Gynecomastia Treatment Should it be Individualized? A Prospective Study

Abstract

ABSTRACT:BACKGROUND :Gynecomastia is a benign enlargement of the male breast.OBJECTIVE:Was to analyze a 3 year period of gynecomastia patients in Al-Kindy Teaching Hospital and the surgical approach used and its outcome.PATIENTS AND METHODS:A prospective study of patients consulting Al-Kindy Teaching Hospital because of gynecomastia between October 2007and August 2010 was done. Data were analyzed for etiology, grade of gynecomastia, surgical technique, complications, and histological results.RESULTS:A total of 64 patients with 86 operations were included. Techniques included subcutaneous mastectomy alone, or subcutaneous mastectomy with wise pattern "inverted T incision". Atypical histological findings were found in 3.12% of the patients (papilloma, fibrolipoma). Surgical revision among all patients was 7%. Body mass index higher than 25kg/m2 was found as significant risk factors for complications (p.043), and that higher grades of gynecomastia (grade IIb and III )had higher statistical incidence of complications p= 0.01CONCLUSION:The treatment of gynecomastia requires an individualized approach. Caution must be taken in high BMI and higher grades, which are associated with increased complication rates. Histological tissue diagnosis should be routinely performed in all true gynecomastia corrections, because histological results may reveal atypical cellular pathology.

Histopathological Study of Escherichia coli O157:H7 Isolated from Children with Bloody Diarrhea in Mice

Abstract

Escherichia coli O157:H7 is well documented with Shiga toxin–producing serotypes of E. coli. Infection with this type of pathogenic bacteria may lead to hemorrhagic diarrhea and kidney failure. This study was carried out to detect the pathogenicity of E. coli O157:H7 isolated from patients with bloody diarrhea. A total of 200 bloody diarrhea samples were collected from children of both sexes, with age between 3 to10 years in the period from beginning of September to the end of December 2016 in Al-Eskan Pediatrics Hospital and Children Safe Hospital (Baghdad/Iraq). The samples were cultured aerobically on enrichment and selective media, then the isolates were identified by Vitek 2 and they were confirmed by latex agglutination test. Eight isolates were diagnosed and identified as Escherichia coli O157:H7. The pathogenicity of the stool recovered isolates were study to recognize the alterations in some organs of mice after experimentally infected with this pathogen. Twelve mice provided by animal house of AL-RAZI center in Baghdad, divided into two groups each group consist of 6 mice. The first group injected with 0.2 ml of de-ionized water and left as control group, whereas the second group infected orally with 0.2 ml of 1.8x106cfu/ml. The animals of two groups sacrificed 24-48 hours post infection. The histopathological examination of intestine for infected mice showed infilteration of inflammatory cells, focal lining epithelial stratification, with multiple layer basal lamina degeneration, then distention of villi appeared with increase inflammatory cells infiltration. The liver showed accumulation of lymphocyte, hemorrhage in central vein with sinuses expansion and, hepatocyte cells showed, degeneration, increase nuclear size and increase hyperchromasia with irregular chromatin distribution. The histopathological examinations of the control group were naïve.

Abstract

Acute appendicitis is one of the commonest surgical emergencies. The diagnosis is stilldoubtful in a number of cases. Neutrophil/Lymphocyte ratio (N/L R) is a simple, applicablehematological test which carried out to find if there is any relationship between this ratio andacute appendicitis and to see could the ratio be used as a diagnostic tool rather than whiteblood cell count (WBC) alone.This study was carried out on 70 patients aged (9-45) years who was admitted to hospital withsuspicion of acute appendicitis, latter on, they underwent appendectomy. Preoperative bloodsample was collected from each patient for WBC and differentials leukocyte count.Postoperatively appendectomy specimens were taken to histopathological examination, whichshows 52 cases of inflamed appendix and 18 normal appendices.When the (N/L R) and (WBC) were correlated with results of histopathology, it was found thatthere is a relationship between acute appendicitis and (N/L R), especially if we use the ratio atcut off point which was>2.6(p value<0.05=0.0001), The sensitivity was 80.76% while thespecificity was 72.22% and accuracy was 78.57%, while the results showed that the (WBC)alone had a limited role in diagnosis of acute appendicitis (p value >0.05=0.219).In conclusion, there is a strong relationship between (N/L R) and acute appendicitis.

