Open Reduction and Internal Fixation Compared to Closed Reduction and External Fixation in Distal Radial Fractures Arandomized Study of 40 Patients

Abstract

Objective: to investigate if open reduction and internal fixation would produce better outcome than external fixation of distal radial fractures.Patients and Methods: 40 patients with unstable or comminuted distal radial fracture where divided randomly into two groups using 2 methods of treatment, in group one closed reduction &bridging external fixation was used ,in group tow open reduction &internal fixation was used. The primary outcome parameter was grip strength, but the patients were followed for 1 year with objective clinical assessment, subjective outcome and radiographic examination.Results & Discussion: At one year postoperatively , grip strength was 89%(standard deviation 15) of the injured side in the internal fixation group and 75%(15) in the external fixation group. Pronation /supnation was 150 (15)in the internal fixation group &136 in(20) in the external fixation group at 1year.4 patients in the external fixation group where reoperated due to malunion as compared to 2 in internal fixation group.6 others are classified as radiographic malunion, 5 in the external fixation ,1 in the internal fixation group.Conclusion: internal fixation gave better grip strength &and better range of motion at one year, and less malunion than external fixation. No difference could be found regarding subjective outcome.

Abstract

Background: Numerous methods have been described for achievement of Intermaxillary fixation in the treatment offractures of facial skeleton. Conventional methods like Erich arch bars and eyelet wires are currently the most commonmethods for achieving intermaxillary fixation (IMF), however, they have their own disadvantages. Since 1989, IMF usingintraoral self-tapping IMF screws has been introduced for treatment of mandibular fractures. The aim of this study wasto evaluate the efficacy, advantages, disadvantages and potential complications associated with using of selftappingIMF screws in the treatment of mandibular fractures.Material and Methods: Twenty patients with favorable mandibular fractures, attended to Oral and Maxillofacial Surgeryunit, AL-yarmuk Teaching Hospital between November 2014 and October 2015, they were treated with IMF screws. Theparameters considered were duration of the procedure, perforations in the gloves, patient acceptance, oral hygiene,iatrogenic dental injuries, mouth opening, healing outcome, occlusal discrepancy and needle stick injuries during IMF.Results: The patients included in this study were 20 (17 males and 3 females). The extremes of age in this study rangedfrom 12 to 37 years. Three patients had 2 fracture lines. Assault was the most common cause of fractures. The mostcommon site of fracture was the body and parasymphysis regions. Two screws (2.5%) from 80 screws becameloosened. One patient (5%) ended with malunion and malocclusion. One patient (5%) developed infection at screwsite. Three patients (15%) developed screws soft tissue burying.Conclusion: IMF screws considered to be a useful modality of treatment to establish maxillomandibular fixation. It issafe, and time-sparing technique; however, it is not without limitations or potential consequences in which the surgeonmust be aware of in order to provide safe and effective treatment

The Use of Simple Monolateral Bar External Fixation as a definitive treatment of Humeral Shaft Fractures

Abstract

Background: Dynamic Compression Plating (DCP) has been the most accepted available surgical fixation method for fractures of humerus shaft in our country. DCP has biological shortcomings. By giving priority to biology, the "internal fixator" principle, has been popularized, bearing some similarity to an external fixator (EF).Aim of the study: EF is being evaluated as an acceptable definitive fixation for this injury.Methods: Between January 2005 and April 2009, 31 patients with acute fracture of the humeral shaft requiring surgical stabilization were treated using simple unilateral bar EF. Reduction achieved either closed or open. Time till union, time till removal of EF, and elbow & shoulder range of motion were recorded, and compared with corresponding values from studies using DCP or EF. Results: Average time till surgery was 5.32 days. Average time for union and dynamization of EF was 11.55 weeks. And for removal of EF was 15.1 weeks.Conclusion: Monolateral bar EF for treatment of humerus shaft fractures is reliable, effective, low risk, with maximum protection of soft tissue. Considering the renewed attention in biological approaches, it’s a reasonable alternative to DCP, the only surgical alternative available in our country.

