Abstract

ABSTRACTBACKGROUND:Colorectal carcinoma is the most common gastrointestinal tract cancer worldwide. In men, it is the third cancer after lung and prostate cancer, while in women; it is the third common cancer after lung and breast cancer. Despite the clear relationship with aging, colorectal carcinoma is not strictly a disease of elderly and 6-8 % of cases occur in patients below 40 years of age. Colorectal cancers are of favorable prognosis provided they are diagnosed and treated in early stage.OBJECTIVE:This study aims to assess the patterns of presentation, distribution, and management of colorectal carcinoma in Gastroenterology and Hepatology Teaching Hospital.METHODS:From November 2005 to November 2007, 80 patients with colorectal carcinoma 50 males and 30 females were admitted to the Gastroenterology and hepatology center. The age, sex, presentation, modes of investigation, stage of the cancer, treatments as well as complications have been described.RESULTS:Male: female ratio about 1.6:1 with peak age of incidence is 60-69 year age group, 17.5% of cases are below age of 40 most of them were with worse histopathological types and advanced stage. The main presenting symptom was bleeding per rectum 63%.The mean period between onset of presenting symptoms and final diagnosis was 7 months .The most common sites were rectum 35% . Fiftythree percent were moderately differentiated ,47.5% were Duke's-C.CONCLUSION:The study highlights the distribution of colorectal carcinoma, presenting symptoms, modalities of treatment, keeping in mind the increasing incidence of colorectal carcinoma in younger age groups.

Abstract

Abstract:-Fifty paraffin blocks of colorectal carcinomas cases included in this study were randomly selected from January 2007 to Juan 2007 from the histopathological department in AL-Sadr Teaching Hospital.Of the fifty paraffin blocks of the studied colorectal carcinomas' cases; 29 cases (58%) were males and 21 cases (42%) were females. Their ages range was (16-80) years with an overall mean age (56.44±13.558) years , (58%) cases in distal colon (26%) cases were rectal , well differentiated carcinoma in (40%) cases , their stages by Dukes they were (36%) Dukes C followed by (30%) Dukes B then (18%) Dukes D and (16%) Dukes A. Fifty cases were submitted for immunohistochemical stain for P53 antibody, 39 cases (78%) were positive , and positive cases classified as weak positive in 18 cases (36%) and strong positive in 21cases (42%) ,also as faint staining pattern in 15 cases (38.4%) and dense staining pattern in 24 cases (61.6%).TP53 expression was more in distal colon cases (82.7%) , higher stages Dukes C (83.3) and D ( 88.8%) and poorly differentiated cancer (94.7%) , With no significant correlation of TP53 expression and site and stage of tumor but with significant relationship with grade of tumor

اهدف الدراسة هو البحث عن التعبير المناعي النسيجي الكيميائي لجين أل P53 كمؤشر للتغيرات الجينية الطارئة في ألخلايا السرطانية لأورام القولون ومقارنته في النسيج الحميد ولمعرفة ترابط هذا التغير مع ثوابت أخرى مثل درجة تمايز الورم، موقع الورم في الأمعاء الغليضه وأخيرا درجة انتشاره. منذ بداية كانون الثاني 2007 وحتى حزيران 2007 تم جمع 50 عينة من مرضى مصابين بسرطان القولون (21أنثى و 29 ذكور) تراوحت أعمارهم بين 16 و80 سنه مع معدل عمر 56,44 سنه ،وقد قورنت هذه المجموعة مع مجموعه أخرى (عشرون حاله) مصابين بأمراض القولون الحميدة. أظهرت الدراسة المناعية النسيجية أن تعبير أل-TP53 كان موجبا في 78% من سرطان القولون, بينما لم يلاحظ أي تعبير لهذا ألجين في النسيج الطبيعي (قيمة ألفا < 0,05). كان تعبير أل-TP53 أكثر في المرضى في القولون البعيد أكثر منه في القولون المستقيم والقولون القريب ، وفي أعلى مراحل الانتشار[Dukes C&D] و في الأورام ذات التمايز الضعيف (الدرجة الثالثة ) عنه في الأورام ذات التمايز القوي والمتوسط (الدرجة الأولى والثانية). مع عدم وجود ترابط بين ألتعبير المناعي النسيجي ل TP53 مع موقعه ودرجة انتشاره (قيمة ألفا > 0.05) كما انه توجد علاقة واضحة لتعبير أل-TP53 مع درجة التمايز (قيمة ألفا < 0.05).

