Meningitis In Sulaimani Pediatric Teaching Hospital: A Retrospective Study

Abstract

Background: Meningitis in children is one of the life threatening condition in pediatric age group, which needs proper and early diagnosis in the emergency department. Meningitis morbidity and mortality depend on causative agent, time of diagnosis, proper and sufficient duration of therapy in the hospital. For diagnosis, many parameters can be used from history, physical examination and laboratory investigation, which needs sophisticated laboratory to perform.Objective: To analyze CSF findings in children admitted to hospital as meningitis (bacterial and non-bacterial). Also, assess frequency and percentage of the bacterial species isolated from positive CSF culture and determine the resistance patterns of the different bacterial pathogens. Method: A retrospective study carried out in Sulaimania, Iraq/pediatric teaching hospital from January to December 2014. lumber puncture done in 96 cases aged between 29 days to 15 years that have been diagnosed as meningitis CSF parameters; WBC total count, Sugar and protein level were examined, also culture done for all cases with CSF leukocytosis . Results: According to CSF findings and clinical diagnosis, 51% (49cases) considered bacterial while 49% (47 cases) considered non bacterial meningitis. Deaths recorded only among Bacterial meningitis patients with a percentage of 6.1%. Results of CSF culture determined a 24 % (22 cases) as positive for bacterial growth. Streptococcal pneumonia, was on the top of the isolated bacteria with a high resistance against penicillin and Azithromycin.Conclusion: Bacterial meningitis are still occupying large percentage of cases admitted to hospital, and if un-diagnosed promptly will have a high mortality. High index of suspicion for bacterial meningitis is recommended.

Abstract

Objective: To assess the changes of cerebrospinal fluid (CSF) acetyl cholinesterase(AChE) activity in children with acute bacterial meningitis in comparison to healthyage and sex matched controls.Patients and methods: Out of 41 cases admitted to Alkhansaa pediatric and diliveryhospital (Mosul city) between January 2010 and July 2010, 28 cases proved to be acase of Haemophilus influenzae meningitis, 13 child proved lack from any type ofmeningitis with age and sex matched subjects taken as a control group. Initially fromall of the studied children (patients and controls) about 5 ml CSF samples were takenand assayed for appearance, leucocytes, red blood cells, sugar, protein and AChEactivities by spectrophotometric method.Results: There were a significant increase in CSF WBCs count, RBCs count, proteinlevels, AChE activity with a decrease in CSF glucose level for children with acutebacterial meningitis in comparison to the control group.Conclusion: There is an important value for measurement of CSF AChE activity forconfidential diagnosis of children with acute bacterial meningitis specially for thosewith equivocal decision by using the traditional parameters.

أهداف البحث: لتحديد أهمية قياس مستوى التغيير لنشاط خميرة الكولين استراز في سائل النخاع ألشوكي للمرضى المصابين بالتهاب السحايا البكتيري الحاد من الاطفال وبالمقارنة مع مجموعة ضبط من اصحاء ذوي اجناس و اعمار مقاربة لمجوعة المرضى.المرضى وطرق العمل: من أصل 41 مريضا ادخلوا الى مستشفى الخنساء للأطفال و التوليد في مدينة الموصل للفترة مابين كانون الثاني 2010 وتموز 2010 ، كان هناك 28 حالة شخصت على انها التهاب سحايا بكتيري بجرثومة Haemophilus influenzae. كذلك انتخب 13طفلا من أعمار و أجناس مقاربة من الاصحاء اتخذت كمجموعة ضبط. في البداية تم سحب 5 مل من سائل النخاع ألشوكي من المجموعتين ( مجموعة المرضى والضبط) وتم معاينة النماذج و فحص كريات الدم البيض, كريات الدم الحمر ,مستوى السكر,مستوى البروتين و قياس مستوى نشاط خميرة الكولين استراز في سائل النخاع ألشوكي باستخدام طريقة امتصاص الطيف الضوئي.النتائج: هنالك زيادة معنوية في كريات الدم البيض, كريات الدم الحمر, مستوى البروتين و مستوى نشاط خميرة الكولين استراز في سائل النخاع ألشوكي مع نقص معنوي في مستوى السكر للأطفال الذين لديهم التهاب السحايا الحاد بالمقارنة مع مجموعة الضبط.الاستنتاج: كان هنالك فائدة مهمة من قياس مستوى نشاط خميرة الكولين استراز في سائل النخاع الشوكي للتأكد من صحة تشخيص التهاب السحايا البكتيري الحاد عند الأطفال الذين لم يكتمل تشخيصهم للمرض باستخدام طرق الكشف الاعتيادية .

The Effect of Furosemide on Some Physiological and Immunological Parameters In Male Albino Rats

Abstract

The present study was performed to examine the effect of furosemide (laxis) on certain hematological and immunological criteria in rats. The animals were divided into three groups( 5 replicates for each) : first and second treatment groups gives orally suspended furosemide 0.1 and 0.2 mg/kg/day respectively ,while the third group (control) gives orally normal saline (0.9%), and all groups primed for 5 days. The results showed a significant decreases (P<0.05) in RBCs count, platelets count, WBCs count, concentration of Hb and PCV% in treatment groups compare with control group ; furthermore, results revealed a non significant differences (P<0.05) between treatment and control groups in concentration of total protein, while there was a significant increases (P<0.05) in concentration of the cytokine GM-CSF in first treatment group compared with second treatment and control groups.

