Abstract

Background:Breast carcinoma is the most common malignant tumor and the leading cause of carcinoma death in women, with more than 1,000,000 cases occurring worldwide annually.(1) as a matter of fact , the mortality rate for breast carcinoma changed very little from the 1930s to the early 1990s, because of the combined action of earlier diagnosis and improved therapy .
Materials and methods: The prospective study included 500 cases of breast carcinoma who went total mastectomy , between October 2006- April 2007, where taken from private pathology laboratory , sections(4microns) are taken and stained with H&E stain and over-reviewed.
Results: Clinicopathological analysis of the 500 cases of breast carcinoma, including the age, clinical presentation, the site of the tumor, the mammography results, the microscopic morphology, the stage and the grade of the tumor. The age in relation to the morphological classification also analyzed.
Conclusion: The commonest breast carcinoma in women is invasive ductal carcinoma, followed by lobular carcinoma, with mean age 44.5 year, mainly in the left breast, and that mammography approach is so useful in early detection of breast carcinoma.

Abstract

Background: The type of treatment of carcinoma
of the breast depends on its stage at presentation. Its
early detection is important.
Objective: Analysis of treated patients for
carcinoma of breast in regards to their stages,
considering it as an indicator for patient's presentation
and discusses the results in this collection.
Methods: A review study of cases treated surgically
for the last 10 years by the authors. Their stages were
recorded in addition to their age, gender and type of
histopathology.
Results: A collection of 110 patients was studied.
Those in the 5th decade were affected more. Females
were 107 and males were only 3. The right breast was
affected more than the left one. Infiltrative ductal
carcinoma was the commonest type of pathology
followed by the lobular carcinoma. Stage 2 was the
commonest stage at time of diagnosis and treatment.
Conclusion: In our collection, more than half of
our patients present during stage 2. This is a late
presentation. Earlier detection is necessary. An
important Program for early detection directed by the
authorities, using the periodic physical examination
and self breast examination and considering the start
of mammography screening program for those with
age range of (40-60). Programs for educating women
in regards to the risk factors and other information
about this disease through the internet should be
considered.

Abstract

Abstract:Background: Breast carcinoma is the most common malignant tumor in females at the age group (15-74) years. Because neoadjuvant therapy including preoperative chemotherapy and tamoxifen is becoming increasingly common for treating early breast carcinoma, itُ s desirable to grade the tumor before surgery to select the appropriate treatmentAim of the study: 1-Grading the cytological material obtained by FNA performed on malignant breast lesions. 2-Assess the reliability of FNA by comparison with histopathology.Material & Method: One hundred and three malignant cases of breast lesions diagnosed by FNA were reviewed in Al-Kadhmiah Teaching Hospital from 1994-1998. Their histopathological confirmation was present in 91 cases only. Those aspirates were stained by H&E stain & were graded according to the Robinson grading system using six cytological parameters; cell dissociation, cell size, cell uniformity, nucleoli, nuclear margin & chromatin pattern and then grade them into three grades according to the total scores.Results: We were able to correctly correlate 74 cases (81.3%) of FNA cytological material graded according to the mentioned criteria with their histological grading for ductal carcinoma. For lobular carcinoma only histopathological confirmation was done. Performing Chi-square testX²=67.4 (p≤0.001).Conclusions: Cytological grade does not correspond exactly to histological grade but it is equally good if the variability in histological grade is considered due to tumor heterogeneity. Also the value of Robinson´s grading system in cytological grading since it is simple, take little time & the information obtained is of clinical use & may be of prognostic value especially when combined with mammographic findings. الملخص:يعتبر سرطان الثدي من الأورام الشائعه في الإناث للعمر15-74 سنه . وحيث أن العلاج المواد الكيمياويه والتاموكسفين قبل إجراء التداخل الجراحي لأورام الثدي أصبح ظاهره معروفه لعلاج حالات سرطان الثدي في مراحله الأولى فقد أصبح من المفضل تقييم درجة خبث السرطان عن طريق الفحص الخلوي وذلك قبل أجراء أي تداخل جراحي ليتم أختيار أفضل نظام علاجي.الهدف من البحث : 1-معرفة درجة خبث الخلايا السرطانيه لسرطان الثدي بالفحص الخلوي (الرشف بالأبره الدقيقه ). 2-تقييم الفحص الخلوي وذلك بمقارنة نتائجه ومدى مطابقتها للفحص النسيجي لنفس الورم.المواد وطريقة العمل :تم مراجعة 103 حاله لسرطان الثدي تم تشخيصها عن طريق الفحص الخلوي بواسطة الرشف بالأبره الدقيقه في مستشفى الكاظميه التعليمي للفتره من 1994-1998 . صبغت هذه الحالات بصبغة الهيماتوكسلين والأيوسين وتم تقييم درجة خبث الخلايا السرطانيه فيها بواسطة نظام روبنسون لتقييم درجة الخبث معتمدًا على 6 صفات خلويه وهي : 1-التنافر بين الخلايا .2-حجم الخليه. 3-مدى التشابه بين الخلايا. 4-ظهور النويه. 5-حافات النواة. 6-مادة الكروماتين. وصنفت درجة الخبث الى ثلاثة درجات معتمدين الصفات المذكور وقورنت في 91 حاله مع مقاطعها النسيجيه. النتائج : أظهرت النتائج تطابق الفحصين الخلوي والنسيجي في 74 حاله (3و81%) من حيث تقييم درجة خبث الورم السرطاني معتمدين فيها على نظام روبنسون للفحوص الخلويه ونظام ريتشاردسون وبلوم المبدل لتقييم درجة خبث الأورام السرطانيه القنويه والتطابق النسيجي فقط (دون تقييم درجة الخبث ) للاورام السرطانيه الفصيصيه .وأختلفت نتائج الفحصين الخلوي والنسيجي في 17 حاله ( 6و18%) .وتبين من الفحص الأحصائي أن قيمة P ≤ 001و0 الذي يدل على أن تقييم درجة الخبث بواسطة الفحص الخلوي لاينطبق 100% مع التقييم النسيجي لكنه يفي بالغرض اذا ما اخذ بنظر الاعتبارأختلاف الوضع الخلوي للورم السرطاني . الاستنتاج : ان نظام روبنسون لتقييم درجة خبث أورام الثدي السرطانيه بواسطة الرشف بالأبره الدقيقه يعتبر نظام سهل التطبيق وسريع، والمعلومات المستمده منه معتمده في التشخيص السريري وقد تكون ذات أهميه في معرفة تطور الورم خصوصا اذا ما أستعمل مع تصوير الثدي الشعاعي لأعطاء نتائج أكثر دقه.