Colorectal cancer comprises several distinct histological types includingadenocarcinoma which forms 85%-95% of all colorectal cancer cases. Pathogenesis ofcolorectal cancer is a multistep process characterized by involvement of many geneticalterations, including p53.The aim of this study is to detect the expression of p53 protein in colorectal carcinomaand to show its relationship with some clinicopathological features including age,gender, histological types, histological grades and staging.Forty paraffin-embedded tissue blocks of colectomy specimens were used in thisretrospective study. They were collected from the Department of Pathology in SulaimaniaTeaching Hospital, Shorsh Hospital and Shehid Saifeddin Private Clinic from January2007 to July 2008.Two sections of 4 micrometer thickness were taken from each paraffin embedded tissueblock. First section was taken for hematoxylin and eosin stain and the other one forimmunohistochemistry [anti-p53 monoclonal antibody] by using DakoCytomationEnvision + Dual Link System-HRP (DAB+). The relationship between p53 over expressionand the dependent variable (age, gender, histological types, histological grades, andstaging) were evaluated statistically using an analysis of variance (ANOVA) with STATA 8soft ware (College station, Tx). A positive reaction for p53 was scored on a semiquantitativebase as score 0 (no staining), score 1+ (weak staining), score 2+ (moderatestaining), and score 3+ (strong staining).Staining was negative for p53 (score 0) in 16 cases (40%). Positive cases were scored as(1+) in 4 cases (10.0%); (2+) in 8 cases (20.0%); and (3+), in 12 cases (30.0%). There wereno significant relationships between p53 over expression and age (p=0.682), gender(p=0.924), histological types (p=0.30), histological grades (p=0.516), and the stage of thedisease (p=0.281). Conclusions: Considering the p53 protein over expression in arelatively high percentage of patients, it seems that p53 mutation may play an importantrole in the development of colorectal carcinoma. There were no significant relationshipsbetween p53 protein expression and some clinicopathologic variables such as age,gender, histological types, histological grades, and the stage of the disease.

Abstract

Background: Quantitation of serum immunoglobulins (IgG, IgA and IgM) provides useful information for the evaluation of certain cancers.Objectives:to estimate serum immunoglobulins level before and after surgery, and to shed light on the correlation of immunoglobulins with progression of CRC.Patients and Methodes. By single radial immune diffusion method IgG, IgA, IgM were estimated in 100 CRC patients preoperatively, and in 20 patients postoperatively compared with 35 patients control with ulcerative colitis (UC) and 50 healthy control.Results: The study showed significantly increased the serum IgM level in patients group preoperatively (p<0.001) compared with control group. On the other hand, no significant differences were observed in respect to the mean of IgG, IgA, and IgM after surgical excision of the tumor in 20 patients.Conclusion: These finding suggests that high level of IgM might be considered as prognostic indicator.Keywords: serum immunoglobulins, Colorectalcancer cases