Abstract

Twenty female rats white were used in this study, which divided in to two groups ,the first group (control) , while the Second group(10) rat were in number the group was infected by toxoplasma gondii was injected 0.5 cm3 intraperitonially in pregnant rats at 7 days after gestation . Examination the of rats was have after injection, to show the effected parasite on it in comparsion white rats control.

استخدمت في هذه الدراسة عشرون من اناث الجرذان البيض حيث قسمت الى مجموعتين المجموعة الاولى مجموعة السيطرة والمجموعة الثانية هي المجموعة التي تم حقنها بطفيلي المقوسات الكوندية Toxoplasma gondii وجرى حقن 0.5 سم3 من عالق المشيمة في التجويف البريتوني في اليوم السابع من الحمل وجرى فحص الراشح البريتوني للجرذان بعد الحقن للتأكد من اكتساب الجرذان للاصابة بالطفيلي وذلك لمعرفة التأثير المرضي على الاعضاء المصابة مقارنة مع مجموعة السيطرة .

Abstract

The present study was carried out in AL – Sadder education hospital – AL – Najaf province of histopathological Lab. ( 100 ) patients were examined, ( 31 ) patients were infected with colon cancer, the age of patients ranged between ( 25 – 84 ) years old. This work was involved The relationship between colon cancer and many factors such as gander, age, smoking, and blood groups, and also histopathological description. our study was revealed, the colon cancer was affected the age ranged between ( 60-64 ) at percent ( 19.4% ) and more these age ,and patients had blood groups (O+) and (B+) were more than other blood groups, the blood group (O+) patients with colon cancer at percent ( 41.9% ) and blood group (B+) at percent ( 29% ). Our finding not appears any effect of smoking, and gender factors on the prevalence of colon cancer, Histopathological studying was revealed, damage in epithelial lining of colon mucosa, hemorrhage, hyperplasia in the intestinal glands structures and connective tissue that found in sub mucosa of colon as well as polymorphic leucocytic infiltration, especially lymphocytes, and increased in the goblet cells component in the mucosa of colon.

وقد أجريت هذه الدراسة في AL - مستشفى التعليم حزنا - محافظة النجف من مختبر الأنسجة - AL. تم فحص (100) مريضا، أصيب (31) المرضى الذين يعانون من سرطان القولون، وتراوحت أعمار المرضى بين (25-84) سنة. وقد شمل هذا العمل العلاقة بين سرطان القولون والعديد من العوامل مثل الاوز، والعمر، والتدخين، وفصائل الدم، ووصف أيضا الأنسجة. دراستنا كشفت، تأثر سرطان القولون تراوح أعمارهم ما بين (60-64) في المئة (19.4٪) وأكثر هذه السن، وكان المرضى فصائل الدم (O +) وكانت (B +) أكثر من فصائل الدم الأخرى، و فصيلة الدم (O +) المرضى الذين يعانون من سرطان القولون في المئة (41.9٪)، ومجموعة الدم (B +) في المئة (29٪). لا يبدو لنا العثور على أي أثر التدخين، والعوامل المتعلقة بنوع الجنس على انتشار سرطان القولون، ودراسة نسيجية مرضية تم الكشف عن وتلف في بطانة الظهارية من الغشاء المخاطي القولون، والنزف، وتضخم في هياكل الغدد المعوية والنسيج الضام التي وجدت في الغشاء المخاطي الفرعي القولون فضلا عن تسلل متعدد الأشكال leucocytic، وخاصة الخلايا الليمفاوية، وزيادة في مكون الخلايا القدح في الغشاء المخاطي للقولون.