مقدمة : تعتبر صفيحة الضغط الديناميكي الخيار الاكثر قبولا و المتوفر لغرض التثبيت الجراحي لكسر ساق عظم العضد. لكن هذه الصفيحة ليست بدون جوانب سلبية من الناحية البايولوجية. في محاولة لأعطاء الاولوية للناحية البايولوجية تم تطوير مبدأ المثبت الداخلي و الذي يشبه من حيث المبدأ المثبت الخارجيهدف الدراسة : دراسة استعمال المثبت الخارجي كتثبيت نهائي لهذا الكسرالطريقة : منذ كانون الثاني 2005 و لغاية نيسان 2009 تم علاج 31 مريض مصاب بكسر ساق عظم العضد و المتطلب تثبيتا جراحيا بواسطة المثبت الخارجي ذو الانبوب و المستوي الواحد. تم ارجاع الكسر بالطريقة المغلقة أو المفتوحة. تم تسجيل الوقت لحين الألتحام, الوقت لحين رفع المثبت الخارجي و مدى حركة الكتف و تمت مقارنتها بالنتائج المقابلة من دراسات أخرى تستعمل صفيحة الضغط الديناميكي أو المثبت الخارجي النتائج : كان معدل الوقت لحين اجراء التداخل الجراحي 5.32 يوم, و معدل الوقت للألتحام و ترخية و تحميل جهاز المثبت الخارجي 11.55 يوم فيما كانت الفترة لحين رفع المثبت الخارجي الأستنتاج: اختيار المثبت الخارجي ذو الانبوب و المستوي الواحد لعلاج كسر ساق عظم العضد يمثل خيارا فعالا , مقنعا , قليل الخطورة مع حماية قصوى للأنسجة المحيطة و عند الاخذ بنظر الأعتبار الأهتمام المتجدد بالناحية البايولوجية فأنه يعتبر بديلا معقولا لصفيحة الضغط الديناميكي

Abstract

ABSTRACT:BACKGROUND:High energy pelvic ring fractures are serious injuries result from motor vehicle accidents or fall from height, unstable pelvic ring injuries usually associated with sacroiliac disruptions or sacral fractures. The ideal treatment for unstable pelvic fractures remains a matter of debate. The main purpose of the treatment is to save the patient s life and then to achieve an excellent functional outcome. Percutaneous fixation of Sacroiliac joint disruption or sacral fractures is evolved recently as a minimal invasive technique with low complication rate and morbidities.OBJECTIVE: A preliminary study to evaluate prospectively the effectiveness of the percutaneous cannulated screws fixation of posterior pelvic ring injures in unstable fractures with associated injuries .PATIENTS AND METHOD:Six patients have been treated in Medical City/Baghdad (during the period of Jan.2012 till July 2012) by one surgeon, four males and two females with mean age 27 years (range 20-36 years), who have unstable pelvic ring injuries associated with other skeletal, neural or visceral injuries are treated by gradual closed reduction using skeletal traction through lower femur and percutaneous sacroiliac fixations using a 7 mm Cancellous partially threaded screws following the technique described by Matta and Saucedo using C-arm fluoroscopy. A data for the duration of procedure, blood loss, intra-operative and post-operative complications and post-operative rehabilitation were collected analyzed and compared to data for open reduction and internal fixation for the same injuries from other studies . pre and post-operative radiographs and CT scans of the pelvis were studied to evaluate the procedure. RESULTS: A total of 13 screw where inserted percutanously to a six patients . The duration of the surgical procedure was 30 minutes to 60 minutes including the preparation time. Wound size was 5mm-10mm, Blood loss was negligible, no anesthetic complications no intra or post-operative complications, no wound complications, and a very good post-operative rehabilitation. post-operative X-rays and CT scans show no complications and a very good reduction and fixation for the Sacroiliac joints and sacral fractures.CONCLUSION:Gradual closed reduction and Percutaneous sacroiliac fixations using cannulated screws have demonstrated a very good minimally invasive technique with no intra-operative or post-operative complications. This technique may be advantageous as it avoids using extensive approaches, bleeding, wound complications and prolonged surgeries.