Abstract

Objective: Assessment the quality of life concerning physical problems for patient after colorectal cancer in Mosul Governorate.Methodology A descriptive cross-sectional design is employed through the present study from 1st June 2011 to 15th December 2011 in order to assess the quality of life concerning physical problems for patient after colorectal cancer in Mosul Governorate.A purposive (non probability) sample is selected for the study which includes (60) patients diagnosed with colorectal cancer were treated in Mosul Oncology and Nuclear Medicine hospital or the patients who visited the outpatient clinic in the same hospital for medical follow–up and further treatment. Data were gathered through the patients` interviewed. Assessment questionnaire consist of two parts contains demographic characteristic, the other part concerning of quality of life concerning physical problems after colorectal cancer. Reliability and validity of this tool is determined through application of a pilot study and panel of experts. Data were analyzed through the application of descriptive statistical (frequencies and percentages) and inferential statistical (mean of score).Content validity of the instrument was done through eliciting the opinions of a panel of (13) experts and reliability through a pilot study by using Internal consistency reliability is determined through the computation of the cronbach alpha correlation coefficient of the scale on data gathered from patients.Results: The Overall results revealed that the quality of life related to physical problems for patients after colorectal cancer were acceptable. So, there is no significant correlation between the quality of life related to physical problems for these patients and some demographic characteristics such as gender, age, marital status and education level.The study concludes that colorectal cancer are most common in married than in unmarried, half patients age are between 40 and 60 years old, have a relatively low level of education, many are unemployed and they reside in the urban areas in Mosul. Recommendations: The study recommended to preparation of an educational program for patients with colorectal cancer and to raise awareness and education among families of patients to provide a better life for their patients.

الهدف: تقييم نوعية الحياة ذات العلاقة بالمشاكل الجسدية للمرضى بعد سرطان القولون والمستقيم في محافظة الموصل. المنهجية: دراسة وصفية طبقية استعمل فيها أسلوب التقييم تم استخدامه في الدراسة الحالية للفترة من 1 حزيران 2011 إلى 15 كانون الاول 2011 من تقييم نوعية الحياة ذات العلاقة بالمشاكل الجسدية للمرضى بعد سرطان القولون والمستقيم في محافظة الموصل. اختيرت عينة عمديه قصديه (غير احتمالية) في الدراسة وقد تكونت من (60) مريض في مستشفى الاورام والطب الذري في الموصل وكذلك المرضى الذين زاروا العيادة الخارجية في المستشفى نفسه للمتابعة الطبية وإكمال العلاج. تم جمع بيانات الدراسة عن طريق مقابلة المرضى الراقدين والمراجعين في المستشفى المذكور. تكونت استمارة التقويم من جزئين شملت المعلومات الديموغرافية والسريرية المرضى والجزء الأخر تعلق بنوعية الحياة ذات العلاقة بالمشاكل الجسدية بعد سرطان القولون والمستقيم. تم إجراء المصداقية والثبات لأداة التقييم من خلال الدراسة الاستطلاعية وعرض الأداة على مجموعة من الخبراء. تم تحليل البيانات من خلال أسلوب الإحصاء الوصفي( التكرار والنسب المئوية) والإحصاء ألاستنتاجي(المتوسط الحسابي).تم تحديد ثباتية استمارة الاستبانة من خلال اجراء دراسة مصغرة باستخدام الاتساق الداخلي ومن خلال حساب معامل الارتباط كرونباخ ألفا على البيانات التي تم جمعها من المرضى وحددت مصداقيتها من خلال الحصول على آراء لجنة مكونة من (13) خبيرا. وقد تم تحليل البيانات من خلال تطبيق التحليل الإحصائي الوصفي (النسب المئوية) والتحليل الإحصائي الاستدلالي (متوسط النقاط).النتائج: وبشكل عام كشفت النتائج ان نوعية الحياة ذات العلاقة بالمشاكل الجسدية للمرضى بعد سرطان القولون والمستقيم كانت مقبولة، وليس هناك علاقة معنوية بين نوعية الحياة لهؤلاء المرضى وبعض الخصائص الديموغرافية مثل الجنس والعمر والحالة الزوجية ومستوى التعليم.واستنتجت الدراسة أن الاصابة بسرطان القولون والمستقيم هي أكثر شيوعا بين المتزوجين عن اولئك الغير متزوجين، كذلك كانت اعمار نصف المرضى ما بين 40 و 60 سنة، وكان لديهم مستوى متدني من التعليم، والعديد منهم عاطلين عن العمل ويقيمون في المناطق الحضرية في الموصل. التوصيات: أوصت الدراسة بإعداد برنامج تثقيفي للمرضى المصابين بسرطان القولون والمستقيم وزيادة الوعي والتثقيف بين عوائل المرضى لتوفير حياة افضل لمرضاهم.

