Abstract

Background: Oral lichen planus (OLP) is a chronic immunologic disease. The etiology of OLP is unknown, viralantigens (for example EBV) have been proposed as etiologic agents. OLP may get transformation to malignancy soresearch on the presence of these in OLP lesions seems to be necessary. The aim of this study was to evaluate EBVexpression immunohistochemically in OLP.Materials and Methods: Tissue specimens of 30 formalin fixed, paraffin-embedded tissue Blocks histologicallydiagnosed oral lichen planus was performed to evaluate EBV expression.Results: Expression of EBV was detected in epithelium of (46.6%) in the study samples in (OLP). no statisticallysignificant correlation was found with clinical parameters except for a significantly higher expression in females.Conclusions: Epstein Barr viruses were present in considerable amounts in oral lichen planus. Taking into account thepotential of viruses in OLP proving or disapproving or etiological role of viruses in OLP is continuously need to beexamined in further studies

Abstract

This study carried out to show the association between Epstein-Barr Virus (EBV) infection and asthma. a cohort 65 serum samples were collected from asthma patients within age group of 15-62 years ( 37 females and 28 males) and 20 healthy controls. The levels of IgE and EBV IgM antibodies were measured by ELISA assay. The results showed that IgE levels increased with severity of asthma (327.95 IU/ml) and the high level of IgE in asthma patients with age group (45-55 years) was 306 IU/ml. Sero detection of EBV showed that EBV infection increased with severity of asthma and was highest in females with severe asthma. serum IgE levels were high in asthmatic as compared to healthy controls and the highest levels of it was in severity of asthma. On other hand, there was weak significant correlation between levels of IgE and EBV infections in asthma patients.

جرت هذه الدراسة لإظهار الارتباط بين الإصابة الابشتاين-بار فايروس والربو. تم جمع 65 عينة من مصول مرضى الربو باعمار من 65-15 عاما 37) إناث و 28ذكور) و20 من الاصحاء كمجموعة سيطرة وتم تقدير مستويات الكلوبيولين المناعي IgE والأجسام المضادة لل EBV بواسطة تقنية الامتزاز المناعي المرتبط بالانزيم ELISA. أظهرت النتائج أن زيادة مستوياتIgE مترافقة مع ازدياد شدة الربو 327.95 IU/ml))، وارتفاع مستوى الكلوبيولين في مرضى الربو كان ضمن الفئة العمرية 55-45) سنة) 306 IU/ml. وأظهر الكشف المصلي لل EBVزيادة الإصابات EBV مع ازدياد شدة الربو وكانت أعلى نسبة في الإناث الذين يعانون من الربو الحاد. كان IgE بمستويات عالية في المرضى المصابين بالربو بالمقارنة مع الاصحاء وان أعلى مستويات IgE كانت في حالات الربو الشديدة. ومن جهة اخرى، كان هناك ارتباط ضعيف بين مستوياتIgE واصابات EBV في مرضى الربو.

Abstract

Background: Chronic periodontitis is an inflammatory disease that extends into the tissues supporting the teeth.Recent studies have demonstrated that various human herpesviruses especially Epstein-Barr virus (EBV) may play apart in the pathogenesis of human chronic periodontitis. This study aimed to detect anti EBV IgG and IgA in saliva ofchronic periodontitis patients and healthy control subjects by enzyme linked immunosorbent assay (ELISA) test and todetermine the differences between males and females regarding the periodontal condition and the levels of antiEBV IgG and IgA .Materials and methods: The study sample consisted of sixty chronic periodontitis patients of both gender (32 malesand 28 females) and thirty healthy control subjects of both gender (16 males and 14 females) with age ranged from30 to 50 years. Both groups without any systemic disease.Periodontal parameters used in this study were plaque index (PLI), gingival index (GI), bleeding on probing (BOP),probing pocket depth (PPD) and clinical attachment level (CAL). Unstimulated saliva samples were collected fromall subjects and examined by ELISA test for EBV IgG and IgA antibodies detection.Results: The results of the present study observed that there was no significant difference of PLI and GI betweenmales and females in chronic periodntitis patients. Concerning BOP the number of bleeding sites in females wasmore than in males. For PPD and CAL, there was increased PPD with its different scores (0, 1, 2) in males comparedwith females and there was increased CAL with its different scales (scales 0, 1, 2, 3) in males than females. Thepercentage of control group who were positive for anti- EBV IgG was (36.7%) and in chronic periodontitis was (81.7%). Concerning IgA, 40.0% of healthy group and 68.3% of chronic periodontitis patients showed a positivereaction for anti- EBV IgA .Conclusions: The present study showed that EBV Abs (IgG and IgA) were detectable in saliva from healthy individualsbut they were significantly more frequently found in saliva of chronic periodontitis patients. The present study showedthat there was no significant gender difference regarding the salivary level of anti- EBV IgG and IgA.