صممت هذه الدراسة الحالية لبحث فعالية العقار فيوروسمايد على بعض المعايير الدموية والمناعية في ذكور الجرذان البيض. قسمت الحيوانات الى ثلاثة مجاميع (5 حيوانات لكل معاملة): الاولى والثانية جرعت فمويا بالتركيز من معلق العقار 0.1 و 0.2 ملغم/كغم/يوم على التوالي، اما المجموعة الثالثة (مجموعة السيطرة) فقد جرعت بمحلول الملح الوظيفي (0.9% ). وجرعت كل المجاميع لمدة خمسة ايام. بينت النتائج وجود نقصان معنوي (P<0.05) في عد كريات الدم الحمر، الصفيحات الدموية ، خلايا الدم البيض، تركيز الهيموغلوبين والنسبة المئوية لحجم الخلايا المرصوص (PCV%) في مجاميع المعاملة مقارنة مع مجموعة السيطرة، بالإضافة الى ذلك بينت النتائج عدم وجود فروق معنوية بين مجاميع المعاملة والسيطرة في تركيز البروتين الكلي، بينما وجدت زيادة معنوية (P<0.05) في تركيز الحركي الخلوي العامل المحفز لنمو مستعمرات البلاعم الكبيرة والخلايا البيض المحببه (GM-CSF) في مجموعة المعاملة الاولى مقارنة مع المجموعة الثانية من المعاملة والسيطرة.

Abstract

this paper addresses the adopting of implementations enterprise resource planning ERP system based on critical success factors (CSF). We reviewed the execution of enterprise resource planning (ERP) according to five different areas: USA, Australia, China, Middle East, and EU. During our research, seven CSFs covered to compare five different regions to find the best implementation of ERP. The CSFs that we found are “Fit between ERP and business/process, Project champion ERP strategy and implementation methodology, Communication, Change management culture and programme, Business plan /vision /goals /justification, appropriate business and IT legacy systems”. In these seven CSFs, some were most frequently cited such as training, education,and top management support’ they have great effective on the efficacious application of ERP system.

Abstract

background: Childhood bacterial meningitis is a neurological exigency. Accurate, early, rapid diagnosis and treatment is essential to decrease its morbidity and mortality. Aim of study: This is a retrospective study comparing real-time polymerase chain reaction (RT-PCR), with standard bacterial culture for the diagnosis of bacterial meningitis during cerebrospinal fluid (CSF) examination.Materials and methods: RT-PCR was used for the detection of three most common causes of bacterial meningitis (Streptococcus pneumonia, Haemophilus influenza type b, and Neisseria meningitides) in 100 CSF samples from children aged 2 months to 12 years admitted to Al-Elwiya Pediatric Teaching Hospital during the period from January 2016 to January 2017 Results: Growth was detected in 6% CSF cultures of patients included in the study and Streptococcus pneumoniae was the most prominent isolated bacteria. RT-PCR was positive in 43% CSF samples from which S. pneumoniae was identified in all samples. PCR and culture showed concordance in 6% positive and 57% negative samples. Using culture as a reference method, the sensitivity, specificity, and positive and negative predictive values for PCR on CSF samples were 100 %, 60.64%, 13.93% and 100 %, respectively. RT-PCR was positive in 37% CSF samples whereas culture was negative.Conclusion: RT-PCR is a rapid and sensitive test for the diagnosis of bacterial meningitis. The findings of this retrospective study recommend the use of RT-PCR for the diagnosis of children with a clinical suspicion of bacterial meningitis and as a complement to culture, especially, those who received previous antibiotic treatment before lumbar puncture.