The expression of CD44 in colorectal cancer and association with clinicopathological features

Abstract

Background: Cluster of differentiation number 44 (CD 44) is a family of cell-surface adhesion molecules which exist in several isoforms arising from mRNA alternative and encoded by using gene located at chromosome 11 on the short arm p13. The CD44 gene is composed of 10 constitutively spliced exons and 10 variable exons, residing between constitutive exons 5 and 6.In colorectal carcinoma, the relationship between the expression of variant exons and tumor progression is controversial. Some studies have suggested that the expression of certain variant exons resulted in increased tumor progression, while other has no correlation. Objectives: Investigate CD44 immunoreactivity in colorectal carcinoma and determine the association with clinicopathological features. Material and methods: Paraffin- embedded tumor specimens from (70) patients with colorectal adenocarcinoma and (70) healthy control were assessed by immunohistochemisrty for the expression of CD44. Result: Statistical analysis of CD44 expression revealed highly significant difference in colorectal carcinoma patients than in control group. Also there were a relationship between CD44 expression and range of clinicopathological features. Conclusion: CD44 is robust marker for colorectal carcinoma.

Abstract

The CDKN2A gene (encode cyclin dependent kinase inhibitor 2A) is considered as a tumour suppressor gene involved in cell-cycle control, and methylation in promoter regions of this gene is a frequent event in CRC patients as it associated with the loss of p16 protein expression in cancer cells leading to gene silence. The methylation status of the p16 gene was examined in 35 samples of Iraqi colorectal cancers (primary carcinomas) and the five samples of Iraqi bowel inflammation patients using Methyl Sensitive High Resolution Melting and the correlation between the methylation status and the clinicopathological findings was evaluated. The results show aberrant methylation of the p16 gene was detected in 17 colorectal tumor samples out of the 35 (48.75% ) primary colorectal carcinomas but there no change in methylation status was found, suggesting that the aberrant methylation of p16 was frequently observed in Iraqi colorectal carcinomas. The clinicopathological data were then correlated with these results. Significant differences were observed with age (p<0.01), tumour location (p<0.01), moderately differentiated (p<0.01) and (T3) lymphatic invasion (p<0.01). This study provides evidence for hypermethylation status p16 gene in CRC patients, which may serve as useful information on CRC cancer progression.

يعد جين CDKN2A (الذي يشفر الى مثبط الكاينيز cyclin dependent kinase inhibitor 2A) جين كابح للسرطان والذي تتضمنه السيطرة على الدورة الخلوية, المثيلة بمناطق البادئ غالبا ما تحدث بمرضى اورام القولون والمستقيم وترتبط بفقدان التعبير عن البروتينp16 في الخلايا السرطانية مما يؤدي الى كبت او اخماد الجين.تم التحري عن حالة مثيلة جين p16 في 35 عينة لمرضى اورام القولون والمستقيم العراقيين و5 عينات لمرضى التهاب القولون باستخدام تقنية Methyl Sensitive High Resolution Melting وكذلك تم البحث في علاقة حالة المثيلة والصفات السريرية لهؤلاء المرضى. اظهرت النتائج مثيلة شاذة لجين p16 في 17 من 35 عينة بنسبة48.75% لعينات اورام القولون والمستقيم ولم يلاحظ اي تغيير بحالة المثيلة بعينات مرضى التهاب القولون مما يشير الى ان المثيلة الشاذة غالبا ما تحدث في مرضى اورام القولون والمستقيم العراقيين. بعد ذلك تم البحث بعلاقة الصفات السريرية مع هذه النتائج, وجدت اختلافات معنوية مع العمر(p<0.01) وموقع الورم (p<0.01) و moderately differentiated (p<0.01) وكذلك (T3) الغزو اللمفاوي للورم (p<0.01). هذه الدراسة تعطي ادلة على مثيلة عالية لجين p16 لمرضى اورام القولون والمستقيم العراقيين والتي ربما تفيد كدليل بتطور او تقدم اورام القولون.