Abstract

Objective: To evaluate the results of external fixation for intertrochentric hip fractures in elderly patients with high anesthetic risk.Patients and methods: Prospective study conducted at Department of Orthopedic surgery in Al-Kadhemia Teaching Hospital from January 2007 to October 2013 to evaluate 31 patients, with intertrochentric fracture and high risk for anesthesia, 19 females and 12 males, age between (47 – 90) years. All fractures were stabilized with external fixator under spinal, epidural, general and even local anesthesia.Results: The average time of operation was 35 minutes (25-50), no one needs blood transfusion.No mortality in immediate post operative period .but three died after 10 days of operation because of co morbid disease. Bony union is obtained in the rest of patients, in 16 weeks, most patients had excellent and good results according to judget functional score, Pin tract infection and Deep vein thrombosis is the commonest complication.The use of external fixator was associated with significant less blood loss, shorter operative time, reduction in post operative pain, shorter hospitalization.Conclusion: External fixation in elderly patients with high anesthesia risk is fast, minimally invasive procedure in the treatment of intertrochentric fracture, resulting in fewer pre and post operative complication.

يعتبر كسر بين المدورين الفخذي من الكسور الشائعة بين كبار السن.ونضرا للإمراض المرافقة والمزمنة عند المريض التي تجعل تخدير المريض خطرا وصعبا ,قمنا باستعمال طريقة التثبيت الخارجي في تثبيت الكسر باستعمال جهاز الاشعه وبطريقه مغلقه.أجرينا العملية على واحد وثلاثون من المرضى كبار السن في ردهة الكسور في مستشفى ألكاظميه التعليمي المصابين بكسر بين المدورين والمصابين بإمراض مزمنة وتحتم استخدام طريقه علاج سريعة وأمينهأثبتت المتابعة لمده ستة أشهر إن 100% من الكسور قد التأمت في الوقت المحدد وان التقييم الو ضيفي للنتائج كان كالآتي:ممتازة 21%,جيده 43%,مقبوله29%,ضعيفة 7%إن تثبيت الكسر بين المدورين بالتثبيت الخارجي هي عمليه بسيطة ومعتمده لتثبيت هذا النوع من الكسور خصوصا للمرضى كبار السن والمصابين بإمراض أخرى ويمكن اعتبارها بديلا جيدا للتثبيت الداخلي.

Abstract

Background:The embalmed human cadavers used for teaching anatomy in the medical colleges could be preserved for a very long time, the crucial factors for acheivement of this prolonged preservation was not fully discussed.Objectives:This is an experimental study to evaluate the veracity of using paraffin sectioning feasibility as a test for the degree of damage in the embalmed cadaveric tissue affected by recurrent and prolonged air drying in anatomy laboratories.Methods:Routine paraffin sections were done for tissues obtained from cadaveric organs exposed to recurrent and prolonged air drying and tissues from cadaveric organs preserved most of the time in a hydrated condition.Results:The results of this study showed unblemished paraffin sectioning of the emblamed cadaveric tissue preserved with good hydration. The cadaveric tissues exposed to reccurent air drying could not be sectioned properly, the sectioned tissues were hard and brittled.Conlcusion:The experts dealing with the embalmed cadavers evidently necessitude maintancance of cadaveric hydration as a requisite to prolong durability of the demonstrative details in the dissected cadavers. Accordingly; the comparably more proper paraffin sectioning of the well hydrated cadaveric tissue may be considered as a sign of more durable embalmed cadaveric organs.Keywords:cadaver, paraffin, anatomy, fixation, formalin.

Abstract

Objectives: to assess nurses' practice level toward care of fracture treated with external.Methodology: A descriptive study was carried out at orthopedic wards in Al-Zhrawi Surgical Hospital -Missan Governorate. The study started from November 11th, 2014 to June15th, 2015. A non-probability sample of (50) orthopaedic nurses (male & female) who were working in orthopedic wards. The data were collected through the use of questionnaire, which consists of three parts, part(1)Demographic data form that consists of (8) items, part(2) concerning nurses' practices level toward patient treated with external fixation consist from (25)items, part(3) consist from(9) items concerning with external fixation device care. Checklist was used to collect data for orthopedic nurses. The validity of the checklist was determined through presenting it to (15) specialist expert and its reliability were determined through a pilot study which was carried out through the period from (22Jan-to 10th Feb, 2015). Descriptive statistical analysis procedures inferential analysis procedures were used for the data analysis.Results: Discussions of the study result indicated that the majority of items had low M.S (less than1.66)Conclusion: From the results it conclude that the orthopedic nurses have inadequate or deficit in some aspects related to postoperative care for patient undergoing external fixation surgery in orthopedic wards.Recommendations: The researcher recommend a special training sessions should be designated and presented to all orthopedic nurses that include specific training programs about nurses' intervention concerning care of fracture treated with external fixation. Authorizing and distributing a manual handbook or pamphlet to all nurses who work in orthopedic wards including nursing interventions in postoperative.

