Abstract

The local study were selected 70 Iraqi Arab Patients (39 females and 31 males), who referred to HLA laboratory in teaching laboratories in Baghdad medical city from April 2009 – May 2010 were diagnosed with Behcete's disease, C-reactive protein concentration(CRP) was evaluated by ELISA technique, anti-nuclear antibody (ANA) and anti-neutrophil cytoplasmic antibody (ANCA) were evaluated by Indirect immunofluorescence technology. The result showed significant differences (P<0.05) of CRP concentration in patients group compared with control, negative antinuclear nuclear antibody in all cases while ANCA was positive in 8.5% of patients with type P (PANCA). These results lead to suggest that ANCA auto-antibody may have a role in triggering BD

تضمنت الدراسة المحلية 70 عراقي عربي مريض (39 انثى ,31 ذكر جمعت العينات من المرضى المراجعين لمختبر التطابق النسيجي/ المختبرات التعليمية / مستشفى بغداد التعليمي). قيست بروتينات الطور الحاد بطريقة الاليزا. كما قيست اضداد النواة واضداد سايتوبلازم الخلايا العدلة بطريقة التالق المناغي غير المباشر. اظهرت النتائج وجود فروق معنوية في تركيز بروتينات الطور الحاد بين المرضى والاصحاء .اضداد النواة كانت سالبة التفاعل في المرضى والاصحاء بينما كانت اضداد سايتوبلازم الخلايا العدلة موجبة وذات نمط محيطي في 8.5 % من مرضى بهجت بينما كانت النتيجة سالبة في السيطرة. مما يشير الى ان اضداد سايتوبلازم الخلايا العدلة هي احد الاضداد المناعية الذاتية التي يمكن ان يكون لها دور في حث المرض.

Abstract

Background Behçet’s disease (BD) is a multi-systemic inflammatory disorder characterized by recurrent oral and genital ulceration. Free oxygen radicals may have a role in the pathogenesis of BD and can relate to the existence of the disease.Objectives This study had been designed to assess salivary and serum lipid per-oxidation as malondialdehyde (MDA) and status of antioxidant as total glutathione (GSH) in patients with BD in comparison with healthy subjects to detect the possible involvement of oxidative stress in BD. Materials and methods This is a case–control study, salivary and serum samples were taken from ffty BD patients with an age range 20 to 56 years with mean age 35.3 ± 7.6 years and ffty healthy control subjects with age range 16 to 58 years and mean age 34.8± 9.1 years. All were analyzed for MDA and GSH spectrophotometrically. Data was analyzed using descriptive statistics, t-test, P (ANOVA) test, Chi-square (2X) test, Pearson correlation and Receiver Operating Characteristic statistical analyses. Results The study showed that salivary and serum MDA level was higher in BD patients than in healthy subjects (P<0.05 and P<0.001 respectively). The levels of saliva and serum total GSH were signifcantly lower in BD patients than in healthy subjects. (P <0.001). Conclusion The results of this study revealed that BD patients were subjected to oxidative stress damage and saliva could be used as pain-free alternatively to serum in determining oxidative stress in Behçet’s disease.

Abstract

Background and objectives Behçet’s disease is a chronic relapsing multi-system disease with a high prevalent rate and of severe form in countries along the silk route. For diagnosis, a set of criteria has been adopted. Pathergy test, a unique skin lesion of disease, is an important criterion for diagnosis and classification of it. The aim of study was to assess pathergy test in patients diagnosed as Behçet’s disease according to the International Study Group criteria.Methods The study was conducted in two years between October 2005 and October 2007. Cases were diagnosed as having Behçet’s disease based on the International Study Group criteria for the diagnosis of Behçet’s disease. All cases were patients attending Duhok Centerfor Rheumatic Diseases and Rehabilitation. Controls were volunteers and persons who attended the center for consultation of non specific mechanical strain or sprain and with no history of orogenital ulceration.Results Majority of patients were in the fourth decade of life and in most of them the disease had started in the third decade. The male to female ratio was 1.12:1. History of oral ulcer has been reported by 84.3 % at disease onset and the percentage was 100% at diagnosis.Statistically significant difference in the rate of positive pathergy test during exacerbation and remission of disease was obtained and of high specificity for disease. Gender played no rolein the positive rate during phases of disease. Age extremities and disease duration less than 10 years were associated with a drop in the positive rate during remission.Conclusion Pathergy phenomenon occured in about one half of patients with Behçet’s disease and it is recommended to be performed in every patients of Behçet’s disease especially those with incomplete manifestations for diagnosis as it is one of criteria required diagnostic set

