Effect of tooth brushing technique on oral hygiene in patients with fixed orthodontic appliances

Abstract

Background: This study was conducted to find whether the brushing technique (horizontal technique versus roll technique) would affect the oral hygiene of orthodontic patients or not.Materials and methods: This study was conducted on fifteen patients who were undergoing orthodontic treatment and were submitted to the clinical trial by changing the tooth brushing maneuver throughout the orthodontic treatment course from the patient’s own brushing maneuver to the horizontal and then to the roll brushing technique.Results: The results showed that the roll technique had a superior cleansing effect than the horizontal one, and the gender variation had no effect.Conclusion: Patients' instructions are considered as an important factor for planning good oral hygiene.

Abstract

Fixed orthodontic appliances facilitate plaque accumulation and the consequent development of generalized moderate hyperplastic gingivitis . This longitudinal study was to assess the periodontal health of the patients undergoing orthodontic treatment in Ramadi city.Materials and Methods: A clinical study was conducted by Examining 208 first molars 108 with labial bond and 100 with band. Teeth with orthodontic bands, 58of them cemented with glass ionomer cement (Ketac-Cem, Espe, Germany)& 42 cemented with glass inomer cement with chlorohexidin diglunate in 18% , thenthe plaque index, gingival index & Pocket depth were measured on the first molars. Results: during orthodontics treatment Plaque and Gingival indices and Pocket depth was significantly higher than in pre &post treatment .Adolescents showed more plaque accumulation and gingival inflammation than adults the Plaque and Gingival indices and Pocket depth were all significantly higher for banded maxillary and mandibular molars than for the analogous bonded molars. This study show the band cemented with glass inomer cement only is highly significant in Plaque and Gingival indices and Pocket depth than cemented with chlorohexidine glass inomer cement.The findings of this study are predicated on the delivery of preventive periodontal treatment in conjunction with orthodontic treatment to prevent the anticipated periodontal destruction.

Abstract

Aims: To evaluate the amount of nickel ion release from stainless steel orthodontic wires in different artificial saliva and in different time periods. Materials and Methods: This study included six different commercial stainless steel orthodontic wires with different diameters. They were immersed in deionized water (control) and three different artificial saliva, the immersion periods included 7, 14, and 30 days. A graphite-furnace atomic absorption spectrometer (AAS) was used to determine the amount of nickel ions release. Results: The statistical analysis showed that there is a highly significance in the release of nickel ions for all types of wire except wires 0.18, and 0.16 x 0.22 which gave non-significant 2.45 and 1.37 respectively, and the time period of immersion had no significant effect in all types of wire. Conclusions: The release of nickel ions was influenced by the types of wire and the surface areas that in contact with immersion in solution, in addition to the composition of the orthodontic wires, but this were not proportional to the content of metal in the wire.

Abstract

Background: Fixed orthodontic appliance considered a risk factor that effect oralhealth status. Aims of this study were evaluation of oral hygiene, gingival healthand caries experience among patients with fixed orthodontic appliance atdifferent time intervals of treatment.Material & Methods:- 32 patients (24 female and 8 male) included in this study andthey underwent clinical examination plaque, calculus, gingival health and cariesexperience by using Loe, 1967, Green and vermillion, 1960 and WHO, 1987indices at four time intervals: 2-3 weeks after appliance insertion, 2 months, 4months, 6 months. The statistical analysis of data was by using statisticalPackage for social Sciences (SPSS version 18).Results:- The means of all variables examined increased during six months oforthodontic treatment with high significant difference during visits. 49.6% ofpatients developed new caries lesions after six months of treatment.Conclusion:- Patients underwent orthodontic treatment at high risk for developingoral health problems.

Abstract

Background: Traditional orthodontic treatment takes a long time and this may lead to several disadvantages likehigher predisposition to periodontal diseases and dental caries. Recently, many researchers focus on shorterorthodontic treatment time through different methods like the use of: vibration, surgical approach, adjunctivemedicine, laser and others. This study aimed to determine the effect of Acceledent vibration device on the gingivalhealth condition and the changes in levels of salivary IL-1β and TNF- α among patients under fixed orthodonticappliance during orthodontic treatment.Material and Methods: A total of 32 adult patients with an age ranged between 19-23 years were participated in thisstudy. They were divided into two groups: 14 patients under fixed orthodontic appliance with the use of Acceledentvibration device as study group and 18 patients under fixed orthodontic appliance only as control group. All patientswere with Class I and /or Class II malocclusion cases requiring bilateral extraction of their maxillary first premolarteeth, underwent a session of professional oral hygiene and received oral hygiene instructions before and during theperiod of study. The collection of the unstimulated salivary samples from the individuals was performed at differenttimes after placement of the fixed orthodontic appliance; base line (T0), after 1 hour (T1), after one week (T2), aftertwo week (T3). The levels of salivary IL-1β and TNF-α were estimated by using commercially available ELISA kits. Oralcleanliness were determined by using of gingival index (GI) and plaque index (PI).Results: For both study and control groups, results reported that the mean value of salivary IL-1-β (pg/ml) and TNF-α(pg/ml) were highest at T1, followed by T2, then T3 and lowest T0 with highly statistically significant difference(P<0.01).The mean value of both salivary IL-1-β (pg/ml) and TNF-α (pg/ml) among study group were higher thancontrol group at T1, followed by T2, then T3 with statistically significant difference (P<0.05). Among study group only,results of this study recorded a negative direction correlation between both IL-1-β (pg/ml) and TNF-α (pg/ml) andgingivitis at the (T2 and T3) and (T3) with statistically highly significant and significant respectively.Conclusion: The application of Acceledent device play important role in improving the gingival health condition andincreasing the levels of salivary IL-1β and TNF-α secretion among patients under fixed orthodontic appliance incomparison to other under fixed orthodontic appliance