الهدف: تهدف الدراسة لتقييم مستوى ممارسات الممرضين تجاه العناية بالكسر المعالج بالتثبيت الخارجي.المنهجية: دراسة وصفية اجريت في ردهات جراحة العظام في مستشفى الزهراوي الجراحي- محافظة ميسان للمدة من 11 تشرين الثاني 2014 ولغاية 15 حزيران 2015.اختيرت عينة غرضية "غير احتمالية" مكونة من (50) ممرضا وممرضة من العاملين في ردهات الكسور. جمعت البيانات من خلال استبانة مصممة ومكونة من ثلاثة اجزاء, الجزء الاول شمل الخصائص الشخصية للمرضين ويحتوي على(8) فقرات, والجزء الثاني تخص ممارسات الممرضين تجاه المرضى الذين يجرى لهم جراحة التثبيت الخارجي مكونة من (25) فقرةو الجزء الثالث يتكون من (9) فقرات تختص بممارسات الممرضين بخصوص العناية بجهاز التثبيت الخارجي.جمعت البيانات بطريقة الرصد للمرضين العاملين في ردهات الكسور. حددت مصداقية استمارة الاستبانة من خلال عرضها على (15) خبير من ذوي الاختصاص وحدد ثباتها من خلال اجراء الدراسة المصغرة للمدة من 22كانون الثاني 2014 ولغاية 10 شباط 2015. تم وصف تحليل البيانات باستعمال اساليب الاحصاء الوصفيو الاحصاء الاستنتاجي.النتائج: بينت النتائج ان غالبية فقرات الدراسة قد اظهرت ضعفا في التداخلات التمريضية بعد اجراء جراحة التثبيت الخارجي.الاستنتاج: نستنتج ان هناك نقصا وقلة في ممارسات الممرضين العاملين في ردهات الكسوربعد اجراء جراحة التثبيت الخارجي .التوصيات: يوصي الباحث بضرورة اعداد برامج خاصة لتدريب الملاكات التمريضية العاملة في وحدات الكسور تجاة العناية بالكسر المعالج بالتثبيت الخارجي. تصميم كتيب او كراسحول كيفية تقديم العناية التمريضية للمرضى بعد اجراء جراحة التثبيت الخارجي .

Abstract

Lichen is a Symbiotic system consist of nitrogen fixing Cyanobacteria or and algae andfungus. Cyanobacteria can fix nitrogen in a peculiar differentiated cells called heterocystunder aerobic conditions, those heterocyst are the loci of nitrogenase activity. The organicnitrogen and carbohydrates produced by cyanobacteria utilized by fungi and the fungussupplying the cyanobacteria and algae by inorganic metals coming from dissolved rocksby acids produced by fungi. Culture of Nostocspp treated with MOS excreted, the newlyfixed nitrogen in form of ammonia into liquid media, which is proportions with theconcentration of the analog and detected after 6 h of treatment. Acetylene reductiontechnique (nitrogenase activity) was not affected by analog (MSO) treatment. The growthof Cyanobacteria Nostocspp was slightly inhibited starting after 6 h of treatment. The ratesof carbon fixation were highly enhanced after treatment leading to increase the number of(PGBS) in the Cyanobacteria cells. The combination treatment of Nostocspp culture byMSO and DCMU showed the following: carbon and nitrogen fixation are dependent ofeach other, slight inhibitions in culture growth, 50% inhibition in ammonia release,complete inhibition of carbon fixation and disappeared of extra PGB. Light intensities andcarbon fixation are dependent even in the presence of MSO. The cultivation of Nostocsppculture under Ar/O2/CO2 in presence of MSO gives reverse relationship between thecellular incorporated carbon and time. Electron micrograph showed an increase in PGBs ofthe lichen P. conina treated with MSO through the first hour of treatment and thendisappeared after 24 h.