P53 in breast carcinoma: an immunohistochemical study

Abstract

Objectives: The aims of the present study are; first, to find out the relative frequency of p53 over-expression in different types of breast cancer. Second, to correlate the p53 over-expression with different parameters, including the age and menopausal status of the patient, size, grade, stage, type of the tumor, and the status of axillary lymph nodes. Third, to compare our results with othersMethods: The study was both pro and retrospective and included 60 cases of breast carcinoma. Data were obtained from archives of the pathology department, at Al-jumhuri Teaching Hospital and collected in a period spanning from August 2008 to January 2009. P53 over-expression was assessed immunohistochemically .Results: The patients ages ranged from 25 to 78 years (mean: 51.5 year); most of them were in the fourth decade (41.2%). There was a significant inverse relation between p53 over-expression and the age of the patients (p<0.001), in which the largest percentage of p53 positivity seen in the third decade. P53 over-expression was detected in 38.3% of the cases. P53 over-expression was found in (100%) of medullary carcinoma, 19/47 (40.4%) of invasive ductal carcinoma (NOS), 1/3 (33.3%) of ductal carcinoma in situ, and 1/6 (1.7%) of invasive lobular carcinoma. P53 over-expression was not detected in mucinous and papillary carcinomas. There was a significant direct correlation between p53 over-expression and tumor size (p=0.0274), grade (p=0.032), and stage (p<0.001). There were no statistically significant relations between p53 over-expression and the menopausal status (p=0.262) or axillary lymph node metastasis (p=0.471).Conclusions: Immunopositivity for p53 tumor suppressor protein was detected in 38.3% of the cases in this study. P53 over-expression was significantly correlated with patient's age, tumor grade, stage, and size, but no correlation was found with menopausal status and axillary lymph node metastasis. الأهداف: أجريت هذه الدراسة لتقييم حالة الظهور المناعي لبروتين p53 في سرطان الثدي في مدينة الموصل والربط بينها وبين مختلف الصفات المرضبة السريرية للورم ومقارنة هذه النتائج مع نتائج الدراسات الأخرى.الحالات والطرق: إن هذه الدراسة مستقبلية ورجعية تم من خلالها تجميع 60 حالة من سرطان الثدي. تم تجميع هذه الحالات من قسم الأمراض في مستشفى الجمهوري، خلال فترة إمتدت من شهر تموز 2008 إلى كانون الثاني 2009. لقد تم التحري عن بروتين p53 بطريقة مناعية-نسيجية-كيميائية. النتائج: تراوحت أعمار المرضى بين 25 - 78 سنة. تم تقسيم الحالات نسيجيا إلى 47 حالة لسرطان الثدي الارتشاحي القنوي و6 حالات لسرطان الثدي الفصيفصي وحالتين لسرطان الثدي النخاعي و3 حالات لسرطان الثدي داخل القنوي وحالة واحدة لسرطان الثدي المخاطيني . لقد وجد بروتين p53 في 3, 38% من 60 حالة من سرطان الثدي. وكانت سرطان الثدي النخاعيني كلها ايجابية. وكان 40,4% من سرطان الثدي الارتشاحي القنوي و33,3% من سرطان الثدي داخل القنوي و 1,7% من سرطان الثدي الفصيفصي كلها موجبة لظهور بروتين p53. لقد وجد هناك علاقة معنوية طردية بين ظهور بروتين p53والصفات المرضية (حجم الورم (0,0274p=)، مرتبة المرض) 0,032(p=، النخر (0,001p<)، نوع السرطان (0,001(p<). ولقد كان هناك علاقة معنوية عكسية بين ظهور بروتين p53 وعمر المريضة. لم يكن هناك علاقة معنوية بين ظهور بروتين p53 وسن اليأس والغدد اللمفاوية الابطية (262(p= (0,471(p = بالتعاقب. الاستنتاجات: لقد كان وجود البروتين p53 في سرطان الثدي بنسبة 38,3% بالطريقة المناعية-النسيجية-الكيميائية. لقد وجد هناك علاقة معنوية طردية بين ظهور بروتينp53 والصفات المرضية (حجم الورم، مرتبة المرض، النخر، نوع السرطان). ولقد كان هناك علاقة معنوية عكسية بين ظهور بروتين p53 وعمر المريضة. لم يكن هناك علاقة معنوية بين ظهور بروتين p53 وسن اليأس والغدد اللمفاوية الابطية . إن سرطان الثدي النخاعي كان أكثر ظهورا للبروتين p53 ، أما سرطان الثدي الفصيفصي والمخاطيني فهما أقل ظهورا لهذا البروتين.