Abstract

Aim: This clinical study is to investigate the effect of fasting (Sawm) Ramadan on thecount of streptococcus mutans in plaque surrounding fixed orthodontic appliances.Materials and Methods: Fifteen patients with orthodontic appliances wereparticipated in this study. Plaque specimens were collected from those patients attwo occasions, during Rmadan fasting (Sawm) and after they break there fastingafter Ramadan. The plaque Streptococcus mutans number of colony-formingunits (CFU) was analyzed by using Dentocult SM Strip mutans test.Results: This study demonstrated highly significant difference in Streptococcusmutans (CFU) counts in plaque between the values obtained from fasting patientsduring Ramadan and when they are breaking the fast after Ramadan.Conclusions: during Ramadan fasting, persons change timing and types of their foodand beverages consuming as well as teeth cleaning habits. Therefore orthodontistshould be aware of their patients’ oral hygiene during Ramadan by promoting andmaintaining satisfactory teeth cleaning during fasting (Sawm) and Iftar hours.

Abstract

Background:- White spot lesions are common esthetic problem that compromise the success of orthodontic treatment. This study aimed to assess white spot lesions in patients with fixed orthodontic appliance at different time intervals.Materials & Methods:- Thirty two patients (24 females and 8 males) were included in this study and they underwent clinical examination for white spot lesions using enamel decalcification index at four time intervals: (2-3 weeks after appliance insertion, 2, 4 and 6 months).Results:- The patients were free of white spot lesions at the appliance insertion visit. The mean of white spot lesions was 2.22 which were increased significantly during six months to reach 24.59 at the end of study. There was a significant difference between the maxillary and the mandibular arches, however, there was no significant difference found between the right and the left sides in both arches. The total numbers of teeth affected by white spot lesions were 74.61% after six months of treatment. Maxillary second premolar and mandibular canine were the most affected teeth; whereas the gingival area around the orthodontic brackets was the most affected area.Conclusion:-Orthodontic patients had a high risk for development of white spot lesions

Abstract

this study evaluated the effect of fixed orthodontic appliance treatment of anterior cross-bite having a removable posterior bite plate (removable appliance combined with fixed appliance) and fix orthodontic appliance treatment with composite light cure filling material on the occlusal surfaces of lower second premolars and first molars as block by etching and bonding the teeth,24 patients were selected from the medical technical institute clinic in Baghdad from the first of January 2013 until the 30th of December 2013 complaining from anterior cross-bite one tooth or more age range (13-19)11male and 13 female subdivided into three groups depending on the type of treatment used, results showed that there are highly significant difference in treatment time in response to the group of patient with posterior composite etched block(<0.001) even though the displacement space results between groups showed a non significant difference, in conclusion fix orthodontic appliance with posterior composite etched block on premolars and molar teeth is the shortest and best treatment technique we can use for treating such conditions

هذه الدراسة قيمت علاج الاطباق المعكوس الامامي بثلاث طرق هي : استخدام جهاز التقويم الثابت ,استخدام جهاز التقويم الثابت والمتحرك معا",استخدام جهاز التقويم الثابت والحشوةالضوئية على سطح الاطباق للاسنان الخلفية .تم اختيار أربع وعشرين مريضا منذ يوم 1/1/2013 وحتى يوم 30/12/2013 كانو يعانون من الاطباق المعكوس الامامي في سن واحد او اكثر .نتائج الدراسة بينت ان هناك فرق واضح جدا في تقليل فقرة العلاج في الطريقة الثالثة ( استعمال الجهاز المتحرك مع الحشوة الضوئية ) بينما لايوجد فرق ملموس وواضح في الطريقتين الاخريين .الدراسة استنتجت ان جهاز التقويم الثابت مع الحشوة الضوئية على اسطح الاطباق للاسنان الخلفية أقصر وافضل طريقة لعلاج هذه الحالةالمرضية .