Abstract

التهاب النساغ المزمن هو مرض التهابي يمتد إلى الأنسجة الداعمة للأسنان وقد أثبتت الدراسات الحديثة أن. فيروسات قوباء المختلفة وخاصة فيروس ابشتاين بار قد تلعب دورا في التسبب في التهاب النساغ المزمن ويظهر ارتباطا مع شدة أمراض اللثة.هدف الدراسة هو ايجاد العلاقة بين مؤشرات التهاب اللثة وشدة الالتهاب مع المستوى اللعابي لمضادات فيروس ابشتاين بار.تألفت عينة الدراسة من ستين مصابا بالتهاب الأنسجة الداعمة المزمن من الجنسين(32 من الذكور والإناث 28 ) وثلاثين شخصا من الأصحاء من الجنسين ((16 من الذكور والإناث (14 )مع مستوى العمر يتراوح بين 30 إلى 50 سنة. كلا الفريقين دون أي أمراض جهازية. ومعلمات اللثة المستخدمة في هذه الدراسة تشمل النزف عند البحث (BOP)، سبر عمق الجيب (PPD) و مستوى الارتباط السريري (CAL). تم جمع عينات من اللعاب Unstimulated من جميع المرضى وفحصها الفحص المناعي انزيم مرتبط (ELISA test) لمفتش وايغا EBV وكشف الأجسام المضادة. أظهرت نتائج الدراسة أن متوسط المضادة للمفتش EBV ومستويات ايغا اللعابية في المرضى الذين يعانون التهاب اللثة المزمن هو 13،8992 HU / مل و U 20،8902 / مل على التوالي. في هذه الدراسة لم يكن هناك ارتباط كبير بين مستوى مضادات EBV اللعابية وBOP في المرضى الذين يعانون التهاب اللثة المزمن.بالنسبة لPPD، كان هناك ارتباط كبيرو إيجابية قوية مع درجة 1 و 2 مع مستوى مضادات EBV اللعابية، في حين EBV ايغا لديه علاقة إيجابية هامة مع درجة 1 و 2. بشأن CAL، كان هناك ارتباط إيجابي كبير مع مقياس 2 و إيجابية ارتباط كبير مع مقياس 3.أظهرت هذه الدراسة أنه قد يكون هناك وجود ارتباط بين الإصابة ب EBV و شدة التهاب اللثة المزمن وبالتالي عدوى مرافقة مع EBV قد تلعب دورا في تدمير الأنسجة اللثوية

Abstract

Background: Infectious mononucleosis (IM) is a lymphoproliferative disease caused primarily by theEpstein-Barr virus (EBV) infection. The initial viral infection by EBV occurs in B lymphocytes and isfollowed by an extensive proliferation of T lymphocytes. Previous studies on immunity to EBV(including IM) have mainly focused on activation of peripheral blood T cells, which are responsible forthe lymphocytosis in blood during acute IM.Patients and Methods: Indirect immunofluorescence technique analysis was performed to detect thepercentage of CD3, CD4, CD8, CD19, and CD56 positive lymphocytes.Results: Our results on the phenotype of T cells in samples from patients with infectious mononucleosisshowed that there is a significant increase in percentage of CD8+ T-cells when compared with healthygroup. In addition, increased in percentage of CD3 T-cells and moderately increase in CD56 (NK) cells.CD4 and CD19 percentage were significantly decrease in comparison with healthy control.Conclusion: Acute infectious mononucleosis is characterized by a marked increase in the percentage ofCD3 and CD8 T- cells with a slight increase in CD56 percentage. A marked decrease in CD19 percentageand CD4/CD8 ratio was noticed.