Abstract

The aim of study is to study the pathological types and the immunohistochemical properties of breast carcinoma young women and compared it with older womenThis study included 80 paraffin embedded samples from female patients with breast carcinoma which were collected randomly from period of November 2010- November 2013 in AL-Hilla Teaching hospital. The clinical informations were collected including ages of the patients, histopathological types, & tumor grades from clinical reports of the hospital. A manual avidin biotin peroxidase complex procedure (ABC) system was used in the imunohistochemical analysis (Dako Cytomation Copenhagen, Denmark).In this study, the histopathological examination of the 80 cases revealed that , 5 (6.25%) cases were of the pure in situ ductal type, 65 (81.25%) cases were of the invasive ductal carcinoma not otherwise specified, 7 (8.75%) cases were of the invasive lobular carcinoma, and 1 (1.25%) case was of each of the medullary, mucoid, squamous (metaplastic) types.In the 80 cases of breast carcinoma that were studied, ER expression was positive in 8 cases (17.39%), PR expression was positive in 22 cases (27.5%), HER2/neu expression was positive in 20 cases (23.75%). There were higher proportion of ER and Her2 negative breast carcinoma in early aged patients than the older patients so the early onset breast according to the immuno -histochemical parameters have the worse prognonsis than the late onset group.

Abstract

Background: In breast carcinoma, amplification &/or over-expression of HER-2/neu has been associated with a group of unfavorable prognostic factors. The Food & Drug Administration Agency approved Trastuzumab (Herceptin) for the therapy of metastatic breast cancer but only in patients with amplification &/or over-expression of this gene. Because of these advances, evaluation of HER-2/neu status in breast cancer specimens is of vital importance.
Patients and methods: thirty eight females with breast carcinoma were included in this study. All histologic sections were stained routinely with the hematoxylin and eosin stains and immunohistochemically for HER2/neu. All cancers were graded and subcategorized into ductal and lobular carcinomas, with or without an insitu component, and with or without Paget disease. The assessment of HER-2/neu over-expression was quantitated.
Results: Of the twenty four cases of infiltrative duct carcinoma (IDC), HER-2/neu staining was positive in ten (41.7%) cases. None of the fourteen cases of infiltrative lobular carcinoma (ILC) showed a positive reaction. Statistical analysis revealed no significant differences in respect to HER2/neu expression between IDC with and without ductal carcinoma in situ (DCI). All cases of IDC with overlying Paget disease (5 cases; 13.2%) were positive for the stain. All grade I cancers were negative for HER-2/neu over-expression, in contradistinction to a positive staining in 41.7% grade II tumors and 62.5% grade III tumors.
Conclusion: there is significant correlation between HER2/neu overexpression and cancer histopathological type. The presence or absence of an in situ component had no effect on the marker overexpression. Significant correlation was observed between HER-2/neu overexpression and higher histological grades of IDC. No correlation was found between the positivity of the marker and axillary lymph node status. Similarly, no association was found with the other variables except the size of tumor. It is recommended that HER-2/neu assessment to be included as a routine test in the screening program of all breast cancer Iraqi patients.

