The effect of some antifungal agents and chlorhexidine on Candida albicans adherence on acrylic resin denture base surface (In vitro study)

Abstract

The adherence of Candida albicans on acrylic resin denture base surface is considered as the first step in the pathogenicity of candida–associated denture stomatitis which is the most prevalent form of oral candidiasis in denture wearers. This study was an in vitro study aimed to evaluate the anifungal effect of fluconazole, nystatin and chlorhexidine and their antiadherent effect on Candida albicans cells that attached on the fitting surface of denture base. It was determined, using broth microdilution method at two concentrations for each drug, and the results were determined using spectrophotometer. All drugs had a highly significant antifungal effect on Candida albicans isolate at their two concentrations. The effect of saliva on the adherence of Candida albicans on the fitting surface of denture base was also considered. This study concluded the importance of saliva in increasing the microbial adherence; i.e., the mean number of adherent cells on acrylic resin samples in the absence of saliva was 7.4 + 1.9 cell/mm2 and with the presence of saliva the number was 29.1 + 2.7 cells/mm2. The number of remaining adherent cells was counted using fluortescent microscope. The mean number of remaining cells which adhered on acrylic resin samples after the immersion in first concentration fluconazole, nystatin and chlor-hexidine for one hour respectively were 4, 5 and 2.6 cells/mm2 and for the second concentration/1 hour the numbers were 3.2, 4.1 and 1.7 cells/mm2 compared to the normal number of adherent cell 29.1 + 2.7 at p<0.01.

Abstract

This study aims to isolate the pathogenic yeasts from genital tract and investigate their relationship with the age .The results clarified that the most pathogenic yeast isolated from genital tract was Candida albicans , also the results of C.albicanas isolates susceptibility test, to different antifungal revealed that they were sensitive to Miconazole, Ketoconazole and Clotrimazol and were resistant to Nystatin and Grisofulvin. The study of relationship of vaginal infection with the age showed that the incidence of infection with Candida was high among females age group (19-39 years).

هدفت الدراسة إلى عزل الفطريات المرضية من القناة التناسلية الأنثوية و توضيح العلاقة ما بين الالتهابات المهبلية الفطرية والعمر.أو ضحت النتائج أن Candida albicanis هو المسبب الرئيس للالتهابات المهبلية الفطرية. فيما دلت نتائج اختبار حساسية عزلات C. albicans لمختلف ا لمضادات الفطرية على أن العزلات حساسة لكل من Miconazole , Ketoconazole و Clotrimazolو مقاومة إلى Nystatin وGrisofulvin , وعند دراسة علاقة العمر بالالتهابات المهبلية أكدت النتائج أن الإصابات تركزت في الفئة العمرية المحصورة ما بين (19-39) .

Abstract

Degradation of oral health is often assumed to progress with aging. However, significantly higher total counts and greater varieties of Candida species can be detected in wearers of removable dentures compared with nondenture wearers.Poorly fitting or unhygienic dentures leads to the presence of yeast like fungi (Candidaalbicans ) attached to it, and cause inflammation. Although candidiasis is highly resistant to antifungal agents, systemic drugs usage are necessary. But the systemic use of these drugs can cause side effects like liver toxicity, drug interactions etc.Usingpropolseasantifungal by localapplication, found that it suppresses the Candida associated dry socket and denture stomatitis. The Propolispast was used in the following manner. It was applied to the fitting surface of denture, twice a day for 2 weeks. The Propolisputty was putted in dry socket after complete removal of inflammatory tissuesfrom it.The results showed pain, redness area and inflammation related stomatitis were disappear gradually after 48 hours until 2 weeks that the denture stomatitis is healing by using a Propolispast.Propolisputty treat the fungal inflammation of dry socket that gradual decreases of the pain and inflammation.After the treatment, all patientswith denture stomatitis were subjected to examination of the palatal mucosa and socket and quantitative culture of Candida from the palatal mucosa and denture fitting surface and diagnosed by three method :Germ tube.GramStain.andChromAgar media

تشهد صحة الفم غالبا تدهورا مع تقدم العمر علاوة على ذلك زيادة اعداد من انواع كثيرة مختلفة من مستعمرات المبيضات البيضاء مع الاشخاص الذين يستخدمون طقوم الاسنان المتحركة مقارنة مع الذين لايستخدمونها. و المبيضات البيضاء (G+) قادرة على العيش كأحياء طبيعية في فم الاشخاص الاصحاء. عدم ثباتية الطقم او عدم نظافة الطقوم يؤدي الى وجود الفطريات Candida albicans والالتصاق بها وتؤدي الى الالتهابات الفموية. بالرغم من ان الميضات البيضاء عالية المقاومة لمضادات الفطريات السطحية فان النظام الدوائي يكون ضروريا ولكنه يؤدي الى مضاعفات جانبيه مثل تسمم الكبد والتداخل الدوائي والخ واستخدم البروبولس موضعيا كمضاد للفطريات لقمع المبيضات البيضاء المصاحبه (dry socket and stomatitis) . كما يلي : يوضع معجون البروبولس على السطح النسيجي للطقم مرتين في اليوم لمدة اسبوعين اما عجينة البروبولس فانها تستخدم في مكان قلع السن الملتهب بعد تنظيفه من الأنسجة الملتهبة. اظهرت النتائج اختفاء تدريجي للألم واحمرار المنطقة الملتهبة من سقف الفم ومكان قلع السن بعد يومين واختفائها نهائيا بعد اسبوعين من استخدام البروبولس. بعد العلاج تم فحص سقف الفم ومكان قلع السن واخذت عينات منها لفحصها مختبريا بثلاثة طرق وهي: Germ Germ tube . Abstract

Background Due to the increase in the microorganism resistance to most of conventional antifungal drug, studies have encouraged to discover novel treatments for infections caused by Candida species. Hence, alternative therapies are crucial and the use of herbal medicines seems being a promising solution for the candidiasis treatment. Materials and Methods the antifungal effect of some medicinal plant extracts (peppermint and pomegranate) was confirmed by some studies. But there was still a lack of knowledge associated with the specificity of these extracts on sensitivity and viability counts of candida albicans, so for this reason this study was conducted. Water extract (peppermint and pomegranate) was prepared, different concentration (5, 10, 15, 20, and 25mg/ml) from them were taken, tested candida albicans was used to evaluate these extracts on its sensitivity, viability counts, and minimum inhibitory concentration (MIC) (in vitro). Results showed that all concentration of pomegranate showed zone of inhibition, there were highly significant differences between concentrations except between (10, 15) and (20, 25), but peppermint revealed zero zone of inhibition on Sabouraud dextrose agar medium. All concentrations of both extracts were effect in reducing viability counts of candida albicans with highly significant differences with Negative control (Distilled water) P<0.05, but there were no significant difference P >0.05with positive control (Nystatin) and with each other. Conclusions All plant extracts used in this study had antifungal effect and influence on candida albicans. For this reason can be used as disinfectants, mouth wash or soaking solutions and as an alternate to drug in the treatment of denture stomatitis.