Abstract

Background: Except for the tight correlation to nasopharyngeal carcinoma, accumulating evidences show that Epstein-Barr virus (EBV) is correlated to other carcinomas. This study was to investigate the association of EBV with colorectal tumors.Materials & methods: Forty paraffin embedded blocks of colorectal tumors (thirty were adenocarcinoma and ten were benign tumors) were all examined for the presence of EBV DNA with the application of In Situ hybridization.Results: In Situ EBV DNA signals was detected in 6 out of 30 (20%) of colorectal carcinoma with no observed signals in the sections from benign group.Conclusion: Our results showed that infection of EBV exists in human colorectal adenocarcinoma, which indicates that EBV may be involved in the carcinogenesis process.Key words: EBV, In Situ Hybridization, colorectal tumors.

Abstract

Background: This study aimed to detect EBV quantitatively in plasma using real-time polymerase chain reactiontechnique in chronic periodontitis and gingivitis patients and to compare the finding with control subjects (healthyperiodontium) and to investigate the relationship between the presence of EBV & the severity of periodontaldiseases using the clinical periodontal parameters (PLI ,GI , BOP ,PPD and CAL) between each of (chronicperiodontitis and gingivitis) patients and compare with control (healthy periodontium) subjects .Materials and methods: The study sample consisted of (101) individual of both genders, (61) chronic periodontitispatients which were subdivided into (mild, moderate & severe) depending on the scores of clinical attachmentlevel, (20) gingivitis patients and (20) control subjects (healthy periodontium) with age ranged from (30-50) years. Allthe groups were without any history of systemic diseases. Clinical periodontal parameters used in this study were (PLI,GI , BOP ,PPD and CAL) .Blood samples were collected from all individuals and examined by Real-Time PCRtechnique for the detection of EBV.Results: The result of comparison for the occurrence of EBV among study and control groups according to sequentialresponding of EBV appears to be highly significant at negative level of EBV, significant in (100 - 500 copy/105 cells)level and the results of leftover levels appear non significant difference. The result of correlation between the actualoccurrence of EBV and PPD scores in severe chronic periodontitis subgroup appears to be significant at PPD score(1) and non significant at scores (2&3). The correlations between EBV and PPD scores in moderate and mild chronicperiodontitis subgroups appear to be non significant with all scores. The results of correlation between EBV and CALparameter appear to be non significant among all scores of chronic periodontitis subgroup. Concerning plaqueindex, the correlation appears to be significant in mild chronic periodonttis subgroup and highly significant in controlgroup. In case of gingival index, the correlation appears to be significant in severe chronic periodontitis subgroupand control group .The result of correlation with (B.O.P. score 1) appears to be highly significant in severe subgroup ofchronic periodontitis group and significant in gingivitis group, while in case of (B.O.P. score 0), the correlationappears to be significant only in severe chronic periodonttis subgroup.Conclusions: The present findings revealed that there may be an association between EBV infection and the severityof periodontal diseases and thus coinfection with EBV may play a role in increase destruction of periodontal tissues