Abstract

Background: Colorectal cancer is a common disease in old patients, but it has become increasingly evident in young patients.Objective: To compare the colorectal carcinoma, between young and old patients.Patients and Methods: a prospective study of 87 patients with colorectal cancer treated in the department of surgery at Al-Yarmouk Teaching Hospital, From Jan.2007-Jan.2012. The patients were divided according to age into group A; below 40years, and group B; above 40years. These two groups were compared regarding; clinical presentations, pathological features of the tumor, staging of the tumor, and sex.Results: Twenty three patients (26.44%) in group A; their age ranging from 12 to 40 years, with male to female ratio was 1.3:1 the commonest affected age group was 31-40 years old, positive family history was significantly higher than that in group B (39%), the commonest presenting features were altered bowel habits (87%) and abdominal pain (65%), left side colonic tumors were more common than right side tumors, Moderately differentiated tumors were constitute 52% and60.8% were stage C2, there was no significant difference between the two groups.Conclusion: there was an increasing incidence of colorectal cancer in young patients. Family history was a risk factor for group A patients. Diagnosis was usually delayed and this needs more attention because of the lack of alarming symptoms

Abstract

Background and objective: The products of BCL2 and P53 genes are involved in the regulation of proliferation and apoptosis and have been associated with prognosis in several malignancies, including colorectal carcinoma. This study aimed to investigate the expression of antiapoptotic BCL2 protein in colorectal carcinoma and to examine its association with one of the important mediators of apoptosis (P53 protein) and with clinicopathological factors, and to assess the prognostic value of the combined BCL2 /P53 phenotypes by studying their relation with the two most important prognostic factors of grading and staging.Methods: A retrospective and prospective study was carried out from the period of August 2010 to June 2011. Sixty eight formalin-fixed, paraffin-embedded blocks of colorectal carcinoma cases were evaluated regarding BCL2 expression, P53 nuclear accumulation and their concomitant expression using immunohistochemistry by Dako Cytomation.LSAB + System-HRP.Results: The study shows that 29.41% of the colorectal tumors were positive for BCL2 protein expression and associated with earlier stage tumors (P = 0.021), while 72.06% were positive for P53 protein expression and associated with later stage tumors (P = 0.006) and male gender (P = 0.005). There was a trend toward an inverse correlation between BCL2 and P53 expression (P = 0.0013). Tumors that did not express detectable levels of BCL2 but exhibited nuclear accumulation of P53 were most common and associated with later stage tumors (P = 0.003).Conclusion: Concomitant assessment of both BCL2 and P53 status may be valuable in predicting the aggressiveness of tumors in patients with colorectal carcinomas.

Abstract

Background: Increasing evidence arguing the association between metabolic syndrome andcolorectal cancer in general population is in progress. Since there is a lack of information aboutthis issue in Duhok population, the present study was conducted to investigate the associationbetween metabolic syndrome and its components in patients with colorectal cancer.Methods: A case control study was conducted on 158 subjects, 79 patients with histologicallydiagnosed colorectal cancer and 79 apparently healthy subjects. Demographic information wascollected for all subjects through an interview. Components of metabolic syndrome includingabdominal waist circumference (WC), blood pressure (BP), fasting serum glucose (FSG),triglyceride (TG) and high density lipoprotein-cholesterol (HDL-ch) were measured.Results: Of the seventy nine patients with colorectal cancer, 23 (29.1%) had metabolicsyndrome as compared to 20 (25.3%) of the healthy subjects, with rates of 20.2% and 7.6% formales and 8.9% and 17.7% for females, respectively. Patiexhibited a significantly higher prevalence of metabolic syndrome than did the healthy subjects(odds ratio= 2.08, p= 0.024).Conclusion: Patients with colorectal cancer may be associated with increased risk of metabolicsyndrome in a Duhok population, particularly among males and older age group.