Abstract

Background: Chronic myeloid leukemia (CML) is a clonal myeloproliferative disorder of primitivehaemopoietic progenitor cells. The cytogenetic hallmark of CML is the Philadelphiachromosome, created by a reciprocal translocation between chromosomes 9&22 (t[9;22][q34;q11]). Survival and amplification of hematopoietic progenitors are controlled by anumber of regulatory molecules (hematopoietic growth factors). The role of Granulocytemacrophagecolony-stimulating factor (GM-CSF) and its receptor in pathology arises largely asa result of abnormal signaling leading to deregulated myelopoiesis. The development of theBCR-ABL-targeted Imatinib mesylate represents a paradigm shift in the treatment of CML.Data indicate that the level of immune responses against CML is low before Imatinib, rises astreatment is administered and declines again when the BCR-ABL transcript numbers fall to lowlevels.Objective: To assess and evaluate the significance of levels difference in GM-CSF through newlydiagnosed patients, different responder groups (optimal, suboptimal and failure cytogeneticresponse) and advanced stages (acceleration and crisis groups) of CML Iraqi patients whomreceiving Imatinib mesylate (TKI), as an indicator to assess the role of growth factor inpathogenesis of CML disease development and response to treatments.Patients and methods: In this study 128 Iraqi CML patients asses before and after receiving imatinibmesylate treatment which categorized by complete blood picture and fluorescent in situhybridization (FISH) analysis in to different response groups and stages, then used an ELISAtechnique to assess serum level of GM-CSF in each response group and advance stage(acceleration and transformed) of CML patients, in comparison to level in 32 healthy controlsubjects.Results: Out of 128 patients the mean of GM-CSF serum level (pg/ml) for the newly diagnosed,optimal responded, suboptimal responded, failure cytogenetic and advance stage of CML were399.53±104.50, 325.23±66.37, 428.90±45.70, 347.12±54.45, 521.56±83.73, respectively. Whilehealthy was 269.25±86.27.Conclusion: Uncontrolled granulopoiesis in newly diagnosed patients with CML may be mediated byincreased plasma CSF concentrations caused by BCR-ABL activity which may give an idearegarding the role of colony stimulating factors in the pathogenesis of CML disease.

Abstract

Background:Rheumatoid arthritis (RA) is a chronic, inflammatory autoimmune disorder that causes the immunesystem to attack the joints. It is a disabling and painful inflammatory condition, which can lead tosubstantial loss of mobility due to pain and joint destruction. RA is a systemic disease, often affectingextra-articular tissues throughout the body including the skin, blood vessels, heart, lungs, and muscles.Patients and Methods: Enzyme immunoassay for Determination of human TNF- , IL-1 and GM-CSFin serumsamples from50 patients with a diagnosis of rheumatoid arthritisResults: of cytokines showed a significant increase in TNF-alpha, IL-1 beta and GM-CSF in patientswith rheumatoid arthritis (70.98 12.08) pg/ml,(238.6 116.4)pg/ml and (96.1 12.08)pg/ml respectively.When compared with the control group (7.0 3.09)pg/ml, (15.4 3.8)pg/ml and (6.8 3.03)pg/mlrespectively.Conclusion: Increased serum levels of proinflammatory cytokine such as TNF- , IL-1 and GM-CSFprobably play important role in driving inflammatory process and promoting joint destruction inrheumatoid arthritis. Regulation of these cytokines is a crucial importance in the RA disease showingpleiotropic actions and many different targets.Keywords: Rheumatoid arthritis, TNF- , IL-1 and GM-CSF.

Abstract

A prospective study of 50 patients with obstructive hydrocephalus complicating medulloblastoma in posterior fossa (malignant tumors formed from primitive or poorly developed cells at a very early stage of life) with a histopatho-logically proved medulloblastoma. The patients had different age group and from different geographical regions in Iraq. The aim of this study was to clarify the best method for treating hydrocephalus associated with these tumors.And compare in outcome with patient treated with preoperative shunt and those with direct attack with safety burr hole or direct attack only and our results. In those patients that shunt operation was conducted before tumor resection there is improvement in their clinical condition before tumor resection leading to lax brain during operation.CSF diversion was recommend before tumor resection is recommended for most patients especially for those with midline solid tumors.

Abstract

A prospective study of 50 neonates with hydrocephalus and meningocele (one of the important congenital malformations involving defective development of spinal neural tube). This study to compare between patients with V-P shunt before repair of meningocele and those with repair first and the results shows that preoperative shunt procedure in patient with meningocele and hydrocephalus is very important aspect to prevent post-operative CSF leak and wound infection.

Abstract

ABSTRACT:BACKGROUND: Cerebellopontine angle is an area with extremely critical structures, therefore understanding its tumors behavior with the implications on the surgical outcome will help in reduction of the possible surgical complications, eventually improves the patients quality of life.OBJECTIVE:Our object is to illustrate the various aspects of clinical presentation and surgical complications with linkage to the tumor size regarding cerebellopontine angle (CPA) tumors in Iraq. PATIENTS AND METHOD:A prospective cohort study for thirty patients at the Surgical Specialty Hospital (Al Shaheed Ghazi Al Hariri Hospital) have been studied over a 1-year period (2014 / 2015), and followed up postoperatively for 6 months. Their mean age of was 36 years. The suboccipital retrosigmoid approach has been utilized for CPA tumors removal.RESULTS: The most common presentation was vertigo followed by tinnitus. There were two cases (6%) of cerebrospinal fluid leak. Facial nerve function was measured by the House Brackmann (HB) system and function further classified into three categories: excellent (HB I-II), intermediate (HB III-IV), and poor (HB V-VI) outcome following surgery, showing that all cases below 20 mm tumor size have an excellent outcome and (67%) of cases with > 20 mm tumor size shows poor outcome. One death in this study (mortality rate, 3%) was due to respiratory cessation. CONCLUSION: A female preponderance noticed. Significant relationship between tumor size and facial nerve postsurgical functional outcome, with the larger C.P.A. tumors yield worse outcomes. Tumor size also influences the development of CSF leak and operative morbidity and mortality..