Abstract

Autoimmune diseases are complex and happens when the human immune system attacks tissue host for unknown etiology or maybe by some other genetic cofactors or environmental agents (mainly viral particles, bacterial and other infectious agents; by acting as a major cofactors and serving as important initiators in the disease progression for a specific autoimmune sickness and not causing illness on their own) or even as complications or opportunistic infections associated with the disease. The present study aimed to: (I) study the linking of certain infectious viral agents with human autoimmune diseases. (II) Determine possible roles of some studied clinico-pathological factors like; gender, age and to investigate the prevalence of Seropositivity of the antibodies titer of EBV, Rubella virus and CMV with autoimmune diseases development in human. In this study, four groups candidates of Celiac diseases (CD) n=31, Diabetes Mellitus –I (DM-I) n=30, Rheumatoid Arthritis (RA) n=30 & Systemic lupus erythematous (SLE) n=30 were studied from 1st – 30th of January 2017 at Medical City Baghdad. Demographic and laboratory data were collected. All the samples were examined for the presence of EBV VCA-IgG, Rubella IgG and CMV IgM by Indirect and Sandwich ELISA techniques by using a commercial kits from DIAGNOSTIC AUTOMATIONS CO. USA. A total of 121 autoimmune disease patients were studied , the overall positive results for EBV VCA IgG was (68.6 %) while for Rubella IgG was (50.4%) and for CMV IgG was (52.1%) with significant differences in comparing with the negative results (p < 0.05). Celiac disease and diabetic type-I patients had been detected with high Seropositivity for EBV VCA IgG as (22.3%) for each of them, While the high positive results of Rubella IgG was detected in Celiac disease patients as (16.5%). Finally, the diabetic Type-I patients showed the most high Seropositivity for CMV IgG as (17.4%) in significant differences with the negative results regarding to all the other positive ones (p < 0.05).

أمراض المناعة الذاتية معقدة وتحدث عندما يهاجم جهاز المناعة أنسجة المضيف لسبب غير معروف أو ربما لبعض العوامل الوراثية الاخرى أو مؤثرات بيئية (خاصة الفيروسات والبكتيريا والكائنات الحية الدقيقة المعدية الاخرى ؛ وذلك بالقيام بدور العوامل المساعدة الرئيسية والتي تعمل كمحفز هام في تطور المرض إلى مرض محدد من إمراض المناعة الذاتية ولا تسبب )I المرض بنفسها) أو حتى من الممكن إن تكون مصاحبة للمضاعفات أو االلتهابات الانتهازية للمرض. هدفت الدراسة إلى: ( ) تحديد الادوار المحتملة IIمناقشة الادلة التي تربط بعض العوامل المعدية (الفيروسات) مع أمراض المناعة الذاتية في البشر. ( لبعض العوامل المرضية الاكلينيكية المدروسة مثلا ؛ الجنس والعمر والتحري عن انتشار إيجابية المصل لمعيارية األجسام المضادة لفيروس ابشتاين بار، فيروس الحصبة الالمانية والفيروس المضخم للخلايا مع تطور أمراض المناعة الذاتية في البشر. في هذه 31 الدراسة ، أربعة مجاميع مشاركة من الداء الزالقي عدد = ، مرض السكري من النوع الاول عدد ، التهاب المفاصل 31= 30 الرثوي عدد 30 = و الذئبة الحمراء عدد 30 = ، درست من تاريخ ( يناير – 1) في مدينة الطب بغداد. وقد تم 2017 EBV VCA جمع البيانات الديموغرافية و المختبرية. وفحصت جميع العينات لوجود -IgG ،Rubella virus IgG ،C MV . الـ الغير المباشرة والساندويش باستخدام معدات مختبريه تجارية من ELISA بواسطة تقنية IgM DIAGNOSTIC من مرضى المناعة الذاتية التي تم دراستها ، كانت النتائج اإليجابية 121من مجموع AUTOMATIONS CO. USA us دRubella vir)، في حين %68.6(EBV VCA IgGالشاملة لـ 50.4 كانت IgG 52.1 كانت CMV IgM ( و ٪ ) )٪( مع وجود اختالفات كبيرة مقارنتا مع النتائج السلبية . ( تم الكشف عن الداء الزلاقي وداء السكري من النوع p>0.05) EBV VCA الاول باستجابة مصلية عالية لـ Rubella ) لكل منهما، في حين أن النتائج ألايجابية العالية من %22.3 - كــ ( IgG ). وأخيرا، أظهر المرضى بداء %16.5 تم الكشف عنها في المرضى الذين يعانون من مرض الداء الزالقي كـنسبة ( virus IgG ) وفي اختالفات كبيرة مع النتائج السلبية وعلاقتها %17.4 كـنسبة ( CMV IgMالسكري من النوع الاول استجابة مصلية عالية لـ .)p < 0.05 بالنتائج الايـــــــجابية

Assessment of Co-Infection of Human Cytomegalovirus DNA and Epstein Barr-Virus (ZEBRA-Genes) in Tissues of Ovarian Tumors

