A Study on the Relationship between Thyroid Hormones and Adenosine Deaminase Enzyme Activity in Patients with Auto- Immune Hyperthyroid Disease

Abstract

ABSTRACT :BACKGROUND :Adenosine deaminase ADA catalyzes the deamination of adenosine in purine metabolism . Adenosine deaminase deficiency is an inborn error resulting in immunodeficiency condition . clinical interest in this enzyme has been revived by the discovery of a syndrome of severe humoral and cellular immunodeficiency associated with deficiency of ADA in somatic erythrocyte cell .AIM OF STUDY:The aim of present study is conducted to find the relationship between human hyperthyroid activities and immune etiology in graves disease.MATERIALS AND METHODS :Forty individuals with hyperthyroidism autoimmune state were included in this study in comparison with forty normal persons .Thyroid hormones levels were estimated by HPLC and serum ADA specific activity was estimated using spctrophotometric method at 265 nm . Serum protein value was determined by Bradford method .RESULTS :Serum ADA activity was measured for 40 hyperthyroid patients as well as 40 normal individuals . The mean activity of ADA in patients was higher than in the controls . A significant increase in T3 / T4 ratio and level of TSH were noticed .CONCLUSION :A positive correlation between thyroid level and ADA enzyme activity was noticed .

EVALUATION OF IMMUNOHISTOCHEMICAL EXPRESSION OF CK19 IN PAPILLARY THYROID CARCINOMA AND GRAVE'S DISEASE WITH PAPILLARY CHANGES

Abstract

Background:Immunohistochemistry seems to be important method for differentiation between papillary thyroid carcinoma which is the commonest thyroid cancer and Grave's disease which may be challenging by light microscopic features only.Objectives:The aim of the study was to evaluate the immunohistochemical expression of CK 19 antibody that is considered a marker of malignancy in papillary thyroid carcinoma and in Grave's disease and to determine whether CK19 is effective in the discrimination between these two pathological conditions.Methods:In this retrospective study paraffin embedded archival materials from 40 cases including 20 papillary thyroid carcinoma and 10 Grave's disease in addition to ten normal thyroid tissue were used as a control group, collected from the department of pathology of Al-Yarmook Teaching Hospital for the period between Jan 2009 to Jan 2011. The immunohistochemical expression of CK19 markers was assessed for intensity and extent of staining in semiquantative method.Results:Sixteen of the 20 papillary carcinomas showed diffuse and intense cytoplasmic staining with CK19 (80%), 4 cases showing diffuse faint staining (20%). Seven of the 10 Grave's cases (70%) are completely negative. The remaining 3 cases showing focal weak staining with CK19 (30%). There was a significant difference in the extent of staining between papillary thyroid carcinoma and Grave's disease and there was highly significant difference in intensity of staining between them.Conclusions:The staining features of CK19 may be helpful in the differential diagnosis between papillary carcinoma and Grave's disease with papillary carcinoma like structures. This immunoreactivity should be evaluated with histopathological findings in order to prevent over diagnosis of papillary carcinoma.Keywords:CK19 immunohistochemical expression, papillary thyroid carcinoma, Grave's disease.

Abstract

AbstractBack ground: Autoimmune thyroid disease commonly presents as a goiter which may be diffuse or nodular and is usually associated with raised titers of thyroid antibodies, and usually affects 2-5% of the western Population.Aim of study: To know the rate of autoimmune thyroid disease in thyroidectomised patients in Al-yarmouk teaching hospital.Patients and methods: A prospective study performed in Al-yarmouk teaching hospital and Al-mustansiriya medical college, in which patients who underwent thyroidectomy for thyroid gland enlargement were included during a period of one year from March 2011 to March 2012. These cases have been assessed both clinically and by investigations including thyroid function tests, thyroid auto-antibodies (anti-thyroglobulin antibodies TG) and (anti-thyroid peroxidase antibodies TPO) , ultra-sound of thyroid gland and FNAC of thyroid gland, the results were compared to the histopathological results of the same patients, and P value calculated. Results: sixty cases with goitre (operated upon) in the surgical wards of Al-yarmouk teaching Hospital were included, 10 cases had positive serological results for autoimmune thyroid disease (3 cases 5% Hashimoto's thyroiditis, and 7 cases 11.6% Grave's disease) were diagnosed and the diagnosis was proved by post-operative biopsy results. the commonest symptom was thyroid gland enlargement and the commonest age incidence was in the 4th decade.Conclusion: These findings suggest the possibility of using quantitative indicators of thyroid auto-antibodies as predictors of the presence or absence of autoimmune thyroid disease in patients planned for thyroidectomy.

الخلاصةالخلفية العلمية :- امراض الغدة الدرقية المناعية الذاتية عادة ما تظهر على شكل دراق ( تضخم الغدة الدرقية ) بصورة منتشرة او عقدية و عادة ما ترتبط بارتفاع اعداد الاجسام المضادة للغدة الدرقية و التي تصيب 2-5% من السكان الغربيين.الهدف من الدراسة:- معرفة معدل امراض الغدة الدرقية المناعية الذاتية لدى المرضى المجراة لهم عملية رفع الغدة الدرقية في مستشفى اليرموك التعليمي.المرضى و الاساليب:- تم اجراء دراسة استطلاعية في مستشفى اليرموك التعليمي وكلية طب المستنصرية والتي شملت المرضى الذين خضعوا لعملية استئصال الغدة الدرقية خلال فترة سنة واحدة من اذار 2011-اذار 2012 . تم تقييم هذه الحالات من الناحية السريرية و المختبرية باجراء فحوصات وظائف الغدة الدرقية و الاجسام المضادة الذاتية للغدة الدرقية ( الاجسام المضادة لكلوبين الغدة الدرقية , الاجسام المضادة لبيروكسيديز الدرقية ) اجراء فحص موجات فوق الصوتية وفحص الابرة النسيجي الدقيق للغدة الدرقية , وتمت مقارنة هذه النتائج مع النتائج النسيجية لنفس المرضى النتائج:- تم ادراج ستون (60) حالة اصابة تضخم الغدة الدرقية (مجراة لهم عملية استئصال) في الاقسام الجراحية لمستشفى اليرموك التعليمي . تم تشخيص عشرة (10) من هذه الحالات لها نتائج ايجابية مصلية لامراض الغدة الدرقية المناعية الذاتية [ ثلاث (3) حالات 5% التهاب الغدة الدرقية (هاشيموتو) , سبع (7) حالات 11.6% (مرض كريفيز) ] والتي تم تاكيد التشخيص عن طريق نتائج خزعة الغدة الدرقية بعد العمليات الجراحية . كان تضخم الغدة الدرقية في العقد الرابع هو اكثر الاعراض شيوعا لدى هذه الحالات المرضية.الخلاصة:- تشير النتائج الى امكانية استخدام المؤشرات الكمية للاجسام المضادة الذاتية للغدة الدرقية كمؤشر لوجود او عدم وجود أمراض الغدة الدرقية المناعية الذاتية في المرضى المصابين بتضخم الغدة الدرقية و المخطط لهم اجراء عملية استئصال الغدة الدرقية
