientss; asthma with overweight; asthma with metabolic syndrome; allergic rhinitis with normal weight; allergic rhinitis with over weight than in control subjects. Conclusion: Leptin was significantly higher in patients with asthma and allergic rhinitis as compared to healthy non allergic individuals and it was influenced by BMI and presence of metabolic syndrome in allergic patients. BMI influence FEV 1 in both asthmatic and allergic rhinitis patients.