خلفية واهداف البحث: مرض بهجت من الأم ا رض المزمنة ذات انتكاسات على أعضاء الجسم المتعددة. و يكثر انتشارها في المناطق التي تقع على الطريق الحريري القديم. من السمات الفريدة للمرض هو ظهور بثور جلدي أو تقيحي أو احم ا رر جلدي في موقع الحقن و تدعى تفاعل بثارجي. الهدف من الد ا رسة هو تقييم ايجابية اختبار بثارجي لدى المرضى المصابين بمرض بهجت.طرق البحث: أجريت الد ا رسة في محافظة دهوك خلال فترة زمنية امتدت من كانون الثاني 2005 و الى كانون الثاني 2007 . حيث تم جمع 70 حالة من المصابين بمرض بهجت و 70 من المتطوعين الذين ليس لديهم مرض بهجت. وتم اج ا رء اختبار بثارجي عليهم.النتائج: تبين من الد ا رسة أن نسبة الذكور الى الاناث هي 1.12:1 . معظم المرضى كانوا في العقد ال ا ربع. كانت لدى %84.3 من المرضى تقرحات فموية و شفوية منذ بداية المرض. كما أظهرت الد ا رسة سالبية اختبار بثارجي لدى جميع المتطوعين. كان هناك اختلاف في نسبة ايجابية بثارجي خلال فترة الانتكاس و الركود. الجنس لم يكن له دور في تقليل نسبة الايجابية لاختبار بثارجي اثناء فترة الركود. من جانب آخر، كانت لطرفي العمر وفترة م ا رضة أقل من 10 سنوات دور في تقليل نسبة الايجابية أثناء فترة الركود.الاستنتاج: اختبار بثارجي مهم في تشخيص مرض بهجت حيث تظهر في حوالي نصف المرضى. لذا نقترح اج ا رء اختبار بثارجي لكافة المرضى الذين لديهم مرض بهجت و خاصة المرضى الذين تكون أع ا رضهم غير كافية للتشخيص.

Abstract

Background: Extensive reports denote the biological role of trace elements (Cu Zn ", Se ') in many various physiological and pathological conditions, one of the list is shown to be Behcet's disease (BD).The aim of this study was directed towards the distribution of trace elements (Cu 2,Zn, 2,Se 2) in sera of BD patients, and control groups.Methods: Fifty Arab Iraqi patients who fulfdled the ISO criteria for diagnosis BD.compared with 21 patients control with recurrent oral ulcer (ROU), healthy control groups (2Ihealthy relative and 21healthy volunteer). Typing HLA Class I was carried out at Alkarama hospital, by using microlympho-cytotoxicity test. Abstract

Behcet's disease (BD) is a chronic inflammation with characterized lesion, including oral ulcer, skin lesion, genital lesion and ocular manifestation. It affects both genders. The etiologies of BD is unknown genetic, bacterial and viral have proposed as causative agents, for that reason the aim of this study to detect Anti-Herpes Simplex Virus-1(HSV-1) IgG antibody and Anti-Chlamydia pneumonia IgG antibody by ELISA in sera of seventy Iraqi patients and twenty Iraqi healthy control. 34.2% of patients have Anti-HSV-1 IgG antibody while 21.4% of patients have Anti-Chlamydia pneumonia IgG antibody, this lead to suggest that HSV-1 and Chlamydia pneumonia may be one of the etiological factor in BD.

يعد مرض بهجت من الأمراض الالتهابية المزمنة المصاحبة بظهور تقرحات فموية، معوية ، تناسلية وإصابات أخرى متضمنة العين ، ويمكن أن يصيب كلا الجنسين. إمراضية مرض بهجت غير معروفة لحد الآن ولكن يعتقد أن العامل المسبب قد يكون فايروسي أو بكتيري في الأشخاص ذوي الاستعداد الوراثي، لذا فأن الدراسة الحالية تهدف الى التحري عن أضداد Chlamydia pneumonia و HSV-1 من نوع IgG في مصول 70 مريض عراقي مصاب بهذا المرض وبطريقة الأليزا وبالمقارنة مع 20 شخص عراقي سليم . أظهرت النتائج إن 34.2 %في مصول المرضى موجبة لفحص أضداد HSV-1 نوع IgG و 21.4 %موجبة لفحص أضداد Chlamydia pneumonia نوع IgG والتي من الممكن أن تلعب دورا كعامل مسبب لمرض بهجت .

Abstract

Behçet's disease (BD) is a systemic vasculitis identified by recurrent oral and genitalulcerations, skin inflammations and uveitis, with unknown etiology. This study had beendesigned to assess the level of salivary and serum uric acid (UA) and C - reactive protein(CRP) markers in BD patients as activity markers and to explore their relations in diseaseexistence. Fifty BD patients :( 32 males and 18 females) with mean age (35.3 ± 7.6) yearsand fifty healthy subjects (30 males and 20 females) with mean age (34.8± 9.1) years wereinvolved in this study that extend during the period of (2014-2016). All of patients andhealthy subjects were analyzed for UA by (colorimetric spectrophotometric assay that basedon oxidation of UA by uricase enzyme to allantoin) and CRP by (qualitative analysis:antigen-antibody interaction Latex agglutination test) in both saliva and serum. Data wasanalyzed using descriptive statistics, t-test, P (ANOVA) test, Chi-square (2) test, andPearson’s linear correlation coefficient statistical analyses. Results of this study show thatsalivary uric acid was significantly elevated in BD patients than in healthy subjects.(P<0.001). Although serum UA was higher in patients than the healthy subjects butstatistically not significant. (P=0.57). On the other hand, out of fifty patients there werethirteen patients (26%) significantly expressed CRP positive in their serum and saliva. Abstract

To assess the effect of Nigella sativa oil as a topical preparation for treatment of oro- genital ulceration of Behcet's disease. This is a double blinded therapeutic trial in which 40 patients with Behcet's disease manifested as recurrent episodes of oral and genital ulceration were enrolled into 2 groups; Nigella sativa oil was applied as 10% in glycerin topically 3 times / daily to the first 20 patients. The other 20 were given glycerin only. Assessment was done 4 days then 8 days after treatment.Results were recorded as they appeared in clinical examination and those records were arranged in tables for both groups concerning oral and genital ulceration cases.Nigella sativa oil is a new topical agent for promotion of healing of oro- genital ulceration of Behcet’s disease which may act through Pits anti-inflammatory effect.