Abstract

Objective: To evaluate the overexpression of Her-2/neu in patients with breast cancer in Mosul (both invasive and intraductal) against other prognostic parameters of mammary carcinomas, such as histological type, grade, tumor size, patient age and number of lymph nodes involved.
Methods: This is a retrospective study conducted in the Pathology laboratory of Al-Jamhoori Teaching Hospital and at private laboratories. A total of 36 breast cancer cases and 4 benign cases were diagnosed and collected in a period spanning from April 2006 to April 2007. We used Immunohistochemistry to evaluate the overexpression of Her-2/neu against the age, tumor size, type and grade and the axillary lymph node status.
Results: The mean age of all cases was 47.5 years ranging from 28 to 72 years. Regarding the age, Her-2 positivity was shown in 2 peaks (4th and 7th decades), while Her-2 negativity was found in older age group (p=0.004). Overall Her-2 overexpression was in 37% of the cases; it was overexpressed in 34.5% of invasive ductal carcinoma, in 100% of medullary carcinoma and in 100% of ductal carcinoma in situ. Whereas no expression (0.0%) was seen in 3 cases of invasive lobular carcinoma and one case of colloid carcinoma. Her-2 positivity was associated with large size (T2 and T3) rather than small size tumors (p=0.015). There was a correlation between Her-2 positivity and high grade tumor [G3 in 69.2% of the positive cases (p=0.045)]. Her-2 positivity was associated with axillary lymph node metastasis in 84.6% of cases, but not reaching a statistical significance. The benign lesions included in this study (two fibroadenoma, one fibrocystic disease and one duct ectasia) all showed negativity for Her-2 stain.
Conclusion: Her-2 overexpression was found in 37% of breast cancer in Mosul. Regarding age incidence Her-2 overexpression was noted at 2 peaks 3rd and 6th decades. Large size and high grade breast carcinomas were associated with high percentage of Her-2 positivity, and the majority of Her-2 positive cases had axillary lymph node metastases.

الهدف: تقييم شدة إظهار مستلمات Her-2/neu لمريضات سرطان الثدي في مدينة الموصل و لكلا نوعيه(المقيم والمنتشر). كذلك مقارنته مع عناصر أخرى للعوامل الانذارية للمرض مثل نوع السرطان النسيجي, درجة تقدم المرض, حجم المرض, عمر المريضة ووجود انتشار المرض إلى العقد اللمفاوية.تصميم الدراسة: دراسة رجعية لحالات سرطان الثدي عند النساء في مدينة الموصل.طريقة إجراء البحث: تمت الدراسة في مختبر المستشفى الجمهوري التعليمي /قسم النسيج المرضي مع جمع عينات من المختبرات الأهلية فكان العدد الكلي للعينات ست وثلاثون حالة لسرطان الثدي مع جمع أربع عينات من آفات الثدي الحميدة للفترة مابين نيسان 2006 إلى نيسان 2007. تم استخدام صبغة خاصة (صبغات مناعية نسيجية) لإبراز درجة إظهار مستلمات Her-2/neu ومقارنتها مع العوامل الانذارية الأخرى.النتائج: كان معدل أعمار الحالات المدروسة 47.5 سنة (تتراوح مابين 28 إلى 72 سنة). اغلب الحالات التي أظهرت ايجابية لمستلمات Her-2/neu كانت في العقدين (الثالث و السادس). إما أكثر الحالات التي أظهرت سلبية لمستلمات Her-2/neu ففي الفئات العمرية الأكبر. كانت نسبة ايجابية إظهار مستلمات Her-2/neu هو 37% من الحالات. 34.5% من الحالات الإيجابية هي من نوع سرطان الثدي القنوي الانتشاري, كما كانت شدة الظهار للمستلمات أكثر مع الإحجام الأكبر للسرطان ومع الدرجات المتقدمة للسرطان. 84.6% من الحالات التي أظهرت ايجابية لمستلمات Her-2/neu كانت منتشرة للعقد اللمفاوية. الاستنتاج: نسبة ايجابية إظهار مستلمات لسرطان الثدي في مدينة الموصل كانت 37%. وخاصة في العقدين الثالث والسادس وفي الأورام الأكبر حجما وفي الدرجات المتقدمة للسرطانات. معظم الحالات الإيجابية كانت منتشرة للعقد اللمفاوية.