Abstract

Infection by Epstein Barr-Virus (EBV) begins with a short replication phase. The virus remains in a latent phase, only entering the lytic phase in response to a cascade of transcriptional signals. These signals are triggered by the ZEBRA protein (BZFL1) along with Rta (BRLF1). Human cytomegalovirus (HCMV) is responsible for a lifelong persistent infection which ranged from 50% to 90% in adult population, and is related to either socioeconomic status or geographic location. HCMV infection might lead to buildup tumor cells via the protection of certain tumor cells from apoptosis and modulating angiogenesis. The study was designed to determine ZEBRA-EBV gene and DNA-HCMV infections in tissues from ovarian tumors. An 150 ovarian tissues were examined for ZEBRA-EBV gene with DNA-HCMV. Those samples belonged to (45) patients diagnosed with ovarian cancer ; (45) from benign ovarian tumors and (20) patients with borderline ovarian tumors as well as (20) apparently normal ovarian tissues. The detection of ZEBRA-EBV gene and DNA-HCMV were done by chromogenic In situ hybridization (CISH). The positive results of ZEBRA - EBV -CISH detection in malignant ovarian tumors, where 64.4% (29 out of 45 tissues) showed positive signals. While ,in the benign ovarian tumors group were 37.8 % (17 out of 45 tissues) ,followed by borderline ovarian tumors & the apparently healthy ovarian control tissues were 30% (6 out of 20 cases) and 7.5% (3 out of 40 cases), respectively. The present study shows the positive results of HCMV-CISH detection in malignant ovarian tumors, where 55.6% (25 out of 45 cases) showed positive signals, while, 44.4% negative signals which represented 20 out of 45 cases in this group. While, in the benign ovarian tumor group was 35.6% (16 out of 45 cases). Negative signals which in benign group represented 29 out of 45 cases constituted 64.4% .Whereas ,the positive results in borderline ovarian tumor group were 40% (8 out of 20 tissues), followed by the apparently healthy ovarian control tissues was 12.5% (5 out of 40 tissues). We concluded from this study, ZEBRA -EBV genes as well as HCMV-DNA positive signals in malignant, borderline and benign tumors tissues ,they suggest an important role for these viruses in the development of ovarian tumors in Iraqi patients.

Abstract

The frequency of Epstein-Barr Virus (EBV) antigen; latent membrane protein-1 (LMP-1) expression in cases of laryngeal carcinomas was investigated by immunohistochemical staining technique. Fifty-six cases were included in this work; all cases of laryngeal carcinomas were squamous cell carcinomas with different degrees of differentiation. Immunohistochemical staining results revealed 60.7% positive staining results in overall cases, distributed according to the degree of differentiation almost equally (60% in well differentiated, 64.5% in moderately differentiated and 60% in poorly differentiated). The concordance of cigarette smoking with LMP-1 expression was high (96%). These results indicate the highly possible oncogenic effect of EBV infection on the development of laryngeal carcinomas in Iraqi patients.

في هذا البحث تم الاستقصاء عن عدد مرضى سرطان الحنجرة المصابين بفايروس EBV من خلال التعرف على احد المستضدات الخاصة به (Latent Membrane Protein-1: LMP-1) باستخدام تقنية المعلمات المناعية النسيجية. تم ادراج ست و خمسين حالة من حالات سرطان الحنجرة في هذا البحث. كانت كل الحالات من نوع السرطان الخلايا الحرشفية مع درجات مختلفة من التمايز. أظهرت نتائج استخدام تقنية المعلمات النسيجية المناعية نسبة ايجابية في 60,7% في مجمل الحالات، مع نسبة متشابهة تقريباً لجميع درجات التمايز (60% للتمايز الجيد، 64,5 % للحالات متوسطة التمايز و 60% للحالات ضعيفة التمايز). كما أظهرت الدراسة نسبة عالية من التزامن بين التدخين و الاصابة بفايروس EBV من خلال التعرف على مستضد LMP-1 البالغة نسبتها 96%. هذه النتائج تشير إلى التأثير المسرطن المحتمل لفايروس EBV في نشوء سرطان الحنجرة في العراق.