اظهرت المزارع الخلوية للنوعالوسط الزرعي السائل بعد ستة ساعات من المعامله على شكل امونيا وان الكمية المفروزة تناسب طردياأ مع تركيزفي حين كان هناك تثبيط بسيط في النمو مما ادى الى ,MSO لم تتاثر فعالية انزيم النتروجينتز بمعاملة ال .MSO مادةبمادة Nostoc spp. في السايتوبلازم للخلايا. ان تداخل معاملة الخلايا للنوع PGBS زيادة في اعداد جسيماتاوضحت الاتي : ان عملية تثبيت الكاربون والنتروجين معتمدان بعضهما على البعض , DCMU وب MSOتثبيط بسيط في نمو الخلايا, تثبيط 50 % من امونيا المفروزة خارج الخلية , تثبيطكامللتثبيت الكاربون واختفاء جسيماتان , MSO الزائدة , شدة الاضاءة وتثبيت الكربون يعتمدان على بعضها حتى في حالة معاملة الخلايا بمادة PGBادى الى علاقة Ar/ O2/ CO تحت خليط من 2 MSO المعاملة بمادة Nostoc spp. تنمية المزارع الخلوية للنوععكسية بين الكاربون الخلوي التركيبي والزمن المعتمد في تنمية الخلايا , كما تبين دراسة المجهر الالكترونيخلال الساعة الاولى من PGBS ازدياد عدد جسيمات MSO المعاملة بمادة P. canina لعينات لآسنات من النوعالمعاملة واختفائها كلياأ بد 24 ساعة من المعاملة .

Abstract

AbstractThe basic goal of fracture fixation is to stabilize the fractured bone, to enable fast healing ofthe injured bone, and to return early mobility and full function of the injured extremity [1]. Thecomputerized tomography scan slices of the femur bone of one patient were translated to theANSYS V.(10) program and then the solid model (3-D) model was built and analysed by the finiteelement method under different loading conditions for each method of fixation. Finite elementanalysis would help to reveal stress pathways across the shaft of the femur bone and the sites of the8 screws or pins fixed at the shaft of the femur above and below the fracture site.In the internal fixation method two types of screws' materials can be studied. Firstlystainless steel and secondly cobalt chromium. The stress distributions were studied for each screwmaterial for different body weights. While in the external fixation method the stress distributions foronly stainless steel pins were studied.The stress distributions for six pins or screws fixed at the fractured bone above and belowthe fracture site for each method of fixation were studied.The von mises stresses of the cobalt chromium screws fixed internally are less than the vonmises stresses of the stainless steel for each of the body weight. The stresses of the pins fixedexternally are greater than the stresses of the same screws fixed internally.When decreasing the number of the screws or the pins the stress values will increase;therefore when using eight screws or pins the stresses are less than those when used six screws orpins for each method of fixation (internal and external).

Evaluation of Functional Outcome after Open Reduction and Internal Fixation of Capitellar Fracture with K. Wires

Abstract

ABSTRACT :BACKGROUND :Isolated capitellar fractures are uncommon. Authors continue to differ about the preferred method of treatments and it's results on the post operative outcome.OBJECTIVE:Of This study: Is to evaluate the clinical and functional out come of operatively treated capitellar fracture with K.Wire , and furthermore to define the impact of fracture type on the out come.PATIENTS AND METHOD:A retrospective study carried out on 15 patients Sustaind closed capitellar fracture , 11 females , 4 males. The main age (16 – 58 years). the Operative treatment performed at a mean of 10 days, by open reduction through relatively limited lateral approach and internal fixation with k. wires. The evaluation of the functional outcome was by clinical and radiographic examination . The range of motion (ROM) by using of Mayo Elbow Performance Index (MEPI), and The American Shoulder and Elbow Surgeons (ASES) scales , and self assessment questionnaires form.RESULTS:Eight patients type I , one type II , four type III , two type IV ,and three patients had ipsilateral radial head fracture,. two in association with type III , and one with type IV. All fractures healed , with no significant clinical instability or weakness. The mean ulno-humeral motion (UHM) was (95o-132o) . All patients had full forearm rotation, average flexion contracture was 15o (range (8o-35o). (P=0.05). The mean (MEPI) was (90±10) points, with seven excellent results and six good , and the mean (ASES) was 36±3 (30 to 40 points) in seven patients. Ipsilateral radial head fracture had no significant effect on the functional outcome.CONCLUSION:Good to excellent outcome with functional ulnohumeral motion (UHM) can be achieved following open reduction and minimal fixation with k. wires in the treatment of capitellar fractures.
