Abstract

Urinary stone fragmentation with long pulsed Ho:YAG laser (wave lengthequal 2.1 μm) and Er:YAG laser (wave length equal 2.94 μm) investigated in thispaper.Fragmentation efficiency of these two lasers is measured by using various energysettings. Laser induced crater depth and ablation volume for both lasers wereexamined and compared using mathematical model. Theoretical results werecompared with experimental results obtained by Hyun Wook Kang.The study shows that the theoretical results and experimental results arecomparable, and the crater depth when using Er:YAG laser was more than that onHo:YAG laser.

Abstract

The laser-tissue interaction and its thermal effects depend on the parameters of both of the tissue and the used laser. In this paper the laser ablation efficiency of prostate tissue has been studied theoretically by using pulsed Nd:YAG, Ho:YAG and Thulium fiber surgical lasers. Different levels of energies were considered for these lasers, which operate in 20 and 40 Hz pulse repetition frequencies (prf) to evaluate the prostate tissue temperature rise up to the required ablation state. The Gambit program was used first to simulate the geometrical shape of the treated prostate tissue, then the radiative transfer equation (RTE) of the ANSYS Fluent program was applied to simulate and solve the ablation process equations. The results showed that the low rates of repetition frequencies and low laser energies increase the allowed time for safe ablation operation before reaching the carbonization state. It was found also that although the Ho:YAG laser vaporizes prostate tissue faster than the Thulium fiber laser did, the pulsed Thulium fiber laser produced much lower temperature side effects like coagulation, hyperthermia and carbonization of the rest of the prostate tissue, which is surrounding the ablated zone for the same applied average powers by the other two lasers.

يعتمد التاثير المتبادل بين الليزر والنسيج الحيوي ومايولده من تاثيرات حرارية على مواصفات كل من النسيج والليزر معا. وفي هذا البحث درست امكانية وكفاءة استئصال نسيج البروستات باستخدام الليزرات النبضية من نوع النيوديميوم ياك والهولميوم ياك وليزر ليف الثوليوم التي تستخدم في العمليات الجراحية. ولهذا الغرض أستخدمت طاقات متعددة من هذه الليزرات في الدراسة وبترددين نبضيين هما 20و40 هرتز لاجل رفع درجة حرارة النسيج الى حد تبخره واستئصاله. لغرض نمذجه هذه العملية استخدم برنامج الكامبت لرسم نموذج هندسي يمثل شكل البروستات ثم أستخدم برنامج الانسس واختيرت معادلات نقل الاشعاع التي تقع ضمن برنامج اخر اسمه الفلونت لحل ونمذجة عملية الاستئصال هذه. وقد اوضحت النتائج التي تم الحصول عليها انه باستخدام الطاقات الواطئة وبترددات تكرار النبضات الواطئة ايضا يزداد الوقت المتاح خلال العملية للحصول على استئصال آمن من دون حدوث تفحم في نسيج البروستات المحيط بمنطقة الاستئصال ووجد ايضا ان ليزر ليف الثوليوم افضل من الليزرين الاخرين في الاستئصال فهو اقل توليدا للتاثيرات الحرارية الجانبية مثل التخثر وفرط الحرارة وتفحم النسيج و ذلك بغض النظر عن كون ليزر الهولميوم ياك اسرع في تبخير النسيج واستئصاله من ليزر ليف الثوليوم.

Safety and Outcomes of Retrograde Semi-Rigid Ureteroscopy in the Treatment of Renal Pelvic Stones by Using Holmium: YAG Laser.

Abstract

The aim of this study was to evaluate the clinical utility, safety and outcomes of semi-rigid ureteroscopy in the treatment of single renal pelvic stones by using Holmium: YAG laser lithotripsy.Hundred patients (56 females, 44 males), aged between (11) and (67) years (mean 41.5 years), with single renal pelvic stone range from 13-27 millimeter (mean size 21.8 millimeter). Inclusion criteria (any patient with single renal pelvic stone less than 30 millimeter in size regardless its composition and previous history of any urological intervention and the stone is visualized by uretroscope), treated with semi-rigid ureteroscopy by using Holmium: YAG laser for stones pulverization. DJ-stent had been inserted to all patients following stone fragmentation. Preoperative data, as well as stone free rate, operative time and associated perioperative complications were evaluated.The primary stone free rate (which was assessed by Ultrasonography and plain abdominal X-ray) was sixty-eight percent (68 patients) at the first postoperative day, which increased to eighty-six percent (86 patients) after one month by using ancillary procedures such as Extra Corporeal Shock Wave Lithotripsy and medical treatment. the operative time was ranges from 44.3 to 97.8 minute (mean of 72.4 minute), there was no major perioperative complications.The study can conclude that Reterograde semi-rigid ureteroscopy using holmium: YAG Laser is effective and safe option for managing single renal pelvic stone less than 27 mm.