Abstract

Background: Matrix metalloproteinases are enzymes that are involved in the digestion of the components of the extracellular matrix (ECM), cell surface receptors for soluble factors and junctional proteins and physiological processes such as tissue remodeling, but also in the stimulation of tumor growth, invasion, and metastasis.Objectives: To assess the immunohistochemical expression of MMP-3 and MMP-8 in breast carcinoma and to correlate this expression with clinicopathological parameters including patient’s age, tumor size, grade, subtype, lymph node status and lymphovascular invasion.Method:Sixty-two tissue blocks of breast carcinoma specimens were collected from Al-Kadhimiya Teaching hospital and Teaching Laboratories of the Medical City Center. Three sections of 5µm thickness were taken from each block and stained with H&E and Immunohistochemically for MMP-3 and MMP-8.Results: MMP-3 and MMP-8 expression were statistically correlated with patient's age, tumor grade, tumor size, histological subtype, lymph node involvement, and lymphovascular permeation with the exception of MMP-8 and age. Strong expression for MMP-3 was noticed in invasive carcinoma, high grade tumors, large size tumors, cases associated with positive lymph nodes and lymphovascular permeation (positive correlation). Negative expression for MMP-8 was noticed in most of the cases associated with lymphovascular permeation and positive lymph node(s) involvement by the metastatic cells (inverse relationship). Also a negative expression for MMP-8 was noticed in most cases associated with high grade, large size tumors.Conclusions: Assessment of MMP-3 in breast carcinoma reflects the grade of tumors and can predict progression of insitu to invasive cancer, lymph node involvement and lymphovascular permeation so that it may be useful additional prognostic factor. Expression of MMP-8 correlates with a lower incidence of lymph node metastasis and lymphovascular permeation and can be utilized as a marker indicating a good prognosis to these patients.Keywords: Breast carcinoma, MMP-3, MMP-8.

Background: Tumoral angiogenesis is essential for the growth and spread of breast cancer cells.Objective: To evaluate angiogenesis by measuring microvessel density (MVD) with CD34 and its maturity with smooth muscle actin (SMA) immunohistochemistry and to study invasion of basement membrane by tumor cells using type IV collagen.Methods: In the present study microvascular quantification was undertaken on 52 cases of breast carcinoma and 5 cases of benign breast lesions after immunohistochemical staining of tumor vessel, using CD34 antibody and SMA antibody. Microvessel quantification was performed at x400 magnification in the three most vascular areas of the tumors (hot spots).Results: The difference in MVD between benign and malignant cases is significant (P= 0.001). MVD is significantly correlated with L.N. involvement (P=0.004) and lymphovascular permeation (P=0.001), no statistical significant correlation between MVD and age of patient (P=0.656), tumor size (P=0.052), tumor grade (P=0.324).Conclusion: Measurements of angiogenesis may have clinical utility in the evaluation of breast cancer, particularly for estimation of metastatic risk. A high MVD may be a poor prognostic marker of breast carcinoma and a target for antiangiogenic therapy.Key words: Angiogenesis, CD34, SMA, Collagen IV, breast carcinoma.

Abstract

Background and Objective: This study aimed to assess the immunohistochemical expression of stromal CD10 in invasive breast carcinoma and determine its relationship with some clinicopathological parameters.Methods: A cross-sectional study was conducted on 91 female breast cancer cases collected from the archive of pathology department in Rizgary Teaching Hospital and some private labs during the period from January 2012 to August 2015. CD10 expression was detected by immunohistochemistry using labeled polymer and enhanced polymer systems (Dako EnVision™ Flex) Dakoprotocol. The scoring was based on the intensity and percentage of the stained stromal cells. Then CD10 immunoexpression was correlated with the age of patients, tumor size, histopathological types, tumor grade, lymphovascular invasion, lymph node status, and tumor stage.Results: Stromal CD10 expression was observed in 70.4% of the cases. A statistically significant positive correlation was seen between stromal CD10 and the tumor subtype (P = 0.02), tumor grade (P = 0.025), lymphovascular invasion (P = 0.002), and lymph node status (P = 0.047), however, no statistically significant association was identified between stromal CD10 expression and age, tumor size and tumor stage.Conclusion: Stromal CD10 is expressed in the majority of cases of invasive breast carcinoma and concerning the positive correlations between stromal CD10 expression and tumor grade, lymphovascular invasion, and lymph node status suggest a strong effect of stromal CD10 expression on aggressive behavior of breast carcinoma.
